    • The evolution of reading frame maintenance must have been an early event, and presumably preceded the emergence of the three domains Archaea, Bacteria and Eukarya. Features evolved early in reading frame maintenance may still exist in present-day organisms. We show that one such feature may be the modified nucleoside 1-methylguanosine (m(1)G37), which prevents frameshifting and is present adjacent to and 3' of the anticodon (position 37) in the same subset of tRNAs from all organisms, including that with the smallest sequenced genome (Mycoplasma genitalium), and organelles. We have identified the genes encoding the enzyme tRNA(m(1)G37)methyltransferase from all three domains. We also show that they are orthologues, and suggest that they originated from a primordial gene. Lack of m(1)G37 severely impairs the growth of a bacterium and a eukaryote to a similar degree. Yeast tRNA(m(1)G37)methyltransferase also synthesizes 1-methylinosine and participates in the formation of the Y-base (yW). Our results suggest that m(1)G37 existed in tRNA before the divergence of the three domains, and that a tRNA(m(1)G37)methyltrans ferase is part of the minimal set of gene products required for life.

    • OBJECTIVE: The aim of the Antihypertensive Treatment and Lipid Profile in a North of Sweden Efficacy Evaluation study was to compare the long-term effect of the commonly used inexpensive medication with a low-dose diuretic (hydrochlorothiazide), alone or in combination with a beta-adrenoceptor (atenolol), with that of more modern but also more expensive antihypertensive treatment with an angiotensin-II-receptor blocker (candesartan), alone or in combination with a calcium antagonist (felodipine), and to do so in newly diagnosed patients with primary hypertension. The objectives included comparisons of the effects on the glucose metabolism, lipoprotein metabolism, electrolytes, blood pressure, and subjective symptoms.DESIGN: A 1-year, prospective randomized, double-blind, controlled trial.SUBJECTS: In an investigator-initiated study, we included 392 patients (mean age 55 years, 48% men); 370 patients (94%) had never been treated with antihypertensive drugs before the study. No patient was lost to follow-up.RESULTS: Both treatment regimens lowered blood pressure well (23/13 mmHg in the hydrochlorothiazide group and 21/13 mmHg in the candesartan group), with a majority of patients needing two drugs. Fasting levels of both serum insulin and plasma glucose increased in the hydrochlorothiazide group in contrast to unaffected levels in the candesartan group. Diabetes mellitus was diagnosed in nine patients during follow-up, in eight patients in the hydrochlorothiazide group (4.1%) and in one patient (0.5%) in the candesartan group (P = 0.030). Triglycerides increased and high-density lipoprotein-cholesterol decreased more in the hydrochlorothiazide group than in the candesartan group. Both the low-density lipoprotein/high-density lipoprotein and the apolipoprotein B/apolipoprotein A-I ratios increased in the hydrochlorothiazide group. At 12 months, 18 patients in the hydrochlorothiazide group versus five in the candesartan group had a 'metabolic syndrome', as defined by the World Health Organization (P = 0.007) despite 1 year of active blood pressure-lowering therapy. There were less (P = 0.020) adverse events in the candesartan group, but no major differences in the subjective symptoms assessment profile. One subject in each group had a myocardial infarction.CONCLUSION: Antihypertensive treatment with a diuretic, if needed combined with a beta-adrenoceptor blocker, was associated with an aggravated metabolic profile; this was not so for patients treated with an angiotensin-II-receptor blocker, if needed combined with a calcium antagonist. An antihypertensive treatment strategy that costs more in the short run but has no metabolic adverse effects may have a health economic impact in the long term.

    • This report reviews the pharmacology, toxicity and abuse pattern of gamma-hydroxybutyrate (GHB). The legislative changes pertaining to this substance are also addressed. Examples of abuse, driving under the influence and fatal intoxication are given. It is concluded that GHB is widely abused, particularly among the younger generation, and that further cases of severe intoxication are likely to occur as long as the substance is easily available from countless sources, including via the Internet. Despite the classification of GHB as a narcotic in Sweden and several other countries, continued problems are expected since the precursors gamma-butyrolactone (GBL) and 1,4-butanediol (BD) are widely--and legally--available.

    • This thesis examines usage of English colour terms in context, based on an extensive computerised text corpus, the Bank of English. It describes the ways in which English colour terms may be used to refer to nuances outside their normal area of designation and to attributes outside the colour domain. Usage patterns are analysed on three different levels: with regard to the overall frequency of occurrences, nominal domains and individual tokens, respectively.Cognitive linguistics supplies the theoretical framework employed in the analyses of the observed patterns. The study identifies three types of usage where colour terms refer to peripheral colour nuances or to concepts outside the colour domain: classifying, figurative and marked usage.When a colour term has a classifying function, it can be used outside the normal area of designation. This usage is analysed as a type of reference-point construction where a term referring to a salient point in the colour domain is used to subcategorise an entity whose actual colour may be only a peripheral member of the category named by the colour term. An analysis of the OED and the Bank of English shows that this type of usage is primarily restricted to a few of the most salient basic terms.This study points to the close affinities between classifying and figurative usage. Figurative expressions of colour terms frequently have a classifying function. I argue that figurative meanings are derived through two types of metonymy: +SALIENT ATTRIBUTE FOR OBJECT+ and +SALIENT CONCRETE ATTRIBUTE FOR SALIENT ABSTRACT ATTRIBUTE+.Marked usage arises when specific colour terms are used in nominal domains where the specificity is not expected. This phenomenon is consequently confined to non-basic colour terms.On the basis of the established patterns of usage and the frequency of occurrences, this thesis suggests that the colour category may be analysed as a radial category, with the basic colour terms forming the centre.
