    • Accessible summary The burden of caring for a person with mental illness has been transferred to families and close friends. Daily life for young informal carers is unpredictable and they need to stay alert and prepared for events beyond their control. Young informal carers are concerned young adults who support a family member or friend unconditionally. Supporting a friend is related to as a great a sense of individual responsibility as supporting a family member. Abstract This study describes the daily life and management strategies of young informal carers of family members or friends with mental illness. Twelve young adults (three men and nine women; 1625 years old) in Sweden were voluntarily recruited between February and May 2008. Data collected through eight individual semi-structured interviews and one focus group interview were analysed using qualitative content analysis. The findings revealed nine subthemes that were further grouped into three main themes: showing concern, providing support and using management strategies. Participants lived in constant readiness for something unexpected to happen to the person they cared for, and their role in the relationship could change quickly from family member or friend to guardian or supervisor. Supporting a friend was considered as large a personal responsibility as supporting a family member. Their management strategies were based on individual capacities and their ability to step aside should the situation become too demanding. These young informal carers need support in caring for the mentally ill. As the internet becomes increasingly fundamental to daily life, support could be provided most effectively through person-centred web sites.

    • The aim of this study was to explore how young (16-25 year old) informal carers of a person with a mental illness experience and use support. In a mixed method approach, we interviewed 12 young carers, and 241 completed a self-administered questionnaire. While the young carers strive to maintain control, their main support seems to be others in their lives, who often define the situation differently. The carers said web-support, counseling, and group counseling might be helpful, yet very few had any professional support. Young carers are greatly in need of support and it should be provided.

  • Referral to an electronic screening and brief alcohol intervention in primary health care in Sweden : Impact of staff referral to the computer

    • The aim of this paper was to evaluate whether primary health care staff's referral of patients to perform an electronic screening and brief intervention (e-SBI) for alcohol use had a greater impact on change in alcohol consumption after 3 month, compared to patients who performed the test on their own initiative. Staff-referred responders reported reduced weekly alcohol consumption with an average decrease of 8.4 grams. In contrast, self-referred responders reported an average increase in weekly alcohol consumption of 2.4 grams. Staff-referred responders reported a 49 reduction of average number of heavy episodic drinking (HED) occasions per month. The corresponding reduction for self-referred responders was 62. The differences between staff- and self-referred patient groups in the number who moved from risky drinking to nonrisky drinking at the followup were not statistically significant. Our results indicate that standalone computers with touchscreens that provide e-SBIs for risky drinking have the same effect on drinking behaviour in both staff-referred patients and self-referred patients.     • The emerging information society challenges relations between public agencies and citizens in many ways. Providing e-services on the Internet and using other forms of information and communication technologies are basic components of e-government. E-services as such are innovations – even if the service itself existed before – as they are a new way of producing and organizing the service. For secure use and successful implementation of innovations in public contexts, the innovation must be considered legitimate and related to policies.   The European Union and all other levels of government form policies, which are translated among the various levels to manage the useful, secure praxis of e-services. Translation in organisational terms takes place across governmental levels in the multi-level governance chain and secondarily from technical to administrative settings. Since the Swedish public administration relies on a dual steering approach, with strong, constitutionally mandated regional and local autonomy, such policies cannot be forced onto regional and local public agencies. Instead, European and national policy statements become soft policy instruments in the local context, and their implications rely on local uptake in the specific setting and on the competencies of the professionals in local public administration.    Since this is a new, emerging field of innovative policy and practice, our analysis will build on an inductive methodological approach. The theoretical framework of policy and technology translation allows inclusion of this broad process of changes. Our focus here on the translation process is to highlight translations both across levels in multi-level settings and as constructions of meanings of security. The conclusion is that the organizational settings of multi-level governance are greater constraints than new technology for implementation of public e-services.

    • Fatigue after myocardial infarction (MI) has been found to be distressing. A person's self-efficacy will influence his/her health behavior and plays an active role in tackling illness consequences. This study investigated associations between fatigue, disturbed sleep, general self-efficacy, and health-related quality of life (HRQoL) in a sample of 145 respondents admitted to hospital for MI two years earlier. The aim was to identify the predictive value of general self-efficacy and to elucidate mediating factors between self-efficacy and HRQoL. General self-efficacy measured four months after MI was positively related to HRQoL after two years. In tests of indirect effects, fatigue meditated the effects between self-efficacy and the physical and the mental dimension of HRQoL, respectively. The indirect effect of disturbed sleep went through that of fatigue. To conclude, patients who suffer from post-MI fatigue may need support aimed at helping them increase their self-efficacy as well as helping them adapt to sleep hygiene principles and cope with fatigue, both of which will have positive influences on HRQoL.

    • I artikeln lyfts frågan om tolkningens roll i pedagogisk och didaktisk forskning och specifikt prövas Paul Ricoeurs kritiska hermeneutik i vid empiriska studier. Ricoeurfilosofiska projekt handlar framför allt en kommunikation, där han bygger broar mellan det som kan förefalla vara motsägelsefulla ståndpunkter. I artikeln belyses några möjligheter som kan erbjudas forskare med empiriska data inom ramen för denna inriktning. Mer specifikt prövas här först möjligheten att använda begreppet tid så som Ricoeur framställer det, därefter tolkning där spän-ningsfält mellan att förklara och att förstå står i centrum samt, slutligen några frågeställningar runt transkription av inspelade dialoger. Artikelförfattarna kommer till slutsatsen att kritisk hermeneutik kan lämpa sig väl för praxisnära pedagogisk och didaktisk forskning och att det kritiska hermeneutiska förhållningssättet i många fall kan utgöra en god grund för förståelse och förklaring av fenomen i skolans komplexa och motsägelsefulla värld. Författarna menar också att kritisk hermeneutik öppnar för en reflektiv hållning i relation till empiriska data. Slutligen hävdas att tolkningsprocessen kan och bör uttryckas explicit vid empiriska studier.

    • Aim:To explore the influence of personality traits and beliefs about medicines on adherence to treatment with asthma medication.Methods:Respondents were 35 asthmatic adults prescribed controller medication. They answered questionnaires about medication adherence, personality traits, and beliefs about medicines.Results:In gender comparisons, the personality traits “Neuroticism” in men and “adherence to medication” were associated with lower adherent behaviour. Associations between personality traits and beliefs in the necessity of medication for controlling the illness were identified. Beliefs about the necessity of medication were positively associated with adherent behaviour in women. In the total sample, a positive “necessity-concern” differential predicted adherent behaviour.Conclusion:The results imply that personality and beliefs about medicines may influence how well adults with asthma adhere to treatment with asthma medication.

  • From novice towards self-care expert : studies of self-care among persons using advanced medical technology at home

    • The use of advanced medical technology at home has increased in most industrialized countries. The overall aim of this thesis was to develop knowledge of self-care and transition and issues that influence daily life and health among persons using advanced medical technology at home.Three qualitative studies were performed to describe the structure of self-care (I) and elucidate meanings of health-illness transition experiences among persons using long-term oxygen, or a ventila-tor, or performing blood or peritoneal dialysis (II), and to gain a deeper understanding of the meaning of living with an adult family member in this context (III). Ten interviews with adult patients (I-II) and ten with adult next of kin (III) in this context were performed and analysed with descriptive phenome-nological (I), phenomenological hermeneutical (II) and hermeneutical (III) methods. A quantitative, descriptive, comparative, cross-sectional design was used to describe and find factors that influence self-care agency and perceived health in a larger group of persons (180 patients) using the enumerated types of advanced medical technology at home (IV).In the results, (I) self-care among persons using long-term oxygen, a ventilator, or equipment for blood or peritoneal dialysis at home was described at a generic level, independent of the specific type of technology used. The general description of self-care in this context involved prerequisites for, activities for and consequences of self-care; (II) the health-illness transition among adult persons in this context was interpreted as contentment at being part of the active and conscious process towards transcending into a new state of living, in which the individual and the technology were in tune. The successful and healthy transition experience was characterized by human growth and becoming; (III) living with a family member who is using advanced medical technology at home was interpreted as meaning rhythmical patterns of being closely connected to but also separated from him or her, and of sorrow versus reconciliation. Dependence on others was reflected in a need for support from the healthcare professionals and significant others; (IV) health-related and technology-related variables in daily life were rated as satisfactory to quite a high extent, but participants using long-term oxygen perceived their health as significantly lower compared to the other technology groups. Further, a significant difference in sense of coherence was found between users of long-term oxygen and peri-toneal dialysis. Factors that contributed to self-care agency and sense of coherence were found.In conclusion, self-care in a high-tech home context means more than simply mastering the technology. With the goal of maintaining an active, social life, the health-illness transition involves a learning process of accepting and integrating the technology into daily life. With knowledge and support, patients and next of kin are able to assume substantial responsibility for self-care/dependent-care. Daily life seems to be manageable for patients using this kind of technology at home.
