  • Scandcleft randomised trials of primary surgery for unilateral cleft lip and palate: 4. Speech outcomes in 5-year-olds - velopharyngeal competency and hypernasality

    • Background and aim: Adequate velopharyngeal function and speech are main goals in the treatment of cleft palate. The objective was to investigate if there were differences in velopharyngeal competency (VPC) and hypernasality at age 5 years in children with unilateral cleft lip and palate (UCLP) operated on with different surgical methods for primary palatal repair. A secondary aim was to estimate burden of care in terms of received additional secondary surgeries and speech therapy. Design: Three parallel group, randomised clinical trials were undertaken as an international multicentre study by 10 cleft teams in five countries: Denmark, Finland, Sweden, Norway, and the UK. Methods: Three different surgical protocols for primary palatal repair were tested against a common procedure in the total cohort of 448 children born with a non-syndromic UCLP. Speech audio and video recordings of 391 children (136 girls, 255 boys) were available and perceptually analysed. The main outcome measures were VPC and hypernasality from blinded assessments. Results: There were no statistically significant differences between the prevalences in the arms in any of the trials. VPC: Trial 1, A: 58%, B: 61%; Trial 2, A: 57%, C: 54%; Trial 3, A: 35%, D: 51%. No hypernasality: Trial 1, A: 54%, B: 44%; Trial 2, A: 47%, C: 51%; Trial 3, A: 34%, D: 49%. Conclusions: No differences were found regarding VPC and hypernasality at age 5 years after different methods for primary palatal repair. The burden of care in terms of secondary pharyngeal surgeries, number of fistulae, and speech therapy visits differed.

  • Amplitude and phase control of attosecond light pulses

    • We report the generation, compression, and delivery on target of ultrashort extreme-ultraviolet light pulses using external amplitude and phase control. Broadband harmonic radiation is first generated by focusing an infrared laser with a carefully chosen intensity into a gas cell containing argon atoms. The emitted light then goes through a hard aperture and a thin aluminum filter that selects a 30-eV bandwidth around a 30-eV photon energy and synchronizes all of the components, thereby enabling the formation of a train of almost Fourier-transform-limited single-cycle 170 attosecond pulses. Our experiment demonstrates a practical method for synthesizing and controlling attosecond waveforms.

    • Although vaccines against influenza can reduce maternal morbidity and mortality, large-scale data on adverse effects in the offspring are scarce. Historical cohort study in Stockholm County, Sweden. We linked H1N1 vaccination data (Pandemrix(A (R)), a mono-valent AS03 adjuvanted H1N1 vaccine) with pregnancy and birth data from 21,087 women with singleton offspring conceived between February 2009 and January 2010 (vaccinated during pregnancy: n = 13,297 vs. unvaccinated: n = 7,790). Data were analysed by conceptualizing the observational cohort as a series of nested cohorts defined at each week of gestation. Logistic regression estimated odds ratios (ORs) for low birth weight (LBW, < 2,500 g), preterm birth (< 37 completed weeks), small-for-gestational age (SGA, < 10th percentile of the gestational age-specific birth weight within the cohort), low 5-min Apgar score (< 7), and caesarean section. Data were adjusted for potential confounders, including maternal age, body mass index, smoking, parity, civil status and comorbidities. Compared with infants of non-vaccinated women, infants of vaccinated women had similar adjusted ORs (95 % CI) for LBW (0.91; 0.79-1.04), preterm birth (0.99; 0.89-1.10), SGA (0.97; 0.90-1.05), low Apgar score (1.05, 0.84-1.31), and a marginal risk reduction for caesarean section (0.94, 0.89-0.99). H1N1 vaccination during pregnancy, using an AS03-adjuvanted vaccine, does not appear to adversely influence offspring risks of LBW, preterm birth, SGA, or low Apgar score. Our results suggest that this vaccine is safe for the offspring when used in different stages of pregnancy.

    • In 2016, the Swedish Transport Administration, commissioned by the government, conducted an investigation to prepare for the introduction of control of sobriety in some ports. The Swedish Transport Administration submitted its report to the Government Offices on 31 January 2017 and proposed that an innovative concept of sobriety controls be developed and implemented in strategically important places in the traffic environment, such as ports, car parks and selected road sections.In a government mission, Trafikverket is commissioned to develop and implement facilities for automated sobriety checks at strategic locations in the traffic environmentIn order to describe the respective roles, responsibilities and forms of cooperation of the authorities in a joint project, an agreement has been signed by the National Police Commissioner and the Director General of the Swedish Transport Administration.The aim of the project is to create a new method that allows for legal and automated sobriety controls in the road traffic environment. The method must be designed in such a way that it requires a limited presence of police and/or representatives of other supervisory authorities.The project will investigate technical, legal and organizational conditions for such activities and then develop, procure and implement the new method. The tool/operations can be used at different strategic locations in the road traffic environment such as ports, car parks, border crossings, selected road routes, etc.The purpose of the new project is to enable regular and resource-efficient sobriety controls, thus increasing sobriety and reducing the number of dead and seriously injured in road trafficThe project is expected to be completed by 31 December 2020.

  • Oscillator strengths for high-excitation Ti II from laboratory measurements and calculations

    • This work reports new experimental radiative lifetimes of six 3d2(3F)5s levels in singly ionized titanium, with an energy around 63 000 cm−1 and four 3d2(3F)4p odd parity levels where we confirm previous investigations. Combining the new 5s lifetimes with branching fractions measured previously by Pickering et al., we report 57 experimental log gf values for transitions from the 5s levels. The lifetime measurements are performed using time-resolved laser-induced fluorescence on ions produced by laser ablation. One- and two-step photon excitation is employed to reach the 4p and 5s levels, respectively. Theoretical calculations of the radiative lifetimes of the measured levels as well as of oscillator strengths for 3336 transitions from these levels are reported. The calculations are carried out by a pseudo relativistic Hartree–Fock method taking into account core-polarization effects. The theoretical results are in a good agreement with the experiments and are needed for accurate abundance determinations in astronomical objects.

  • Scandcleft Project Trial 2—Comparison of Speech Outcome in 1- and 2-Stage Palatal Closure in 5-Year-Olds With UCLP

    • Objective: To investigate in-depth speech results in the Scandcleft Trial 2 with comparisons between surgical protocols and centers and with benchmarks from peers without cleft palate. Design: A prospective randomized clinical trial. Setting: Two Swedish and one Finnish Cleft Palate center. Participants: One hundred twelve participants were 5-years-old born with unilateral cleft lip and palate randomized to either lip repair and soft palate closure at 4 months and hard palate closure at 12 months or lip repair at 3 to 4 months (Arm A), or a closure of both the soft and hard palate at 12 months (Arm C). Main Outcome Measures: A composite measure dichotomized into velopharyngeal competency (VPC) or velopharyngeal incompetency (VPI), overall assessment of velopharyngeal function (VPC-Rate), percentage of consonants correct (PCC score), and consonant errors. In addition, number of speech therapy visits, average hearing thresholds, and secondary surgeries were documented to assess burden of treatment. Results: Across the trial, 53.5% demonstrated VPC and 46.5% VPI with no significant differences between arms or centers. In total, 27% reached age-appropriate PCC scores with no statistically significant difference between the arms. The Finnish center had significantly higher PCC scores, the Swedish centers had higher percentages of oral consonant errors. Number of speech therapy visits was significantly higher in the Finnish center. Conclusion: At age 5, poor speech outcomes with some differences between participating centers were seen but could not be attributed to surgical protocol. As one center had very few participants, the results from that center should be interpreted with caution. © 2019, American Cleft Palate-Craniofacial Association.

    • Formulation of mandatory requirements in procurement projects has significant influence on opportunities for development and deployment of Open Source Software (OSS). The paper contributes insights on a widespread practice amongst public procurement projects which causes problematic lock-in effects and thereby inhibits opportunities for use of OSS solutions. Through a systematic investigation of 30 randomly selected procurement projects in the software domain the paper highlights illustrative examples of mandatory requirements which cause lock-in and presents five recommendations for how requirements instead should be formulated in order to avoid causing lock-in. Findings show significant lock-in caused by current procurement practices with a stark preference for proprietary software and SaaS solutions amongst procuring organisations.

    • Heterogeneous tool environments are often a reality and it is therefore increasingly important to be able to interchange model information between tools. This is not only true concerning the natural heterogeneity resulting from distributed development contexts; the need may also arise in a tool chain and for legacy reasons. Without this possibility, there is significantly reduced flexibility, and a danger of tool lock-in. In this study we explore the use of the standardised interchange format XMI for supporting interchange of model information between heterogeneous tools. We report on the current state regarding XMI version 2.0 and greater. We find that there seems to be better support for model interchange using XMI 2.0 than for earlier versions of XMI, and speculate that one contributing factor may be the recent integrations of the Eclipse platform in UML modelling tools.

  • Active steering of laser-accelerated ion beams

    • A technique for optical control of the spatial distribution of laser-accelerated ion beams is presented. An ultra-short laser pulse, tightly focused to relativistic intensities on a thin foil target, drives a beam of MeV ions. An auxiliary, nanosecond laser pulse drives a shock and locally deforms the initially flat target prior to the main pulse interaction. By changing the properties of the shock-driving laser pulse, the normal direction of the ion emitting surface is locally manipulated and the emission direction is thereby controlled. In the future, this method could be used to achieve dynamic control of the ion beam divergence.
