    • Studies assessing the effects of vitamin D or calcium intake on breast cancer risk have been inconclusive. Furthermore, few studies have evaluated them jointly. This study is the largest so far examining the association of dietary vitamin D and calcium intake with breast cancer risk in the European Prospective Investigation into Cancer and Nutrition. During a mean follow-up of 8.8 yr, 7760 incident invasive breast cancer cases were identified among 319,985 women. Multivariable Cox proportional hazards regression was used to estimate hazard ratios (HR) and 95% confidence intervals (CI) for pre- and postmenopausal breast cancer risk. Comparing the highest with the lowest quintile of vitamin D intake, HR and 95% CI were 1.07 (0.87-1.32) and 1.02 (0.90-1.16) for pre- and postmenopausal women, respectively. The corresponding HR and 95% CIs for calcium intake were 0.98 (0.80-1.19) and 0.90 (0.79-1.02), respectively. For calcium intake in postmenopausal women, the test for trend was borderline statistically significant (P(trend) = 0.05). There was no significant interaction between vitamin D and calcium intake and cancer risk (P(interaction) = 0.57 and 0.22 in pre- and postmenopausal women, respectively). In this large prospective cohort, we found no evidence for an association between dietary vitamin D or calcium intake and breast cancer risk.

  • Skeletal muscle PGC-1α1 and KAT enzymes at the intersection between depression and metabolic disease

    • Depression and metabolic diseases are leading causes of disability and major contributors to socioeconomic burden worldwide. Physical activity exerts many beneficial effects that confer direct health improvement in individuals suffering from these disorders. However, the molecular mechanisms underlying the influence of different components of exercise interventions remain unknown. To isolate the impact of skeletal muscle conditioning on stress-induced depression, we used transgenic models that exhibit many of the chronic adaptations to aerobic exercise in skeletal muscle. Here, we show a mechanism by which skeletal muscle PGC-1α1 modulates kynurenine metabolism and mediates resilience to stress-induced depression. PGC-1α1 acts in concert with PPARα/δ transcription factors to induce the expression of kynurenine aminotransferases in skeletal muscle. The activity of this pathway diverts the metabolism of stress-induced kynurenine to kynurenic acid. Since kynurenic acid is unable to cross the blood brain barrier, this peripheral shift protects the brain from stress-induced accumulation of kynurenines. In addition, we further show that skeletal muscle PGC-1α1 and kynurenine aminotransferases are part of the physiological adaptations to aerobic exercise in both rodents and humans. Given that exercise-mediated activation of this pathway leads to peripheral accumulation of kynurenic acid, we evaluated the physiological role of this metabolite in modulating energy homeostasis. Here, we describe that kynurenic acid plays a role in systemic energy homeostasis through the regulation of adipose tissue function and inflammation. Kynurenic acid induces the expression of lipid metabolism, thermogenic and anti-inflammatory gene networks in the white adipose compartment. This reduces body-weight gain and improves glucose tolerance in animals fed a high-fat diet. Mechanistic studies in primary adipocytes show that kynurenic acid activates the G protein-coupled receptor 35. Downstream signaling of this activation is mediated through Ca2+, ERK, CREB and PGC-1α1 stabilization. Finally, activation of GPR35 by kynurenic acid induces the expression of RGS14, which sensitizes β-adrenergic response to specific agonists. In sum, this work uncovers a previously unknown function of PGC-1α1 in skeletal muscle and kynurenic acid in white adipose tissue. Targeting this metabolic node has great potential for the treatment of depression and metabolic diseases such as type-2 diabetes.

    • Objective: To describe and compare the consumption of the main groups and subgroups of vegetables and fruits (V&F) in men and women from the centres participating in the European Prospective Investigation into Cancer and Nutrition (EPIC). Design: Cross-sectional analysis. Dietary intake was assessed by means of a 24-hour dietary recall using computerised interview software and standardised procedures. Crude and adjusted means were computed for the main groups and sub-groups of V&F by centre, separately for men and women. Adjusted means by season, day of the week and age were estimated using weights and covariance analysis. Setting: Twenty-seven centres in 10 European countries participating in the EPIC project. Subjects: In total, 35 955 subjects (13 031 men and 22 924 women), aged 35-74 years, randomly selected from each EPIC cohort. Results: The centres from southern countries had the highest consumption of V&F, while the lowest intake was seen in The Netherlands and Scandinavia for both genders. These differences were more evident for fruits, particularly citrus. However, slightly different patterns arose for some sub-groups of vegetables, such as root vegetables and cabbage. Adjustment for body mass index, physical activity, smoking habits and education did not substantially modify the mean intakes of vegetables and fruits. vegetable and fruit intake follows a south-north gradient in both Conclusions:. Total, genders, whereas for several sub-groups of vegetables a different geographic distribution exists. Differences in mean intake of V&F by centre were not explained by lifestyle factors associated with VF intake.

    • Hereditary hemochromatosis (HH) is a strong risk factor for hepatocellular cancer, and mutations in the HFE gene associated with HH and iron overload may be related to other tumors, but no studies have been reported for gastric cancer (GC). A nested case-control study was conducted within the European Prospective Investigation into Cancer and Nutrition (EPIC), including 365 incident gastric adenocarcinoma and 1284 controls matched by center, sex, age and date of blood collection. Genotype analysis was performed for two functional polymorphisms (C282Y/rs1800562 and H63D/rs1799945) and seven tagSNPs of the HFE genomic region. Association with all gastric adenocarcinoma, and according to anatomical localization and histological subtype, was assessed by means of the odds ratio (OR) and 95% confidence interval (CI) estimated by unconditional logistic regression adjusted for the matching variables. We observed a significant association for H63D with OR (per rare allele) of 1.32 (CI = 1.03-1.69). In subgroup analyses, the association was stronger for non-cardia anatomical subsite (OR = 1.60, CI = 1.16-2.21) and intestinal histological subtype (OR = 1.82, CI = 1.27-2.62). Among intestinal cases, two tagSNPs (rs1572982 and rs6918586) also showed a significant association that disappeared after adjustment for H63D. No association with tumors located in the cardia or with diffuse subtype was found for any of the nine SNPs analyzed. Our results suggest that H63D variant in HFE gene seems to be associated with GC risk of the non-cardia region and intestinal type, possibly due to its association with iron overload although a role for other mechanisms cannot be entirely ruled out.

  • Impact of Cigarette Smoking on Cancer Risk in the European Prospective Investigation into Cancer and Nutrition Study

    • Purpose Our aim was to assess the impact of cigarette smoking on the risk of the tumors classified by the International Agency for Research on Cancer as causally associated with smoking, referred to as tobacco-related cancers (TRC). Methods The study population included 441,211 participants (133,018 men and 308,193 women) from the European Prospective Investigation Into Cancer and Nutrition. We investigated 14,563 participants who developed a TRC during an average follow-up of 11 years. The impact of smoking cigarettes on cancer risk was assessed by the population attributable fraction (AF(p)), calculated using the adjusted hazard ratios and 95% CI for current and former smokers, plus either the prevalence of smoking among cancer cases or estimates from surveys in representative samples of the population in each country. Results The proportion of all TRC attributable to cigarette smoking was 34.9% (95% CI, 32.5 to 37.4) using the smoking prevalence among cases and 36.2% (95% CI, 33.7 to 38.6) using the smoking prevalence from the population. The AF(p) were above 80% for cancers of the lung and larynx, between 20% and 50% for most respiratory and digestive cancers and tumors from the lower urinary tract, and below 20% for the remaining TRC. Conclusion Using data on cancer incidence for 2008 and our AF(p) estimates, about 270,000 new cancer diagnoses per year can be considered attributable to cigarette smoking in the eight European countries with available data for both men and women (Italy, Spain, United Kingdom, the Netherlands, Greece, Germany, Sweden, Denmark).

  • Prognostic value of comprehensive intracoronary physiology assessment early after heart transplantation.

    • We evaluated the long-term prognostic value of invasively assessing coronary physiology after heart transplantation in a large multicentre registry.Comprehensive intracoronary physiology assessment measuring fractional flow reserve (FFR), the index of microcirculatory resistance (IMR), and coronary flow reserve (CFR) was performed in 254 patients at baseline (a median of 7.2weeks) and in 240 patients at 1year after transplantation (199 patients had both baseline and 1-year measurement). Patients were classified into those with normal physiology, reduced FFR (FFR≤0.80), and microvascular dysfunction (either IMR≥25 or CFR≤2.0 with FFR>0.80). The primary outcome was the composite of death or re-transplantation at 10years. At baseline, 5.5% had reduced FFR; 36.6% had microvascular dysfunction. Baseline reduced FFR [adjusted hazard ratio (aHR) 2.33, 95% confidence interval (CI) 0.88-6.15; P=0.088] and microvascular dysfunction (aHR 0.88, 95% CI 0.44-1.79; P=0.73) were not predictors of death and re-transplantation at 10years. At 1year, 5.0% had reduced FFR; 23.8% had microvascular dysfunction. One-year reduced FFR (aHR 2.98, 95% CI 1.13-7.87; P=0.028) and microvascular dysfunction (aHR 2.33, 95% CI 1.19-4.59; P=0.015) were associated with significantly increased risk of death or re-transplantation at 10years. Invasive measures of coronary physiology improved the prognostic performance of clinical variables (χ2 improvement: 7.41, P=0.006). However, intravascular ultrasound-derived changes in maximal intimal thickness were not predictive of outcomes.Abnormal coronary physiology 1year after heart transplantation was common and was a significant predictor of death or re-transplantation at 10years.

    • In recent years, the European air transport industry has seen a number of important changes, with more on the horizon. This comprehensive work presents a multi-faceted analysis of the air industry in Europe, how it has developed in recent years, and how it is set to develop further into the future. The work brings together leading experts in the field from across the continent to address the burning issues affecting this significant sector of the European transportation arena. It begins with a scene-setting contribution that outlines the detail of the regulatory context within which the European airline sector operates. We then move on to look at some of the strategic responses to this evolving regulatory context. In this respect, the specifics of some relevant business models, the competitive context (particularly with respect to pricing and profitability), changes in the productivity of European airlines relative to the global industry and the issue of joint ventures in the transatlantic market are all dealt with in some detail. The focus then shifts to the analysis of more niche sectors within the European aviation sector, specifically the business aviation and air cargo markets. The volume finishes with more specific works which address key issues in today’s European airline market, namely the adoption of new propulsion technology and the ever-present environmental impacts associated with aircraft noise.

    • We investigate the effect of manganese on lattice stability and magnetic moments of paramagnetic Fe-Cr-Mn steel alloys along the Bain path connecting the body-centered cubic (bcc) and face-centered cubic (fcc) structures. The calculations are carried out using the ab initio exact muffin-tin orbital method, in combination with the coherent potential approximation, and the paramagnetic phase is modeled by the disordered local magnetic moment scheme. For all Fe-Cr-Mn alloys considered here, the local magnetic moments on Fe atoms have the minimum values for the fcc structure and the maximum values for the bcc structure, whereas the local magnetic moments on Mn have almost the same value along the constant-volume Bain path. Our results show that Mn addition to paramagnetic Fe-Cr solid solution stabilizes the bcc structure. However, when considering the paramagnetic fcc phase relative to the ferromagnetic bcc ground state, then Mn turns out to be a clear fcc stabilizer, in line with observations.

    • Using the exact muffin-tin orbital (EMTO) method in combination with the coherent potential approximation (CPA), we studied the influence of alloying elements Al, V, Zr, Tc, Re and Ir on the phase stability, lattice parameters, densities, and elastic properties of NbMoTaWMx (M = Al, V, Zr, Tc, Re, Ir; 0 ≤ x ≤ 1.0) refractory high entropy alloys. The predicted lattice parameter and elastic parameters agree with the available experimental and theoretical data. We calculated the atomic size difference (δ), the valence electron concentration (VEC) and the total energy of face-centered cubic (fcc), body-centered cubic (bcc) and hexagonal closed-pack (hcp) structures. We found that all studied NbMoTaWMx (M = Al, V, Zr, Tc, Re, Ir; 0 ≤ x ≤ 1.0) systems have the bcc structure. We showed that Al and Ir decrease the stability of the bcc structure and Zr significantly enlarges the lattice parameter of the host. Vanadium increases the ductility of NbMoTaW system. On the other hand, Al and Ir have strongly non-linear effect on the ductility, with clear minimum at x = 0.55. The present results provide consistent theoretical data and call for experimental investigation for these alloys.
