How Accurately Does the Information on Motor Development Collected During Health Checkups for Infants Predict the Diagnosis of Neurodevelopmental Disorders? – A Bayesian Network Model-Based Study

- Purpose: We investigated to what extent early motor development problems predict a future diagnosis of neurodevelopmental disorders (NDDs)/Early Symptomatic Syndromes Eliciting Neurodevelopmental Examinations (ESSENCE) by using a Bayesian network model (BN). Subjects and methods: Subjects were the children who had participated in the 18-and 36-month checkups in two cities in Japan between April 2014 and March 2015. Their motor development data at the 4-, 10-and 18-month-checkups were collected with ethical consideration. The diagnosis was confirmed at the age of six, after regular assessment in all developmental areas at a neurodevelop-mental clinic. The accuracy of prediction of NDD based on posterior probabilities determined using the BN was evaluated using the area under the receiver operating characteristic (ROC) curve (AUC). The posterior probability (the optimal cut-off value) yielding the maximum Youden Index (sensitivity + specificity – 1) is determined with the ROC curve, and the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and utility index (UI) were computed. Results: BN models showed associations between early motor items and developmental coordination disorders, borderline intelli-gence/intellectual disability, and speech and language disorder. The ROC curve for any NDD had an AUC of 0.735. The posterior probability with the maximal Youden Index was 0.138; at the optimal cut-off value, the sensitivity, specificity, PPV, NPV, UI+, and UI-were 0.619, 0.761, 0.250, 0.940, 0.155 and 0.715, respectively. Conclusion: We utilized a novel approach in detailing the associations between certain early motor problems and specific NDDs. We showed that the presence of motor development problems early in development increases the probability of a future diagnosis of any NDD. Still, the sensitivity of early motor development problems as a screening tool was not high enough to be the sole instrument for detecting NDDs. The need for a broad, holistic ESSENCE perspective when looking at the course of motor development problems was stressed. © 2022 Hatakenaka et al.

However, few studies have been conducted with a view to following up a clinically representative cohort of children coming for assessment of motor delay before age two years. We performed a prospective clinical cohort study to examine whether or not early motor delay is often an indication of ESSENCE.

- The study of subcellular targets and biochemical processes within a living cell is valuable for biological and medical research. Secretory vesicles, one such important intracellular target, are nanoscale lipid structures that are capable of storage, transport, and secretion of, for example, neurotransmitters, hormones, proteins or waste products. Vesicles play an essential role in intercellular communication systems, as they facilitate the release of chemical messaging agents. If deregulated, these communication processes can be a central part in the pathogenesis of some neurodegenerative diseases or diabetes. Generally, due to their nanometer size and intracellular location, the analysis of single vesicles and their content is a great challenge. It requires sensitive techniques, micro/nanoscale tools and sensitive instruments with extreme spatio-temporal resolution. This review focuses on electrochemical sensors to study the biochemistry and quantification of messenger molecules and other species (e.g., reactive oxygen and nitrogen species) stored in organelles, providing new trends and developments in this field. Furthermore, we review the effect of the chemical environment of single cells (e.g., treatment with chemicals, drugs, lipids, and ions) on regulation of the physical and chemical properties of vesicles. Finally, unsolved challenges of and perspectives on vesicle electroanalysis are discussed.

- Recent work has shown that chemical release during the fundamental cellular process of exocytosis in model cell lines is not all-or-none. We tested this theory for vesicular release from single pancreatic beta cells. The vesicles in these cells release insulin, but also serotonin, which is detectible with amperometric methods. Traditionally, it is assumed that exocytosis in beta cells is all-or-none. Here, we use a multidisciplinary approach involving nanoscale amperometric chemical methods to explore the chemical nature of insulin exocytosis. We amperometrically quantified the number of serotonin molecules stored inside of individual nanoscale vesicles (39 317 +/- 1611) in the cell cytoplasm before exocytosis and the number of serotonin molecules released from single cells (13 310 +/- 1127) for each stimulated exocytosis event. Thus, beta cells release only one-third of their granule content, clearly supporting partial release in this system. We discuss these observations in the context of type-2 diabetes.

- Carbon nanotube (CNT)-modified electrodes are used to obtain new measurements of vesicle content via amperometry. We have investigated the interaction between CNTs and isolated adrenal chromaffin vesicles (as a model) by vesicle impact electrochemical cytometry. Our data show that the presence of CNTs not only significantly increased the vesicular catecholamine number from 2,250,000 +/- 112,766 molecules on a bare electrode to 3,880,000 +/- 686,573 molecules on CNT/carbon fiber electrodes but also caused an enhancement in the maximum intensity of the current, which implies the existence of strong interactions between vesicle biolayers and CNTs and an altered electroporation process. We suggest that CNTs might perturb and destabilize the membrane structure of intracellular vesicles and cause the aggregation or fusion of vesicles into new vesicles with larger size and higher content. Our findings are consistent with previous computational and experimental results and support the hypothesis that CNTs as a mediator can rearrange the phospholipid bilayer membrane and trigger homotypic fusion of intracellular vesicles.

- Background: Immediate/early prosthetic rehabilitation of the edentulous mandible with four or more implants for support of a fixed bridge is a well-documented procedure. Only a few studies have reported on the outcome of using three implants for this purpose. Aim: To retrospectively analyze the clinical outcome when using three implants for support of an immediately loaded fixed bridge in the totally edentulous mandible. Materials and methods: In total, 132 patients (67 females and 65 males, mean age 62.6 years, range 35-85 years) treated with 396 implants (Brånemark System) over an 11-year period in one clinic were retrospectively analyzed with regard to implant survival. Implants with both machined and oxidized surface had been used. Eighty percent of the implants were 13 mm in length or longer. The permanent bridge with acrylic teeth was delivered to the patient the same day. The patients were re-called annually for clinical examinations. Results: All patients had been followed for at least 1 year, with the longest follow-up of 10 years and a mean follow-up of 5 years. Thirteen implant failures in 10 patients were experienced, all occurring during the first 12 months of loading. The overall survival rate was 96.7% for implants and 92.4% for original bridges. More machined than oxidized implants failed, 7% vs. 1.2%. Fourteen additional implants were placed at the time of removal of the failed implants when also the fixed bridge was adjusted on the same day. Thus, all patients received and maintained a fixed bridge, except for a few hours in the 10 failure cases when the bridges were adjusted. Conclusions: This retrospective survival analysis shows that three implants can be used to support a fixed bridge for immediate loading in the edentulous mandible. - This paper reflects our conversation at the border - a dividing line but also a potential meeting place - of organizational culture theory and institutional theory. First, we discuss the border between institutional theory and organizational culture theory by exploring two notions central to both - taken for grantedness and meanings. We ask what is taken for granted about institutions and organizational culture and how institutions and organizational cultures materialize? Our conversation reveals that although the notion of the taken for granted is important to institutional theory and organizational culture theory, what this means and implies is quite different for each. We also found that even though institutions and cultures involve meaning and evolve through meaning making, the two are understood and hence explored methodologically in quite different ways. So what seemed to be similar in these two theoretical frameworks actually differentiates them. Nevertheless, and still optimistically, we move on to suggest possible ways to bridge organizational culture theory and institutional theory, specifically through the notion of identity - both individual, organizational and interorganizational. © SAGE Publications 2012.
