    • The aim of this study was to explore experiences of undergoing a diagnostic workup in patients with irritable bowel syndrome (IBS) at a unit for functional gastrointestinal disorders. Research has been sparse in addressing such experiences and the impact on well-being. Patients with IBS were invited to perform a workup of gastrointestinal tests. Of 120 patients who completed the tests, 20 were invited for an interview. Analysis of interviews was conducted through interpretative phenomenological analysis. One master theme emerged: validation of IBS experience inferred from three subthemes: the duality of suffering in IBS, coping with inflicted discomfort and pain, and capacity for resilience.Patients reported long-term suffering from symptoms including poor management within the healthcare organization. Despite inconvenience associated with the tests, patients expressed appreciation for professional attributes such as attentiveness that were perceived as a sense of being cared for and seen as a "person." During the workup, patients acquired greater knowledge of what IBS means, including knowledge about their own body functions and experienced relief that symptoms were not caused by any "dangerous" disease. Validation of IBS experience surfaced in the data implying that in such context, patients with IBS appear to find personal solutions to cope with everyday experiences and enhance autonomy.

  • Acute confusional state in elderly orthopaedic patients: factors of importance for detection in nursing care

    • OBJECTIVES: The aims of this study were to identify factors of significance in the development of acute confusional state (ACS) and the differences between patients who developed ACS and those who did not. METHOD AND RESULTS: Assessment, observations and interviews with 505 patients admitted to an orthopaedic clinic revealed that 51 patients developed ACS during their in-hospital stay. Patients admitted for hip fracture had a higher incidence of ACS (20.2%) than patients admitted for elective surgery for coxarthros or gonarthros (3.6%). The highest hazard ratio for ACS was several other physical diseases 15.94 (CI: 4.60-55.31 and p-value <0.00001) and the lowest was age 1.10 (CI: 1.04-1.15 and p-value <0.0002). The ACS lasted from 1 to 9 days, and patients had one (N=42), two (N=8) or three episodes (N=1) of confusion during their stay on the ward. More patients who developed ACS before surgery had two or more confusional episodes and emergency patients developed ACS more rapidly. The ACS lasted longer in patients with a higher score on the OBS scale at admittance and with rapid development of ACS. CONCLUSIONS: Acuteness in the situation seems an important risk indication for ACS in the elderly. Awareness of factors associated with the development of ACS makes it possible to more systematically identify those at risk, for instance by systematic assessment in the first interview with the patient on admission to hospital.

  • Wear impact on rolling contact fatigue crack growth in rails

    • Wear and rolling contact fatigue of rails are major problems for the railway industry and it is of high value to develop models for prediction of rail life.In this paper, a 2D FE model is used to simulate crack growth of a single head check--like RCF crack in an idealized rail. The presented model takes into account wear through crack truncation and allows for crack curving.The influence of a number of parameters on the net crack growth rate is presented for different values of the dissipated contact energy. Studied parameters include surface friction, contact pressure, fluid pressurization and wear coefficient. From the studies it is concluded that the net growth rate is highly dependent on all the studied parameters. This highlights the need for accurate estimates of contact forces and material parameters in order to accurately determine wear impact on crack growth in rails.

  • Tracking science : An alternative for those excluded by citizen science

    • In response to recent discussion about terminology, we propose "tracking science" as a term that is more inclusive than citizen science. Our suggestion is set against a post-colonial political background and large-scale migrations, in which "citizen" is becoming an increasingly contentious term. As a diverse group of authors from several continents, our priority is to deliberate a term that is all-inclusive, so that it could be adopted by everyone who participates in science or contributes to scientific knowledge, regardless of socio-cultural background. For example, current citizen science terms used for Indigenous knowledge imply that such practitioners belong to a sub-group that is other, and therefore marginalized. Our definition for "tracking science" does not exclude Indigenous peoples and their knowledge contributions and may provide a space for those who currently participate in citizen science, but want to contribute, explore, and/or operate beyond its confinements. Our suggestion is not that of an immediate or complete replacement of terminology, but that the notion of tracking science can be used to complement the practice and discussion of citizen science where it is contextually appropriate or needed. This may provide a breathing space, not only to explore alternative terms, but also to engage in robust, inclusive discussion on what it means to do science or create scientific knowledge. In our view, tracking science serves as a metaphor that applies broadly to the scientific community-from modern theoretical physics to ancient Indigenous knowledge.

    • Det pågår en intensiv diskussion om hur skogsbruket bör bedrivas, som visar att det finns tydliga motsättningar mellan intressegrupper och rättighetshavare. Dessa motsättningar syns tydligt i konflikterna mellan renskötare och skogsägare, och de grundar sig i brister i dagens skogsvårdslagstiftning. En genomlysning av skogsvårdslagen behövs för att komma till rätta med problemen.
