  • Midwives' experiences of doula support for immigrant women in Sweden : A qualitative study

    • Objective: to describe and analyse midwives’ experiences of doula support for immigrant women in Sweden. Design: qualitative study, analysed using content analysis. Data were collected via interviews. Setting: interviews were conducted at the midwives’ workplaces. One midwife was interviewed at a cafe. Participants: ten midwives, who participated voluntarily and worked in maternity health care in western Sweden. Findings: the interview data generated three main categories. (1) ‘A doula is a facilitator for the midwife’ has two subcategories, ‘In relation to the midwife’ and ‘In comparison with an interpreter’, (2) ‘Confident women giving support, ’has two subcategories, ‘Personal characteristics and attitudes’ and ‘Good support,’ (3) ‘Doulas cover shortcomings’ has two subcategories, ‘In relation to maternity care’ and ‘In relation to ethnicity’. Key conclusion and implications for practice: The findings of this study show that midwives experience that doulas are a facilitator for them. Doulas provide support by enhancing the degree of peace and security and improving communication with the women in childbirth. Doulas provide increased opportunities for transcultural care. They may increase childbearing women’s confidence and satisfaction, help meet the diverse needs of childbearing women and improve care quality.

  • Effects of measurements on correlations of software code metrics

    • ContextSoftware metrics play a significant role in many areas in the life-cycle of software including forecasting defects and foretelling stories regarding maintenance, cost, etc. through predictive analysis. Many studies have found code metrics correlated to each other at such a high level that such correlated code metrics are considered redundant, which implies it is enough to keep track of a single metric from a list of highly correlated metrics.ObjectiveSoftware is developed incrementally over a period. Traditionally, code metrics are measured cumulatively as cumulative sum or running sum. When a code metric is measured based on the values from individual revisions or commits without consolidating values from past revisions, indicating the natural development of software, this study identifies such a type of measure as organic. Density and average are two other ways of measuring metrics. This empirical study focuses on whether measurement types influence correlations of code metrics.MethodTo investigate the objective, this empirical study has collected 24 code metrics classified into four categories, according to the measurement types of the metrics, from 11,874 software revisions (i.e., commits) of 21 open source projects from eight well-known organizations. Kendall's tau-B is used for computing correlations. To determine whether there is a significant difference between cumulative and organic metrics, Mann-Whitney U test, Wilcoxon signed rank test, and paired-samples sign test are performed.ResultsThe cumulative metrics are found to be highly correlated to each other with an average coefficient of 0.79. For corresponding organic metrics, it is 0.49. When individual correlation coefficients between these two measure types are compared, correlations between organic metrics are found to be significantly lower (with p <0.01) than cumulative metrics. Our results indicate that the cumulative nature of metrics makes them highly correlated, implying cumulative measurement is a major source of collinearity between cumulative metrics. Another interesting observation is that correlations between metrics from different categories are weak.ConclusionsResults of this study reveal that measurement types may have a significant impact on the correlations of code metrics and that transforming metrics into a different type can give us metrics with low collinearity. These findings provide us a simple understanding how feature transformation to a different measurement type can produce new non-collinear input features for predictive models.

    • Technical debt refers to various weaknesses in the design or implementation of a system resulting from trade-offs during software development usually for a quick release. Accumulating such debt over time without reducing it can seriously hamper the reusability and maintainability of the software. The aim of this study is to understand the state of the technical debt in the development of self-driving miniature cars so that proper actions can be planned to reduce the debt to have more reusable and maintainable software. A case study on a selected feature from two self-driving miniature car development projects is performed to assess the technical debt. Additionally, an interview study is conducted involving the developers to relate the findings of the case study with the possible root causes. The result of the study indicates that "the lack of knowledge" is not the primary reason for the accumulation of technical debt from the selected code smells. The root causes are rather in factors like time pressure followed by issues related to software/hardware integration and incomplete refactoring as well as reuse of legacy, third party, or open source code.

    • Social robots are expected gradually to be used by more and more people in a widerrange of settings, domestic as well as professional. As a consequence, the features and qualityrequirements on human–robot interaction will increase, comprising possibilities to communicateemotions, establishing a positive user experience, e.g., using touch. In this paper, the focus is ondepicting how humans, as the users of robots, experience tactile emotional communication with theNao Robot, as well as identifying aspects affecting the experience and touch behavior. A qualitativeinvestigation was conducted as part of a larger experiment. The major findings consist of 15 differentaspects that vary along one or more dimensions and how those influence the four dimensions ofuser experience that are present in the study, as well as the different parts of touch behavior ofconveying emotions.

    • Advanced practice nurses: A new resource for Swedish primary health-care teams This study is associated with the first evaluation of the four first advanced practice nurses (APNs), part of a primary health-care team in Sweden. The aim is to describe health-care teams' experiences of the new APN role and investigate what opportunities and barriers to the role exist. Eighty-one respondents answered a 14-question questionnaire with a Likert scale and one open-ended question. The Kruskal-Wallis test was used to investigate differences between professional groups' answers. The qualitative material was analysed using deductive content analysis. Though mostly positive experiences of the role were found, differences did exist between the professional groups. APNs are considered a resource in that access to care, cooperation and patient flow increased. Barriers include APNs' limited autonomy and inability to prescribe medication. To further develop the role, the right to prescribe medication and strategic leadership within the Swedish health-care system, policy and legislation are needed.

  • The early chain of care in bacteraemia patients: Early suspicion, treatment and survival in prehospital emergency care

    • Introduction: Bacteraemia is a first stage for patients risking conditions such as septic shock. The primary aim of this study is to describe factors in the early chain of care in bacteraemia, factors associated with increased chance of survival during the subsequent 28 days after admission to hospital. Furthermore, the long-term outcome was assessed. Methods: This study has a quantitative design based on data from Emergency Medical Services (EMS) and hospital records. Results: In all, 961 patients were included in the study. Of these patients, 13.5% died during the first 28 days. The EMS was more frequently used by non-survivors. Among patients who used the EMS, the suspicion of sepsis already on scene was more frequent in survivors. Similarly, EMS personnel noted the ESS code "fever, infection" more frequently for survivors upon arriving on scene. The delay time from call to the EMS and admission to hospital until start of antibiotics was similar in survivors and non-survivors. The five-year mortality rate was 50.8%. Five-year mortality was 62.6% among those who used the EMS and 29.5% among those who did not (p < 0.0001). Conclusion: This study shows that among patients with bacteraemia who used the EMS, an early suspicion of sepsis or fever/infection was associated with improved early survival whereas the delay time from call to the EMS and admission to hospital until start of treatment with antibiotics was not. 50.8% of all patients were dead after five years.   • Developing a multivariable prediction model of global health-related quality of life in patients treated for rectal cancer : a prospective study in five countries

    • Purpose Rectal cancer and its treatment have a negative impact on health-related quality of life (HRQoL). If risk factors for sustained low HRQoL could be identified early, ideally before the start of treatment, individualised interventions could be identified and implemented to maintain or improve HRQoL. The study aimed to develop a multivariable prediction model for global HRQoL 12 months after rectal cancer treatment.Methods Within COLOR II, a randomised, multicentre, international trial of laparoscopic and open surgery for rectal cancer, a sub-study on HRQoL included 385 patients in 12 hospitals and five countries. The HRQoL study was optional for hospitals in the COLOR II trial. EORTC QLQ-C30 and EORTC QLQ-CR38 were analysed preoperatively and at 1 and 12 months postoperatively. In exploratory analyses, correlations between age, sex, fatigue, pain, ASA classification, complications, and symptoms after surgery to HRQoL were studied. Bivariate initial analyses were followed by multivariate regression models.Results Patient characteristics and clinical factors explained 4–10% of the variation in global HRQoL. The patient-reported outcomes from EORTC QLQ-C30 explained 55–65% of the variation in global HRQoL. The predominant predictors were fatigue and pain, which significantly impacted global HRQoL at all time points measured.Conclusion We found that fatigue and pain were two significant factors associated with posttreatment global HRQoL in patients treated for rectal cancer T1-T3 Nx. Interventions to reduce fatigue and pain could enhance global HRQoL after rectal cancer treatment.

    • This thesis investigates the development of root and non-root meanings of modal verbs through an empirical study of the verbs må and kunna in Old Swedish. Modality is taken to refer domains in which possibility contrasts with necessity. In Old Swedish, må (magha) and kunna 'may' and kunna 'can' have the potential to express most of the meanings relating to possibility and necessity, as well as lexical meanings. The two verbs are investigated in an Old Swedish corpus consisting of legal and religious texts. Certain default correlations between type of modal and semantic and syntatic enviroments, which have been proposed in the litterature, are investigated. The result shows that må frequently expresses participant-internal possibility, 'ability' and deonic possibility, 'permission', whereas kunna is mainly associated with participant-internal and general external possibility. Of particular interest is the use of må in conditions, which provides evidence that this construction type may have played a bridging role in the development on non-root modality. Using the framework of force dynamics, the development of modal meaning is analysed as a metamorphical transfer of one consistent image-schema between different conceptual domains. the schematic components are described in terms of forces and barriers. The proposed transfer from a physical to a social domain and further on to a mental and a conversional domain provides a good account of the diachronic development.

  • Attitudes Regarding Participation in a Diabetes Screening Test among an Assyrian Immigrant Population in Sweden

    • Immigrants from the Middle East have higher prevalence and incidence of type 2 diabetes (T2D) compared with native Swedes. The aim of the study was to describe and understand health beliefs in relation to T2D as well as attitudes regarding participation in a screening process in a local group of Assyrian immigrants living in Sweden. A qualitative and quantitative method was chosen in which 43 individuals participated in a health check-up and 13 agreed to be interviewed. Interviews were conducted, anthropometric measurements and blood tests were collected, and an oral glucose tolerance test was performed. In total, 13 of the 43 participants were diagnosed with impaired glucose metabolism, 4 of these 13 had TD2. The interviewed participants perceived that screening was an opportunity to discover more about their health and to care for themselves and their families. Nevertheless, they were not necessarily committed to taking action as a consequence of the screening. Instead, they professed that their health was not solely in their own hands and that they felt safe that God would provide for them. Assyrians' background and religion affect their health beliefs and willingness to participate in screening for TD2.
