  • Vilka är ute i naturen? Delresultat från en nationell enkät om friluftsliv och naturturism i Sverige

    • Detta är en av fyra rapporter som ger en översiktlig bild av resultaten från en nationell enkätundersökning om friluftsliv och naturturism i Sverige. Studien är genomförd av forskningsprogrammet Friluftsliv i förändring. Resultaten som presenteras här fokuserar bl a på vilka som är ute i naturen och vilka aktiviteter man ägnar sig åt. Nästan alla uppfattar skogar, fjäll, sjöar och hav som ”natur” men det är inte alls lika självklart med t.ex. badstränder, trädgårdar och elljusspår. Mer än hälften av de svarande är ute i naturen ganska ofta, eller mycket ofta, under vardagar. Under längre ledigheter ökar denna andel till 89 %. Naturen är också en central miljö för barns uppväxt och t.ex. har hela 94 % av de svarande ofta, eller mycket ofta, tillbringat lov och semestrar i naturen. Nöjes- och motionspromenader är vanligast att ha ägnat sig åt åtminstone en gång under en 12- månadersperiod (nästan 95 % av de svarande), följt av strövat i skog och mark (90 %), arbetat i trädgården (84 %), solbadat (81 %), och haft picknick eller grillat i naturen (80 %). Även om vi ser till de aktiviteter som är vanligast att ägna sig åt ofta (mer än 60 gånger per år) så toppas listan av nöjes- och motionspromenader (ca 32 %), följt av cykla på vägar, arbeta i trädgården, ströva i skog och mark och promenera med hund. Vi har också gjort en fördjupad analys när det gäller de tio vanligaste aktiviteterna som de svarande angett att de utövat mer än 20 gånger under en 12-månadersperiod (ca 1-2 gånger per månad). Det visar sig då att kvinnorna generellt är mer aktiva. Jogging/terränglöpning, jakt och fiske är mer vanligt bland män medan solbad, picknick och stavgång är mer representerat hos kvinnor. Gruppen under 30 år skiljer ut sig något genom att promenera med hund är vanligare och arbete i trädgården mindre vanligt, jämfört med övriga grupper. Joggning/terränglöpning tycks också vara en aktivitet främst hos yngre och medelålders, medan det omvända gäller för stavgång. Föräldrarnas uppväxtland tycks inte ha så stor betydelse för vilka aktiviteter man väljer, men svarande med svenska föräldrar utövar aktiviteterna i större utsträckning jämfört med personer vars föräldrar vuxit upp utomlands. Ett undantag finns emellertid för picknick eller att grilla i naturen vilket är vanligare hos svarande med utländska föräldrar.

  • Comparison of oral health assessments between nursing staff and patients on medical wards

    • The maintenance of good oral health is essential for nutrition, recovery and well-being. This requires the involvement of the nursing staff, especially in cases where oral care and any necessary dental treatment are vital to ensure medical treatment. The aim of this study was to evaluate the validity of oral assessments performed by nursing staff using the revised oral assessment guide (ROAG), using comparisons with patients' self-assessment of oral problems. When a comparison was made of how the staff and patients assessed their oral status, a high level of agreement was found. In these assessments, with the exception of oral mucosa and teeth, the percentage agreement was >80. The kappa coefficient revealed slight to moderate agreement. When there was a disagreement, the staff assessed the oral health as being significantly poorer than the patients did. In the present study, it was shown that few oral assessments performed by the nursing staff and patient disagreed. The ROAG may therefore be useful for the nursing staff to make the patients' oral health problems visible. © 2007 The Authors; Journal compilation © 2007 Blackwell Publishing Ltd.

  • Comparison of clinically diagnosed asthma with parental assessment of children's asthma in a questionnaire

    • Epidemiological evaluations of the prevalence of asthma are usually based on written questionnaires (WQs) in combination with validation by clinical investigation. In the present investigation, we compared parental assessment of asthma among their preschool children in response to a WQ with the corresponding medical records in the same region. An International Study of Asthma and Allergies in Childhood (ISAAC)-based WQ was answered by 75% of the parents of 6295 children aged 1–6 yr. Clinically diagnosed asthma, recorded in connection with admissions to the hospital or a visit to any of the outpatient clinics in the same region, were analysed in parallel. Finally, a complementary WQ was sent to the parents of children identified as asthmatic by either or both of this approaches. In response to the WQ 5.9% were claimed to suffer from asthma diagnosed by a doctor. According to the medical records, the prevalence of clinically diagnosed asthma was 4.9%. The estimated prevalence among children requiring treatment for their asthma was 4.4%. The sensitivity of the WQ was 77%, the specificity 97.5%. In the 1–2 yr age group the sensitivity was only 22%. This WQ was able to identify 54% of the children with a medical record of asthma. Forty percent of the children claimed by their parents to be asthmatic had no medical record of asthma. An ISAAC-based parentally completed WQ provided an acceptable estimation of the prevalence of asthma in children 2–6 yr of age, although only half of the individual patients identified in this manner are the same as those identified clinically.

    • The objective of this study is to portray the illness-related threats experienced by parents of children after the diagnosis of central nervous system (CNS) tumor. Parents were asked to rate the extent to which they experienced a set of specific fears related to their child's brain tumor and its treatment. Outcomes for parents of CNS tumor patients (n = 82) were compared with those of reference parents of patients treated for acute lymphoblastic leukemia (n = 208). The fears about an illness recurrence and the late effects of treatment were most prominent among parents of CNS tumor patients. For 7 out of 11 kinds of fear, parents of CNS tumor patients expressed a stronger fear than the reference group. More than a quarter of the parents of children treated for CNS tumors feared a complete decline of the child. Parents of CNS tumor patients experience relatively heightened cancer related fears in several domains. The fear of devastating consequences felt by one fourth of parents signals the need of individualized psychological support and information at diagnosis and follow-up to facilitate parental coping with the posttreatment situation.

    • Overload training resulting in an overreached state is common in elite sports, and if undetected can develop into an overtraining syndrome. This risk is accentuated by the lack of reliable measures of overreaching. Coaches and scientists therefore have to use a combination of tests in the monitoring process. This article presents a case study of the recovery from underperformance of a young elite endurance athlete and the work of a multidisciplinary sport science support team. When it was determined that the athlete's performance had deteriorated, and that this was due solely to the stress of training, training load was radically reduced for a period of 14 days. A combination of physiological, biochemical, and psychological measurements were then used to monitor the recovery process. The purpose of this article is to describe how coaches and sport science teams can help in monitoring training and recovery in practical settings, allowing detection of the early signs of overreaching before a more serious overtraining syndrome develops.

    • Background: Skilled and specialist registered nurses (RNs) are central to evolving elderly care. The past decades' organisational and structural changes have altered RNs' roles and work situations in municipal elderly care in Sweden. This calls for appropriate educational preparation. However, a substantial proportion of RNs in municipal elderly care lack adequate specialist competence. Aim: The focus of this study was to describe RNs' perceptions of needs and possibilities for competence development in municipal elderly care and compare the perceptions of RNs' who work solely in dementia care (DC) with those who work in general elder care (GC) where older persons have diverse diagnoses. Design: A non-experimental, descriptive and comparative design was used. Settings: Sixty special housing with subunits in a large city in the middle of Sweden. Participants: Participating RNs were a total of 213, with a response rate of 62.3%. Of the 213 RNs, 95 (44.6%) worked in DC and 118 (55.4%) in GC. Method: A structured questionnaire that was specifically designed for this study and focused on needs and possibilities for competence development in nursing. Results: The RNs were on average not lacking or were hardly lacking knowledge in examined domains. However, RNs in GC lacked knowledge of dementia, falls, and fall injures to a greater extent than RNs in DC. RNs in DC perceived greater possibilities for competence development at work. Most RNs requested a better organisation for competence development, especially in GC. The majority of RNs had no supervision. The use of RNs' competence was high, although they used their highest competence about half of the working hours. The employers' financial contribution to RNs' continuing education was poor. Conclusion: A better organisation and greater possibilities for RNs' competence development is needed. The employers need to make a greater contribution financially to RNs' continuing education. Aim: The main focus was to describe RNs' education and their view of competence development in municipal elderly care. Another aim was to compare RNs working solely in dementia care (DC) with those working in general elder care (GC) of older persons with diverse diagnoses. Design: A non-experimental, descriptive design with a survey research approach was used. Settings: Sixty special housing with subunits including those offering daytime activities in a large city in the middle of Sweden. Participants: The number of participating RNs was a total of 213, with a response rate of 62.3%. Of the 213 RNs, 95 (44.6%) worked in DC, and 118 (55.4%) in GC. Method: A questionnaire survey. Results: The findings showed that RNs possessed a broad range of competence. The majority lacked a bachelor's degree in nursing. Few had adequate specialist competence. RNs' in DC wanted to invest more in competence development whereas RNs in GC were more motivated to attain greater authority in the making of important decisions and to seek another position. Conclusion: An important future prospect is to develop the competence of RNs in elderly care. In order to ensure high quality and security in elderly care, it is also essential to increase the number of RNs with specialist competence. (c) 2005 Elsevier Ltd. All rights reserved.

This change has had a significant impact on the work situation of registered nurses (RNs). Aim: The main focus was to describe RNs' work situation and their characteristics in municipal elderly care. Another aim was to compare RNs working solely in dementia care with those working in general care of older persons with diverse diagnoses. Design: A non-experimental, descriptive design with a survey research approach was used. Settings: Sixty special housing units with underlying units including those offering daytime activities in a large city in the middle of Sweden. Participants: The number of participating RNs was a total of 213, with a response rate of 62.3%. Of the 213 RNs, 95 (44.6%) worked in dementia care, and 118 (55.4%) in general care. Method: A questionnaire survey. Results: The results indicated high levels of time pressure in both groups. Greater knowledge and greater emotional and conflicting demands were found in dementia care. The majority perceived a greater opportunity to plan and perform daily work tasks than to influence the work situation in a wider context. Support at work was perceived as generally high from management and fellow workers and higher in dementia care.   • Factors associated with poor general health after stem-cell transplantation

    • Aims: To describe functional status (FS), general health (GH) and symptom distress (SD) from admission to 1 year post-SCT and to identify medical, demographic, and/or patient-reported outcome variables associated with patient-perceived GH. Material and Methods: Forty-one patients (27 women) with a median age of 44 (18-65) years answered three questionnaires (SIP, SWED-QUAL, and SFID-SCT) from admission to 1 year post-SCT. Results: At discharge, 59% of the patients reported poor FS and GH, and 24% reported > 10 simultaneous symptoms. After 1 year post-SCT, 22% still reported poor FS, 32% poor GH, and 12% > 10 simultaneous symptoms. Compared with admission, significantly larger proportions of the patients reported poor GH at discharge (20 vs 59%, p = .001), poor FS at 6 months (24 vs 59%, p = .004), and poor GH [The number of symptoms was found to be significantly associated with poor GH at discharge (OR 1.330, p = .009) and at 1 year post-SCT (OR 2.000, p = .010)]. Patients reporting "poor GH" at discharge and at 1 year post-SCT reported a median of 7 and 10 symptoms, respectively. Patients with "good GH" reported a median of three symptoms both at T1 and T4. "Tiredness", "anxiety", "mouth dryness", "loss of appetite", and "diarrhoea" were reported by a larger proportion of the patients reporting "poor GH". Conclusions: The results confirm that some patients who have undergone a SCT have a negatively affected life situation. The study indicates that actively asking for symptoms and applying the best treatment for symptom alleviation are among the most important measures that SCT teams can take to help the patients perceive better general health and an improved life situation.
