  • Evaluation of the Post Stroke Checklist: a pilot study in the United Kingdom and Singapore

    • Background There is currently no standardized process for long-term follow-up care. As a result, management of post-stroke care varies greatly, and the needs of stroke survivors are not fully addressed. The Post Stroke Checklist was developed by the Global Stroke Community Advisory Panel as a means of standardizing long-term stroke care. Since its development, the Post Stroke Checklist has gained international recognition from various stroke networks and is endorsed by the World Stroke Organization to support improved stroke survivor follow-up and care. Aims The aim of this study was to evaluate the feasibility and usefulness of the Post Stroke Checklist in clinical practice and assess its relevance to stroke survivors in pilot studies in the United Kingdom and Singapore. Methods The Post Stroke Checklist was administered to stroke survivors in the United Kingdom (n = 42) and Singapore (n = 100) by clinicians. To assess the feasibility of the Post Stroke Checklist in clinical practice, an independent researcher observed the assessment and made notes relating to the patient-clinician interaction and their interpretations of the Post Stroke Checklist items. Patient and clinician satisfaction with the Post Stroke Checklist was assessed by three questions, responded to on a 0-10 numerical rating scale. Clinicians also completed a Pragmatic Face and Content Validity test to evaluate their overall impressions of the Post Stroke Checklist. In the United Kingdom, a subset of patients (n = 14) took part in a concept elicitation interview prior to being administered the Post Stroke Checklist, followed by a cognitive debriefing interview to assess relevance and comprehension of the Post Stroke Checklist. Results The Post Stroke Checklist identified frequently reported problems for stroke survivors including cognition (reported by 47.2% of patients), mood (43.7%), and life after stroke (38%). An average of 3.2 problems per patient was identified across both countries (range 0-10). An average of 5 and 2.6 problems per patient were identified in the United Kingdom and Singapore, respectively. The average time taken to administer the Post Stroke Checklist was 17 mins (standard deviation 7.5) in Singapore and 13 mins (standard deviation 7.6) in the United Kingdom. Satisfaction ratings were high for patients (8.6/10) and clinicians (7.7/10), and clinician feedback via the Pragmatic Face and Content Validity test indicated that the Post Stroke Checklist is 'useful', 'informative', and 'exhaustive'. All concepts measured by the Post Stroke Checklist were spontaneously discussed by patients during the concept elicitation interviews, suggesting that the Post Stroke Checklist is relevant to stroke survivors. Cognitive debriefing data indicated that the items were generally well understood and relevant to stroke. Minor revisions were made to the Post Stroke Checklist based on patient feedback. Conclusions The findings suggest that the Post Stroke Checklist is a feasible and useful measure for identifying long term stroke care needs in a clinical practice setting. Pilot testing indicated that the Post Stroke Checklist is able to identify a wide range of unmet needs, and patient and clinician feedback indicated a high level of satisfaction with the Post Stroke Checklist assessment. The items were generally well understood and considered relevant to stroke survivors, indicating the Post Stroke Checklist is a feasible, useful, and relevant measure of poststroke care.

  • A cohort study of sex differences and prognostic biomarkers in colorectal cancer

    • Colorectal cancer (CRC) is one of the most common forms of cancer worldwide, with an annual incidence of more than 1 million cases. Despite advancements in the management of CRC, mortality remains high. Accumulating evidence indicates that CRC is a heterogeneous disease, which affects outcome beyond what can be predicted by disease stage and other conventional prognostic factors. Thus, there is a great need to identify new biomarkers to enable a more accurate prognostication, and to help select patients for adjuvant treatment. The aim of this thesis was to investigate the associations of a series of putative biomarkers with survival, treatment response and clinicopathological factors in a large cohort of incident CRC, with special attention to sex differences. The study group consisted of tumours from 557 incident cases of CRC in the Malmö Diet and Cancer Study (MDCS). The tumours, assembled in tissue microarrays, were evaluated for expression of cyclin D1, mismatch repair proteins, beta-catenin and epidermal growth factor receptor (EGFR) by immunohistochemistry, and further, EGFR gene copy number (GCN) alterations by brightfield double-in situ hybridization. In addition, KRAS and BRAF mutational status was assessed by pyrosequencing. Associations with clinicopathological and investigative factors were explored by Chi Square and Spearman’s correlation tests, and Kaplan-Meier analysis and Cox proportional hazards modelling were applied for survival analysis. We hereby found that nuclear cyclin D1 expression was associated with female sex and a favorable prognosis, although not independently, in male, but not in female, CRC. Microsatellite instability (MSI) correlated to distinctive clinicopathological features, cyclin D1 expression, and independently predicted a good prognosis in stage III-IV CRC. Moreover, beta-catenin overexpression correlated independently with a prolonged survival from stage III-IV CRC, and was associated with microsatellite stable (MSS) tumours. We also observed that KRAS codon 13 mutation predicted a poor prognosis in female CRC, but not independently of established prognostic factors. KRAS and BRAF mutations were mutually exclusive, and correlated with MSS and MSI, respectively. BRAF mutation was independently associated with a reduced survival in male patients with MSS CRC. Furthermore, both EGFR protein expression and GCN alterations were associated with a reduced survival in stage III-IV CRC, the latter, however, not independently of established prognostic factors. EGFR protein expression correlated significantly with EGFR GCN alterations, although a substantial proportion of EGFR expressing tumours displayed a normal GCN, and vice versa. Finally, EGFR alterations were significantly associated with a reduced survival in curatively treated patients with stage III-IV disease receiving adjuvant oxaliplatin. In conclusion, the results from this thesis demonstrate several relevant associations of the investigative biomarkers with prognosis and treatment response in CRC. Moreover, substantial sex differences in the clinicopathological correlates and prognostic significance of some of the biomarkers were observed.

    • This study is intended as a contribution to the ongoing US and European debate on housing inequality and homelessness. We do this by focusing on the problems faced by the "precariously housed", or people repeatedly threatened with eviction, in the advanced welfare state of Sweden. Our empirical analysis is based on a sample of individuals repeatedly threatened with evictin in Sweden in the period 1982-1986. Using proportional hazard regressions, we show that the increase in risk for a new eviction threat for these individuals can best be explained by a combination of unfavorable social circumstances and difficult personal problems, both of which need to be addressed by housing policy.

    • Background Weighted blankets (WBs) have been suggested as a treatment option for insomnia and are commonly prescribed despite lack of evidence of efficacy. Aim To investigate prescription pattern, return rate and cost of WBs. Material and methods This observational cohort register-based study in western Sweden included every individual who, in a 2.5-year period, was prescribed and received at least one WB (n = 4092). A cost evaluation was made by mapping prescription processes for WBs and sleep medication. Results Individuals diagnosed with dementia, anxiety, autism or intellectual disability (ID) retained the WB longer than others. Individuals younger than six and older than 65 years had shorter use time. The cost evaluation showed that the prescription process for WBs was longer and resulted in a higher cost than for sleep medication. Conclusions Some individuals had longer use time, indicating a possible benefit from using a WB. Due to low risk of harm but high economic cost, a revision of the WBs prescription process could be recommended to identify those who might benefit from WB. Significance Our result points towards a need for revision of the prescription process, to implement standardized sleep assessments, and create a more efficient prescription process to lower the cost.

    • The special quasirandom structure (SQS) approach is a successful technique for modelling of alloys, however it breaks inherently the point symmetry of the underlying crystal lattice. We demonstrate that monocrystalline and polycrystalline elastic moduli can scatter significantly depending on the chosen SQS model and even on the supercell orientation in space. Also, we demonstrate that local disturbances, such as vacancies or interfaces change the SQS configuration in a way, that significantly affects the values of the calculated physical properties. Moreover, the diversity of local environments in random alloys results in a large variation of the calculated local properties. We underline that improperly chosen, generated or handled SQS may result in erroneous theoretical findings. The challenges of the SQS method are discussed using bulk Ti0.5Al0.5N alloy and TiN/Ti0.5Al0.5N multilayer as model systems. We present methodological corrections for the mindful application of this approach in studies of advanced properties of alloys.
