    • This study aimed to investigate medical healthcare utilization 3-5 years following the decision about long-term care at home vs. in special accommodation in older people. A total of 1079 people who were granted long-term care the years 2001, 2002 or 2003 were studied regarding the number of hospital stays and the number of contacts with physicians in outpatient care in the 3-5 subsequent years. Those living at home and those in special accommodation were compared regarding medical healthcare utilization during the 3-5 subsequent years. Data were collected through the study Good Aging in Skåne (GAS) and through the registers, Patient Administrative Support in Skåne (PASiS) and PrivaStat. Utilization of medical healthcare decreased slightly in the years following the decision about long-term care. Despite younger age and less dependency in activities of daily living (ADL), those living at home utilized hospital and outpatient care to a greater extent than those in special accommodation; these differences remained over time. Thus, it seems as long-term care needs to become more effective in the prevention of medical healthcare utilization among those cared for at home. More, older people who are granted long-term care at home may otherwise imply increased utilization of medical healthcare. © 2009 Elsevier Ireland Ltd. All rights reserved.

    • Aim: To investigate the utilization of medical healthcare, hospital care and outpatient care, during a 1-year period in relation to informal care, multimorbidity, functional status and health complaints and to long-term care at home or in special accommodation among people aged 65+, with one or more hospital admissions and receiving long-term care.Method: A total of 694 people receiving long-term care during the year 2001 were studied. Data were collected by means of the administrative registers Patient Administrative Support in Skåne and PrivaStat and through the study Good Ageing in Skåne. Those at home and those in special accommodation were compared regarding utilization of medical healthcare, informal care, multimorbidity, functional status and health complaints. Multiple logistic regression analysis was performed using at home vs. in special accommodation as the dependent variable and also two multiple linear regression analyses using the number of hospital stays and the number of contacts with the physician in outpatient care separately as dependent variables.Findings: Those at home were significantly younger (mean age: 81 vs. 84 years) and less dependent in personal and instrumental activities of daily living (PADL/IADL) than those in special accommodation. A larger proportion of those at home was admitted to hospital three times or more (21 vs. 14%) and they had significantly more contacts with physicians in outpatient care (md: 10 vs. md: 7). Informal care was associated with care at home (OR = 0.074) and with utilization of outpatient care (B = 2.045). Dependency in PADL was associated with care in special accommodation (OR = 1.375) and with utilization of hospital care (B = -0.060) and outpatient care (B = -0.581).Conclusion: Medical healthcare seems more accessible to those who live at home are younger, less dependent and who have access to informal caregivers.© 2009 Condelius et al.     • This paper describes a case of collaboration between academia and business firms in supporting innovation processes based on the CILO method, named after the CILO project (communication in learning organizations). The CILO method is an example of firm-supportive research approach. We describe the outcomes related to enhancing enterprise performance from applying the CILO-method in relation to one of the partner companies. The aim of the paper is to describe and discuss the concept of firm-supportive research in relation to the concept of innovation and to demonstrate how the specific characteristics of this research approach can enhance enterprise performance in one of our partner companies, Flower Systems Ltd.

    • Bipolar disorder (BD) is a severe illness that has a serious impact on the lives of those affected and on their families. The aim of this study was to elucidate what it means for family members to live with an adult person who has BD, with reference to their views concerning the condition of the person affected and the future. During 2005, 17 family members of people with the disorder were interviewed, and the data obtained were subjected to a qualitative content analysis. The findings showed that family members felt alone with their experiences and struggled to make sense of and to maintain normality, as their life was encroached on by the condition. Bearing the burden of responsibility and control made it difficult for family members to focus on their own future. To build hope, they needed opportunities to share their experiences with others, increased understanding of the condition, and relief from the burden they bore. This study underlines the importance of strengthening support holistically for family members living with an adult person with BD. Support and interventions concerning these families' needs have to be developed and should be provided by all mental health-care services.

    • Clonal plants from poor habitats benefit less from morphologically plastic responses to heterogeneity than plants from more productive sites. In addition, physiological integration has been suggested to either increase or decrease the foraging efficiency of clonal plants. We tested the capacity for biomass production and morphological response in two closely related, rhizomatous species from habitats that differ in resource availability, Carex arenaria (from poor sand dunes) and C. disticha (from nutrient-richer, moister habitats). We expected lower total biomass production and reduced morphological plasticity in C. arenaria, and that both species would produce more ramets in high nutrient patches, either in response to signals transported through physiological integration, or by locally determined responses to nutrient availability. To investigate mineral nutrient heterogeneity, plants were grown in boxes divided into two compartments with homogeneous or heterogeneous supply of high (H) or low (L) nutrient levels, resulting in four treatments, H-H, H-L, L-H and L-L. Both C. arenaria and C. disticha produced similar biomass in high nutrient treatments. C. disticha responded to high nutrients by increased biomass production and branching of the young parts and by altering root:shoot ratio and rhizome lengths, while C. arenaria showed localised responses to high nutrients in terms of local biomass and branch production in high nutrient patches. The results demonstrated that although it has a conservative morphology, C. arenaria responded to nutrient heterogeneity through morphological plasticity. An analysis of costs and benefits of integration on biomass production showed that young ramets of both species benefited significantly from physiological integration, but no corresponding costs were found. This suggests that plants from resource-poor but dynamic habitats like sand dunes respond morphologically to high nutrient patches. The two species responded to nutrient heterogeneity in different traits, and this is discussed in terms of local and distant signalling of plant status.

    • This study investigated how temporal occupational patterns, operationalized as time use and daily rhythm, are related to occupational balance, in terms of the value and satisfaction that people with persistent mental illness derive from daily occupations. The respondents, 103 individuals visiting an outpatient psychosis unit, completed a time-use diary and questionnaires targeting occupational value and satisfaction. Spending more total time in non-rest occupations (TTNR), in the categories of self-care/maintenance, work/education and play/leisure, was related to perceiving more concrete value, such as making something or learning new things. TTNR was also related to symbolic and self-reward value and to having satisfying daily occupations. A subgroup with a daily rhythm that meant being active during the day and sleeping at night time perceived more symbolic value and greater satisfaction with their daily occupations than another characterized by low activity during the day and having turned the clock around by mostly sleeping and resting during the day. Temporal occupational patterns seemed important for perceived occupational value and satisfaction with daily occupations, seen as facets of occupational balance, and a spiral type of relationship was assumed. © 2010 Taylor & Francis Group, LLC.

    • The aim of this study was to gain knowledge about the specific needs that a web-based support system for young carers (YCs) must meet. Twelve young people with experience of caring for and supporting a close friend, partner or relative with mental illness (MI) were interviewed about their life situation, support needs and opinions about a hypothetical web-based support system. The transcribed interviews were analysed using content analysis. The analysed data were organised into three themes relating to support needs, each including a number of sub-themes: knowledge - understanding MI, managing the mentally ill person and self-care; communication - shared experiences, advice and feedback, and befriending; and outside involvement - acute relief, structured help and health care commitments. Web-based support for YCs may be a suitable way to meet the need for knowledge and to meet some of the needs for communication. We have outlined a concept of a geographically anchored web support to meet the need for befriending, facilitate connections to health and social care, and increase understanding and interaction between the parties involved. Further research is needed to corroborate the results.

    • Changes in society are often discussed on an aggregated level and it is argued that society is changing from a modern industrial society to a post-industrial society. Through studying changes on a local level it is possible to study the complexity and variations of the general and aggregated theories of societal changes, and place changes in a context of time and place. The main object of this study is to analyse change in a locality. The study is a case study which studies Halmstad, a municipality on the west coast of Sweden, between the years of 1968 and 2006. The empirical material consists of analysis of demographic statistics and labour market statistics, using official statistical databases. The empirical material also consists of an analysis of official and historical documents as well as interviews with persons active in Halmstad. Three time periods are identified in the analyses which are characterised by different patterns in the population. The periods are also characterised by changes in the labour market and the employment of the population. These changes are analysed through the interplay between actors in the locality and actors and influences from outside the locality. Through the analysis of reflexivity it is shown how change and stability are interrelated through the interplay between the new and existing or innovation and tradition.

    • While Aristotle has conventionally been perceived as a precursor for the taxonomy of modern biological systematics, such as that of Carolus Linnaeus, this essay investigates the relationship between Aristotelian metaphysics and natural history in order to demonstrate the problem of anomalies in any system of this kind. In particular, the problem involves the place that may be given to the unnatural (deviations form the norm of nature) in a system for nature – if there can be a taxonomy of anomalies (namely, a teratology), given the logic of Aristotelian philosophy.
