    • Purpose: The primary aim of this study was to analyse the predictive value of cognitive and behavioural factors, in relation to pain, disability and quality of life (QoL) one year after lumbar disc surgery.Method: The study design was prospective. Fifty-nine patients scheduled for first time lumbar disc surgery were included. Pain, disability, QoL, coping, fear avoidance beliefs, expected outcome and sick leave were assessed preoperatively and 12 months after surgery. Multiple backward stepwise logistic regression analyses were performed to study the contribution of the preoperatively measured independent behavioural/cognitive factors (coping, fear avoidance beliefs and assessed chance to return to work within 3 months) to the dependent variables pain, disability and quality of life at 12 months after surgery.Results: Low expectations on work return within 3 months after surgery was significantly predictive for residual leg pain, odds ratio (OR)¼8.2, back pain, OR¼9.7, disability, OR¼13.8 and sick leave, OR¼19.5. Low QoL, was best predicted by preoperatively high scores on fear avoidance beliefs OR¼6.6 and being a woman OR¼6.0. The regression model explained 26–40% of the variance in pain, disability, QoL and sick leave.Conclusions: Eliciting patients’ expectations on work return after surgery could contribute to early identification of those who run the risk of developing long-term disability and sick-leave.

    • The effectiveness of physiotherapy after first-time lumbar disc surgery is still largely unknown. Studies in this field are heterogeneous and behavioural treatment principles have only been evaluated in one earlier study. The aim of this randomised study was to compare clinic-based physiotherapy with a behavioural approach to a home-based training programme regarding back disability, activity level, behavioural aspects, pain and global health measures. A total of 59 lumbar disc patients without any previous spine surgery or comorbidity participated in the study. Clinic-based physiotherapy with a behavioural approach was compared to home-based training 3 and 12 months after surgery. Additionally, the home training group was followed up 3 months after surgery by a structured telephone interview evaluating adherence to the exercise programme. Outcome measures were: Oswestry Disability Index (ODI), physical activity level, kinesiophobia, coping, pain, quality of life and patient satisfaction. Treatment compliance was high in both groups. There were no differences between the two groups regarding back pain disability measured by ODI 3 and 12 months after surgery. However, back pain reduction and increase in quality of life were significantly higher in the home-based training group. The patients in the clinic-based training group had significantly higher activity levels 12 months after surgery and were significantly more satisfied with physiotherapy care 3 months after surgery compared to the home-based training group. Rehabilitation after first-time lumbar disc surgery can be based on home training as long as the patients receive both careful instructions from a physiotherapist and strategies for active pain coping, and have access to the physiotherapist if questions regarding training arise. This might be a convenient treatment arrangement for most patients.

    • The starting point for this article is the need to develop empirical insights about contemporary societal risk and safety management practice and executive structures. In order to facilitate insights about societal risk and safety management in a Scandinavian welfare context we use Sweden and its local governmental level (municipal) as an empirical frame in this paper. The aim for this article is to analyse how a variety of risk and safety management tasks are divided within the Swedish municipalities. The objectives are to frame the current directions for internal allocations of risk and safety issues by providing an empirically based executive typology and to contemplate the implications and future research needs that arise from that management pattern.The analysis is based on statistical analysis of information from a web-survey with chief officials (n = 1283) with responsibilities for different municipal functions and sectors. In this study the responses to one of the survey questions are used for statistical analysis. The analyzed question focused on the degree that the respondents estimated that their administrative sector or function handled a selection of risk and safety management assignments (n = 45). A factor analysis was applied to identify patterns in the dataset. The analysis resulted in an eight factor solution with a high degree of explained variance (74.3%). The results provide an elementary contribution to the understanding of the current societal risks and safety management directions.

  • The appropriateness of tight budget control in public sector organizations facing budget turbulence

    • In the public sector, budget deviations are an important performance dimension. Because of political and institutional pressures, it is crucial that public sector organizations neither overspend, nor underspend. Budget deviations actualize the issue of tight budget control. In this article we hypothesize that when public sector organizations face budget turbulence, the implementation of tight budget control is a functional response that increases the likelihood of meeting budget targets. Our study, combining survey and archival data from 196 Swedish municipalities, confirms our hypothesis. If budget turbulence is substantial, public sector organizations benefit from tight budget control as they seek to control budget deviations, but if turbulence is only marginal, they can conduct activities in the same manner as last year and additional direction from tight controls has no effect on budget deviations. A more general contribution of the paper is the evaluation of the effect of environment and tight budgetary control fit on budgetary performance.

  • The relationship between supplier control and competition in public sector outsourcing

    • The aim of this article is to develop theory and contribute to empirical studies about how the effectiveness of bureaucratic controls in public sector outsourcing is contingent upon supplier competition, and why and how this interaction plays out differently for hard and soft types of outsourced services. In previous inter‐organizational management control (IOMC) research there is a contradiction between theory and empirical results concerning how bureaucratic control and supplier competition interacts in aligning suppliers. While IOMC theory suggests competition reduces the need for bureaucratic control, empirical studies clearly indicate the opposite. We extend previous research and theorizing by differing between the outsourcing of hard and soft types of services and by testing the joint effect of bureaucratic control and competition on supplier alignment. The empirical case for testing theory is outsourcing by competitive tendering in the public sector. We use transaction level data from 166 local government suppliers in Sweden. In accordance with our prediction, the effect of bureaucratic control in aligning suppliers decreases with supplier competition when hard types of services are outsourced. For soft types of services, our results indicate that bureaucratic control is not contingent upon supplier competition. Furthermore, we show that when supplier competition is low the effect of bureaucratic control on supplier alignment is stronger for hard than for soft types of services. These results constitute an important contribution to the central notion of the interplay between bureaucratic control and competition in the IOMC literature.

    • This article presents the results from a video-based evaluation study of a social robotic telepresence solution for elderly. The evaluated system is a mobile teleoperated robot called Giraff that allows caregivers to virtually enter a home and conduct a natural visit just as if they were physically there. The evaluation focuses on the perspectives from primary healthcare organizations and collects the feedback from different categories of health professionals. The evaluation included 150 participants and yielded unexpected results with respect to the acceptance of the Giraff system. In particular, greater exposure to technology did not necessarily increase acceptance and large variances occurred between the categories of health professionals. In addition to outlining the results, this study provides a number of indications with respect to increasing acceptance for technology for elderly.

    • In this paper, we focus on spatial formations when interacting via mobile robotic telepresence (MRP) systems. Previous research has found that those who used a MRP system to make a remote visit (pilot users) tended to use different spatial formations from what is typical in human-human interaction. In this paper, we present the results of a study where a pilot user interacted with ten elderly via a MRP system. Intentional deviations from known accepted spatial formations were made in order to study their effect on interaction quality from the local user perspective. Using a retrospective interviews technique, the elderly commented on the interaction and confirmed the importance of adhering to acceptable spatial configurations. The results show that there is a mismatch between pilot user behaviour and local user preference and that it is important to evaluate a MRP system from two perspectives, the pilot user’s and the local user’s.

    • This article presents a method for measuring the quality of interaction in social mobile robotic telepresence. The methodology is in part based on Adam Kendon's theory of F-formations. The theory is based on observations of how bodies naturally orient themselves during interaction between people in real life settings. In addition, two presence questionnaires (Temple Presence Inventory and Networked Minds Social Presence Inventory), designed to measure the users' perceptions of others and the environment when experienced through a communication medium are used. The perceived presence and ease of use are correlated to the spatial formations between the robot and an actor. The proposed methodology is validated experimentally on a dataset consisting of interactions between an elder (actor) and 21 different users being trained in piloting a mobile robotic telepresence unit. The evaluation has shown that these tools are suitable for evaluating mobile robotic telepresence and also that correlations between the tools used exist. Further, these results give iportant quidlines on how to improve the interface in order to increase the quality of interaction.

    • The field of mobile robotic telepresence for social communication is in rapid expansion and it is of interest to understand what promotes good interaction. In this paper, we present the results of an experiment where novice users working in health care were given a guided tour while maneuvering a mobile robotic telepresence system for the first time. In a previous study, it was found that subjective presence questionnaires and observations of spatial configurations based on Kendon’s F-formations were useful to evaluate quality of interaction in mobile robotic telepresence. In an effort to find more automated methods to assess the quality of interaction, the study in this paper used the same measures, with the addition of objective sociometric measures. Experimental results show that the quantitative analysis of the sociometric data correlates with a number of parameters gathered via qualitative analysis, e.g. different dimensions of presence and observed problems in maneuvering the robot.
