  • Technological and informational frames : explaining age-related variation in the use of patient accessible electronic health records as technology and information

    • Purpose Data from a national patient survey (N = 1,155) of the Swedish PAEHR "Journalen" users were analysed, and an extended version of the theory of technological frames was developed to explain the variation in the technological and informational framing of information technologies found in the data.Design/methodology/approach Patient Accessible Electronic Health Records (PAEHRs) are implemented globally to address challenges with an ageing population. However, firstly, little is known about age-related variation in PAEHR use, and secondly, user perceptions of the PAEHR technology and the health record information and how the technology and information-related perceptions are linked to each other. The purpose of this study is to investigate these two under-studied aspects of PAEHRs and propose a framework based on the theory of technological frames to support studying the second aspect, i.e. the interplay of information and technology-related perceptions.Findings The results suggest that younger respondents were more likely to be interested in PAEHR contents for general interest. However, they did not value online access to the information as high as older ones. Older respondents were instead inclined to use medical records information to understand their health condition, prepare for visits, become involved in their own healthcare and think that technology has a much potential. Moreover, the oldest respondents were more likely to consider the information in PAEHRs useful and aimed for them but to experience the technology as inherently difficult to use.Research limitations/implications The sample excludes non-users and is not a representative sample of the population of Sweden. However, although the data contain an unknown bias, there are no specific reasons to believe that it would differently affect the survey’s age groups.Practical implications Age should be taken into account as a key factor that influences perceptions of the usefulness of PAEHRs. It is also crucial to consider separately patients’ views of PAEHRs as a technology and of the information contained in the EHR when developing and evaluating existing and future systems and information provision for patients.Social implications This study contributes to bridging the gap between information behaviour and systems design research by showing how the theory of technological frames complemented with parallel informational frames to provide a potentially powerful framework for elucidating distinct conceptualisations of (information) technologies and the information they mediate. The empirical findings show how information and information technology needs relating to PAEHRs vary according to age. In contrast to the assumptions in much of the earlier work, they need to be addressed separately.Originality/value Few earlier studies focus on (1) age-related variation in PAEHR use and (2) user perceptions of the PAEHR technology and the health record information and how the technology and information-related perceptions are linked to each other.

    • Despite the fact that patient accessible electronic health records (PAEHRs) have been around for many years in several countries, there is a lack of research investigating patient preferences for receiving bad news, including through PAEHRs. Little is also known about the characteristics of the patients who prefer to receive bad news through the PAEHR in terms of e.g. medical diagnosis, age and educational level. This study, based on a national patient survey in Sweden (N=2587), investigated this. Results show that, generally, receiving bad news by reading in the PAEHR is still among the least preferred options. Additionally, a higher proportion of men want to receive bad news in the PAEHR compared to women (p=0.001), and the same goes for those who are not working/have worked in healthcare (p=0.007). An effect of disease groups was also found, showing that diabetes patients in particular, want to receive bad news through the PAEHR.

  • Differences in Health Information Literacy Competencies Among Older Adults, Elderly and Younger Citizens

    • To address the research gap on age-based differences in health information literacy (HIL), we investigated how younger (born 1960–) and older adults (1946–1960), and elderly citizens (–1945) differed from each other by their HIL competencies. Data were collected with an online survey of patients using the Swedish national electronic health record system. Altogether, 2,587 users responded. One-way ANOVA with post hoc tests revealed several differences between the groups: younger adults were less likely to value health information than older adults; older adults and elderly were least likely to compare information from multiple sources and had trouble in determining health information needs; older adults were most likely to have trouble understanding health terminology and the elderly to have difficulties in understanding medicinal package labels. The study shows that HIL is not necessarily improving or declining but adapting to challenges of advanced age. © 2019, Springer Nature Switzerland AG.

    • There is a broad consensus that better models for assessing the impact of information efforts are needed to inform information and knowledge management and digital preservation. In contrast to measuring the quality of information, less attention has been directed to the assessment of knowledge and information processes as its constituent. Using archaeology and archaeological information as a sample context, the aim of this conceptual paper is to probe in to the evaluation of the impact and usefulness of information by taking into account the practices of how it is produced, managed and used. On a basis of a review and discussion of earlier literature on the impact of information and evaluation and management of information in archaeology, it is proposed that a better understanding of how the impact of information unfolds as a part of its production, management and use could contribute to the development of infrastructures, repositories and procedures for the management of the preservation and use of these resources.

    • Patients' online access to their EHR together with the rapid proliferation of medical information on the Internet has changed the way patients use the information to learn about their health. It is well documented that patients often turn to the Internet to find information about their health and care. However, little is known about patients’ information seeking behaviour when using online EHRs. By using information horizons as an analytical tool this paper aims to investigate the information behaviour of cancer patients who have chosen to view their EHRs (readers) and to those who have not made that option (non-readers). Thirty interviews were conducted with patients. Based on information horizons, it seems that non-reading is associated with living in a narrower information world in comparison to readers. The findings do not suggest that the smallness would be a result of an active avoidance of information, or that it would be counterproductive for the patients.

  • Organizational changes, trust and information sharing : an empirical study

    • PurposeWhile there is relatively plenty of evidence for the positive impact of communication on the perceptions of organizational change, how organizational changes affect information sharing is relatively unknown. The purpose of this paper is to investigate if a favorable perception of ongoing organizational changes has a positive impact on information sharing and whether trust mediates this relationship.Design/methodology/approachA questionnaire (n=317) was administered to the employees of a large Finnish multinational organization. Partial least square structural equation modeling was used to test the hypotheses based on earlier research findings.FindingsThe results show that a positive perception of recent organizational changes improves information sharing both directly and indirectly, mediated by trust. Consequently, when changes are perceived negatively, employees recoil from information sharing which is known to have negative implications for organizations.Research limitations/implicationsData were collected in a single organization. The nature of the specific changes in the studied organization and its particularities undoubtedly had an effect on respondents’ perceptions.Originality/valueThis paper contributes to organizational information management research by elaborating on the relationship between organizational changes and interpersonal information sharing between employees. To the authors’ knowledge, this is the first quantitative study confirming the impact of the perception of organizational changes on employee information-sharing behavior.

  • Figurations of Digital Practice, Craft, and Agency in Two Mediterranean Fieldwork Projects

    • Archaeological practice is increasingly enacted within pervasive and invisible digital infrastructures, tools, and services that affect how participants engage in learning and fieldwork, and how evidence, knowledge, and expertise are produced. This article discusses the collective imaginings regarding the present and future of digital archaeological practice held by researchers working in two archaeological projects in the Eastern Mediterranean, who have normalized the use of digital tools and the adoption of digital processes in their studies. It is a part of E-CURATORS, a research project investigating how archaeologists in multiple contexts and settings incorporate pervasive digital technologies in their studies. Based on an analysis of qualitative interviews, we interpret the arguments advanced by study participants on aspects of digital work, learning, and expertise. We find that, in their sayings, participants not only characterize digital tools and workflows as having positive instrumental value, but also recognize that they may severely constrain the autonomy and agency of researchers as knowledge workers through the hyper-granularization of data, the erosion of expertise, and the mechanization of work. Participants advance a notion of digital archaeology based on do-it-yourself (DIY) practice and craft to reclaim agency from the algorithmic power of digital technology and to establish fluid, positional distribution of roles and agency, and mutual validation of expertise. Operating within discourses of labour vs efficiency, and technocracy vs agency, sayings, elicited within the archaeological situated practice in the wild, become doings, echoing archaeology's anxiety in the face of pervasive digital technology.

    • Introduction. Researchers’ data related information needs are growing. This paper reports the findings of a study with archaeologists and cultural heritage professionals focussing on data reuse related meta-information needs.Method. Interviews with (N=)10 archaeologists and cultural heritage professionals. Analysis. Qualitative coding and content analysis.Results. Four types of paradata needs (data on processes, e.g. data creation) are identified, including 1) scope, 2) provenance, 3) methods and 4) knowledge organisation and representation paradata. Knowledge organisation and representation paradata has been least explored both in research and practises so far. The findings point to a need to develop the understanding of the needs and means of documentation of knowledge organisation and representation.Conclusions. The findings contribute to the data literacy of researchers producing and using data descriptions, and to the study of how paradata can be created and used. Further, the findings indicate that distance-to-data is a significant parameter in determining whether information needs are continuous or discrete. Further, the most likely type of reuse should guide the level and type of paradata. Finally, the findings underline that in spite of the comprehensiveness of available meta-information, it will be incomplete. Complementary means — including collaboration with data creators and meta-information extraction approaches — are needed to increase information reusability.

    • Digitalisation of research data and massive e!orts to make it findable, accessible, interoperable, and reusable has revealed that in addition to an eventual lack of description of the data itself (metadata), data reuse is often obstructed by the lack of information about the data- making and interpretation (i.e. paradata). In search of the extent and composition of categories for describing processes, this article reviews a selection of standards and recommendations frequently referred to as useful for documenting archaeological visualisations. It provides insight into 1) how current standards can be employed to document provenance and processing history (i.e. paradata), and 2) what aspects of the processing history can be made transparent using current stan- dards and which aspects are pushed back or hidden. The findings show that processes are often either completely absent or only partially addressed in the standards. However, instead of criticising standards for bias and omissions as if a perfect description of everything would be attainable, the findings point to the need for a comprehensive con- sideration of the space a standard is operating in (e.g. national heri- tage administration or international harmonisation of data). When a standard is used in a specific space it makes particular processes, methods, or tools transparent. Given these premises, if the standard helps to document what needs to be documented (e.g. paradata), and if it provides a type of transparency required in a certain space, it is reasonable to deem the standard good enough for that purpose.
