    • PURPOSE Information about the patient's current drug list is a prerequisite for safe drug prescribing. The aim of this study was to explore general practitioners' (GPs) understandings of who is responsible for the patient's drug list so that drugs prescribed by different physicians do not interact negatively or even cause harm. The study also sought to clarify how this responsibility was managed. METHODS We conducted a descriptive qualitative study among 20 Swedish physicians. We recruited the informants purposively and captured their view on responsibility by semistructured interviews. Data were analyzed using a phenomenographic approach. RESULTS We found variation in understandings about who is responsible for the patient's drug list and, in particular, how the GPs use different strategies to manage this responsibility. Five categories emerged: (1) imposed responsibility, (2) responsible for own prescriptions, (3) responsible for all drugs, (4) different but shared responsibility, and (5) patient responsible for transferring drug information. The relation between categories is illustrated in an outcome space, which displays how the GPs reason in relation to managing drug lists. CONCLUSIONS The understanding of the GP's responsibility for the patient's drug list varied, which may be a threat to safe patient care. We propose that GPs are made aware of variations in understanding responsibility so that health care quality can be improved.

  • Decision-Making Is in the Making! Aspects of Decision-Making in the Area of Assistive and Welfare Technology : A Qualitative Study

    • Assistive and welfare technology (AT/WT) has been introduced as a way of facing an ageing population and providing support for older adults in their daily lives. There is much research concerning the assessment and recommendation of AT/WT to individual end-users. However, few studies have explored AT/WT decision-making from a managerial perspective. This study explores what aspects influence decision-making in assistive technology organizations concerning new technology procurements. The study is based on interviews with 24 managers engaged in assistive technology organizations, representing 13 of 21 regions in Sweden. The interview data consisted of the participants’ experiences deciding on AT/WT procurement. A reflexive inductive thematic analysis was used to identify aspects that influenced decision-making. The main findings show that decision-making is in the making, meaning that decision-making is a constant on-going managerial process. Furthermore, the findings show that managers experience uncertainty in the decision-making, sometimes make ad hoc decisions and request an evidence-based, person-centred approach to improve decision-making. The study concludes that supportive, technology, patient, and knowledge aspects influence managers’ decisions.

    • BACKGROUND: Digital assistive technology (DAT) may support time management in people with dementia or mild cognitive impairment (MCI), but research on DAT for time management is limited. We aimed to explore how everyday could be supported by DAT for time management in persons with dementia or MCI from informal carers' perspectives. This study focused on a DAT device for time management called MEMOplanner (MMP).METHOD: Using a mixed-methods design, we utilized the Time-Proxy© questionnaire and a study-specific interview guide to investigate the perspectives of informal carers (n = 8) regarding the use of MMP by individuals with dementia or MCI.RESULT: The MMP was helpful in keeping track of time and activity. It helped to maintain an active lifestyle and facilitated communication. However, the MMP did not reduce the need for assistance from the informal carers, and it took time to learn the different functions of the device. Further research into employing a more extensive array of DAT for time management or other areas to assist individuals with dementia will yield valuable insights into enhancing and sustaining a higher quality of life despite cognitive decline.

  • The Policies of Provision of Assistive and Welfare Technology : A Literature Review

    • Background: Due to the increasing ageing population worldwide, humanity is facing global demographic challenges. For many people, their later years are often lived with changed functioning and the need for support in daily living activities. Assistive technology and welfare technology (AT/WT) constitute a partial solution to the demographic challenges. Objectives: The purpose of this literature review is to identify the policy features, such as decision models and guidance for the provision of AT/WT as reported in the current scientific literature. Methods: A literature review with a deductive approach using the structure of the Global Cooperation on Assistive Technology (GATE) model was conducted. The data were searched from the databases: PubMed, Cinahl Plus, and the Web of Science, resulting in nine included articles. Results: The findings are presented according to the structure of the eight domains from the GATE model— policy, assessment, procurement, technology, environment, usability, sustainability and rights— which are further grouped into categories exploring the variations in every domain. Conclusion: The review reveals a nascent and growing interest in the area; however, evidence-based decision models and other guidance on AT/WT are lacking. Recent scholarships and policy reforms can form the basis of stronger guidance to organise complex AT/WT systems, but further and future research is needed.

  • Views of Swedish Elder Care Personnel on Ongoing Digital Transformation : Cross-Sectional Study

    • Background: Swedish municipalities are facing demographic challenges due to the growing number of older people and the resulting increased need for health care services. Welfare technologies are being launched as possible solutions for meeting some of these challenges.Objective: The aim of this study was to explore the perception, experimentation, evaluation, and procurement of welfare technology practices among professionals working in municipal elder care in relation to their gender, age, and profession.Methods: Data for this explorative cross-sectional study were collected from 393 responses to a web-based survey on municipal elder care in Sweden. Chi square tests were performed to determine the associations.Results: The results revealed gender, age, and professional differences in perspectives of municipal elder care workers. Differences were particularly evident in attitudes toward technology, both the use of technology in general and in the workplace, and involvement and participation in decision making regarding the procurement of new welfare technologies. Men (37/53, 70%) expressed a more positive attitude toward and curiosity regarding new technologies than women (157/336, 46.7%) (P=.03). Regarding age, the younger respondents (18-24 years old) perceived the digital transformation in the workplace as "too slow" (4/4, 100%), whereas the majority of older respondents (65-74 years old) perceived it as happening at the "right pace" (4/7, 57%). The elder care personnel felt encouraged by management to explore and experiment with new welfare technologies, but never did so either for management or with patients. Even though the majority of the respondents were women, more men (4/7, 57%) were involved in the procurement process for welfare technology devices and solutions than women (98/336, 29.2%) (P<.001).Conclusions: Personnel working within municipal elder care were generally very positive toward new technologies. However, both gender and age differences may influence these perspectives such as the personnel's resistance to welfare technology and patients' participation in welfare technology usage and deployment. Different levels of participation in the decision-making process regarding new technology deployment may negatively affect the overall digital transformation within municipal elder care.

  • Classification of physiological signals for wheel loader operators using Multi-scale Entropy analysis and case-based reasoning

    • Sensor signal fusion is becoming increasingly important in many areas including medical diagnosis and classification. Today, clinicians/experts often do the diagnosis of stress, sleepiness and tiredness on the basis of information collected from several physiological sensor signals. Since there are large individual variations when analyzing the sensor measurements and systems with single sensor, they could easily be vulnerable to uncertain noises/interferences in such domain; multiple sensors could provide more robust and reliable decision. Therefore, this paper presents a classification approach i.e. Multivariate Multiscale Entropy Analysis-Case-Based Reasoning (MMSE-CBR) that classifies physiological parameters of wheel loader operators by combining CBR approach with a data level fusion method named Multivariate Multiscale Entropy (MMSE). The MMSE algorithm supports complexity analysis of multivariate biological recordings by aggregating several sensor measurements e.g., Inter-beat-Interval (IBI) and Heart Rate (HR) from Electrocardiogram (ECG), Finger Temperature (FT), Skin Conductance (SC) and Respiration Rate (RR). Here, MMSE has been applied to extract features to formulate a case by fusing a number of physiological signals and the CBR approach is applied to classify the cases by retrieving most similar cases from the case library. Finally, the proposed approach i.e. MMSE-CBR has been evaluated with the data from professional drivers at Volvo Construction Equipment, Sweden. The results demonstrate that the proposed system that fuses information at data level could classify 'stressed' and 'healthy' subjects 83.33% correctly compare to an expert's classification. Furthermore, with another data set the achieved accuracy (83.3%) indicates that it could also classify two different conditions 'adapt' (training) and 'sharp' (real-life driving) for the wheel loader operators. Thus, the new approach of MMSE-CBR could support in classification of operators and may be of interest to researchers developing systems based on information collected from different sensor sources.

    • Supporting resource sharing in multiprocessor architectures is one of the major problems that limit the potential performance benefits of using such architectures for real-time systems. Many approaches and algorithms have been proposed to support resource sharing, however, most of them impose either high blocking times on tasks or require a large memory allocation. In this paper we investigate the possibility of combining the lock-based approaches and wait-free approaches (using multiple buffers) in order to decrease both the blocking times that may affect the schedulability of tasks and the required memory. To achieve this, we propose a solution based on evaluating the maximum allowed blocking time on each task according to the schedulability analysis, and then find the minimum memory allocation for each resource that limits the blocking times on tasks to be less than the maximum allowed blocking times.
