  • Readmission Rates Due to Post-Tonsillectomy Haemorrhage in Sweden: SIGNIFICANT DIFFERENCES BETWEEN SURGICAL CENTRES : Significant Differences Between Surgical Centres

    • Objectives: Post-surgical haemorrhage is the major complication of tonsil surgery. Post-tonsillectomy haemorrhage often results in readmission to hospital, sometimes demands return to theatre and can be a life-threatening occurrence. There are large differences in previously reported rates of post-tonsillectomy haemorrhage. The aim of this study is to determine readmission rates due to post-tonsillectomy haemorrhage in Sweden 2012-2014.Method: This is a population-based cohort study based on matched data from two healthcare registries in Sweden; the Swedish National Patient Register (NPR) and the National Tonsil Surgery Register in Sweden (NTSRS). All patients who underwent tonsillectomy, with or without simultaneous adenoidectomy, on benign indications from 2012 to 2014 were included in the study. The unique Personal Identity numbers were used to follow patients over time in NPR and identify readmission due to haemorrhage within 30 days from surgery. In NTSRS, readmission was reported by the patient in a questionnaire 30 days after surgery.Results: By matching the two registries a total of 22 800 unique patients were identified and included in the study. A total of 1726 patients (7.6%) were readmitted to hospital due to post-surgical haemorrhage. Tonsil surgery was conducted at 56 different surgical centres. Readmission rates due to post-tonsillectomy haemorrhage ranged from 0% to 22.4% at different surgical centres. 11 of 56 surgical centres had significant lower readmission rates compared to the average in Sweden. 12 of 56 surgical centres had significant higher readmission rates than the average.Conclusions: The rate of hospital readmissions due to post-surgical haemorrhage following tonsillectomy in Sweden 2012-2014 was 7.6%. There were significant differences in readmission rates between different surgical centres. This suggests that quality improvement interventions should be explored in units with high readmission rates to decrease post-tonsillectomy morbidity.

    • This paper describes the design of MEFISTO (Mercury Electric Field In-Situ Tool) and WPT (Wire Probe Antenna) electric field sensors for Plasma Wave Investigation (PWI) on the BepiColombo Mio spacecraft (Mercury Magnetospheric Orbiter, MMO). The two sensors will enable the first observations of electric fields, plasma waves and radio waves in and around the Hermean magnetosphere and exosphere. MEFISTO and WPT are dipole antennas with 31.6 m tip-to-tip length. Each antenna element has a spherical probe at each end of the wire (15 m length). They are extended orthogonally in the spin plane of the spacecraft and enable measurements of the electric field in the frequency range of DC to 10 MHz by the connection to two sets of receivers, EWO for a lower frequency range and SORBET for higher frequencies. In the initial operations after the launch (20 Oct. 2018), we succeeded to confirm the health of both antennas and to release the launch lock of the WPT. After Mercury orbit insertion planned at the end of 2025, both sensors will be fully deployed and activate full operations of the PWI electric field measurements.

  • Metonymy in the semantic field of verbal communication : A corpus-based analysis of WORD

    • Language about language has been studied above all from the perspective of metaphor, neglecting metonymy. This study tests the hypothesis that metonymy is also central within the semantic field of communication. A bottom-up, empirical study of word—one of the most frequent nouns in English—was carried out, based on 4000 tokens from the British National Corpus. word was found to be highly figurative, with metonymic uses (55%) being considerably more frequent than metaphorical uses (24%), but with some overlap, and with the two forms of the lemma displaying different profiles. Although largely an abstract noun,word is even richer in figurative meaning than previously studied body part nouns. The core meaning of wordrefers to the grammatical word, but it is frequently extended through metonymy—and especially synecdoche—to stand for units of communication of varying scope; these are mapped out in the study. The metonymic meanings were found to be more conventionalized in nature than the basic meanings. Metonymy is shown to be a robust phenomenon that, at least in the semantic field of verbal communication, may be more significant than the vastly more-studied phenomenon of metaphor.

  • Patients’ experiences of eHealth in palliative care : an integrative review

    • Background: With a growing world population, a longer life expectancy, and more deaths due to chronic diseases, the need for palliative care is increasing. Palliative care aims to alleviate suffering and to promote well-being for patients with progressive, incurable disease or injury. E-Health entails using of information and communication technology for healthcare provision. It is unclear to how patients experience use of eHealth technology within palliative care.Methods: The aim of this study was to describe patients’ experiences of eHealth in palliative care. A systematic integrative review was performed using six databases: Cinahl Complete; MEDLINE; PubMed; Psychology and Behavioral Sciences Collection; Nursing and Allied Health; and PsycINFO. Twelve studies met the inclusion criteria of adult patients in palliative care, English language, published 2014–2019: comprising 397 patients. Six studies were from European countries, four from North America, one from South America and one from Oceania. Seven were feasibility or pilot studies.Results: The findings are synthesized in the main theme: E-health applications – promoting communication on patients’ and families’ terms, and three sub- themes: usability and feasibility of eHealth applications; symptom control and individualized care; and use of eHealth applications increased sense of security and patient safety. Patients’ experiences were that eHealth promoted individualized care, sense of security, better symptom management and participation in care. Communication was facilitated by the inherent flexibility provided by technology.Conclusions: E-Health applications seem promising in promoting equal, individualized care, and may be a tool to endorse accessibility and patient participation in palliative care settings. Indications are that eHealth communication resulted in patients and families receiving more information, which contributed to experiences of patient safety and feelings of security. At organizational and societal levels, eHealth may contribute to sustainable development and more efficient use of resources.
