  • Support during labour : first-time fathers' descriptions of requested and received support during the birth of their child

    • Objective:  to explore how first-time fathers describe requested and received support during a normal birth. Design:  qualitative research design. Ten first-time fathers were interviewed during the first postpartum week.  Individual  open-ended  interviews  were  used  to  explore  the  fathers’  descriptions,  and  the interviews were analysed using qualitative analysis. Setting:  a labour ward at one hospital in a south-western county of Sweden in November and December 2006. Participants:  first-time  fathers  who  had  experienced  a  normal  birth  at  the  hospital  during  the study period. Findings:  the support described is presented as one main theme, ‘being involved or being left out’, which included four underlying categories: ‘an allowing atmosphere’, ‘balancing involvement’, ‘being seen’ and ‘feeling left out’. Key conclusions:  fathers perceived that they were given good support when they were allowed to ask questions during labour, when they had the opportunity to interact with the midwife and their partner, and when they could choose when to be involved or to step back. Fathers want to be seen as individuals who are part of the labouring couple. If fathers are left out, they tend to feel helpless; this can result in a feeling of panic and can put their supportive role of their partner at risk. Implications  for practice:  the  results  of  this  study could  initiate  discussions  about  how  health-care professionals can develop support given to the labouring couple, with an interest in increasing paternal involvement.

    • The supply chain strategy research has generated many frameworks for matching the supply chain with the nature of demand, market, consumers, or products. Recently many companies have implemented innovative new product development processes in order to increase revenue. However, research on the importance of the retailers for an original equipment manufacturer (OEM) adopting a leagile supply chain strategy for innovative products, is scarce. Using the case study method, this research investigates the need of collaboration between an OEM and its retailers. This research aims to richen the knowledge about demand supply chain management (DSCM), and the coordination of demand and supply processes between companies in the demand-supply chain. This research shows that there are several measures that may be used to monitor performance, and that collaboration is important for the case company.

    • Purpose – The purpose of this paper is to form an understanding of how new product development (NPD) relates to supply chain management (SCM), why the two fields should be coordinated, and how this may be done.Design/methodology/approach – This research uses a literature review and case study research. The case study considers a Swedish company that operates on a global basis in the furniture industry. Empirical data have been collected mainly from in-depth interviews with key persons representing senior and middle management in the case company.Findings – This paper stresses the need to produce innovative, value-adding products, as well as the necessity to quickly deliver them to the market. Companies that face mature business environments may encounter problems due to a high emphasis on either the value-creation processes, or on the value delivery processes. Therefore, NPD activities need to be coordinated with SCM activities on a strategic level, lest competitiveness will be lost.Research limitations/implications – The research is limited to one case company; replication studies would enhance understanding of the studied phenomenon. There is a wide need for research exploring how various parts of demand and supply chains should be managed in order to fully utilize the advantages of the consumer-oriented enterprise.Practical implications – This paper provides insights for researchers and practitioners on how to coordinate and balance NPD (demand side) with SCM (supply side) activities. It highlights that companies should organize themselves around understanding how consumer value is created and how these processes may be coordinated to provide that value. The two processes must be given equal attention and importance to avoid sub-optimization.Originality/value – The need for coordinating NPD and SCM activities has been emphasized in the literature but still remains relatively unexplored. This paper contributes by investigating this issue further.

  • Afterpains : A Comparison Between Active and Expectant Management of the Third Stage of Labor

    • Background: Management of the third stage of labor, the period following the birth of the infant until delivery of the placenta, is crucial. Active management using synthetic oxytocin has been advocated to decrease blood loss. It has been suggested, but not studied, that oxytocin may increase afterpains. The aim of this study was to compare womens experience of pain intensity when the third stage of labor was managed actively and expectantly and their experience of afterpains. Methods: A single-blind, randomized, controlled trial was performed at two delivery units in Sweden in a population of healthy women with normal, singleton pregnancies, gestational age of 34 to 43 weeks, cephalic presentation, and expected vaginal delivery. Women (n = 1,802) were randomly allocated to either active management or expectant management of the third stage of labor. Afterpains were assessed by Visual Analog Scale (VAS) and the Pain-o-Meter (POM-WDS) 2 hours after delivery of the placenta and the day after childbirth. Results: At 2 hours after childbirth, women in the actively managed group had lower VAS pain scores than expectantly managed women (p = 0.014). Afterpains were scored as more intense the day after, compared with 2 hours after, childbirth in both groups. Multiparas scored more intense afterpains, compared with primiparas, irrespective of management (p < 0.001). Conclusions: Active management of the third stage of labor does not provoke more intense afterpains than expectant management. (BIRTH 38: 4 December 2011)

  • Assessing women's sexual life after childbirth : the role of the postnatal check

    • Objective: to describe midwives' reflections on counselling women at the postnatal check with special focus on sexual life after childbirth. Design: qualitative descriptive study with focus group discussions as the method of data collection. Setting: antenatal clinics in Stockholm greater catchments' area. Participants: during 2006 and 2007, a voluntary sample of midwives from 10 different antenatal clinics were invited. Analysis: content analysis was undertaken through identification of codes, categories and themes. Findings: Two themes emerged: balancing between personal perceptions of the woman's needs and the health system restrictions and strategies for counselling about sexual life after childbirth'. The midwives tried to create a picture for themselves of the woman coming for the postnatal check and her needs before the consulation. This picture guided the midwives, but lack of time and knowledge restricted them when counselling on sexual life after childbirth. Two different strategies in counselling were identified, one task-oriented and one subject-oriented. Demands and time restrictions led midwives to distance themselves from their clients. A task-oriented approach was more visible in midwives' encounters with foreign-born women, where linguistic difficulties, cultural diversity and narrow time frames restricted the midwives' effectiveness and/or sensitivity as caregivers. In contrast, the subject-oriented strategy meant 'getting in tune', i.e. listening to the woman when she expressed her feelings and emotions, encouraging the woman to be an active participant in decisions involving her care. This strategy is used for women who arouse midwives' empathy and when there is some form of recognition and understanding. Conclusion: The picture created of the woman and her needs guided the midwives, but lack of knowledge and time limitations restricted counselling on sexual life after childbirth. Two counselling strategies were identified, one task-oriented and one subject-oriented. Balancing these two counselling strategies improves both the ethical aspects and the quality of the counselling. (C) 2009 Elsevier Ltd. All rights reserved.
