Lise-lotte.edwall@epost.tidanet.se AIM: The aim was to elucidate the lived experience of regular diabetes nurse specialist check-ups among patients with type 2 diabetes. BACKGROUND: Diabetes care with diabetes nurse-led clinics in primary care has been established in Sweden since the 1980s. Information about patients' lived experience of these regular check-ups is important in the further development of diabetes nursing in primary care. METHODS: Narrative interviews were conducted with 10 women and 10 men with type 2 diabetes regarding their lived experience in everyday life and the regular check-ups by the diabetes nurse specialist. A phenomenological-hermeneutic method inspired by Ricoeur was used in the analysis. RESULTS: Patients'- with type 2 diabetes - lived experience of regular check-ups showed an overall positive influence on the patients' way of living with the disease by underpinning and developing their understanding and management of daily life. This influence was interpreted as an interlinked chain comprising the following elements; being confirmed, being guided within the disease process, becoming confident and independent and being relieved. Every link contained a positive influence from being dependent-independent, autonomous and a feeling of freedom in patients striving for an everyday life with a disease under control. RELEVANCE TO CLINICAL PRACTICE: The development of diabetes-nurse-led clinics from the perspective of the patient must consider the patient's individual need for support and continuity with the diabetes nurse specialist. Consultations with collaborative partnership and specific expertise that is easy to access implied autonomous disease self-management in terms of patients' own control of the disease over time. In view of the increasing number of patients with diabetes, this could positively impact the availability of diabetes nursing care.

    • Police authorities in a number of countries have recently introduced body-worn cameras (BWC). With the use of body-worn cameras, the police have gained access to new forms of wearable and powerful law enforcing technologies. The cameras enable collection of large volume of personal information and in some cases even sensitive information that must be managed and stored within the organisation in line with rules of law. As is often the case when technology develops faster than societal norms and values, a range of questions concerning issues related to regulation of these practises are still uninvestigated. Therefore, this paper will analyse what actually regulates individual police officers’ body-worn camera practice. Empirically, we use the Swedish police as a case and our study is based on qualitative interviews. Theoretically we draw upon Lawrence Les-sig’s four modality model - law, norms, market, and architecture - and we conclude that i) law is considered important although law regarding BWC is still in its infancy, ii) while law and official directives have a more macro applicability, norms are developed and maintained more locally, iii) market regulate indirectly via availability and cost, and iv) architecture is not necessarily as self-executed as often stated.

Det lärandeklimat som finns på arbetsplatsen är ofta avgörande för att åstadkomma ett långsiktigt utvecklingsarbete med positiva effekter. Det är mycket som pekar på att ett hållbart utvecklingsarbete handlar om att komma in i en positiv lärandespiral. Genom att avsätta tid för att uppriktigt och förutsättningslöst försöka förbättra hur arbetet leds och organiseras så skapas också ett engagemang hos medarbetare för att delta i förbättringsarbetet. Detta i sin tur medför olika positiva resultat för både individen och organisationen. Avsikten med den här handboken är att stödja arbetsplatser i att utveckla en gynnsam lärandespiral.   • Hopelessness among adults with congenital heart disease : Cause for despair or hope?

    • BackgroundAdults with congenital heart disease (CHD) face unique life courses and challenges that may negatively influence their psychological functioning. The aims of this study were to (1) examine the level of hopelessness among adults with CHD in comparison with non-CHD participants and (2) identify correlates of elevated hopelessness among adults with CHD.MethodsWe enrolled 347 patients with CHD (18–64 years, 52.2% female) and 353 matched (by sex/age) non-CHD persons in this cross-sectional study. Hopelessness was assessed by Beck Hopelessness Scale. Hierarchical multiple logistic regression analyses were performed to explore correlates of elevated hopelessness.ResultsThe mean total hopelessness score did not significantly differ between the CHD and non-CHD groups. Twenty-eight percent of CHD patients had elevated hopelessness scores. Within the CHD patient sample, regression analyses revealed that being male (odds ratio = 2.62), not having children (odds ratio = 3.57), being unemployed (odds ratio = 2.27), and elevated depressive symptoms (odds ratio = 1.21) were significantly associated with hopelessness. Regular physical activity (odds ratio = 0.36) emerged as a protective factor and all CHD disease parameters were unrelated to hopelessness. The final model explained 43% of the variance in hopelessness.ConclusionsAdult CHD teams are encouraged to continue to explore strategies to support patients to live as rich and full as lives as possible by pursuing relationships, employment and physical activity, as well as managing depression and hopelessness.

    • This article explores the experiences of reciprocity and conflicts in social relationships, and how these experiences are related to financial support (loans and gifts) in the wider social network. The balance of exchange within the social network as a whole is considered in relation to socio-demographic features, financial conditions and the availability of social relationships. The data are drawn from a study of financial support exchange in Sweden based on a representative survey among Swedish citizens (N=500). The results reveal that people living in economic hardship and who have more problematic life circumstances appear to be at risk of receiving more limited informal financial support by experiencing more feelings of conflicts and nonreciprocal social relations.

  • For better or for worse? : Transformation of social network relations due to long-term illness

    • This paper explores whether, and if so how, long-term illness affects and changes personal social relations and how these transformations are related to diagnosis, length of the sick leave, age and gender. It is based on qualitative in-depth interviews (N = 10) and a broader representative survey among long-term ill people (n = 319). The results reveal that illness affects the social relationships in both positive and negative directions. On the one hand, the proximity and mobilization of support from family and friends increases during the period of illness and new relationships are included in the networks. On the other hand, the distance within some relationships increases, and some disappear or become less support-rewarding. While the need for social relationships seems, for the majority, to be essential and constant, the results show that the circle of close personal relationships is shifting and that emotional intensity and reciprocal exchange vary.
