  • Health care staff's opinions about existential issues among patients with cancer.

    • OBJECTIVE: The objective of this study was to explore health care staff's opinions about what existential issues are important to patients with cancer and staff's responsibility when existential issues are raised by patients. METHOD: Four focus group interviews were conducted with health care staff (N = 23) at an in-patient hospice, on an oncology ward, on a surgical ward, and with a palliative home health care team. The focus group interviews focused on two questions, first, about health care staff's opinions about patients' important existential questions and, second, about health care staff's responsibility when existential issues are raised by the patient. The interviews were tape-recorded, transcribed verbatim, and analyzed by qualitative content analysis into subcategories and categories. RESULTS: Four categories and 11 subcategories emerged from the first question. The first category, "life and death," was based on joy of living and thoughts of dying. The second category "meaning," consisted of acceptance, reevaluation, hope, and faith. The third category, "freedom of choice," consisted of responsibility and integrity, and the fourth and last category, "relationships and solitude," consisted of alleviation, dependency, and loss. One category emerged from the second question about the health care staff's responsibility, "to achieve an encounter," which was based on the subcategories time and space, attitudes, and invitation and confirmation. SIGNIFICANCE OF RESULTS: The strength of this study was that the findings were fairly congruent in different settings and in different geographical areas. Health care staff were aware of the importance of existential issues to patients. The existential issues, mentioned by health care staff, are similar to findings from studies conducted among patients, which is another strength of the present study. Health care staff are also confident about how to act when these issues are raised by the patients. The challenge for the future is to implement the findings from this study among health care staff in different settings.

Hur resonerar män som väljer att stanna hemma och ta ut lång föräldra­ledighet? Det moderna faderskapet visar sig vara en komplex historia.

  • Sick leave after traumatic brain injury. The person or the diagnosis - Which has greater impact?

    • Purpose: The aim of this study was to describe if and when a sample of traumatic brain injury (TBI) patients could finish their time of sick leave during a 4-year follow up and to explore which factors that influenced the time for sick leave. Materials and methods: All persons, 1999—2002, between 18 and 64 years of age (250 in total), admitted to the emergency room and diagnosed according to ICD 10 as S062 and S063, were included. Demographic data were gathered from medical charts and data concerning sick leave 1 year before trauma and 4 years after trauma, were collected from the Swedish social insurance system. To explore predictors of sick leave, two logistic regressions were performed. Results: The sample (mean age 39.68) consisted of 78% men. More than half of the accidents were due to fall. In the sample, 28 % was on sick leave on the day of trauma and 96 % of these were still on sick leave 4 years after trauma, compared with 39 % in the group not on sick leave on the day of TBI. Sick leave at the day for trauma was found to be a predictor for sick leave 4 years after trauma for the whole group (p = 0.000) together with Glasgow Coma Scale (GCS) (p = 0.002) and length of stay (p = 0.049). In the logistic regression with only the group not on sick leave, the only significant variable was GCS (p = 0.003). Conclusion: The findings support the necessity to consider premorbid and social factors in the TBI rehabilitation.

    • The aims of this study were to describe and compare quality of life (QOL) before and after designation to a palliative homecare team (PHT) in patients with different cancer diagnoses and to identify pre-designation predictors of post-designation global QOL. We measured patients� QOL one week before designation and 11 days (median time) after with the Assessment of Quality of life at the End of Life (AQEL). Of 163 eligible patient 63 participated without attrition. Patients� QOL improved in the physical, psychological, medical and global areas. Six items significantly improved: hours recumbent during the day (p=.009), nausea (p=.008), anxiety (p=.007), getting hold of staff (p=.000), received care (p=.003) and global QOL (p=.023). Depression/low in mood (r=.55) and meaningfulness (r=.70) associated to global QOL. Furthermore, pain (p=.028) and meaningfulness (p=.028) predicted global QOL. In the existential area, it is important to further explore how meaningfulness is associated to and predicts global QOL.

    • Background: Depression often develops undetected; to make treatment possible, a single-item screening question may be useful. Patients and methods: We attempted to compare the accuracy of the single-item question ‘Are you depressed?’ with the seven-item Depression subscale of the Hospital Anxiety and Depression Scale (HADS-D) among 1192 Swedish testicular cancer survivors. Results: We obtained information from 974 men (82%). Fifty-nine men (6%) answered ‘Yes’ to the question ‘Are you depressed?’ while 118 (12%) answered ‘I don't know’ and 794 (82%) answered ‘No’. Among the 794 men who answered ‘No’ to the question ‘Are you depressed?’, 790 (99.5%) were not considered as depressed according to HADS-D 11+. Of those answering ‘Yes’, 34% (20/59) were identified as depressed according to the same cut-off. Sensitivity of ‘Yes’ compared with HADS-D ≥11 was 61%, rising to 88% when ‘Yes’ and ‘I don't know’ were combined. Conclusion: In a population of men with a prevalence of depression similar to that of the normal population, almost none of those responding ‘No’ to the written question ‘Are you depressed?’ were depressed according to HADS-D ≥11. Adding the category ‘I don't know’ increases sensitivity in detecting depression.
