    • The goal of the research presented is to create a computational framework and system architecture for clinical decision support in palliative care. The application focused is the classification of depression. The method under investigation is case-based reasoning, motivated by the complexity of the domain and a lack of generalized principles of sufficient coverage and strength for diagnosis and treatment. A system architecture is described and exemplified through an implemented prototype. The outcome of the research so far is a system that captures the properties intended, and for which a clinical test set-up has been defined. © 2010 IEEE.

  • Impact of Abdominal Subcutaneous Fat Reduction on Glycemic Control in Obese Patients with Type 2 Diabetes Mellitus

    • Background: The effect on type 2 diabetes mellitus (T2DM) when adipose tissue is removed is controversial. This study aimed to evaluate and compare the effect of the abdominoplasty and bariatric surgery on glycemic control in patients with T2DM. Methods: Patients with T2DM undergoing abdominoplasty for cosmesis were studied (n=25). Subjects were 36.91.3 years with a preoperative body mass index (BMI) of 40.60.5kg/m(2) and mean glycated hemoglobin (HbA1c) of 7.4%+/- 0.2%. Fifteen matched patients undergoing bariatric surgery were selected as a comparator group. Weight, BMI, waist circumference (WC), random blood glucose (RBG), and HbA1c were evaluated at baseline and 3, 6, and 12 months postsurgery. Results: By 12 months, abdominoplasty reduced weight by 5.6 +/- 0.3kg p<0.01), and HbA1c was reduced to 6.8%+/- 0.3% (p<0.01). After 12 months, bariatric surgery reduced BMI from 42.2 +/- 1kg/m(2) to 26.6 +/- 0.4kg/m(2) (p<0.01). HbA1c reduced from 7.9%+/- 0.4% to 5.5%+/- 0.2% (p<0.01). WC was similar between both groups at 3 months, although HbA1c reductions were superior after bariatric surgery. Conclusions: Reducing subcutaneous adipose tissue with abdominoplasty results in a small improvement in glycemic control in patients with T2DM. Despite equivalent WC at 3 months, bariatric surgery outperformed abdominoplasty on all metabolic parameters then and thereafter.

    • We consider a Cyber-Physical System (CPS) as a network of components that are either physical plants with continuous behaviors or discrete controllers. To build CPS's in a systematic manner, the TIMES-Pro tool is designed to support modeling, analysis and code generation for real-time simulation and final deployment. In this paper, we present our decisions in designing the modeling language, the tool architecture and features of TIMES-Pro, and also a case study to demonstrate its applicability.

    • During vascular development, endothelial platelet-derived growth factor B (PDGF-B) is critical for pericyte recruitment. Deletion of the conserved C-terminal heparin-binding motif impairs PDGF-BB retention and pericyte recruitment in vivo, suggesting a potential role for heparan sulfate (HS) in PDGF-BB function during vascular development. We studied the participation of HS chains in pericyte recruitment using two mouse models with altered HS biosynthesis. Reduction of N-sulfation due to deficiency in N-deacetylase/N-sulfotransferase-1 attenuated PDGF-BB binding in vitro, and led to pericyte detachment and delayed pericyte migration in vivo. Reduced N-sulfation also impaired PDGF-BB signaling and directed cell migration, but not proliferation. In contrast, HS from glucuronyl C5-epimerase mutants, which is extensively N- and 6-O-sulfated, but lacks 2-O-sulfated L-iduronic acid residues, retained PDGF-BB in vitro, and pericyte recruitment in vivo was only transiently delayed. These observations were supported by in vitro characterization of the structural features in HS important for PDGF-BB binding. We conclude that pericyte recruitment requires HS with sufficiently extended and appropriately spaced N-sulfated domains to retain PDGF-BB and activate PDGF receptor β (PDGFRβ) signaling, whereas the detailed sequence of monosaccharide and sulfate residues does not appear to be important for this interaction.

  • Smith Predictor Sliding Mode Closed-loop Glucose Controller in Type 1 Diabetes

    • Type 1 diabetic patients depend on external insulin delivery to keep their blood glucose within near-normal ranges. In this work, two robust closed-loop controllers for blood glucose control are developed to prevent the life-threatening hypoglycemia, as well as to avoid extended hyperglycemia. The proposed controllers are designed by using the sliding mode control technique in a Smith predictor structure. To improve meal disturbance rejection, a simple feedforward controller is added to inject meal-time insulin bolus. Simulation studies were used to test the controllers, and shown the controllers ability to regulate the blood glucose within the safe limits in the presence of errors in measurements, modeling, and meal estimation.

    • Neuroscience, including research on Alzheimers disease, is hampered by the lack of suitable in vitro models to study the human nervous system. To counteract this, many attempts to differentiate cell lines into more neuron-like cells have been performed, resulting in partial expression of neuronal features. Furthermore, it has been reported that neuroblastoma cell lines lack mature isoforms of tau. Our aim was to develop an improved in vitro model, generating sustainable cells with morphology and biochemistry of human, mature neurons. To obtain cells with neuronal differentiation and function, we investigated the effect of combining three-dimensional culturing of SH-SY5Y cells in extracellular matrix (ECM) gel with several factors reported to have neuro-differentiating effects. This resulted in cells with apparent neuronal morphology with long, extensively branched neurites. Further investigation revealed expression of several neurospecific markers including synapse protein Sv2 and nuclear marker NeuN, as well as the presence of synapses and axonal vesicle transport. In addition, these cells expressed mature tau isoforms, and tau protein expression was significantly increased compared to undifferentiated cells, reaching levels found in adult human brain. In conclusion, we found that pre-treatment with retinoic acid followed by ECM gel culturing in combination with brain derived neurotrophic factor, neuregulin beta(1), nerve growth factor, and vitamin D-3 treatment generated sustainable cells with unambiguous resemblance to adult neurons. These cells also expresses adult splicing forms of tau with neuronal localization, making this cellular in vitro model useful in many areas of neuroscience research, particularly the Alzheimers disease field.

    • Spherical brushes composed of diblock polyampholytes (diblock copolymers with oppositely charged blocks) grafted onto solid spherical particles in aqueous solution are investigated by using the primitive model solved with Monte Carlo simulations and by lattice mean-field theory. Polyampholyte chains of two compositions are considered: a copolymer with a long and a short block, A(100)B(10), and a copolymer with two blocks of equal length, A(50)B(50). The B block is end-grafted onto the surface, and its charge is varied, whereas the charge of the A block is fixed. Single-chain properties, radial and lateral spatial distributions of different types, and structure factors are analyzed. The brush structure strongly depends on the charge of the B block. In the limit of an uncharged B block, the chains are stretched and form an extended polyelectrolyte brush. In the other limit with the charges of the blocks compensating each other, the chains are collapsed and form a polyelectrolyte complex surrounding the particles. At intermediate charge conditions, a polyelectrolyte brush and a polyelectrolyte complex coexist and constitute two substructures of the spherical brush. The differences of the brush structures formed by the A(100)B(10) and A(50)B(50) polyampholytes are also analyzed. Finally, a comparison of the predictions of the two theoretical approaches is made.

  • Evaluation of Irvingia kernels extract as biobased wood adhesive

    • Irvingia tree species have been earmarked for domestication in many countries due to their potential as raw materials for various applications, which include biodiesel, cosmetics, perfume, soap, etc. Presently, there is no information on the utilization of kernel seed extract as a potential source of green wood adhesive. This study is focused on investigating the properties of adhesives produced from kernel seeds of two Irvingia wood species i.e. Irvingia gabonensis (IG) and Irvingia wombolu (IW), as well as investigating the improved properties derived from the effect of modification using a few selected modifying agents including glutaraldehyde, glyoxal, epichlorohydrin (EPI) and an acid/base type process modification. Polyethylene (PE) was used along with the glutaraldehyde, glyoxal and epichlorohydrin modifiers in the modification process. Fourier transform infrared spectroscopy (FTIR), differential scanning calorimetry (DSC), scanning electron microscopy (SEM) and thermogravimetric analysis (TGA) were conducted to study the effect of modification on adhesive properties. The glycosidic carbon of the unmodified extracts and that of the EPI modified sample were not sensitive to chain conformations. Principal components (PC) 1 and 2 explained 85.19 and 9.54%, respectively, of the total variability in FTIR spectra among the modified and unmodified adhesives. The unmodified samples for IG and IW exhibited one peak with crystallization temperatures of 18.7 and 14.4 °C, respectively, indicating only one component exhibits some low degree crystallinity. The adhesive properties of the modified extracts were tested on wood veneers according to ASTM standard. The shear strength of the modified adhesives ranged from 1.5 to 3.93 MPa and 1.7 to 4.05 MPa for IG and IW, respectively. The modified samples containing PE showed marked improvement in the shear strength. The highest values were about 63% higher than the shear strength of unmodified samples with least shear strength. The results indicated that the modification of Irvingia-based adhesives had a great contribution to their performance as natural wood adhesives.
