  • Two sides of breastfeeding support : experiences of women and midwives

    • Background: Midwives’ support of breastfeeding in maternity wards has been proven to provide an impact on women’s breastfeeding experiences. In previous studies women describe professional support unfavourably, with an emphasis on time pressures, lack of availability or guidance, promotion of unhelpful practices, and conflicting advice. Thus, the present study aims to investigate women’s experiences and reflections of receiving breastfeeding support and midwives’ experiences and reflections of giving breastfeeding support. Methods: This study was carried out in a county in southwestern Sweden during 2003-2004. A qualitative method, content analysis, was chosen for the study. The data came from interviews with women as well as interviews with midwives who were experienced in breastfeeding support. Results:  The women’s and midwives’ experiences and reflections of receiving and giving breastfeeding support were conceptualized as one main theme: “Individualized breastfeeding support increases confidence and satisfaction.” This theme contained three categories: “The unique woman,” ”The sensitive confirming process,” and “Consistency of ongoing support.” In order to feel confident in their new motherhood role, the women wanted more confirmation as unique individuals and as breastfeeding women; they wanted to be listened to; and they wanted more time, understanding, and follow-up from health professionals. In contrast, the midwives described themselves as encouraging and confirming of the women’s needs.  Conclusions:  If health care professionals responded to the woman’s unique needs, the woman felt that the breastfeeding support was good and was based on her as an individual, otherwise a feeling of uncertainty emerged. The midwives, however, expressed that they gave the women individual support, but they also expressed that the support came from different points of view, because the midwives interpreted women’s signals differently.

    • In higher education for professions, there still seems to be an inherent problem regarding how to integrate academic studies with practical training. There are reasons to highlight the problems for students studying to be engineers, teachers, and nurses, for example. This study aims to describe and explain, from a content perspective, how students understand their own studies where Work Integrated Learning is a part of the education, by using qualitative theory and methodology with data from questionnaires and interviews. The conclusion is that higher education, in such meaning. is an arena where best practice meets scientific argumentation, enabling some students to understand their studies from a dichotomized point of view. On the other hand, we also find that students' can develop into skilled professionals by moving beyond the current dichotomization discourse. The fruitful attachment of practical training demonstrates the idea of multiple perspectives and how education inspired by Worked Integrated Learning could constitute an arena in which situations are reflected on and experienced. It is therefore relevant to discuss the issue within an expanding zone of educational content that develops practical skills through concretization, application, identification, and students adopting a critical approach.     • Effective implementation of the new product development (NPD) process not only enables management to coordinate the efficient flow of new products, but will also assist to support ramp-up of various supply chain activities and other related activities supporting the commercialisation of the product. Thus, companies need to address all these issues in parallel to be successful and this requires some kind of integrative product development approach. The purpose of this research is to increase the understanding of how NPD is connected to supply chain management (SCM) by investigating how the NPD process is structured and executed in two international manufacturing companies seeking to realise an innovative, predictable and efficient product development. Several essential linkages between NPD and SCM have been derived from case study findings concerning important issues for successful product development.

    • Most highly accurate predictive modeling techniques produce opaque models. When comprehensible models are required, rule extraction is sometimes used to generate a transparent model, based on the opaque. Naturally, the extracted model should be as similar as possible to the opaque. This criterion, called fidelity, is therefore a key part of the optimization function in most rule extracting algorithms. To the best of our knowledge, all existing rule extraction algorithms targeting fidelity use 0/1 fidelity, i.e., maximize the number of identical classifications. In this paper, we suggest and evaluate a rule extraction algorithm utilizing a more informed fidelity criterion. More specifically, the novel algorithm, which is based on genetic programming, minimizes the difference in probability estimates between the extracted and the opaque models, by using the generalized Brier score as fitness function. Experimental results from 26 UCI data sets show that the suggested algorithm obtained considerably higher accuracy and significantly better AUC than both the exact same rule extraction algorithm maximizing 0/1 fidelity, and the standard tree inducer J48. Somewhat surprisingly, rule extraction using the more informed fidelity metric normally resulted in less complex models, making sure that the improved predictive performance was not achieved on the expense of comprehensibility.     • When predictive modeling requires comprehensible models, most data miners will use specialized techniques producing rule sets or decision trees. This study, however, shows that genetically evolved decision trees may very well outperform the more specialized techniques. The proposed approach evolves a number of decision trees and then uses one of several suggested selection strategies to pick one specific tree from that pool. The inherent inconsistency of evolution makes it possible to evolve each tree using all data, and still obtain somewhat different models. The main idea is to use these quite accurate and slightly diverse trees to form an imaginary ensemble, which is then used as a guide when selecting one specific tree. Simply put, the tree classifying the largest number of instances identically to the ensemble is chosen. In the experimentation, using 25 UCI data sets, two selection strategies obtained significantly higher accuracy than the standard rule inducer J48.

    • The globalization movement in recent decades has meant rapid growth in trade, financial transactions, and cross-country ownership of economic assets. In this article, we examine how the globalization of national business systems has influenced the framing of corporate social responsibility (CSR). This is done using text analysis of CEO letters appearing in the annual reports of 15 major corporations in Sweden during a period of transformational change. The results show that the discourse about CSR in the annual reports has changed from a national and communitarian view of social responsibility (cf. a negotiated view of CSR) toward an international and individualistic view of social responsibility (cf. a self-regulating view of CSR). The article contributes theoretically (1) by adding a national–global dimension to previous conceptualizations of CSR and (2) by showing that the rise of CSR discourse and activities in the last 10 years does not have to imply an increased commitment and interest in corporate responsibility per se, only that there are increased societal expectations that corporations should develop the capability to act more independently as moral agents.

    • Background: Becoming a parent is overwhelming for most men  and  women  and  alters  the  sexual  relationship  for many couples. Aim: To describe fathers’ experience about sexual life after childbirth within the first 6 months after childbirth. Method: A descriptive design, using content analysis with a qualitative approach, based on focus group discussions and one-to-one interviews. Participants: Eight  first-time  and  two  subsequent  fathers participated. Results: Three subthemes were identified: Struggling between stereotypes  and  personal  perceptions  of  male  sexuality  during transition to fatherhood; new frames for negotiating sex; a need to feel  safe  and  at  ease  in  the  new  family  situation. The over-arching  theme  emerged  as  ‘transition  to  fatherhood  brings sexual life to a crossroads’ and guided us to a deeper understanding  of  the  difficulties  men  experience  during  the transition  to  fatherhood.  To  get  sexual  life  working,  a number  of  issues  had  to  be  resolved,  such  as  getting involved in the care of the baby and the household and getting  in  tune  with  their  partners  in  regard  to  sexual desire. The men needed to be reassured and prepared for this new situation by health care professionals. Conclusions: New fathers in our study put the baby in focus in early parenthood and were prepared to postpone sex until  both  parties  were  ready,  although  they  needed reassurance to feel at ease with the new family situation. The fathers’ perceptions of sexual life extended to include all kinds of closeness and touching, and it deviated from the stereotype of male sexuality. This is important information for health care providers and midwives to be aware of for their encounters with men (and women) during the transition  to  fatherhood,  and  parenthood  and  can  contribute  to  caring  science  with  a  gender  perspective  on adjustment of sexual life after childbirth.
