  • Influence of dietary supplementation with Lactobacillus reuteri on the oral flora of healthy subjects

    • Avsikten med studien var att undersöka förekomsten av Lactobacillus reuteri i saliv ef- ter användandet av ett probiotiskt kosttillskott innehållande L. reuteri samt fastställa om L. reuteri har effekt på plackindex (PlI) samt på den supra- och subgingivala mikrofloran. Studien var dubbelblind, randomiserad, placebokontrollerad och löpte över 12 veckor. Totalt deltog 23 friska individer randomiserade i test- respektive kontrollgrupp. I försöket testades placebotuggummi mot tuggummi med L. reuteri (ATCC 55730 och ATCC PTA 5289). Tuggummi togs 2 gånger per dag, morgon och kväll efter tandborstning. Startda- gen och efter 12 veckor samlades vilosaliv samt plack supra- och subgingivalt. Dessutom bedömdes plackindex enligt Silness & Löe på 4 indextänder mesialt och distalt. Efter ytterligare 4 veckor gjordes en uppföljning med prov på saliv och bedömning av plack på indextänderna. Mikrobiologisk analys utfördes med hjälp av checkerboard DNA-DNA hybridiseringsteknik samt odling på selektiva medier för bestämning av laktobacill- populationen. Totala laktobacillhalten i saliv ökade signifikant i båda grupperna (p<0,05) med en signifikant ökning av L. reuteri i testgruppen (p=0,008). Efter 16 veckor påträffades ingen L. reuteri i saliv. Kontrollgruppen visade en signifikant ökning av PII efter 12 veckor (p=0,023), medan testgruppen inte visade någon förändring. I båda grupperna regist- rerades en signifikant ökning supra- och subgingivalt av de flesta bakteriearter, men det fanns ingen signifikant skillnad mellan grupperna för undersökta bakterier. Kvoten mellan ”onda/goda” supragingivala bakterier minskade i testgruppen men var inte sig- nifikant. Motsvarande kvot för subgingivala bakterier minskade signifikant i båda grup- perna. Slutsats: Intag av L. reuteri resulterade i förekomst av L. reuteri i saliv men bakterien måste tillföras kontinuerligt. Ingen signifikant förändring i den undersökta supra- och subgingivala mikrofloran kunde påvisas. Den signifikanta ökningen i PlI i kontrollgruppen utan motsvarande förändring i testgruppen kan tyda på en probiotisk effekt av L. reuteri i denna population

  • Cost of composite and glass ionomer class II molar restorations and theoretical analyses of cost per year of function at Public Dental Services in Sweden

    • The aim was to evaluate the cost of direct composite and glass ionomer class II molar restorations, and the theoretical cost per year of function, at Public Dental Services (PDS) in Sweden, years 2000 and 2005. Costs for patients, Social Insurance Offices (SI; Försäkringskassan), and total cost, were calculated based on fee schedules from all PDS in Sweden. Theoretical cost per year calculations were based on the median survival times (MST) of failed direct composite and glass ionomer class II molar restorations, derived from a set of clinical studies conducted in Nordic general practices. Due to lack of national statistics from SI, the number of direct restorations including more than one surface, made in adults, in general dentistry at PDS in the county of Halland were studied. From the year 2000 to year 2005, the total cost of composite class II molar restorations increased by 25 %, whereas the total cost of glass ionomer restorations more than doubled. Theoretical calculations implied a higher cost per year of function for composite restorations in year 2000, whereas in year 2005, glass ionomer restorations had a higher cost per year of function. The cost of direct composite and glass ionomer class II molar restorations increased from year 2000 to 2005, at PDS in Sweden. In the context of planning public health care funding, theoretical models for cost prediction may prove useful.

  • Dental care of young adults in west Sweden.

    • 107 Swedish subjects, all 20 years old, were studied for the first three years (1990-1992) after they had left the organised dental care for children and adolescents (which is free of charge for all youth through the age of 19). They were registered in four different risk-grouping systems in order to estimate the amount of their future dental care. Three of the systems used registrations from the Public Dental Service records and in the fourth one a dentist made a subjective estimation. The follow-up used dental insurance claims to study performed treatments, courses of treatments and cost. The risk group system that used subjective estimations appeared to be the one that most accurately predicted the actual dental care consumption. Approximately 70% of the subjects received some kind of dental care during the three years. The distribution was not confined to any particular risk group. Ten per cent had received complete dental care annually. Twenty-five per cent went to a private dentist and 75% continued to go to the Public Dental Service. Those who went to a private dentist received substantially more treatment and the annual cost was a little more than twice as much as in the Public Dental Service.

    • Patients considering undergoing combined orthodontic-orthognathic treatment for correction of malocclusion require appropriate information, particularly with respect to duration of treatment The primary aim of this study was to determine the duration of orthodontic treatment carried out in conjunction with orthognathic surgery and to analyze the influence of selected explanatory variables. A further aim was to compare the duration of orthodontic treatment in patients treated by orthodontists at Linkoping University hospital orthodontic clinic and six regional orthodontic clinics This is a retrospective study, based on data from the orthodontic records of patients who had undergone orthognathic surgery at the University Hospital Maxillofacial Unit in Linkoping, Sweden, from 2000 to 2005 The subjects comprised 207 out of 233 patients (107 women and 100 men), aged 15 8 - 56.9 years, median 24.2 years at the time of surgery 26 patients were excluded, either diagnosed with a syndrome, or because it was impossible to follow up the entire treatment. 59 subjects had undergone orthodontic treatment at the University hospital orthodontic clinic and 148 at the 6 regional orthodontic clinics. The variables recorded were gender, malocclusion, the number of appointments (scheduled and emergency) and treatment setting The median value for pre-operative orthodontic treatment time was 19.2 months (range 2.4 - 68.4); for postoperative orthodontic treatment 4 6 months (range 0 - 18 8) and for total orthodontic treatment 278 months (range 5.9 - 79 1). For the University hospital orthodontic clinic, pre-operative (16.7 months) and total orthodontic treatment times (25 months) were significantly shorter than for the regional clinics; the duration of postoperative treatment was significantly longer (4.1 months). No other explanatory variables had a significant influence on the duration of orthodontic treatment. Compared with the regional orthodontic clinics, there were significantly fewer scheduled appointments at the University hospital orthodontic clinic, but no significant difference in emergency attendance.

    • The preventive effect of a modified fluoride (F) toothpaste technique was investigated. Both the incidence and progression of approximal caries among Saudi adults with a high caries prevalence were evaluated after 2 years. A total of 175 adults were randomly assigned to a test group and a control group and 106 completed the study. In the test group (n = 54), the patients were asked to use the provided F toothpaste twice a day. They were instructed to use the "modified F toothpaste technique" as follows: (1) to use 2 cm of the toothpaste, (2) to brush for 2 min, (3) to swish the toothpaste slurry around the dentition with active movements of the cheeks, lips and tongue, forcing the slurry into the approximal area for about half a minute, before spitting it out, and (4) no post-brushing rinsing and no eating/drinking for 2 hr. The patients in the control group (n = 52) were instructed to continue using their regular F toothpaste twice a day without any further instructions. Approximal caries and filled surfaces were scored on bitewing radiographs at baseline and after 2 years. The mean (SD) total caries incidence for the test and control group was 1.15 (1.49) and 3.37 (2.57) respectively (p < 0.001). In general, the control group displayed a higher progression rate than the test group with regard to enamel lesions to dentine (NS), enamel lesions to filled surfaces (p < 0.05) and filled surfaces that had recurrent caries (NS). To conclude, the "modified fluoride toothpaste technique", as practised over the 2 years in a Saudi population with a high caries prevalence, had a caries preventive effect on the incidence of approximal caries, but not on the progression.

    • The aim of the present study is to analyse factors influencing the patterns of utilization of dental care in an adult urban Swedish population aged between 30 and 80 in the years 1993 and 2003.This study is part of two large epidemiological dental studies of randomly selected Individuals, performed in 1993 and 2003. It comprises clinical and questionnaire data from 555 and 505 individuals in the two study years. In 1993, more patients visited the private dental care system more often than the public system. This difference still existed in 2003, but to a 50% lower extent. All age groups had less utilization of dental care in 2003 than in 1993. The largest change was seen in the 30-year age group in utilization patterns of dental care between the two study years. A hierarchical block regression method was performed in four steps, analysing utilization for the two study years separately. The dental service system influenced the utilization patterns significantly for both years. The health factors strengthened this main result. Irrespective of dental health, patients were predicted to visit the private dental ca re service system more often in both 1993 and 2OO3. The socio-economic differences between people in Sweden were of no consequence for dental care utilization. As to age and attitude towards treatment costs, a substantial change was observed and might reflect a change in social patterns with greater inequalities, but the overall picture of equal access in dental care in Sweden remains.

  • Salivary IgA in minor-gland saliva of children, adolescents, and young adults

    • Immunoglobulin A (IgA) is the principal immunoglobulin in human saliva and minor salivary glands contribute with approximately 30-35 % of the total salivary IgA in whole saliva. Salivary IgA is considered to be a vital component of the defence of the oral tissues and the concentration seems to increase with age. However, data on the salivary IgA-concentration in minor gland saliva of children, compared to adults, is not available. Objectives: To study the salivary IgA-concentration in minor gland saliva in defined mucosal areas, and in un-stimulated whole saliva, in children in different ages and young adults. Methods: 90 individuals were recruited: 3-year-old children (n = 30), 14-year-old children (n = 30), and young adults (n = 30). Minor gland saliva was collected on labial and buccal mucosa with filter papers and the volume was determined using a Periotron 8000. The whole saliva was collected by draining into a tube. Assessment of the salivary IgA-concentration was carried out in a sandwich ELISA. One-way ANOVA was used testing the differences between the age-groups (at the 0.05 level of significance). Results: The salivary IgA-concentration in labial gland saliva was significantly lower (p<0.05) in the 3-year-olds (3.7 mg/100 ml +/- 3.5) compared with the 14-year-olds (12.6 mg/100ml +/- 12.8) and the adults (12.8 mg/100ml +/- 13.4). In the un-stimulated whole saliva, a significantly lower salivary IgA-concentration was noted for the youngest group (9.0 mg/100ml +/-9.1) compared with the 14-year-olds (17.9 mg/100ml +/-14.9) and the adults (17.0 mg/100ml +/-9.9). Conclusion: The salivary IgA-concentration in labial and un-stimulated whole saliva seems to increase with age. This may reflect a developing specific immune response in saliva of young children.   • Parental attitudes and experiences of dental care in children and adolescents with ADHD - a questionnaire study

    • Attention deficit hyperactivity disorder (ADHD) is a common psychiatric condition characterized by age-inappropriate levels of inattention, hyperactivity-impulsiveness or a combination of these. The aim of this study was to analyze parental attitudes to and experience of dental care, oral hygiene and dietary habits in children/adolescents with ADHD. Twenty-six parents of 31 subjects, 20 boys and 11 girls, aged 5-19 years with ADHD registered at the Gothenburg Child Neuropsychiatric Clinic, were invited. The parents answered a questionnaire regarding different oral problems when visiting the Clinic of Pediatric Dentistry, Gothenburg, for an oral examination of their child. The parents felt the dental care at the Public Dental Service was good, but noted a lack of knowledge regarding child neuropsychiatry among the dental staff which may influence the dental treatment. Fifteen parents reported their children had experienced mouth pain and 15 reported their child had suffered from both discomfort and pain from local anesthesia. Thirteen of the children had a dental trauma and 12 parents reported pain in connection to the dental treatment. Pain related to filling therapy was stated by 11 parents. According to the parents, five children suffered from dental fear but 15 reported the child had a general fear. Pursuant to the parents, the beverage for dinner was mainly milk or water, while sweet drinks were more frequent when thirsty. Seventeen; parents reported their children had poor oral hygiene or could not manage to brush their teeth and 14 of the 31 children only brushed once a day or less. The results show that the parents experience a lack of child neuropsychiatric knowledge, care and patience from the dental staff, which may influence the treatment. Oral hygiene/tooth brushing is neglected and the frequent consumption of sugar is difficult for the parents to handle.
