  • Likhet inför lagen
  • 2005

  • Person-Centeredness in Clinical Reasoning of Interprofessional Stroke Teams
  • 2023
  • Ingår i: Patient Education and Counseling. - : Elsevier. - 0738-3991 .- 1873-5134. ; 109:Suppl., s. 70-70
  • Tidskriftsartikel (övrigt vetenskapligt/konstnärligt)abstract
    • Background: Although person-centered care is prioritized in healthcare, challenges remain before such care is integrated in everyday communication and practice. One way to strengthen person-centered care is that health professionals’ clinical reasoning (i.e., assessment and management) is pervaded by patient participation and individualized to patient needs. Interprofessional team meetings, focusing on goals and management planning, is an opportunityt o improve person-centeredness. However, there is a lack of understanding of how person-centeredness is created in the clinical reasoning of teams. This study aims to explore how clinical reasoning is performed from a person-centered perspective in team meetings with patients with stroke and next of kin. Methods: Explorative qualitative design employing a thematic analysis of audio recorded communication at three team meetings. In total, three patients, two next of kin, and 15 professionals representing eight health professions, participated in the meetings.Findings: Four themes and eight subthemes were established: a) Emphasizing the patient’s resources; b) Struggling to find a common understanding, including subthemes: Unite the person’s narrative, the relative’s view, and the expertise of the interprofessional team, missed opportunities to clarify patient needs and wishes, and active listening and receptiveness; c) Balancing the patient’s goals and professionals’ goals, including subthemes: Shared goals, the professionals’ view of appropriate goals, and the professionals’ assessment governs achievement of goals; d) Ambiguity in decisions about the management plan, including subthemes: Initiating shared decision making and lack of clarity and consensus.Discussion: This study reveals how the patient, next of kin and team contribute to shared understanding of the patient’s problem. Goals were guided by the patient’s and professionals’ expertise. To improve person-centeredness in clinical reasoning, the team need to further put the patient’s personal goals and perception of goal-achievement in the forefront as well as improve communication skills to catch patient needs and facilitate shared decisions.

  • Facing negative emotions : Evaluating the effects of training in affirmative communication for contact nurses in cancer care
  • 2022
  • Ingår i: Annals of Oncology. - : Elsevier. - 0923-7534 .- 1569-8041. ; 33:7, s. S1368-S1368
  • Tidskriftsartikel (övrigt vetenskapligt/konstnärligt)abstract
    • Background: In cancer care, contact nurses daily meet people who deal with strong, aversive emotions in relation to that they have or may have cancer where the risk ofdying is constantly present. This places demands on the ability of contact nurses tomaster difficult conversations with strong emotional expressions. One communicationmethod known for its regulating effects on emotions is affirmative communication, socalled validation. The overall aim of the current study was to evaluate effects of atraining in validating communication for contact nurses in cancer care, aimed tostrengthen their ability to work in a person-centered way. Methods: This study had a within-group design with pre-, post- and follow-up mea-surements (2 months). Specifically, the study aimed to evaluate whether the vali-dation training coincided with an increase in validation and a decrease in invalidation.Contact nurses from six regions (n ¼ 17) with a generally long work experience as anurse participated in a digital validation training for six weeks (three training sessionsplus three pre-recorded theoretical lectures). Communicative behaviors wereassessed through video-recordings of interactions between the nurses and fictitiouspatients. The videos were encoded by independent coders and analyzed withdependent MANOVA.Results: The contact nurses showed a significant increase in validation and a signif-icant decrease in invalidation after the training. These effects remained at the 2-month follow-up.Conclusions: Experienced healthcare professionals’ affirmative communication skillscan be improved after a brief training in validation.

  • Facing negative emotions : Evaluation of a brief training in validating communication for contact nurses in cancer care
  • 2023
  • Ingår i: European Journal of Oncology Nursing. - : Elsevier. - 1462-3889 .- 1532-2122. ; 66
  • Tidskriftsartikel (refereegranskat)abstract
    • Background: Contact nurses in cancer care (CNCC) often face challenges when communicating with patients and their families. The overall aim was to evaluate a brief digital validation training for CNCC, to test whether it was associated with increased validation and decreased invalidation. Associations between communication skills in validation and markers of work-related stress were also investigated. Methods: This intervention study investigated associations between the training and validation skills using a within-group design with repeated measures (at pre, post, and eight-week follow-up). Additionally, associations between the training and occupational self-efficacy, self-validation, and exhaustion symptoms were explored. Results: Seventeen CNCCs (all female with relatively long work experience) completed a five-week digital training program. Results indicated a statistically significant increase in validation and a statistically significant decrease in invalidation, showing that the nurses improved their communication skills following the training. No changes were found in markers of work-related stress. Conclusion: The results indicate that digital validation training for CNCCs is potentially beneficial by means of improving communication skills. This study further demonstrates that a brief digital training in validating communication to CNCC is associated with improved person-centered communication regarding strong negative emotions.

  • Bereaved siblings' perception of participating in research : a nationwide study
  • 2013
  • Ingår i: Psycho-Oncology. - : John Wiley & Sons. - 1057-9249 .- 1099-1611. ; 22:2, s. 411-416
  • Tidskriftsartikel (refereegranskat)abstract
    • ObjectiveThe objective of the present study is to examine bereaved siblings' perception of research participation.MethodsA Swedish nationwide study on avoidable and modifiable health care‐related factors in paediatric oncology among bereaved siblings who lost a brother or sister to cancer between the years 2000 and 2007 was conducted. Data are presented as proportions, and the differences between groups were statistically tested at the 5% significant level using Fisher's exact test.ResultsOut of 240 eligible siblings, 174 responded (73 %). None of the siblings (0/168) thought their participation would affect them negatively in the long term. However, 13% (21/168) stated it was a negative experience to fill out the questionnaire, whereas 84% (142/169) found it to be a positive experience. Women were more likely to report their participation as positive in a long‐term perspective compared with men (p = 0.018).ConclusionsNone of the bereaved siblings in this Swedish nationwide study anticipated any long‐term negative effect from their research participation. A majority reported it as positive to revisit their needs and experiences throughout their brother or sister's illness and death 2–9 years following the loss. We believe that the stepwise approach used in this study contributed to the high acceptance.

  • Psychological health in siblings who lost a brother or sister to cancer 2 to 9 years earlier.
  • 2013
  • Ingår i: Psycho-Oncology. - : Wiley. - 1057-9249 .- 1099-1611. ; 22:3, s. 683-691
  • Tidskriftsartikel (refereegranskat)abstract
    • BACKGROUND: The objective of this study was to assess long-term psychological distress in siblings who lost a brother or sister to cancer 2 to 9 years earlier, as compared with a control group of non-bereaved siblings from the general population.METHODS: During 2009, we conducted a nationwide follow-up study in Sweden by using an anonymous study-specific questionnaire. Siblings who had lost a brother or sister to cancer between the years 2000 and 2007 and also a control group of non-bereaved siblings from the general population were invited to participate. The Hospital Anxiety and Depression Scale (HADS) was used to measure psychological distress, and to test for differences in the ordinal outcome responses between the groups, we used Wilcoxon-Mann-Whitney rank-sum test.RESULTS: Among the bereaved siblings, 174/240 (73%) participated and 219/293 (75%) among the non-bereaved. Self-assessed low self-esteem (p = 0.002), difficulties falling asleep (p = 0.005), and low level of personal maturity (p = 0.007) at follow-up were more prevalent among bereaved siblings. However, anxiety (p = 0.298) and depression (p = 0.946), according to HADS, were similar.CONCLUSION: Bereaved siblings are at increased risk of low self-esteem, low level of personal maturity and difficulties falling asleep as compared with non-bereaved peers. Yet, the bereaved were not more likely to report anxiety or depression.

  • Patient throughput times and inflow patterns in Swedish emergency departments. A basis for ANSWER, A National SWedish Emergency Registry
  • 2011
  • Ingår i: Scandinavian Journal of Trauma, Resuscitation and Emergency Medicine. - : Springer Science and Business Media LLC. - 1757-7241. ; 19
  • Tidskriftsartikel (refereegranskat)abstract
    • Objective: Quality improvement initiatives in emergency medicine (EM) often suffer from a lack of benchmarking data on the quality of care. The objectives of this study were twofold: 1. To assess the feasibility of collecting benchmarking data from different Swedish emergency departments (EDs) and 2. To evaluate patient throughput times and inflow patterns. Method: We compared patient inflow patterns, total lengths of patient stay (LOS) and times to first physician at six Swedish university hospital EDs in 2009. Study data were retrieved from the hospitals' computerized information systems during single on-site visits to each participating hospital. Results: All EDs provided throughput times and patient presentation data without significant problems. In all EDs, Monday was the busiest day and the fewest patients presented on Saturday. All EDs had a large increase in patient inflow before noon with a slow decline over the rest of the 24 h, and this peak and decline was especially pronounced in elderly patients. The average LOS was 4 h of which 2 h was spent waiting for the first physician. These throughput times showed a considerable diurnal variation in all EDs, with the longest times occurring 6-7 am and in the late afternoon. Conclusion: These results demonstrate the feasibility of collecting benchmarking data on quality of care targets within Swedish EM, and form the basis for ANSWER, A National SWedish Emergency Registry.

  • Anaerobic bioremediation of a soil with mixed contaminants : Explosives degradation and influence on heavy metal distribution, monitored as changes in concentration and toxicity
  • 2009
  • Ingår i: Water, Air and Soil Pollution. - Dordrecht : Springer Netherlands. - 0049-6979 .- 1573-2932. ; 202:1-4, s. 301-313
  • Tidskriftsartikel (refereegranskat)abstract
    • Two soils with explosives and metals were evaluated for the degradation efficiency of explosives by native microorganisms under anaerobic conditions. The commercially available method Daramend®, amended with zero-valent iron (ZVI), was compared with a horse-manure amended compost and a treatment with ZVI alone. In a moderately contaminated soil, Daramend® and ZVI treatment gave significantly higher removal rates compared  to compost and control treatments (Tukey’s test, P<0.05). The largest overall decrease in ecotoxicity, measured with bioluminescent bacteria (Vibrio fischeri), was achieved with ZVI-treatment. In a more contaminated soil no degradation of contaminants and no decline in soil toxicity could be distinguished after the same time period. Problems with establishment of anaerobic conditions during parts of the remediation process and low microbial activity due to acute toxicity of contaminants are plausible explanations. Redistribution that could potentially lead to mobilization of the co-contaminant Pb was not observed in either of the soils during the biological treatments.

  • Bioremediation of a soil industrially contaminated by wood preservatives : degradation of polycyclic aromatic hydrocarbons and monitoring of coupled arsenic distribution
  • 2011
  • Ingår i: Water, Air and Soil Pollution. - Dordrecht : Springer Netherlands. - 0049-6979 .- 1573-2932. ; 214:1-4, s. 275-285
  • Tidskriftsartikel (refereegranskat)abstract
    • Two commercially available aerobic bioremediation methods (Daramend® and BioSan) were utilized to study the aerobic biodegradation of polycyclic aromatic hydrocarbons (PAH) and the effect of the simultaneously present arsenic. The soil was collected at an old wood preservation site and the initial PAH16-concentration was 46 mg/kg, with mainly high molecular weight congeners. The As-concentration was105 mg/kg with low availability as assessed with sequential extraction. To enahce the availability of PAH, the effect of a non-ionic surfactant was evaluated. Degradation of both low and high molecular weight PAH was observed, however after 30 weeks, the degradation was generally low and no treatment was significantly better than the others. The treatments had, on the other hand, an effect on As-distribution, with increased As-concentration in the available fraction after treatment. This may be due to both the microbial activity and the presence of anoxic micro sites in the soil. The overall efficiency of the biological treatment was further evaluated using the standardized ecotoxicity test utilizing Vibrio fischeri (Microtox®). The toxicity test demonstrated that the bioremediation led to an increase in toxicity, especially in treatments receiving surfactant. The surfactant implied an increase in contaminant availability but also a decrease in surface tension, which might have contributed to the overall toxicity increase.
