Reporting of complex interventions in clinical trials : Development of a taxonomy to classify and describe fall-prevention interventions

- Interventions for preventing falls in older people often involve several components, multidisciplinary teams, and implementation in a variety of settings. We have developed a classification system (taxonomy) to describe interventions used to prevent falls in older people, with the aim of improving the design and reporting of clinical trials of fall-prevention interventions, and synthesis of evidence from these trials.Methods: Thirty three international experts in falls prevention and health services research participated in a series of meetings to develop consensus. Robust techniques were used including literature reviews, expert presentations, and structured consensus workshops moderated by experienced facilitators. The taxonomy was refined using an international test panel of five health care practitioners. We assessed the chance corrected agreement of the final version by comparing taxonomy completion for 10 randomly selected published papers describing a variety of fall-prevention interventions.Results: The taxonomy consists of four domains, summarized as the "Approach", "Base", "Components" and "Descriptors" of an intervention. Sub-domains include; where participants are identified; the theoretical approach of the intervention; clinical targeting criteria; details on assessments; descriptions of the nature and intensity of interventions. Chance corrected agreement of the final version of the taxonomy was good to excellent for all items. Further independent evaluation of the taxonomy is required.Conclusions: The taxonomy is a useful instrument for characterizing a broad range of interventions used in falls prevention. Investigators are encouraged to use the taxonomy to report their interventions.

- The home environment is a primary context for daily activities, especially among older adults and persons with disabilities. Functional and age-related decline can generate problems in relation to certain environmental features, necessitating modifications or other provisions of support. The determination of appropriate home interventions is best served using a person–environment (P–E) fit approach, which considers both environmental barriers and functional limitations in measuring the magnitude of accessibility problems. In the United States (U.S.), there are few valid and reliable instruments utilizing this approach. This study aimed to adapt the environmental component of the Swedish Housing Enabler (HE) for valid use in the U.S., and furthermore investigate the inter-rater agreement of the instrument. Statistical analyses of fifty pairwise home assessments show the environmental component of the U.S. HE to be sufficiently reliable (j = 0.410, percentage of agreement = 81 %) for the region of study. A valid and reliable U.S. HE has the potential to inform appropriate housing environment interventions and therefore improve U.S. housing stock, the majority of which is currently considered inaccessible for residents with functional limitations.

- Older adults prefer to age in place, necessitating a match between person and environment, or person-environment (P-E) fit. In occupational therapy practice, home modifications can support independence, but more knowledge is needed to optimize interventions targeting the housing situation of older adults. In response, this study aimed to explore the accessibility and usability of the home environment to further understand adaptive environmental behaviors. Mixed methods data were collected using objective and perceived indicators of P-E fit among 12 older adults living in community-dwelling housing. Quantitative data described objective P-E fit in terms of accessibility, while qualitative data explored perceived P-E fit in terms of usability. While accessibility problems were prevalent, participants' perceptions of usability revealed a range of adaptive environmental behaviors employed to meet functional needs. A closer examination of the P-E interaction suggests that objective accessibility does not always stipulate perceived usability, which appears to be malleable with age, self-perception, and functional competency. Findings stress the importance of evaluating both objective and perceived indicators of P-E fit to provide housing interventions that support independence. Further exploration of adaptive processes in older age may serve to deepen our understanding of both P-E fit frameworks and theoretical models of aging well.

- Objectives: To describe subjective quality of life and health, self-perceived instrumental activities of daily living (I-ADL) ability and adaptation strategies, and to explore possible relationships between these variables in fibromyalgia patients. Design: Two instruments were used with fibromyalgia patients: the Fibrositis Impact Questionnaire and 'Living with fibromyalgia - adaptation to chronic disease and handicap', and correlative analyses were performed. Subjects: A sample of 34 patients undergoing rehabilitation (in a specific programme). Results: The majority of the patients used many adaptation strategies, demonstrating some significant relationships with perceived I-ADL ability and subjective quality of life and health. Different symptoms correlated highly significantly with self-perceived ability in I-ADL. Conclusion: The significant relationships between adaptation strategies and I-ADL ability and health demonstrated deserve clinical awareness and further research efforts.

- The aim of this study was to investigate use and need for assistive devices (AD) in a cross-national European sample. Data from the ENABLE-AGE research project were utilised involving very old persons in Sweden, Germany, Latvia, Hungary and the United Kingdom. Of the total sample 65% reported that they had and used one or more AD, and 14% reported unfilled need for AD. The most commonly used AD were AD for communication, followed AD for mobility. The result can serve as input for future planning and development of information and services to improve health care and social services for older.
