  • Clinical Reasoning Curricula in Health Professions Education : A Scoping Review

    • OBJECTIVES: This scoping review aimed to explore and synthesize current literature to advance the understanding of how to design clinical reasoning (CR) curricula for students in health professions education.METHODS: Arksey and O'Malley's 6-stage framework was applied. Peer-reviewed articles were searched in PubMed, Web of Science, CINAHL, and manual searches, resulting in the identification of 2932 studies.RESULTS: Twenty-six articles were included on CR in medical, nursing, physical therapy, occupational therapy, midwifery, dentistry, and speech language therapy education. The results describe: features of CR curriculum design; CR theories, models, and frameworks that inform curricula; and teaching content, methods, and assessments that inform CR curricula.CONCLUSIONS: Several CR theories, teaching, and assessment methods are integrated into CR curricula, reflecting the multidimensionality of CR among professions. Specific CR elements are addressed in several curricula; however, no all-encompassing CR curriculum design has been identified. These findings offer useful insights for educators into how CR can be taught and assessed, but they also suggest the need for further guidance on educational strategies and assessments while learners progress through an educational program.

  • Reasoning about reasoning : using recall to unveil clinical reasoning in stroke rehabilitation teams

    • Purpose: The study objective was to investigate how health care providers in stroke teams reason about their clinical reasoning process in collaboration with the patient and next of kin.Materials and methods: An explorative qualitative design using stimulated recall was employed. Audio-recordings from three rehabilitation dialogs were used as prompts in interviews with the involved staff about their clinical reasoning. A thematic analysis approach was employed.Results: A main finding was the apparent friction between profession-centered and person-centered clinical reasoning, which was salient in the data. Five themes were identified: the importance of different perspectives for a rich picture and well-informed decisions; shared understanding in analysis and decision-making - good intentions but difficult to achieve; the health care providers' expertise directs the dialog; the context's impact on the rehabilitation dialog; and insights about missed opportunities to grasp the patient perspective and arrive at decisions.Conclusions: Interprofessional stroke teams consider clinical reasoning as a process valuing patient and next of kin perspectives; however, their professional expertise risks preventing individual needs from surfacing. There is a discrepancy between professionals' intentions for person-centeredness and how clinical reasoning plays out. Stimulated recall can unveil person-centered practice and enhance professionals' awareness of their clinical reasoning.

    • Anthony Giddens is generally considered one of the most prominent sociologists of moderntime. In this paper, we draw upon a micro-process case study to explore how his theory ofstructuration (ST) can be used to analyze the interplay between strategy and accounting inday-to-day-organizational life. In short, we find that strategizing and accounting shouldnot be viewed as two separate practices, but rather as two aspects ofoneand the samepractice, which form and feed each other in a recursive manner over time. Based on thesefindings, we also elaborate on how ST may be usefully applied to understand continuityand transformation of strategizing practices more generally. An overall conclusion is thatST not only provides a strong and consistent ontological framework for theorizing aboutthese practices, but also offers a rich conceptual toolbox which can be usefully appliedto better understand how and why structural continuity and change may coexist and inter-mingle in daily organizational life.

    • Over the past 111 years since Thorstein Veblen (1898) introduced his proposal for evolutionary theories in social science influences from evolutionary theory have been used in marketing from time to time. In a couple of very recent articles in marketing there has been a more explicit use of Darwinian ideas. In this paper, however, reasoning is taken one next step further by introducing not only a theory or a perspective that draws inspiration from evolutionary ideas but actually is evolutionary on the ontological level in the original Darwinian sense that it builds on the three basic evolutionary elements of retention, variation and selection. Thus, the aim of this paper is to show the importance of going from merely flirting with Darwinian ideas by using biology analogies and metaphors to developing a truly evolutionary meta-theory in order to more firmly integrate and create structure in the rather fussy bundle of ideas that in different parts contributes to an evolutionary model of explanation. In this paper we use the evolving relationships in multichannel settings as one illustration to start such a debate.

    • Trends like Internet of Things (IoT), 5G and Cloud are pushing for device connectivity to the Internet, which affects industrial embedded systems with e.g., an increase in code base and functionality. Due to different application requirements, there are relative little reuse between embedded systems with different run-time context (from super loop to multi-threaded), and different types of communication (best effort and real time). In order to improve code reuse and changeability, we propose a flexible communication stack design, that can be configured for time sensitive communication with a real-time operating system (RTOS), or configured for best effort communication with either a super loop or an operating system (OS). Experiments demonstrate the flexibility and simplicity of the design with different configurations, e.g., super loop, single threaded, multi-threaded. Measurements show that the variations in performance related to run-time context scales as expected.

    • A trend in the industrial domain is that the networks are growing and becoming more complex, this is further accelerated by the digitalization trend. In order to address this, there is a need to improve the efficiency when moving between the R&D phases. For example, integrate innovative research findings into industrial systems, shorten time to market, improve product quality and reduce the number of issues. Despite a huge research effort on network simulators and emulators there are still some issues that needs to be addressed. This paper presents challenges that needs to be resolved, in order for the industry to adopt and benefit from using network simulators and emulators. The major challenges streamlining the workflow in and between the different R&D phases while preserving the real-time aspects in the entire industrial distributed control system.

    • Trends like the Cloud, Internet of Things and 5G are pushing for an increase in connectivity, but, introducing a new type of network in an industrial distributed control system is a big investment with high risks. Time to market with sufficient quality is crucial. However, when getting through the Research and Development (R&D) phases, a lot of time is spent on isolated activates, e.g., simulations, collecting requirements, design, coding, debugging, creating testbeds, and performing various tests. Therefore, there is a need to improve efficiency when moving between the R&D phases. For verification and validation of communication software, the most common network evaluation method in industry are real testbeds, mostly since a testbed can be very similar to the deployed system. Testbeds are, however, hard to debug and costly to maintain. Other network evaluation methods like simulators, have some strengths that testbeds are lacking, like repeatability, control over the network, and lower cost. However, code from simulators can seldom be reused, especially in industrial time-sensitive target systems, due to different abstraction levels, run-time behavior and system timing. This paper presents findings from a case study that targets improved efficiency, getting from research theories, to deployed devices in a homogeneous Industrial Wireless Sensor and Actuator Network (IWSAN). We propose a small subset of network simulators features which eases changeability, reuse, and debugging of communication software. The selected simulator features are evaluated with a proof of concept implementation that is customized to a research platform. The findings indicate improved efficiency when moving back and forth between activities in different R&D phases.
