    • Succeeding with continuous improvement is important for manufacturing companies to increase the competitive edge. In order to succeed with continuous improvement, literature shows that communication of improvement indicators need to be integrated with communication of control indicators. This paper identifies divergencies between current practice and theory in the communication of CI, which can be a reason for why manufacturing companies fail in their CI implementation. An integration of control indicators and improvement indicators could improve continuous improvement results, increasing business performance.

  • Adaptation to climate change? : Why business-as-usual remains the logical choice in Swedish forestry

    • The two latest IPCC assessment reports have concluded that knowledge is not sufficient for inducing action on climate change. This study problematizes the issue of going beyond business-as-usual through a study of the forestry sector in Sweden, which is a large economic sector and could be expected to be an early adapter, given that newly planted forest may stand some 70-90 years into the future. Therefore resources, economic motivation in the longer term and environmental foundations for early adaptation action could be expected to exist. This study draws upon the Foucauldian conceptualization of governmentality to explain the particular institutional logics that nevertheless lead to business-as-usual arguments dominating discussion on adaptation in the case of Swedish forestry. The study emphasizes that adaptation must be seen as steered and limited by existing institutional, social system logics, rather than by externally defined "rational" motivations. Efforts on adaptation to climate change must thus be considered in relation to, and seek to change, existing institutionally based motivational and incentive structures, and must thus be conceived through social rather than environmental logics. In fact, social logics may even define the types of actions that may be regarded as adaptations.

    • Objectives/HypothesisOur previous study showed that modified uvulopalatopharyngoplasty (UPPP), including tonsillectomy, significantly improved nocturnal respiration in obstructive sleep apnea syndrome (OSAS) patients. This is a continuous study of changes in daytime sleepiness and quality of life.Study DesignProspective randomized controlled trial (RCT), two parallel arms.MethodsSixty‐five patients with apnea‐hypopnea index ≥ 15, body mass index < 36, Epworth Sleepiness Scale (ESS) ≥ 8, Friedman stage I or II, failing nonsurgical treatment. The intervention group (n = 32) underwent surgery, and the controls (n = 33) had no treatment. At baseline and the 7‐month follow‐up, polysomnography, questionnaires, and vigilance tests were implemented.ResultsAll patients answered the questionnaires, and 48 took the vigilance test. Epworth Sleepiness Scale decreased significantly in the intervention group, from a mean (standard deviation) of 12.5(3.2) to 6.8(3.9), but nonsignificantly in the control group, from 12.9(3.1) to 12.5(3.9), a significant group difference (P < 0.001). The physical and mental component score on the Short Form‐36 questionnaire increased significantly in the intervention group, from a mean 47.8(8.3) to 51.2(8.8) and from 42.1(10.6) to 48.1(9.7), respectively, but with nonsignificant changes in the controls: 49.0(9.0) to 48.3(9.1) and 41.0(10.2) to 42.7(11.5), significant group differences (P = 0.007, P = 0.031), respectively. The sleep latency/vigilance test showed a significant mean increase in the intervention group of 7(12.4) minutes and a decrease in the controls of 2.2(10.6), a significant group difference (P = 0.011). There were significant correlations between changes in subjective outcomes and nocturnal respiration.ConclusionThis RCT shows that modified UPPP was effective in improving daytime sleepiness and quality of life in OSAS patients. It strengthens the body of evidence on the potential effect of surgery offered to selected patients.

    • Background Arterial blood gases (ABG) are essential for assessment of patients with severe illness, but sampling is difficult in some settings and more painful than for peripheral venous blood gas (VBG). Venous to Arterial Conversion (v-TAC; OBIMedical ApS, Denmark) is a method to calculate ABG values from a VBG and pulse oximetry (SpO 2 ). The aim was to validate v-TAC against ABG for measuring pH, carbon dioxide (pCO 2 ) and oxygenation (pO 2 ). Methods Of 103 sample sets, 87 paired ABGs and VBGs with SpO 2 from 46 inpatients eligible for ABG met strict sampling criteria. Agreement was evaluated using mean difference with 95% limits of agreement (LoA) and Bland-Altman plots. Results v-TAC had very high agreement with ABG for pH (mean diff (ABG–v-TAC) -0.001; 95% LoA -0.017 to 0.016), pCO 2 (-0.14 kPa; 95% LoA -0.46 to 0.19) and moderate to high for pO 2 (-0.28 kPa; 95% LoA -1.31 to 0.76). For detecting hypercapnia (PaCO 2 >6.0 kPa), v-TAC had sensitivity 100%, specificity 93.8% and accuracy 97%. The accuracy of v-TAC for detecting hypoxemia (PaO 2 <8.0 kPa) was comparable to that of pulse oximetry. Agreement with ABG was higher for v-TAC than for VBG for all analyses. Conclusion Calculated arterial blood gases (v-TAC) from a venous sample and pulse oximetry were comparable to ABG values and may be useful for evaluation of blood gases in clinical settings. This could reduce the logistic burden of arterial sampling, facilitate improved screening and follow-up and reduce patient pain.
