    • The aim of this study was to describe how patients' participation in the care they received was documented in their health care records during the last three months of their lives. Two hundred and twenty-nine deceased adult persons were randomly identified from 12 municipalities in a Swedish county and their records were selected from different health care units. Content analysis was used to analyse the text. Four categories of patient participation were described: refusing offered care and treatments; appealing for relief; desire for everyday life; and making personal decisions. The most common way for these patients to be involved in their care at the end of their life was by refusal of the treatment and care offered. Characteristic of the different ways of participation were the diverse activities represented. The description of patients' involvement in their life situation at this time indicated their dissociation from the health care offered more than consenting to it.

  • Self-reported physical activity and aerobic fitness are differently related to mental health

    • Background: A relevant, but overlooked question is if self-reported physical activity and aerobic fitness are differently related to mental health. Purpose: To examine the relation between mental health and level of self-reported physical activity (SRPA) and aerobic fitness (AF), and whether AF mediates the relation between SRPA and mental health. Methods: Participating in the study were 177 voluntary subjects (49% men, 51% women) with a mean age of 39 years. Symptoms of depression and anxiety were measured through the Hospital Anxiety and Depression (HAD) scale, and the Shirom-Melamed Burnout Questionnaire (SMBQ) was used to evaluate self-reported symptoms of burnout. Leisure time SRPA during the last three months were measured using a single item. AF was measured by using the Åstrand bicycle test. Results: Self-reported physical activity, but not AF, was significantly related to self-reported symptoms of depression, anxiety, and burnout. Light to moderate physical activity that is performed regularly seems to be associated with more favorable mental health pattern compared with physical inactivity. No support was found for the mediating effect of AF of the physical activityemental health relationship. Conclusions: Self-reported behavior of regular physical activity seems to be more important to monitor than measures of AF when considering the potential preventive effects of physical activity on mental health.

    • The nature of the sustainability challenges currently at hand is such that dominant pedagogies and forms of learning that characterize higher education need to be reconsidered to enable students and staff to deal with accelerating change, increasing complexity, contested knowledge claims and inevitable uncertainty. In this contribution we identified four streams of emerging transformative, transgressive learning research and praxis in the sustainability sciences that appear generative of a higher education pedagogy that appears more responsive to the key challenges of our time: (1) reflexive social learning and capabilities theory, (2) critical phenomenology, (3) socio-cultural and cultural historical activity theory, and (4) new social movement, postcolonial and decolonisation theory. The paper critiques the current tendency in sustainability science and learning to rely on resilience and adaptive capacity building and argues that in order to break with maladaptive resilience of unsustainable systems it is essential to strengthen transgressive learning and disruptive capacity-building. © 2015 Elsevier B.V.

  • The decline of Western European social democracy: Exploring the transformed link between welfare state generosity and the electoral strength of social democratic parties, 1975–2014

    • Social democratic parties are crumbling at the polls. Surprisingly, however, the causes of this demise remain largely unexplored. This article contributes to filling this gap in the research by studying the long-term impact of welfare state generosity on the vote share of social democratic parties in 16 Western European democracies. If the welfare state indeed was a key factor behind social democratic growth in the past, we ask whether the recent plight of these parties is down to a reversal of their previously dominant success factor? The article makes three principal findings. First, we show that social democratic parties primarily benefited electorally from expansive reforms at lower levels of welfare state generosity. Second, we find that this dynamic of diminishing returns also helps explain the demise of the Social Democratic party family in the whole of Western Europe. Lastly, our results reveal that programmatic turns to the right predict electoral losses in the least generous welfare states, whereas such shifts either pass unnoticed or predict vote gains in the most generous ones. We conclude by arguing that the structure of welfare state institutions is one important explanation for variations in the demise of the once powerful Social Democratic party family.

  • Challenges in Delivering Services: The Front-Line Hospitality and Tourism Employee Perspective

    • Significant challenges are faced by front-line hospitality and tourism employees interacting with customers on a daily basis. These challenges and the ways they are dealt with form the focus of this thesis. As they interact, a range of incidents may arise from highly satisfactory encounters to service failures. It has been argued that in order to achieve satisfactory service encounters, employees need to feel in control, satisfied and motivated as well as being given appropriate assistance, information and training. Front-line employees in hospitality and tourism are focused on here due to the complex nature of the industry. It is commonly agreed that it is characterised by poor working conditions such as anti-social working hours, part-time and/or seasonal work, minimal on-the-job training and low levels of pay. These are all factors which may affect employee work satisfaction and motivation as well as recruitment and retention. The thesis is comprised of four studies. In Study I, hospitality and tourism employee work motivation is analysed. Study II examines front-line hotel restaurant employees’ experiences of service encounters. How the skills utilised by employees during these service encounters are acquired and developed is then explored in Study III. Study IV concludes by examining employee informal information sharing activities, which offer one way of developing these skills. The thesis’ findings paint a more positive picture of work and workers in hospitality and tourism than suggested in some of the earlier published research. This could be explained by the fact that the thesis utilises an employee perspective in its truest sense. The findings of the thesis show that employees are motivated (to large extent by factors that have been pointed out in earlier published work motivation research) and that they are active in the pursuit of developing skills necessary to create satisfactory service encounters. Another conclusion of the thesis’ findings is that the interaction between workers and guests in the service encounter provides an important opportunity for workers to feel satisfied at work.

    • Background: In coming decades, the number of people affected by illnesses who need palliative care will rise worldwide. Registered Nurses are in a central position in providing this care, and education is one of the necessary components for meeting coming requirements. However, there is a lack of knowledge about palliative care in undergraduate nursing education curricula, including the extent of the education provided and the related pedagogical methods. Aim: The aim was to investigate the extent, content and pedagogical methods used and to explore lecturers’ experiences of being responsible for teaching and learning about palliative care for undergraduate nursing students on nursing programmes at Swedish universities. Setting: All 24 universities responsible for providing undergraduate nursing education in Sweden participated. Participants: One lecturer with in-depth knowledge about palliative care or end-of-life care education participated in the quantitative (n = 24) and qualitative (n = 22) parts of the study. Method: A mixed-method research study with an explorative design was used. Descriptive statistics were used to analyse quantitative data, and content analysis for qualitative, with both also analysed integratively. Results: Few undergraduate nursing programmes included a specific course about palliative care in their curricula, however, all universities incorporated education about palliative care in some way. Most of the palliative care education was theoretical, and lecturers used a variety of pedagogical strategies and their own professional and personal experience to support students to understand the palliative care approach. Topics such as life and death were difficult to both learn and teach about. Conclusions: There is a need for substantial education about palliative care. Lecturers strive on their own to develop students’ understanding and increase the extent of palliative care education with innovative teaching strategies, but must compete with other topics. Palliative care teaching must be prioritised, not only by the universities, but also by the national authority.

Ett stort antal experter har medverkat i boken varav majoriteten är barnmorskor med olika yrkeskunskaper och akademisk erfarenhet. Boken är unik i det att den tydliggör och lyfter fram, både på ett teoretiskt och praktiskt plan, barnmorskans kunskaper och färdigheter för yrkesutövningen.   Boken har delats in i tre stora huvudområden där den första delen tar upp barnmorskans historia, arbets- och forskningsfält samt globala aspekter. Den andra delen - barnafödande, börjar med ett nytt kapitel om professionellt förhållningssätt som berör barnmorskans roll när det gäller mötet med kvinnan och hennes närstående inom olika verksamhetsgrenar. Vidare finns teorier om bemötande, värdegrund och metoder för samtal under graviditet och förlossning, vid depression, vid föräldraförberedelse och variationer i föräldraskap. Dessutom behandlar den andra delen mödrahälsovård, graviditet, förlossning, barnsängstid samt förlossning med komplicerat förlopp. Det tredje avsnittet - reproduktiv och sexuell hälsa - inkluderar fertilitetsreglering, infertilitet, klimakteriet, ungdomars sexuella utveckling och hälsa samt gynekologi och gynekologisk undersökningsteknik. Sist i boken finns ett avsnitt om gynekologisk cancer samt ett nytt avsnitt om sexualitet.

  • Strategic alliances in Kenyan smallholder farming

    • Implementing and complying with standards and certifi cation oft en increases costs for supply chain actors. Th ese increased costs are caused by upgrading production, logistics and marketing needed to achieve compliance, and can lead to the exclusion of actors from the supply chain. In particular, the exclusion of small-scale growers in developing countries as the result of the expansion and proliferation of private voluntary standards (PVS) used by large procurers has been extensively reported. Costs of PVS are per certifi cation and the unit is usually the individual farm, regardless of its size. In much of the developing world smallholder production dominates domestic food production, and these small farms face proportionately higher costs per unit area for certifi cation and compliance. Benefi ts of PVS are per production unit, giving benefi ts to larger farms. Developing world smallholder production tends to be on less than one hectare, giving relatively small production. Compared with the costs per farm, there is an inherent bias in many standards and certifi cation towards larger farms. Standards in export horticulture can, potentially, incentivize a more active role for the private sector in investing in small-scale growers in ways that are mutually benefi cial for growers and exporters. Such co-investment is a feature of trading relationships and business models that are inclusive of small-scale growers. In Kenyan horticulture, donors, exporters and smallholders have in some cases managed to leverage PVS requirements into profi table local agricultural developments. In this paper, we use resource-based strategic alliance theory to explain the patterns that have evolved. We propose greater use of cooperation theory to help make more effi cient economic development interventions which are complementary with private-sector investments. We explore how standards might be used to accelerate development initiatives.
