  • Refill Adherence in Relation to Substitution and the Use of Multiple Medications: A Nationwide Population Based Study on New ACE-Inhibitor Users

    • Objective Generic substitution has contributed to economic savings but switching products may affect patient adherence, particularly among those using multiple medications. The aim was to analyse if use of multiple medications influenced the association between switching products and refill adherence to angiotensin-converting-enzyme (ACE) inhibitors in Sweden. New users of ACE-inhibitors, starting between 1 July 2006 and 30 June 2007, were identified in the Swedish Prescribed Drug Register. Refill adherence was assessed using the continuous measure of medication acquisition (CMA) and analysed with linear regression and analysis of covariance. The study population included 42735 individuals whereof 51.2% were exposed to switching ACE-inhibitor and 39.6% used multiple medications. Refill adherence was higher among those exposed to switching products than those not, but did not vary depending on the use of multiple medications or among those not. Refill adherence varied with age, educational level, household income, country of birth, previous hospitalisation and previous cardiovascular diagnosis. The results indicate a positive association between refill adherence and switching products, mainly due to generic substitution, among new users of ACE-inhibitors in Sweden. This association was independent of use of multiple medications.

    • Euthyroid sick syndrome characterized by reduced levels of thyroid hormones (THs) is observed in patients with meningococcal shock. It has been found that the level of THs reflects disease severity and is predictive for mortality. The present study was conducted to investigate the impact of THs on host defense during meningococcal infection. We found that supplementation of thyroxine to mice infected with Neisseria meningitidis enhanced bacterial clearance, attenuated the inflammatory responses and promoted survival. In vitro studies with macrophages revealed that THs enhanced bacteria-cell interaction and intracellular killing of meningococci by stimulating inducible nitric oxide synthase (iNos)-mediated NO production. TH treatment did not activate expression of TH receptors in macrophages. Instead, the observed TH-directed actions were mediated through nongenomic pathways involving the protein kinases PI3K and ERK1/2 and initiated at the membrane receptor integrin alpha v beta 3. Inhibition of nongenomic TH signaling prevented iNos induction, NO production and subsequent intracellular bacterial killing by macrophages. These data demonstrate a beneficial role of THs in macrophage-mediated N. meningitidis clearance. TH replacement might be a novel option to control meningococcal septicemia.

  • The Cooperative Challenge : Farmer Cooperation and the Politics of Agricultural Modernisation in 21st Century Uganda

    • The main purpose of this dissertation is to study whether the official rhetoric on the role that cooperatives could play in the quest for agricultural modernisation in Uganda have any resemblance with how farmers view the benefits and problem with cooperation. This question was motivated by the political initiative in the early 21st century to revive the cooperative movement in Uganda, a movement burdened with a history of political intervention and difficulties to adapt to a de-regulated agricultural market system. In the thesis, the move to revive the cooperative movement is linked to another more encompassing political initiative, that of the modernisation and commercialisation of small-scale agriculture. The book consists of two empirical studies, where interviews and text analysis have been important research methods. By first analysing the rhetoric emanating from political institutions and donor agencies, the conclusion is drawn that cooperatives are seen primarily as an institutional device which, it is hoped, will fill spaces that have been abandoned by the state and not filled with private sector initiatives.A focused case study of a dairy cooperative and a number of its members in Uganda shows that farmers’ expectations of the cooperative to some extent converge with those revealed in the first study: for example they expected the cooperative to provide a stable outlet for their production of milk. The interviews and quantitative analysis indicate that farmers’ preferences for both fast cash and a stable income can create incentives for members to free-ride on the collective efforts of their fellow members. This can be cumbersome in a competitive marketing situation where other buyers compete with the cooperative for the members’ produce. The cooperative can find it difficult to secure a necessary level of member commitment if members do not share a common code of conduct.

    • The digital initiatives in the parishes within the Church of Sweden were founded upon local initiatives from the parishes based on local needs and context. In keeping with the diaconal responsibility of making church a place for all, new ways to reach people were welcomed. Widespread internet connectivity and digital literacy, individual skills, and competence among Church staff, combined with a local ability to harbour and develop such initiatives, together with an open atmosphere within the larger Church, paved the way for the digital transformation of the first half of the year 2020. However, the results bring to the fore a question regarding the identity of the Church of Sweden (CoS). Is it a denomination with a solely inward focus towards the already active, or is it also a church reaching new people by preaching the gospel?
