  • Problematic medication use in headache - Epidemiology and qualitative aspects of medication overuse headache and non-adherence to prophylactic migraine treatment

    • Aim: The aim was to analyze problematic medication use among persons with headache, focusing on overuse of acute medications, with subsequent medication overuse headache (MOH), and on underuse of prophylactic medication. Methods: Data in Studies I and II came from a population survey in which 44 300 Swedes were interviewed by telephone about headache and medication use. In Study III, a questionnaire concerning adherence and beliefs about medicines was distributed to 174 consecutive migraine patients at a headache clinic. Study IV was a qualitative study, using grounded theory, in which 14 individual interviews were conducted with persons who fulfilled the diagnostic criteria for MOH. Results: The prevalence of MOH in Sweden was 1.8% (95% C.I. 1.7–1.9). It was 2.8 times more common among women than among men and inversely associated with socioeconomic status. Fewer than half of those with MOH had made a headache-related visit to a physician during the previous year, and almost half used only over-the-counter (OTC) medications to treat their headaches. The proportion only using OTC medications was particularly high among the young. There were several differences indicating that the use of medications and the rate of health care contacts were unequal in relation to educational level. As for use of prophylactic medications, approximately one third of the migraineurs were considered non-adherent. The participants in the qualitative study perceived headaches as something that threatened to ruin their lives, and despite extensive efforts, they were unable to find any effective aid other than the acute medication. Because of this, the acute medication became indispensable to them. Conclusion: Both overuse of acute medication and underuse of prophylactic medication are significant problems among persons with headache. Since both problems may lead to increased headaches, it is likely that many persons with headache carry an unnecessarily high disease burden.

    • Die Geschichte der verschiedenen Richtungen der Frauenbewegungen im 19. und 20. Jahrhundert ist in den letzten Jahrzehnten einer breiten wissenschaftlichen Untersuchung unterzogen worden. Dabei lag der Schwerpunkt der Forschungen weitgehend auf den Methoden, Strukturen und Aktionsformen von nationalen Frauenbewegungen, die in ihren verschiedenen Erscheinungsformen untersucht worden sind. Die historische Analyse in Deutschland hat erst in den letzten Jahren begonnen, sich von dieser nationalen Ebene wegzubewegen und einen ersten vorsichtigen Blick darauf zu werfen, dass die Frauenbewegungen der verschiedenen Länder eine Organisation auf internationaler Bühne anstrebten und diese auch schufen. International organisierte Frauen konnten dabei teilweise auf jahrelanges gemeinsames Engagement zurückgreifen, denn die Frauenbewegungen hatten seit ihren Anfängen international diskutiert und gearbeitet. Es waren gerade diese über die nationalen Grenzen hinausgehenden Diskussionen, die eine wesentliche Grundlage der Frauenbewegungen bildeten. Es greift allerdings zu kurz, wenn man die internationale Dimension der Frauenbewegungen als ihre unveränderbare Grundlage zwischen 1848 und der Zeit nach dem Zweiten Weltkrieg ansehen wollte. So wie nationale Bewegungen historischen Veränderungen ausgesetzt waren und diese beschleunigten oder abbremsten, so unterlag auch die internationale Zusammenarbeit historischen Prozessen. Diese internationale Zusammenarbeit in den Frauenbewegungen, ihre Brüche und Kontinuitäten, sind Gegenstand des vorliegenden Sammelbandes. Die hier versammelten Artikel gehen auf die Tagung "Kommunikation – Mobilität – Netzwerke: Die internationale Dimension der Frauenbewegungen (ca. 1830 bis in die 1960er Jahre)" zurück, die vom 15. bis 17. September 2005 an der Universität Bremen stattfand. Inhaltlich war die Tagung von Mitgliedern des Arbeitskreises Historische Frauen- und Geschlechterforschung konzipiert und durchgeführt worden, insbesondere von Eva Schöck-Quinteros, Jutta Schwarzkopf, Annika Wilmers und Kerstin Wolff.Die Konferenz war eine der ersten in Deutschland, die verschiedene nationale Frauenbewegungen des 19. und 20. hinsichtlich ihrer internationalen Dimensionen untersuchte. Forscherinnen und Forscher aus Deutschland, England, Schweden, den USA, Kanada, Ungarn und den Niederlanden trugen dazu bei, die Geschichte der nationalen Frauenbewegungen um ihre internationale Perspektive zu erweitern.

    • The objective of this study was to analyse sociodemographic differences in medication use, health-care contacts and sickness absence among individuals with medication-overuse headache (MOH). A cross-sectional, population survey was conducted, in which 44,300 Swedes (a parts per thousand yen15 years old) were interviewed over telephone. In total, 799 individuals had MOH. Of these, 47 % ( = 370) only used over-the-counter medications. During the last year, 46 % ( = 343) had made a headache-related visit to their physician and 14 % ( = 102) had visited a neurologist. Among individuals aged < 30 years, the number of days/month with headache was greater than the number of days with medication use, whereas the opposite was true for those a parts per thousand yen30 years. Both the proportion using prophylactic medication and the proportion having consulted a neurologist were smaller among those who only had elementary school education than among those with higher education ( = 0.021 and = 0.046). Those with a lower level of education also had a higher number of days/month with headache and with medication use than those with a higher educational level ( = 0.011 and = 0.018). The MOH-sufferers have limited contacts with health-care and preventive measures thus need to include other actors as well. Particular efforts should be directed towards those with low educational levels, and more research on medication use in relation to age is required.

    • Clothes and textiles were a 'store of value' and a medium of exchange during the early industrialisation in Sweden 1830-1900. The study of second-hand trade at auctions in the small Swedish town of Vasteras serves to problematize the meaning that second-hand trade had for different groups in society. This article presents results from cross-sectional studies of auction sales in Vasteras in 1837, 1860, 1885, and 1898. The results suggest that second-hand clothes retained their position as store of value until the turn of the 19th century. As late as the end of the 19th century there was a shortage of coins and paper money and few secure alternative to money. Despite major changes in the financial market and, from the mid-19th century, an industrial breakthrough in textile production, with falling relative prices and a shift to more breakable fabrics, clothes and textile remained important items at the auctions studied. However, there was a shift in the kind of objects that were traded in this market and in who used them as a store of value and a medium of exchange. Already in 1860, there was a shift from putting up textiles and clothes for sale to mainly selling clothes. In the last decades of the 19th century there was uncertainty about the value of second-hand goods. To compensate for falling average prices for clothes and textiles, people who continued to use auctions to sell clothes at the end of the 19th century managed to choose items that maximised credit or cash earned. Both men and women chose to sell men's clothes of high value. However, the loss of the store of value in second-hand clothes around 1900 might have hit women and less wealthy persons harder and made them more vulnerable, as they did not have the same access to financial institutions that better-off men had.

    • In less than 15 years, child baptism has gone from being a mainstream tradition to a minority practice. This decline is a result of both high unaffiliation, especially with the Church of Sweden, and a more diversified religious society due to migration. Using microdata from parents of children born in 2005 and 2015, we were able to discern that differences in the practice of child baptism in the Church of Sweden are positively associated with the parents' relation to the church, residence in rural areas, and income. Our LPM analysis shows that the probability of a child being baptized are mainly determined by the parents' relation to the church when controlling for all the other variables. The most influential factors are the mother's affiliation and an urban lifestyle. Parents' marital status and socioeconomic circumstances have a strong effect on the decision to baptize a child, therefore affecting who becomes a future member of the church.

    • The purpose of this study is to identify and discuss aspects of Nordic public service tv (NPSTV) research on what values (normative statements on what is at stake and what is considered worth protecting in relation to understandings of the public service remit) are stated as particularly relevant in distribution changes of teletext, digital tv and play services. The historiography shows a movement towards emphasising personalised media, bringing universal access to the many. It is universal service as a trust-worthy PSB service for the majority, not the minority, that protected. The ambitious intentions of NPSTV research to shed light on and not at least protect the Nordic, the European and the general aim of PSB, has motivated safeguarding of the fundamentals: access in the distribution and the financing. These intellectual pleasures in form of considerable anxieties concerning PSB are still predominant. Solidarity towards the few, or caring for under-privileged groups is downplayed. Protecting minority languages or minority issues are not sought after today, as was the case with teletext. Teletext in all Nordic countries included the value of being a special service for the hearing impaired. Research however did not mirror society, doing studies of teletext. DTV and play services are primarily studied as change regarding economic growth, keeping market shares and PSB being relevant in a digital media landscape. The nation-centric and media-centric PSB research approach seem to have become norm again through this development.This can be seen as research being actors of change, as well as being witnesses to society. It is argued that NPSTV research is both part of a slowly crystallising cultural practice of protecting PSB, and particularly rising to the occasion of triggering events: whatever technology that comes PSB will need to survive. Research show a particular interest in the legal, the structure of the industry organizations and the PSB institutions. However, PSB values like the minorities issue, and small Nordic countries, or the community building aspect of PSB is, as mentioned, lacking.What this study shows is the need to search for, see and acknowledge and also act on the gaps and distortions that exist. Why are there no strong critical reactions to NPSTV research excluding countries and minorities? Why is there no insight and directions towards how PSB should be striving for coherence of all publics and how coherence strategies mean not only one for all, but also many variances for differences as well as particular content aiming to create a sense of belonging and community?
