Bladder exstrophy : Clinical and experimental aspects

- Bladder exstrophy is a complex malformation seen in 1/30 000. Most children are subject to repeated operations, including pelvic osteotomy, genital and bladder neck reconstruction and sometimes bladder augmentation or a urinary reservoir, to achieve continence and a normal genital function. A retrospective study of all Swedish children born 1970-89 showed a low continence rate, 17% , and a high rate of wound dehiscence. The results led to concentration of the treatment to fewer pediatric urologists and adoption of a national treatment policy. A prospective study of bladder augmentation and reservoir showed these operations to result in continence and a large bladder volume without major complications. Albumin leakage in the urine and increase rates for skeletal mineralisation need further evaluation. However, intestinal mucosa in the urinary tract is known to imply a risk of malignancy and metabolic complications over time. Therefore, an animal model for bladder augmentation in growing subjects was created. Theoretically, an intestinal patch without mucosa would diminish the risks but was in the pig model shown to shrink to such extent that the bladder volume decreased. A bladder augmented with a full thickness intestinal patch grew faster than the normal bladder which suggests the use of small intestinal patches in a growing child. A child with bladder exstrophy has a wide symphyseal diastasis leading to an outward rotation of the hips and a waddling gait. The fate of this malformation was studied using CT. The rotation of the pelvis was balanced by a torsion within the femur and an initial toeing-out was normalised over time. Incontinence, multiple operations and odd looking genitals implies a risk of psychological problems. However, tests and interviews showed a good self-esteem and a normal behaviour in most cases despite restrictions in social life and feelings of being different. Male teenagers seem to be a risk group and the parents are at risk for sustained psychological problems. Early support to parents and discussion groups for teenagers are suggested.

- Data visualization is of increasing importance in the Biosciences. During the past 15 years, a great number of novel methods and tools for biological data visualization have been developed and published in various journals and conference proceedings. As a consequence, keeping an overview of state-of-the-art visualization research has become increasingly challenging for both biology researchers as well as visualization researchers. To address this challenge, we have reviewed visualization research for the Biosciences and created an interactive web-based visualization tool, the BioVis Explorer. BioVis Explorer allows the exploration of published visualization methods in interactive and intuitive ways, including faceted browsing and associations with related methods.

To Be or Not to Be . . . : Brand Affiliation in the Hotel Industry

- A large proportion of any business’s value comprises intangible assets, and for many businesses a considerable part of these assets' value is attributable to brands with which businesses affiliate. In light of increasing affiliation of hotel properties with hotel chains and the increasing importance of branding in the hospitality industry, senior managers/owners should be aware of the importance of concepts such as brand equity and brand value and, better yet, incorporate them into their strategic decision-making processes. The extent to which hotel management scholars and industry practitioners understand or use the concept of brand value attributable to affiliation (BVAA) is, however, limited.The aim of this research project is to increase our understanding of the costs and benefits connected with operating a hotel independently or as part of an affiliation, using both quantitative and qualitative methods. Such an understanding could be used to spur further research but should also be directly applicable by the practitioners.The results of this research indicate that affiliating with a brand matters to managers/owners in the hotel industry, but also that brand-related concepts are used only to a limited extent. One of the studies comprising this dissertation, which included 51,000 hotels in the U.S. during a full economic cycle, suggests that affiliated hotels produced better financial performance than unaffiliated hotels, especially during the global economic recession of 2007–2009. On the other hand, the results from a study of hotels and organisations in Sweden suggest that brand value–related information attributable to affiliation is not used to any large extent in the industry. In the third study, a method for financially evaluating BVAA is developed, making it possible not only to measure whether affiliating produces a positiveBVAA, but also to demonstrate a practical method for financially assessing a current affiliation and the various affiliation options. Consequently, these findings should contribute new ways of treating the strategic question of affiliation.

Improved Sleep Correlates with Improved Quality of Life and Motor Symptoms with Foslevodopa/Foscarbidopa

- Background: Foslevodopa/foscarbidopa is a subcutaneous infusion of levodopa/carbidopa prodrugs. Objectives: Assess correlations between sleep and efficacy from interim data of a phase 3 trial of foslevodopa/foscarbidopa (NCT03781167). Methods: Pearson correlations between sleep (Parkinson's Disease Sleep Scale-2 [PDSS-2]) and quality of life (QoL; Parkinson's Disease Questionnaire-39), motor experiences of daily living (m-EDL; Movement Disorder Society-Unified Parkinson's Disease Scale Part II), and "Off"/"On" times were calculated for baseline and week 26 improvements. Regression analyses were adjusted for baseline PDSS-2 score. Results: Baseline sleep correlated moderately with QoL (r = 0.44, P < 0.001) and weakly with m-EDL (r = 0.28; P < 0.001). Sleep improvement weakly correlated with improved "Off" time (r = 0.37; P < 0.001) and QoL (r = 0.36; P < 0.001). Regression analyses demonstrated significant positive associations for improved sleep, "Off" time, QoL, and m-EDL. Conclusions: Improved sleep with foslevodopa/foscarbidopa was associated with improved QoL and "Off" time.

- We study the impact of efficient network coding and multiple access techniques in a two-source two-relay one-destination wireless network through the diversity-multiplexing tradeoff (DMT) analysis. We compare the standard relaying protocol without network coding and the protocols using either a binary network coding (BNC) or an appropriately designed non-binary finite-field network coding (FFNC) at the relays. It is shown that the use of the non-binary FFNC strictly outperforms the other two protocols in terms of DMT. In addition, we propose a new transmission strategy based on the non-binary FFNC design and the non-orthogonal multiple access technique to further improve the DMT performance. Our results highlight the advantages of applying appropriate network coding in multi-source multi-relay networks.
