Entrustable professional activities (EPAs) for undergraduate medical education : development and exploration of social validity

- Background: The development of entrustable professional activities (EPAs) as a framework for work-based training and assessment in undergraduate medical education has become popular. EPAs are defined as units of a professional activity requiring adequate knowledge, skills, and attitudes, with a recognized output of professional labor, independently executable within a time frame, observable and measurable in its process and outcome, and reflecting one or more competencies. Before a new framework is implemented in a specific context, it is valuable to explore social validity, that is, the acceptability by relevant stakeholders.Aim: The aim of our work was to define Core EPAs for undergraduate medical education and further explore the social validity of the constructs.Method and material: In a nationwide collaboration, EPAs were developed using a modified Delphi procedure and validated according to EQual by a group consisting of teachers nominated from each of the seven Swedish medical schools, two student representatives, and an educational developer (n = 16). In the next step, social validity was explored in a nationwide survey. The survey introduced the suggested EPAs. For each EPA, the importance of the EPA was rated, as was the rater’s perception of the present graduates’ required level of supervision when performing the activity. Free-text comments were also included and analyzed.Results: Ten Core EPAs were defined and validated. The validation scores for EQual ranged from 4.1 to 4.9. The nationwide survey had 473 responders. All activities were rated as “important” by most responders, ranging from 54 to 96%. When asked how independent current graduates were in performing the ten activities, 6 to 35% reported “independent”. The three themes of the free text comments were: ‘relevant target areas and content’; ‘definition of the activities’; and ‘clinical practice and learning’.Conclusion: Ten Core EPAs were defined and assessed as relevant for Swedish undergraduate medical education. There was a consistent gap between the perceived importance and the certainty that the students could perform these professional activities independently at the time of graduation. These results indicate that the ten EPAs may have a role in undergraduate education by creating clarity for all stakeholders.

- Purpose Since 2006, higher education institutions (HEIs) in Sweden, should according to the Higher Education Act, promote sustainable development (SD). In 2016, the Swedish Government asked the Swedish higher education authority to evaluate how this study is proceeding. The authority chose to focus on education. This paper aims to produce a report on this evaluation. Design/methodology/approach All 47 HEIs in Sweden were asked to write a self-evaluation report based on certain evaluation criteria. A panel was appointed consisting of academics and representatives for students and working life. The panel wrote an evaluation of each HEI, a report on general findings and recommendations, and gave an overall judgement of each HEI in two classes as follows: the HEI has well-developed processes for integration of SD in education or the HEI needs to develop their processes. Findings Overall, a mixed picture developed. Most HEIs could give examples of programmes or courses where SD was integrated. However, less than half of the HEIs had overarching goals for integration of SD in education or had a systematic follow-up of these goals. Even fewer worked specifically with pedagogy and didactics, teaching and learning methods and environments, sustainability competences or other characters of education for SD. Overall, only 12 out of 47 got a higher judgement. Originality/value This is a unique study in which all HEIs in a country are evaluated. This provides unique possibilities for identifying success factors and barriers. The importance of the leadership of the HEIs became clear.

- In the first of two essays in this Journal, I seek to unify the historicalgeography of early modern ‘European expansion’ (Iberia and Latin America)with the environmental history of the ‘transition to capitalism’ (northwestern Europe). The expansion of Europe’s overseas empires and the transitions to capitalism within Europe were differentiated moments within the geographicalexpansion of commodity production and exchange – what I call the commodityfrontier. This essay is developed in two movements. Beginning with a conceptual and methodological recasting of the historical geography of the rise of capitalism,I offer an analytical narrative that follows the early modern diaspora of silver.This account follows the political ecology of silver production and trade from the Andes to Spain in Braudel’s ‘second’ sixteenth century (c. 1545–1648). In highlighting the Ibero-American moment of this process in the present essay, Icontend that the spectacular reorganization of Andean space and the progressive dilapidation of Spain’s real economy not only signified the rise and demise of a trans-Atlantic, Iberian ecological regime, but also generated the historicallynecessary conditions for the unprecedented concentration of accumulation andcommodity production in the capitalist North Atlantic in the centuries thatfollowed.

Review and assessment of self-reports of travel-related emotional wellbeing

- Introduction: Travel behavior research has only started to address how travel affects emotional wellbeing. The development of measurement methods is an important goal of this research.Methods: A review and assessment of methods of measuring travel-related emotional wellbeing is presented guided by a conceptual framework specifying what is measured (cognitive evaluations, emotional responses, or moods), the way it is measured (proactively, instantaneously, or retrospectively), and when it is measured (before, during, or after travel). Anticipated, current, residual and recalled moods are the objects of the measurement. Only studies of commuting or other types of daily travel are addressed.Results: We find that no research has measured anticipated moods, some research has measured current moods before, after and during travel, and most research has measured recalled moods.Conclusions: The most valid and reliable method is to measure current mood instantaneously at several points in time, before, during, and after travel. A measure of emotional wellbeing can then be obtained by objective aggregation. An approximate more feasible method is to retrospectively measure recalled moods for a given specified time period that may not only include travel. The available methods for measuring recalled moods have acceptable psychometric properties but research is needed to validate these methods by comparing the results to an objective aggregation of instantaneous measures of current mood at different points in time.

Vilka är ute i naturen? Delresultat från en nationell enkät om friluftsliv och naturturism i Sverige

- Detta är en av fyra rapporter som ger en översiktlig bild av resultaten från en nationell enkätundersökning om friluftsliv och naturturism i Sverige. Studien är genomförd av forskningsprogrammet Friluftsliv i förändring. Resultaten som presenteras här fokuserar bl a på vilka som är ute i naturen och vilka aktiviteter man ägnar sig åt. Nästan alla uppfattar skogar, fjäll, sjöar och hav som ”natur” men det är inte alls lika självklart med t.ex. badstränder, trädgårdar och elljusspår. Mer än hälften av de svarande är ute i naturen ganska ofta, eller mycket ofta, under vardagar. Under längre ledigheter ökar denna andel till 89 %. Naturen är också en central miljö för barns uppväxt och t.ex. har hela 94 % av de svarande ofta, eller mycket ofta, tillbringat lov och semestrar i naturen. Nöjes- och motionspromenader är vanligast att ha ägnat sig åt åtminstone en gång under en 12- månadersperiod (nästan 95 % av de svarande), följt av strövat i skog och mark (90 %), arbetat i trädgården (84 %), solbadat (81 %), och haft picknick eller grillat i naturen (80 %). Även om vi ser till de aktiviteter som är vanligast att ägna sig åt ofta (mer än 60 gånger per år) så toppas listan av nöjes- och motionspromenader (ca 32 %), följt av cykla på vägar, arbeta i trädgården, ströva i skog och mark och promenera med hund. Vi har också gjort en fördjupad analys när det gäller de tio vanligaste aktiviteterna som de svarande angett att de utövat mer än 20 gånger under en 12-månadersperiod (ca 1-2 gånger per månad). Det visar sig då att kvinnorna generellt är mer aktiva. Jogging/terränglöpning, jakt och fiske är mer vanligt bland män medan solbad, picknick och stavgång är mer representerat hos kvinnor. Gruppen under 30 år skiljer ut sig något genom att promenera med hund är vanligare och arbete i trädgården mindre vanligt, jämfört med övriga grupper. Joggning/terränglöpning tycks också vara en aktivitet främst hos yngre och medelålders, medan det omvända gäller för stavgång. Föräldrarnas uppväxtland tycks inte ha så stor betydelse för vilka aktiviteter man väljer, men svarande med svenska föräldrar utövar aktiviteterna i större utsträckning jämfört med personer vars föräldrar vuxit upp utomlands. Ett undantag finns emellertid för picknick eller att grilla i naturen vilket är vanligare hos svarande med utländska föräldrar.

- With schools and universities closing across Europe, the Covid-19 lockdown left actors in the field of education battling with the unprecedented challenge of finding a meaningful way to keep the wheels of education turning online. The sudden need for digital solutions across the field of education resulted in the emergence of a variety of digital networks and collaborative online platforms. In this joint article from scholars around Europe, we explore the Covid-19 lockdowns of physical education across the European region, and the different processes of emergency digitalization that followed in their wake. Spanning perspectives from Italy, Germany, Belgium, and the Nordic countries, the article’s five cases provide a glimpse of how these processes have at the same time accelerated and consolidated the involvement of various commercial and non-commercial actors in public education infrastructures. By gathering documentation, registering dynamics, and making intimations of the crisis as it unfolded, the aim of the joint paper is to provide an opportunity for considering the implications of these accelerations and consolidations for the heterogeneous futures of European education.

- Recipes are not only part of today's cooking culture, they are also part of the Swedish syllabus of home economics. The aim of this study was to investigate what kinds of difficulties students with mild intellectual disabilities have using recipes during cooking lessons in home economics. We conducted an ethnographically inspired approach, with a total of 44 h of accompanying observations. Three compulsory schools for students with intellectual disabilities were enrolled in the study, and 37 students and three teachers were included. The socio-cultural theory of learning has been used as a theoretical framework. The findings reveal both that recipes are central artefacts during the cooking lessons and that the students have various difficulties using the recipes. The difficulties vary, and they concern both how the recipes are designed and the purport of the recipes. Difficulties in relation to the design included, for example, the separation of ingredients and instructions in the text and the large amount of information given in both the whole and the parts of the recipes. The difficulties in relation to the purport – that is, the meaning or sense of the recipe – were the ingredients, the kitchen utensils and the knowledge of how to perform a specific task. These difficulties can be considered special in relation to the use of the recipes. We suggest the concept of ‘recipe literacy’ to capture the complex knowledge of using recipes.

Well-Being of Early-Career Researchers: Insights from a Swedish Survey

- Several studies have documented the importance of optimal work situation and the general well-being of early-career researchers (ECRs) for enhancing the academic performance of universities. Yet, most studies focused on specific categories of ECRs, or on specific academic disciplines as well as on specific outcomes. With this study, we recognize the need for a broader sample encompassing different categories of ECRs across academic disciplines. In a national survey of Swedish universities, the National Junior Faculty of Sweden (NJF) collected data from ECRs in order to study the influence of work situation and well-being on perceived scientific environment. We observed that work situation and well-being are interdependent and jointly influence each other in shaping the conditions for ideal scientific environment. Importantly, we employ structural equation model (SEM) analysis to account for the endogenous relationship between work situation and personal well-being in predicting perceived scientific environment. Results from SEM indicate that support from the university, work time management, job clarity, contract length and quality of life satisfaction were related to the perceivedpossibility of conducting the best science. Our research also highlighted individual differences across demographic factors and contract length in the perceived work situation and the possibility of conducting the best science.
