  • A paleolimnological assessment of the effects of post-1970 reductions of sulfur deposition in Sweden

    • Analysis of diatoms in sediment cores from 10 acidic (pH < 6) lakes in southern Sweden shows that eight of the lakes have acidified after 1950, while two lakes have not significantly acidified. However, since the 1970s, sulfur deposition has decreased by 50%, and lake water chemistry monitored since 1983 shows an initial reversal of acidification. However, the diatom data do not indicate that a general recovery in pH has occurred yet. The diatoms show that a small recovery has occurred in only one lake (pH increase from 4.7 to 4.9). The 10 lakes vary in total organic carbon content from 2 to 17 mg.L-1. According to the diatoms, the lakes with high total organic carbon (>9 mg.L-1) have not acidified as much as the lakes with lower total organic carbon (<7 mg.L-1). We ascribe this difference in response to acid deposition to the buffering capacity of organic acids. Knowledge of the role of organic acids when combined with anthropogenic acid deposition is important in predicting responses to decreasing acid deposition and the time scales required for recovery. Paleolimnological methods are shown here to provide valuable information for these purposes as well as to provide a long-term perspective on lake acidity changes needed for the evaluation of recovery.

  • Patients with cardiovascular disease revisiting specialist physicians via remote treatment : interview study of experiences

    • Background: Access to health care for an aging population with growing needs presents major challenges in northern Sweden’s sparsely populated regions. Few people, the lack of professionals, and long distances make it difficult to provide health care on equitable terms according to the Swedish legislation. Remote treatment (RT) using information and communication technology has been suggested to overcome these difficulties, and person-centered care (PCC) is a desired philosophy to improve the quality of health care. However, there is scarce knowledge about how patients experience RT meetings.Objective: This study aimed to describe the experiences of patients with cardiovascular disease revisiting specialist physicians via RT guided by a PCC perspective in northern Sweden’s sparsely populated regions.Methods: A qualitative study was conducted based on interviews with 8 patients with cardiovascular disease revisiting their physician through RT, from a digital health room to a health care center or from a health care center to a hospital. The interviews were recorded, transcribed verbatim, and analyzed using inductive content analysis. The results are discussed from a PCC perspective.Results: The analysis resulted in 6 categories: good accessibility, safety with good relationships, proximity and distance with technology, habit and quality of the technology facilitating the meeting, cherishing personal integrity, and participation in own care. These categories were interpreted as the theme, participation and relationships are important for good and close care via RT.Conclusions: The study shows that participation and relationships are important for good and close care via RT. To improve the quality of an RT meeting, PCC can be applied but needs to be extended to the digital domain—electronic PCC, especially the communication component, as it is the most salient difference from a face-to-face meeting. Important factors that should be considered before, during, and after the RT meeting have been identified.

    • Artikeln handlar om 20 ungdomar som inte går till skolan med fokus på deras väg genom den Barn- och ungdomspsykiatriska vården. Ungdomarna i studien var mellan 13 och 17 år gamla och den omfattades av 12 flickor och åtta pojkar. Resultatet visar olika professioner inom BUP ger ungdomarna många olika diagnoser och många olika behandlingsåtgärder under en och samma vårdperiod. Vilka konsekvenser kan då detta få för ungdomarna? Det kan leda till för långa vårdperioder som ger psykisk lidande för ungdomarna, samtidigt som det påverkar deras möjligheter att fullfölja sin skolgång negativt.

    • School non-attendence is often a sign of a complex combination of different kinds of problems, which means that these children and young people are often in need of composite help from several different types of professions within various authorities. The purpose of the study was to examine how school authorities, the Social Services and Child and Adolescent Psychiatry (BUP) collaborate in their work with young people who do not go to school. The study comprised a thematic analysis of qualitative interviews with managerial representatives of the respective authorities. The empirical material consisted of 12 qualitative interviews with heads of units at BUP (5 individuals), section managers from Social Services (3 individuals) and principals from compulsory schools (4 individuals) in three municipalities in western Sweden. According to the results, it seems problematic to manage the positions of power that may arise in collaboration between the parties. A position of power thus implies the right to make a desicion as a profession as well as activities that are related to each other. The right to make a desicion means the mandate to determine which measures should be put in place for the young people and their families. This study also shows that the parties should develop a common knowledge base that is a combination of educational, social and psychiatric perspectives. The common knowledge base can reduce the risk of power imbalance between the parties.

    • Truancy is a problem that normally leads to treatment interventions within different organizations. Within these organizations different perspectives on the causes and consequences of the above problem can be found. The purpose of this literature study is to map out and describe the current state of research within the fields of school refusal, truancy, and school phobia. The investigation was carried out in the form of a systematic review of the relevant research literature. Research shows that approximately 90% of all young people who refuse to go to school have some form of psychiatric diagnosis.

    • Trots den tydliga begreppsliga avgränsningen av vad stödjande samtal är och inte är, kvarstår det faktum att stödjande samtal som klinisk praxis inom Barn- och ungdomspsykiatrin (BUP) rymmer en mängd olika handlingar och förhållningssätt. Syftet med studien var att undersöka vad kuratorer inom BUP beskriver att de gör i praktiken när de har stödjande samtal med patienter och anhöriga. I denna studie intervjuades 11 yrkesverksamma kuratorer från Barn- och ungdomspsykiatrin (BUP) i västra Sverige. Fyra centrala teman beskrivs i resultatet: 1) Det värdefulla lyssnandet, 2) Arbetsallians, 3) Integrerad arbetsform, 4) Hopp. Det stödjande samtalet kan beskrivas som en behandlingsåtgärd med hög komplexitet. I åtgärden integreras grundläggande samtalsmetodik med aspekter från olika psykologiska teorier och metoder. I åtgärden integreras också kuratorns arbets- och livserfarenheter. Kuratorer inom BUP gör följande när det har stödjande samtal med patienter och anhöriga: De skapar förtroende och tillit mellan behandlare, patient och anhörig. De synliggör och bekräftar patientens psykiska lidande och de vardagsproblem som är knutna till deras svåra situation. De integrerar sina arbets- och livserfarenheter i mötet med patient och anhörig. De har en pragmatisk hållning till psykologisk teori. De inger hopp om förbättring av patientens psykiska lidande och vardagsproblem.

  • Does the menstrual cycle influence aerobic capacity in endurance-trained women?

    • Purpose: The aim was to study if aerobic capacity varies during different phases of the menstrual cycle (MC) in endurance-trained female athletes.Methods: Ten endurance-trained eumenorrheic women performed a submaximal test followed by an incremental test until exhaustion three times during one MC, early follicular phase (EFP), late follicular phase (LFP), and midluteal phase (MLP). During the submaximal test, the respiratory exchange ratio (RER) and utilization of fat and carbohydrates were analyzed; and, during the incremental test, VO2 peak, maximal heart rate, utilization of fat and carbohydrates, and RER were analyzed. Lactate levels were analyzed at rest, during the submaximal test, and after the incremental test. The anaerobic threshold was determined at RER = 1.Results: No significant differences (p <.05) between the MC phases were seen in a maximal heart rate or VO2peak. Similarly, VO2, heart rate, RER, fatty acid oxidation, and carbohydrate oxidation at 70, 80, 90, and 100% of VO2peak did not differ significantly between MC phases. There were no significant differences between these phases in resting lactate before the test or during the submaximal tests, though there was a significant difference in lactate concentration 3 minutes after the incremental test between the EFP and the LFP (p =.043).Conclusion: This study did not display variations in physiological parameters between EFP, LFP, and MLP, indicating similar aerobic capacity despite hormonal variations. This knowledge may be useful when planning for competition in aerobic events.

    • Background: Bilirubin, an antioxidant, has been associated with reduced cardiovascular disease risk. A major cause of elevated plasma bilirubin is the common UGT1A1*28 promoter polymorphism in the gene of the bilirubin-conjugating enzyme UDP-glucuronosyltransferase-1A1, which reduces transcription by 70%. Earlier studies reporting a protective effect of bilirubin on stroke, have not included analysis of UGT1A1*28. The purpose of this study is to investigate if bilirubin and UGT1A1*28 are protective against ischemic stroke in a prospective case-referent setting.Methods: Cases with first-ever ischemic stroke (n=231; median lag time 4.9 years), and 462 matched referents from the The Northern Sweden Health and Disease Study Cohort were included. Plasma bilirubin was measured and UGT1A1*28 was analyzed by fragment analysis.Results: Plasma bilirubin was lower in cases than in referents, but the difference reached significance only for women. The UGT1A1*28 polymorphism (allele frequency 30%), showed a strong gene-dose relationship with bilirubin levels both among cases and referents, but was not associated with risk for stroke. Among multiple other variables analysed the strongest correlation with bilirubin was found for plasma iron.Conclusions: There was no evidence for a protective effect of the UGT1A1*28 polymorphism against stroke and consequently neither for bilirubin. The findings suggest that other factors influencing the risk for stroke also might affect bilirubin levels.

    • Currently used methodologies for quality control of residual leukocytes and erythrocytes in leukocyte-depleted plasma are either expensive or time-consuming. It has been proposed that urine dipsticks could be used as a screening method for residual erythrocytes. The aim was, therefore, to evaluate if urine dipsticks could be used to detect residual erythrocytes and also residual leukocytes in leukocyte-depleted plasma. Dilution series ranging over the decision limits for residual erythrocytes and leukocytes were prepared. Positive, negative and overall agreements, as well as the precision and joint frequency distributions, were calculated for five dipstick analyzers and their corresponding dipsticks. Twenty-four consecutive leukocyte-depleted donor plasma samples were also tested. None of the dipstick analyzers had both a high positive and a high negative agreement. Accordingly, none of the analyzers were able to discriminate between cell concentrations close to the decision limits. The inconsistency count revealed differences in precision between the dipstick analyzers. In the 24 consecutive donor samples, no significant correlation between the dipstick analyzers and the reference methods were found. In conclusion, urine dipsticks are not suitable for quality control of residual leukocytes and erythrocytes in leukocyte-depleted donor plasma.
