  • On Oral Health-Related Quality of Life in Swedish Young Adults
  • 2015
  • Doktorsavhandling (övrigt vetenskapligt/konstnärligt)abstract
    • Young adults in Sweden have grown up with dental care free of charge until the age of twenty. Their self-perceived oral health has been reported as being good, but rapid changes in society have led to a weaker economic situation for many young adults, which may influence their dental attendance and priorities concerning oral health and oral health care. The overall aim of this thesis was to explore the views of young adults on dental care, oral health and Oral Health-Related Quality of Life (OHRQoL).The thesis is based on four scientific papers which all apply a qualitative approach. In Paper I, the views of young adults on dental care were explored. Paper II investigated the views of young adults on their oral health and OHRQoL. In Paper III, measures of OHRQoL were described and analysed from a public health perspective, and in Paper IV, the views of young adults on the relevance of three measures of OHRQoL were explored. In Paper I, II, and IV, data collection was performed through qualitative interviews. The selection of informants was strategic with reference to age (21-29 years), sex and education. For Paper III, a literature search for OHRQoL measures was made in the PubMed database. The data in Paper I was analysed in accordance with the constant comparative method (inspired by Grounded Theory), and in Papers II, III and IV, qualitative content analysis was used. The results showed that young adults were satisfied with the dental care that they had received but reported specific views and demands on dental care (Paper I). They perceived their oral health as good, but an array of oral health problems was described (Paper II). The young adults’ perceived control of their OHRQoL depended on their future prospects of oral health, in relation to their perceptions of their past and present oral health. In Paper III, the search for measures of OHRQoL in the PubMed database generated 22 measures. The measures were analysed with regard to their theoretical framework and in relation to four principles of health promotion. Some elements of public health principles were found in all the measures, but most of them originated in disease-oriented theories. The occurrence of oral problems was reflected in young adults’ views on the measures of OHRQoL (Paper IV). The analysed measures were deemed to have both advantages and disadvantages but to be fairly equal. The conclusions are that young adults’ OHRQoL was dependent on their earlier experiences of dental care and their former and present oral health, as well as their future prospects regarding oral health. Elements of public health principles were present to a varying degree in all the measures of OHRQoL. Young adults regarded the frequently used measures of OHRQoL as being equal. The measures were mainly disease-oriented and no specific measures had been developed for young adults.

  • Oral health-related quality of life in Swedish young adults
  • 2015
  • Ingår i: International Journal of Qualitative Studies on Health and Well-Being. - Järfälla : Informa UK Limited. - 1748-2623 .- 1748-2631. ; 10:1
  • Tidskriftsartikel (refereegranskat)abstract
    • The living conditions of young adults in Sweden have changed during the last decades due to the economic and employment situation in society. Although oral health is mainly considered to be good in this age group, their use of dental care has decreased and their priorities and opportunities regarding oral health are little known. The purpose of this study was to describe the views of Swedish young adults on their oral health and oral health-related quality of life (OHRQoL). The design of the study was qualitative, using content analysis. Sixteen young adults, aged 21-29 years, were interviewed. The findings from the interviews were summarized under the theme "Young adults reflected on their OHRQoL in a time perspective'' consisting of three categories: "Past experiences, Present situation, and Future prospects.'' The OHRQoL of young adults is dependent not only on their own experiences of oral health during childhood and their received dental care but also on their present self-perceived oral health, oral health habits, and social life; together with their expectations of future oral health. The findings in this study indicate that the oral health awareness and needs of young adults, as well as their expectations of oral care, merit further follow-up.

  • Young adults' views on the relevance of three measures for oral health-related quality of life
  • 2015
  • Ingår i: International Journal of Dental Hygiene. - Malden, MA : John Wiley & Sons. - 1601-5029 .- 1601-5037. ; 13:3, s. 184-191
  • Tidskriftsartikel (refereegranskat)abstract
    • OBJECTIVE: The aim of this study was to explore the views of young adults on the relevance of three measures of oral health-related quality of life (OHRQoL). METHODS: Sixteen young adults aged 21-29 years were interviewed. The selection was strategic with reference to age (21-25 years.; 26-30 years), sex and education (university degree; upper secondary school). The interview guide covered areas on the content and construction of the measures: The Oral Health Impact Profile (OHIP), the Oral Impacts on Daily Performances (OIDP) and the Oral Health-Related Quality of Life UK (OHRQoL-UK). The data were analysed using qualitative content analysis. RESULTS: A theme expressing the latent content was formulated during the data analysis: 'young adults' own experiences were reflected in their views on the OHRQoL measures'; that is, the experiences of young adults of own oral problems and aspects that were found to be especially important for their age group influenced their view on the measures. The self-reported ability to understand and answer the questions varied and the perceived advantages and disadvantages were almost equally distributed among the three measures. CONCLUSIONS: The OHIP, OIDP and OHRQoL-UK were evaluated as being equal by the young adults in this study, with regard both to their pros and cons. The clarity of the measures was regarded as the most important factor, while the length and assessment period were less significant.

  • The experience of working on a locked acute psychiatric ward
  • 2013
  • Ingår i: Journal of Psychiatric and Mental Health Nursing. - Chichester : Wiley. - 1351-0126 .- 1365-2850. ; 20:4, s. 321-329
  • Tidskriftsartikel (refereegranskat)abstract
    • This study's aim was to elucidate health-care staff experience of working on a locked acute psychiatric ward. In many countries changes in health care has contributed to fewer beds available in inpatient care, and a concentration of patients with severe psychiatric conditions. This implies a changing work environment in acute psychiatric care. Qualitative interviews with health-care staff (n= 10) were carried out on a ward for patients with affective disorder and eating disorder in a Swedish hospital. Qualitative content analysis was used. Four themes were identified from the data: 'undergoing changes in care delivery', 'feeling a need for security and control', 'managing the demands at work' and 'feeling a sense of responsibility'. This study adds to earlier research into how a sense of responsibility can place a significant burden on health-care staff working on a locked psychiatric ward and also contribute to increased control of patients. This study also shows that relationships and power structures among health-care staff need to be addressed when organizational changes are made in care delivery. Further research is needed to reach a comprehensive understanding of care on locked acute psychiatric wards, including a development of nursing and medicine as knowledge domains in one common context. © 2012 Blackwell Publishing.

  • The health-care environment on a locked psychiatric ward: an ethnographic study
  • 2006
  • Ingår i: International Journal of Mental Health Nursing. - Richmond : Wiley-Blackwell. - 1445-8330 .- 1447-0349. ; 15:4, s. 242-250
  • Tidskriftsartikel (refereegranskat)abstract
    • Recent changes in psychiatric hospital care involving a reduction in the number of beds and time spent in hospital motivated the study of conditions of inpatient care on such wards. An ethnographic study of a locked, acute psychiatric ward in a department of psychiatry was performed with the aim of describing the health-care environment in such a ward. The ward admitted patients on both a voluntarily and involuntarily basis. Data were collected by means of 3.5 months of participant observations. The results showed a health-care environment that was overshadowed by control. Staff were in control but they also lacked control; they attempted to master the situation in line with organizational demands and they sometimes failed. At the same time, the staff tried to share the responsibility of caring for patients and next of kin. Patients were controlled by staff; they were the underdogs and dependent on staff for their care and the freedom to leave the ward. Patients tried to make themselves heard and reacted to the control by developing counter-strategies. What this study adds to earlier research is patients' pressure on staff and sometimes quite an open struggle for more control, which may be an expression for an unacceptable imbalance in power between patients and staff.

  • The meaning of care on a locked acute psychiatric ward : Patients' experiences
  • 2009
  • Ingår i: Nordic Journal of Psychiatry. - London : Informa Healthcare. - 0803-9488 .- 1502-4725. ; 63:6, s. 501-507
  • Tidskriftsartikel (refereegranskat)abstract
    • BACKGROUND: The deinstitutionalization of psychiatric care has led to a concentration of patients to fewer wards. This leads to difficulties in separating voluntarily and involuntarily admitted patients, and there is a risk that the interest in safeguarding patients' autonomy will decrease. AIMS: The aim of this study was to elucidate the meaning care has to patients on a locked acute psychiatric ward. The study was performed on a ward for patients with affective and eating disorders who were admitted both voluntarily and involuntarily. METHODS: Interviews were conducted and analysed using qualitative content analysis. RESULTS: The results show that to the patients, care had contradictory qualities some alleviated their suffering while others contributed to stress. The alleviation from suffering originated from the experiences of strengthened integrity and self-determination, from being supported, and having a place of refuge on the ward. In contrast to this, there were experiences of being dependent and trapped because of lack of influence and choice, and being controlled. CONCLUSIONS: In conclusion, this qualitative study shows that to the patients, care can mean being disciplined in a way that risks undermining the alleviation of suffering that care can provide. It is thus important to be observant when it comes to manifestations of control in care. The study also shows that the support of fellow patients has a value as a complement to the care given by staff members and next of kin, something that needs to be further investigated.

  • LEADER and rural development policy - What's the problem represented to be?
  • 2024
  • Ingår i: Journal of Rural Studies. - Oxford : Elsevier. - 0743-0167 .- 1873-1392. ; 108
  • Tidskriftsartikel (refereegranskat)abstract
    • This article problematizes the EU Programme LEADER as a policy to improve opportunities to live and thrive in rural areas. LEADER was initiated in 1991 as a Commission initiative and has, over time, expanded and become an institutionalized public policy in the EU. LEADER is based on a particular development method - the LEADER method - which sets out fundamental principles for conducting development work. The idea is that rural development should start with Local Action Groups at the grassroots level, emphasizing networking and entrepreneurship within a geographic context. Drawing on Bacchi's methodology, What's the problem represented to be, the article aims to examine the LEADER Programme and analyze problem representations and political rationalities. The analysis shows that actors working within LEADER have developed political rationalities that partly constitute obstacles to achieving long-term impacts. It concerns issues related to the bureaucratic framework of LEADER and representations on a fixation on producing added values, an internal logic of project administration, and an emphasis highlighting the role of the individual entrepreneur. The LEADER Programme has not only been institutionalized as part of the EU and member states' administrative systems. Still, it has also created its ideological superstructure consisting of a specific narrative, methodology, language, and common forms of organizing. This gives LEADER advantages but also risks associated with internal closedness that creates subjectification between ‘them and us' and produces lived effects that hinder structural changes promoting rural areas.
