  • Time Is Money, But How Much? The Monetary Value of Response Time for Thai Ambulance Emergency Services

    • ObjectiveTo calculate the monetary value of the time factor per minute and per year for emergency services.MethodsThe monetary values for ambulance emergency services were calculated for two different time factors, response time, which is the time from when a call is received by the emergency medical service call-taking center until the response team arrives at the emergency scene, and operational time, which includes the time to the hospital. The study was performed in two steps. First, marginal effects of reduced fatalities and injuries for a 1-minute change in the time factors were calculated. Second, the marginal effects and the monetary values were put together to find a value per minute.ResultsThe values were found to be 5.5 million Thai bath/min for fatality and 326,000 baht/min for severe injury. The total monetary value for a 1-minute improvement for each dispatch, summarized over 1 year, was 1.6 billion Thai baht using response time.ConclusionsThe calculated values could be used in a cost-benefit analysis of an investment reducing the response time. The results from similar studies could for example be compared to the cost of moving an ambulance station or investing in a new alarm system.

  • Value of Statistical Life and Cause of Accident: A Choice Experiment

    • The purpose of this study is to compare the value of statistical life (VSL) estimates for traffic, drowning, and fire accidents. Using a choice experiment in a mail survey of 5,000 Swedish respondents we estimated the willingness to pay for risk reductions in the three accidents. In the experiment respondents were asked a series of questions, whether they would choose risk reducing investments where type of accident, cost of the investment, the risk reduction acquired, and the baseline risk varied between questions. The VSLs for fire and drowning accidents were found to be about 1/3 lower than that for traffic accidents. Although respondents worry more about traffic accidents, this alone cannot explain the difference in VSL estimates. The difference between fire and drowning accidents was not found to be statistically significant

    • Seamless wireless connectivity in high mobility scenarios (≥ 300 km/h), is one of the fundamental key requirements for the future 5G networks. High speed train (HST) is one of the preferred mid-range transportation systems, and highlights the challenges of providing wireless connectivity in high mobility scenarios for the 5G networks. Advanced version of Long Term Evolution (LTE-A) from the Third Generation Partnership Project (3GPP) with peak data rate up to 100 Mbps in high mobility scenarios paved the road toward high quality and cost effective onboard Internet in HSTs. However, frequent handovers (HO) of large number of onboard users increase the service interruptions that in turn inevitably decrease the experienced quality of service (QoS). In this paper, according to the two-tier architecture of the HST wireless connectivity, we propose a novel and practically viable onboard traffic offloading mechanism among the HST carriages that effectively mitigates the service interruptions caused by frequent HOs of massive number of onboard users. The proposed architecture does not imply any change on the LTE network standardization. Conclusions are supported by numerical results for realistic LTE parameters and current HST settings.

  • First-line managers struggling to lead home care based on the individual’s needs and goals – conflict between ethical principles

    • Purpose: The purpose of this paper is to describe how first-line managers (FLMs) in home care (HC) reason about the opportunities and obstacles to lead the work according to the individual’s needs and goals. Design/methodology/approach: In this participatory appreciative action reflection project, eight managers within one Swedish municipality were interviewed. The data were analysed using a thematic analysis. Findings: The results showed a polarization between two different systems that FLMs struggle to balance when attempting to lead HC that adapts to the needs and goals of individuals. One system was represented by the possibilities of a humane system, with human capital in the form of the individual, older persons and the co-workers in HC. The second system was represented by obstacles in the form of the economic needs of the organization in which the individual receiving HC often felt forgotten. In this system, the organization’s needs and goals governed, with FLMs needing to adapt to the cost-effectiveness principle and keep a balanced budget. The managers had to balance an ethical conflict of values between the human value and needs-solidarity principles, with that of the cost-effectiveness principle. Originality/value: The FLMs lack the opportunity to lead HC according to the needs and goals of the individuals receiving HC. There is a need for consensus and a value-based leadership model based on ethical principles such as the principles of human value and needs-solidarity to lead the HC according to the individual’s needs and goals.
