    • En este estudio partimos de la categorización de tipos de cortesía y de descortesía válidos para conversaciones coloquiales propuesta en Bernal (2007). El objetivo del presente trabajo es probar la aplicabilidad de tal categorización a la interacción judicial española en lo que respecta a actividades de cortesía. Concretamente, trataremos de responder a las siguientes preguntas de investigación, siempre desde el contraste entre contexto informal e institucional, como es el contexto judicial, donde se producen “openly adversarial uses of language” dada la naturaleza básicamente conflictiva del diálogo (cf. Mosegaard Hansen, 2008: 1397): ¿qué tipo de actividades de cortesía se produce durante el interrogatorio judicial? ¿Por parte de quién son realizadas? ¿Es posible ubicar categorías de cortesía estrictamente restringidas a la interacción judicial?

  • Chilean women in midlife : aspects on health, sexuality, migration and gender roles : implications for midwifery

    • Aims: To explore effects of biological and psychosocial factors on climacteric symptoms in Chilean midlife women (I), - to assess the impact of menopause and sociodemographic variables on quality of life (QoL) (II) - to learn about Chilean women s reflection about womanhood, and sexuality during midlife and the possible influence of migration whether they lived in Sweden or in Chile (III) - to explore how Chilean immigrant women living in Sweden perceived and related their life situations and health status during midlife to their migration experiences (IV) and to find out what Chilean midwives and midifery students regard being Chilean women´s health care needs in midlife (V). Material and Methods: Quantitative and qualitative methods were used to collect the data. The inclusion criteria for the studies were healthy Chilean women who accompanied patients to the Primary Health Centres (PHC) in Santiago (I-II), Chilean middle-aged women living in Santiago or who had lived in Stockholm for at least 15-20 years (III and IV). Chilean midwives working in PHC in Santiago and Chilean midwifery students in their last study years (4th and 5th year) (V). The quantitative data were collected in two cross-sectional surveys using face to face structured interviews (I-II); three validated scales and two questionnaires to collect data on biological, psychosocial and climacteric symptoms (I), one quality of life (QoL) questionnaire (II). The qualitative data was collected using Focus Group Discussions (FGDs) (III- V), in depth interviews with key informants (IV) and written narratives (V). Socio demographic data was collected in all the studies. EPI-INFO and STATA softwares were used for analysis of quantitative data (I-II). FGDs, in-depth interviews and written narratives were analysed using manifest and latent content analysis (III-V). Results: Perimenopausal women had a significant increase of stress and climacteric symptoms and perimenopausal state was a risk factor for somatic and vasomotor symptoms (I). Menopausal women had worse quality of life scores than women still menstruating and menopause was the only variable found to cause a significant impairment in QoL (II). A major influential factor in the women´s lives had been the strong influence of a gender imbalanced life since childhood. This was a fact whether the women lived in Stockholm, Sweden or Santiago, Chile. The experienced gender imbalance had made the women socially, economically and biologically more vulnerable to exploitation later in life (III). The Chilean women discussed about their struggle to gain social acceptance and position and they reflected about the discrimination they had met in the Swedish society and within the health care system along with health changes they had had during midlife. They connected some of their health related problems to their hardships of migration. Important for their way of coping with their own health seemed to be a recognition of their level of independence, need for an `own space´, self-acceptance and awareness of power relationships (IV). The midwives considered that women in midlife have special health care service needs; they considered themselves to be the most appropriate health staff to provide health care for women in midlife. Midwives recognized that they lacked communication skills and competences in assisting psychological, social and cultural health care needs of women in midlife such as violence, abuse and sexuality issues. The midwifery students remarked that many midwives focused their attention on fulfilling the biomedical requirements. Some students also questioned disrespectful attitudes by midwives especially towards poor immigrant women and women with psychosocial problems (V). Conclusion: Gender issues, structure of power relationships and empowerment should be incorporated into and critically discussed during midwifery education, training and in clinical settings. More reflections about and attention to the quality of the client provider relationship in clinical practice are needed.

    • We used whole-genome analysis and subsequent characterization of geographically diverse strains using new genetic signatures to identify distinct subgroups within Francisella tularensis subsp. tularensis group A.I: A.I.3, A.I.8, and A.I.12. These subgroups exhibit complex phylogeographic patterns within North America. The widest distribution was observed for A.I.12, which suggests an adaptive advantage.

  • Cardiovascular disease and adrenergic dysfunction in mild primary hyperparathyroidism

    • Primary hyperparathyroidism (PHPT) is associated with cardiovascular morbidity and premature death, but the underlying mechanisms are incompletely understood. The aim of this thesis was to investigate cardiovascular parameters and adrenergic function in mild PHPT compared to healthy controls subjects before and after curative parathyroidectomy (PTX). Forty-nine patients with mild PHPT (serum calcium 2.7 ± 0.1 mmol/L) and 48 control subjects, matched for age and sex, were examined; patients before PTX and up to 1 year postoperatively; control subjects at inclusion. Examinations comprised 24-hour ambulatory blood pressure (BP) measurements, 24-hour electrocardiogram with Holter monitoring, and echocardiography and measurements of circulating catecholamines at rest and after physical stress. At baseline, the patients showed signs of structural and functional heart disturbances and reduced catecholamine response to physical stress, which had not normalized at follow-up 6 months after PTX. The patients also showed signs of impaired heart rate variability (HRV). This, however, normalized after PTX. Patients being treated for hypertension showed an increase in BP after PTX, whereas BP decreased in normotensive patients. Echocardiographic, HRV-related and adrenergic parameters correlated with preoperative serum levels of PTH, contrary to hypertension. The results demonstrate reversible impairment of HRV and a previously unknown adrenergic dysfunction in PHPT, as well as the first clinical evidence of a chronotropic PTH effect. The results also demonstrate a hitherto unknown risk of BP increase after PTX in PHPT patients on antihypertensive treatment. These factors should be considered in the ongoing controversy regarding the management of patients with mild, so-called “asymptomatic” PHPT.

  • Public Provision of Private Goods and Nondistortionary Marginal Tax Rates : Some Further Results

    • The incidence and efficiency losses of taxes have usually been analyzed in isolation from public expenditures. This negligence of the expenditure side may imply a serious misperception of the effects of marginal tax rates. The reason is that part of the marginal tax may in fact be a payment for publicly provided goods and reflects a cost that the consumers should bear in order to face the proper incentives. Hence, part of the marginal tax may serve the same role as a market price in the sense that it conveys information about a real social cost of working more hours. We develop this idea formally by studying an optimal income tax model in combination with a type of public provision scheme not analyzed before; the provision level is individualized and positively associated with the individual’s labor supply. As examples we discuss child care, elderly care, primary education and health care. We show that there is a potential gain in efficiency where public provision of such services replaces market purchases. We also show that it is necessary for efficiency that, other things equal, marginal income tax rates are higher than in economies where the services are purchased in the market. This because the optimal tax should be designed so as to face the taxpayers with the real cost of providing the services. Hence, it might very well be that economies with higher marginal tax rates have less severe distortions than economies with lower marginal tax rates.

The discussion critically dissects these findings, proposing the concept of ‘promising victimhood’ for a nuanced understanding of differential treatment, while urging further theoretical development. This concise systematic review serves as a crucial benchmark for both academics and policymakers, emphasizing the need to address systemic injustices in refugee reception across various levels and sectors.
