  • The preventive effect of a modified fluoride toothpaste technique on approximal caries in adults with high caries prevalence. A 2-year clinical trial.

    • The preventive effect of a modified fluoride (F) toothpaste technique was investigated. Both the incidence and progression of approximal caries among Saudi adults with a high caries prevalence were evaluated after 2 years. A total of 175 adults were randomly assigned to a test group and a control group and 106 completed the study. In the test group (n = 54), the patients were asked to use the provided F toothpaste twice a day. They were instructed to use the "modified F toothpaste technique" as follows: (1) to use 2 cm of the toothpaste, (2) to brush for 2 min, (3) to swish the toothpaste slurry around the dentition with active movements of the cheeks, lips and tongue, forcing the slurry into the approximal area for about half a minute, before spitting it out, and (4) no post-brushing rinsing and no eating/drinking for 2 hr. The patients in the control group (n = 52) were instructed to continue using their regular F toothpaste twice a day without any further instructions. Approximal caries and filled surfaces were scored on bitewing radiographs at baseline and after 2 years. The mean (SD) total caries incidence for the test and control group was 1.15 (1.49) and 3.37 (2.57) respectively (p < 0.001). In general, the control group displayed a higher progression rate than the test group with regard to enamel lesions to dentine (NS), enamel lesions to filled surfaces (p < 0.05) and filled surfaces that had recurrent caries (NS). To conclude, the "modified fluoride toothpaste technique", as practised over the 2 years in a Saudi population with a high caries prevalence, had a caries preventive effect on the incidence of approximal caries, but not on the progression.

  • Dental care utilization in a Swedish county in 1993 and 2003

    • The aim of the present study is to analyse factors influencing the patterns of utilization of dental care in an adult urban Swedish population aged between 30 and 80 in the years 1993 and 2003.This study is part of two large epidemiological dental studies of randomly selected Individuals, performed in 1993 and 2003. It comprises clinical and questionnaire data from 555 and 505 individuals in the two study years. In 1993, more patients visited the private dental care system more often than the public system. This difference still existed in 2003, but to a 50% lower extent. All age groups had less utilization of dental care in 2003 than in 1993. The largest change was seen in the 30-year age group in utilization patterns of dental care between the two study years. A hierarchical block regression method was performed in four steps, analysing utilization for the two study years separately. The dental service system influenced the utilization patterns significantly for both years. The health factors strengthened this main result. Irrespective of dental health, patients were predicted to visit the private dental ca re service system more often in both 1993 and 2OO3. The socio-economic differences between people in Sweden were of no consequence for dental care utilization. As to age and attitude towards treatment costs, a substantial change was observed and might reflect a change in social patterns with greater inequalities, but the overall picture of equal access in dental care in Sweden remains.

  • Salivary IgA in minor-gland saliva of children, adolescents, and young adults

    • Immunoglobulin A (IgA) is the principal immunoglobulin in human saliva and minor salivary glands contribute with approximately 30-35 % of the total salivary IgA in whole saliva. Salivary IgA is considered to be a vital component of the defence of the oral tissues and the concentration seems to increase with age. However, data on the salivary IgA-concentration in minor gland saliva of children, compared to adults, is not available. Objectives: To study the salivary IgA-concentration in minor gland saliva in defined mucosal areas, and in un-stimulated whole saliva, in children in different ages and young adults. Methods: 90 individuals were recruited: 3-year-old children (n = 30), 14-year-old children (n = 30), and young adults (n = 30). Minor gland saliva was collected on labial and buccal mucosa with filter papers and the volume was determined using a Periotron 8000. The whole saliva was collected by draining into a tube. Assessment of the salivary IgA-concentration was carried out in a sandwich ELISA. One-way ANOVA was used testing the differences between the age-groups (at the 0.05 level of significance). Results: The salivary IgA-concentration in labial gland saliva was significantly lower (p<0.05) in the 3-year-olds (3.7 mg/100 ml +/- 3.5) compared with the 14-year-olds (12.6 mg/100ml +/- 12.8) and the adults (12.8 mg/100ml +/- 13.4). In the un-stimulated whole saliva, a significantly lower salivary IgA-concentration was noted for the youngest group (9.0 mg/100ml +/-9.1) compared with the 14-year-olds (17.9 mg/100ml +/-14.9) and the adults (17.0 mg/100ml +/-9.9). Conclusion: The salivary IgA-concentration in labial and un-stimulated whole saliva seems to increase with age. This may reflect a developing specific immune response in saliva of young children.   • Parental attitudes and experiences of dental care in children and adolescents with ADHD - a questionnaire study

    • Attention deficit hyperactivity disorder (ADHD) is a common psychiatric condition characterized by age-inappropriate levels of inattention, hyperactivity-impulsiveness or a combination of these. The aim of this study was to analyze parental attitudes to and experience of dental care, oral hygiene and dietary habits in children/adolescents with ADHD. Twenty-six parents of 31 subjects, 20 boys and 11 girls, aged 5-19 years with ADHD registered at the Gothenburg Child Neuropsychiatric Clinic, were invited. The parents answered a questionnaire regarding different oral problems when visiting the Clinic of Pediatric Dentistry, Gothenburg, for an oral examination of their child. The parents felt the dental care at the Public Dental Service was good, but noted a lack of knowledge regarding child neuropsychiatry among the dental staff which may influence the dental treatment. Fifteen parents reported their children had experienced mouth pain and 15 reported their child had suffered from both discomfort and pain from local anesthesia. Thirteen of the children had a dental trauma and 12 parents reported pain in connection to the dental treatment. Pain related to filling therapy was stated by 11 parents. According to the parents, five children suffered from dental fear but 15 reported the child had a general fear. Pursuant to the parents, the beverage for dinner was mainly milk or water, while sweet drinks were more frequent when thirsty. Seventeen; parents reported their children had poor oral hygiene or could not manage to brush their teeth and 14 of the 31 children only brushed once a day or less. The results show that the parents experience a lack of child neuropsychiatric knowledge, care and patience from the dental staff, which may influence the treatment. Oral hygiene/tooth brushing is neglected and the frequent consumption of sugar is difficult for the parents to handle.

  • Clinical evaluation of local antibiotic treatment in smokers with chronic periodontitis in a maintenance care program

    • Syftet med den här studien var att undersöka om lokal antibiotika gel, 8,8 % Doxycyklin (Atridox®), kan förbättra läkningen av inflammerade tandköttsfickor hos vuxna rökande patienter under stödbehandling. Material och Metod I den här experimentellt blinda randomiserade studien valdes 20 rökande patienter (≥ 10 cigaretter/dag) med diagnos kronisk parodontit ut från ett parodontalt stödbehandlingsprogram. Varje patient hade ≥ 5 tänder, och ≥ 5 tandköttsfickor med fickdjup ≥ 5 millimeter. Patienterna indelades i två grupper, 10 i testgruppen (Doxycyklingel 8,8 %, Atridox®) och 10 i kontrollgruppen (Placebogel). Vid baseline och 3 månader efter behandling utfördes följande kliniska registreringar: fickdjup (PPD), klinisk fästenivå (CAL), blödning vid sondering (BOP), och plackindex (PLI) vid 4 ytor på alla indextänder. Mätning av mängden gingivalexudat (GI) utfördes vid 2 ytor på indextänder. Tandhygienist (KS) utförde scaling och debridering i hela bettet, även molarer (full-mouth). Tandläkare behandlade omgående patienterna i testgruppen med applicering av antibiotikagel Atridox® i minst 5 tandköttsfickor med primärt fickdjup ≥ 5 millimeter och blödning vid sondering. Patienterna i kontrollgruppen fick placebogel applicerad på motsvarande sätt. Efter applikationen av gel sköljde patienterna med 0,1 % klorhexidin i 7-10 dagar då mekanisk munhygien inte fick utföras under den perioden. Statistiska analyser Statistikprogram som användes var StatXact och Excel. Statistiska metoder Wilcoxon rank och Wilcoxon tecken rank test. P-värden ≤ 0.05 har bedömts som statistiskt signifikanta. Medelvärden över alla tänder/ytor har använts för varje patient och med patienten som statistisk enhet. Resultat När studien avslutades var det 16 patienter kvar. 4 patienter exkluderades ur studien på grund av sjukdom. Dessvärre visade det sig att bortfallet endast var i testgruppen som då bestod av 4 kvinnor och 2 män med genomsnittsålder 56,3 år. I kontrollgruppen ingick 5 kvinnor och 5 män med genomsnittsålder 59,4 år. Tre månader efter behandling noterades signifikant förbättring för testgruppen avseende PPD -1.36 mm (p=0.0313), signifikant förbättring för kontrollgruppen avseende PPD -1.07 mm (p=0.0020), CAL -0.65 mm (p=0.0200) och BoP (p=0.0078). Efter 3 månader fanns det ingen signifikant skillnad mellan test- och kontrollgrupp avseende samtliga parametrar. När resultaten av samtliga tänder (inklusive indextänder) analyserades noterades signifikant förbättring av PPD för både test- (p=0.0313) och kontrollgrupp (p=0.0039). Signifikant förbättring noterades även för kontrollgruppen av BOP, CAL och GI. Ingen signifikant skillnad mellan doxycyklin- respektive placebobehandlade bett avseende samtliga parametrar. Slutsats Resultatet av den här randomiserade kontrollerade studien kunde inte visa statistisk signifikant skillnad mellan test- och kontrollgrupp i det här begränsade materialet. Lokal antibiotikabehandling med 8,8 % Doxycyklingel (Atridox®) hade inte någon tilläggseffekt till scaling och debridering hos vuxna rökare med diagnos kronisk parodontit i ett stödbehandlingsprogram. Resultatet visade god läkning och signifikant förbättring av inflammerade tandköttsfickor efter subgingival scaling och debridering i både test- och kontrollgrupp utan någon skillnad mellan antibiotika- och placebogel.

  • Comparing patients with Apert and Crouzon syndromes--clinical features and cranio-maxillofacial surgical reconstruction.

    • Cranio-maxillofacial malformations, as seen in Crouzon and Apert syndromes, may impose an immense distress on both function and aesthetics of the person affected. The aims of this study were to describe and compare the main facial and intraoral features of patients with Apert and Crouzon syndromes, the clinical manifestations that may be present, additionally to the main syndromic traits, as well as the cranio-maxillofacial surgical treatment protocols followed.Twenty-three patients with Apert syndrome (6 males, 17 females), and 28 patients with Crouzon syndrome (20 males, 8 females) were evaluated for general medical aspects, craniofacial characteristics, dentoalveolar traits before and after the final orthognathic surgery, and types and timing of cranio-maxillofacial operations. Mental retardation, associated additional malformations, cleft palate, and extensive lateral palatal soft tissue swellings were more common in children with Apert syndrome. In both syndromes, clinical findings included concave profile, negative overjet, posterior crossbites, anterior openbite, and dental midline deviation, which were corrected in almost all cases with the final orthognathic surgery, with the exception of the lateral crossbites, including more than one tooth pair, which were persisting in about half of the cases. Cranial vault decompression and/or reshaping, midfacial and orbital advancement procedures, often in conjunction with a mandibular setback, were the most frequent cranio-maxillofacial operations performed. In conclusion, Apert syndrome is more asymmetric in nature and a more severe clinical entity than Crouzon syndrome. The syndromic dentofacial features of both conditions could be significantly improved after a series of surgical procedures in almost all cases with the exception of the posterior crossbites, with haIf of them persisting post-surgically.

  • Dental agenesis patterns in Crouzon syndrome

    • Dental agenesis may be present in an isolated familiar manner, or occur as a part of a syndrome.To date, this clinical trait seems to have been overlooked in patients with Crouzon syndrome.The aim of the present study was to investigate dental agenesis and dental agenesis patterns in a population of persons with Crouzon syndrome in Sweden. Serial panoramic radiographs of 26 individuals with Crouzon syndrome (20 males, 6 females) were examined.Third molars were excluded from the assessment. The prevalence of agenesis for at least one tooth was 42.3%. Each affected patient was found to have up to 5 missing teeth. Upper and lower second premolars were the most frequently congenitally missing teeth. Eleven dental agenesis patterns of the entire dentition were identified, as described by the tooth agenesis code (TAC). All patterns were unique and asymmetric,with only one exception, a symmetric pattern of the maxillary and mandibular second premolars. In conclusion, persons with Crouzon syndrome were found to have a high prevalence of dental agenesis and a remarkable variability of dental agenesis patterns. It is important to be aware of this clinical situation, especially when orthodontic treatment planning for these patients is performed as early as in the mixed dentition.

  • Management of tinnitus and jaw-muscle tenderness using an intraoral appliance and acupuncture.

    • Associations between signs and symptoms from the masticatory system and tinnitus have been reported. The aim of the study was to evaluate the effect of intraoral splint therapy and acupuncture on jaw-muscle tenderness and tinnitus. The study comprised 45 patients (24 men, 21 women; mean age 48 ±12 years) with long standing tinnitus (duration 6.5 ±5.9 years), referred from the audiology department at the University hospital in Örebro, Sweden. A complete audiological survey was performed before referral. Jaw muscles were palpated and the subjective tinnitus evaluated on a 100 mm scale (VAS) at baseline and after one year. All patients received stabilization (Michigan type) splints at start of treatment. After 6 months, non-responders (n=25) were subjected to acupuncture (6 sessions with duration of 30 minutes). Standard statistical methods were used. All patients had tender jaw muscles at palpation. Patients reported a significant decrease of the intensity of tinnitus during the observation period (from 78±20 mm to 52±24 mm after one year; P < 0.001). Only 6 (13 %) of the 45 patients did not report any improvement of their tinnitus. The number of jaw muscles tender to palpation also decreased significantly from 7.9±5.9 to 4.6±5.3; (P < 0.001). In conclusion, all 45 patients with tinnitus had tender jaw muscles. Intraoral splint therapy and acupuncture had a favorable effect on tinnitus and the jaw muscle symptoms. One year after the start of treatment, all but 6 of the 45 patients reported improvement of their tinnitus. Based on the results it is suggested that many tinnitus patients with jaw muscle tenderness can benefit from a treatment including intraoral splint and acupuncture.
