  • Cytogenetic and morphologic subgroups of myelodysplastic syndromes in relation to occupational and hobby exposures.

    • OBJECTIVES: This study investigated the association between occupational and hobby exposure and the risk of myelodysplastic syndromes (MDS) while focusing on differential patterns of clonal chromosome aberrations and morphologic subgroups. METHODS: A case-referent study was conducted with 330 MDS patients investigated cytogenetically in 1976-1993 (cases) and matched referents. Telephone interviews with either the person or a next-of-kin were used. The participation rate of the cases and referents was 85% and 60%, respectively. Information was obtained from the next-of-kin more often for the cases (88%) than for the referents (26%). Occupational hygienists assessed the exposure using interview data on worktasks and hobbies. Associations with disease risk were evaluated for 10 exposures with a logistic regression analysis. RESULTS: The investigated exposures were generally not associated with cytogenetically abnormal MDS. Effect estimates for specific cytogenetic or morphologic subgroups were generally imprecise. Occupational exposure to extremely low-frequency magnetic fields (EMF) was associated with MDS with a normal karyotype [odds ratio (OR) 2.0, 95% confidence interval (95% CI) 1.0-4.0]. The exposure-response association was consistent for intensity but inconclusive for duration. A decreased risk was observed for MDS, irrespective of karyotypic pattern, among farmers and farmhands (OR 0.53, 95% CI 0.35-0.81). CONCLUSIONS: Cytogenetically abnormal MDS was generally not associated with occupational or hobby exposure to known or suspected genotoxic agents. However, exposure prevalences and intensities were low for several agents. An association was suggested between occupational exposure to EMF and MDS with a normal karyotype. Biases due to differential information quality and selective participation cannot be ruled out.

  • National policies and social inequalities in exit paths from working life in Sweden

    • We investigated the impact on work life exits from reduced access to disability pension (2006), and financial incentives to extend working life. Method and materialWe used labour statistics of all employees in Sweden, Social insurance and income data, to compare occupational groups (SSYK, based on ISCO-88), and blue and white collar workers, with regard to i) lost years in working life due to death, disability pension and long-term sick-leave preceding disability pension 2007-2010, ii) granted disability pensions 2007-2011, and iii) premature age pension in 2004 and 2011.ResultsYears lost in working life were similar for men and women in the same 1-digit SSYK occupational group, somewhat higher for those born outside Sweden, but showed a clear gradient from white to blue collar occupations, e.g. for 0.39 versus 2.40 ys lost for Legislators/senior officials/managers and in Elementary occupations, respectively (women born in Sweden).In 2006 the prevalence of disability pension in age 50-64 was 3.61% among women and 2.49% among men, with 10/10 of the highest prevalence occupations (4-digit SSYK code) in men, and 9/10 in women, being blue collar ones. Approved applications decreased 2006-2011 by 74.4% in women, and 64.3% in men; for mental disorders (ICD-10-SE; F00-F99) 64.9% in women and 48.8% in men, for musculoskeletal disorders (M00-M99) 91.1% and 90.0%, respectively. The prevalence of premature age pension increased between 2004 and 2011: men 2.5% to 6.4%, women 1.7% to 5.5%. Blue collar occupations were most affected.ConclusionsThe socioeconomic divide in lost years of working life between white and blue collars prevailed. There was an apparent flow from disability to premature age pension, in particular in female blue collars. The findings indicate the budgetary savings of disability pensions transferred the economic burden of disease to individuals, and mainly to female blue collar workers.

  • DIFFUSE RETINAL VASCULAR LEAKAGE AND CONE-ROD DYSTROPHY IN A FAMILY WITH THE HOMOZYGOUS MISSENSE C.1429G>A (P.GLY477ARG) MUTATION IN CRB1

    • PURPOSE: To describe a specific cone-rod dystrophy phenotype in a family with the homozygous c.1429G>A; p.Gly477Arg mutation in CRB1. The detailed phenotype of subjects with this specific mutation has not been described previously. METHODS: Clinical examination included full-field electroretinography and high-resolution and widefield retinal imaging and uveitis workup. Molecular genetic analysis included next-generation sequencing of known retinal dystrophy genes and Sanger sequencing for segregation analysis. RESULTS: Three affected male siblings (26, 16, and 8 years old) were diagnosed with cone-rod dystrophy, featuring bilateral macular hypoautofluorescent lesions. In addition, the eldest brother was found to have retinal vascular leakage throughout the retina without telangiectasia. Uveitis laboratory workup was unremarkable. The homozygous c.1429G>A; p.Gly477Arg mutation in CRB1 was found to segregate with disease in this family. CONCLUSION: To the best of our knowledge, diffuse vascular leakage without telangiectasia or exudation, with bull's eye maculopathy, has not been reported previously in CRB1-cone rod dystrophy. This expands the phenotype complexity associated with CRB1 mutations and confirms that dystrophies associated with mutations in this gene may appear with features of uveitis.

  • Balancing asymmetrical load using a static var compensator

    • Power electronic controllers are employed in the transmission and distribution levels of the power systems. In transmission level applications, Flexible AC Transmission System (FACTS) controllers, consist of shunt and series static compensators, are applied for power flow control, stability enhancement, voltage regulation and power oscillation damping. A Static Var Compensator (SVC) is a shunt-connected FACTS controller that is able to exchange reactive power with the power system in a controlled way. The SVC can be used for load balancing applications to reduce negative sequence components in the network to an acceptable level. The aim of the paper is to evaluate the dynamic performance of the SVC negative sequence controller. The benefits of the SVC negative sequence controller and characteristics is presented. The simulation illustrates that the SVC allows utilities to balance asymmetrical loads to mitigate the network negative sequence components.

  • Label-free separation of peripheral blood mononuclear cells from whole blood by gradient acoustic focusing

    • Efficient techniques for separating target cells from undiluted blood are necessary for various diagnostic and research applications. This paper presents acoustic focusing in dense media containing iodixanol to purify peripheral blood mononuclear cells (PBMCs) from whole blood in a label-free and flow-through format. If the blood is laminated or mixed with iodixanol solutions while passing through the resonant microchannel, all the components (fluids and cells) rearrange according to their acoustic impedances. Red blood cells (RBCs) have higher effective acoustic impedance than PBMCs. Therefore, they relocate to the pressure node despite the dense medium, while PBMCs stay near the channel walls due to their negative contrast factor relative to their surrounding medium. By modifying the medium and thus tuning the contrast factor of the cells, we enriched PBMCs relative to RBCs by a factor of 3600 to 11,000 and with a separation efficiency of 85%. That level of RBC depletion is higher than most other microfluidic methods and similar to that of density gradient centrifugation. The current acoustophoretic chip runs up to 20 µl/min undiluted whole blood and can be integrated with downstream analysis.
