What healthcare teams find ethically difficult : Captured in 70 moral case deliberations

- Background: Ethically difficult situations are frequently encountered by healthcare professionals. Moral case deliberation is one form of clinical ethics support, which has the goal to support staff to manage ethical difficulties. However, little is known which difficult situations healthcare teams need to discuss.Aim: To explore which kinds of ethically difficult situations interprofessional healthcare teams raise during moral case deliberation.Research design: A series of 70 moral case deliberation sessions were audio-recorded in 10 Swedish workplaces. A descriptive, qualitative approach was applied, using thematic content analysis.Ethical considerations: An advisory statement specifying no objections to the study was provided from an Ethical Review Board, and consent to be recorded was assumed by virtue of participation in the moral case deliberation.Findings: Three themes emerged: powerlessness over managing difficult interactions with patients and next-of-kin, unease over unsafe and unequal care, and uncertainty over who should have power over care decisions. The powerlessness comprised feelings of insufficiency, difficulties to respond or manage patient's/next-of-kin's emotional needs or emotional outbursts and discouragement over motivating patients not taking responsibility for themselves. They could be uncertain over the patient's autonomy, who should have power over life and death, disclosing the truth or how much power next-of-kin should have.Discussion: The findings suggest that the nature of the ethically difficult situations brought to moral case deliberations contained more relational-oriented ethics than principle-based ethics, were permeated by emotions and the uncertainties were pervaded by power aspects between stakeholders.Conclusion: MCD can be useful in understanding the connection between ethical issues and emotions from a team perspective.

- This paper focuses on students in need of special education in mathematics (SEM students) and highlights teachers’ and principals’ reflections upon these students’ construction of knowledge in relation to two educational settings: the regular teaching setting and the test setting. The findings indicate that SEM students’ knowledge is legitimized only when displayed. However, there appear to be differences according to the specific setting. Different settings imply different knowledge representations, norms, and practices that need to be taken into account when reflecting, planning, and carrying out teaching in mathematics in relation to SEM.

- This paper is an exploration of how an educational method, Explicit Instruction (EI), is used and reflected upon in research in relation to special educational needs in mathematics (SEM) in early school years. The current research front is put in relation to the Swedish curricula in mathematics and the Swedish school act (steering documents) to explore potential possibilities and challenges of the research findings in a Swedish context. The analysis was done in three steps: 1) a systematic search of research literature, 2) a content analysis of the literature 3) a relation of the results of to the steering documents. The exploration of the possibilities of EI displays that EI can contribute to every student's learning by providing a distinct structure in relation to the mathematical content. It displays challenges for special education in mathematics regarding collaboration between teachers, the competence of teachers, and time to plan the EI and time for the students working with EI.

- Background. Surveillance is a central activity among mental health nursing, but it is also questioned for its therapeutic value and considered to be custodial. Aim. The aim of this study was to describe how mental health nurses use different approaches to observe patients in relation to the practice of surveillance in psychiatric nursing care. Methods. In this study, Spradley's twelve-step ethnographic method was used. Results. Mental health nurses use their cultural knowing to observe patients in psychiatric care in various ways. Two dichotomous approaches were identified: the latent and the manifest approach. Discussion. Different strategies and techniques for observing patients are structured along two dichotomies. The underlying relationships between these two different dichotomous positions transform the act of observing into surveillance. This is further developed in a theoretical model called the powerful scheme of observation and surveillance (PSOS).

- Mental health nurses use manifest and latent approaches for surveillance and observation of patients in the context of mental health care. Patient spaces in mental health organizations are subtly linked to these different means of surveillance. This article investigates these approaches, focusing in particular on the variety of spaces patients occupy and differences in the intensity of observation that can be carried out in them. The aim is to elaborate on space and surveillance in relation to the patients’ and nurses’ environment in psychiatric nursing care. Places where patients were observed were operationalized and categorized, yielding three spaces: those for patients, those for staff, and neutral areas. We demonstrate that different spaces produce different practices in relation to the exercise of panoptic power and that there is room for maneuvering and engaging in alternatives to “keeping an eye on patients” for nurses in mental health nursing. Some spaces offer asylum from panoptic observations and the viewing eyes of psychiatric nurses, but the majority of spaces in mental health nursing serve as a field of visibility within which the patient is constantly watched.

- Earlier research shows that breast augmentation is positively correlated with positive psychological states. The aim of this study was to explore the shared values, feelings, and thoughts within the culture of breast enlargement among women visiting Internet-based forums when considering and/or undergoing esthetic plastic surgery. The study used a netnographic method for gathering and analyzing data. The findings show that the women used the Internet forum to provide emotional support to other women. Through electronic postings, they cared for and nursed each others' anxiety and feelings throughout the whole process. Apart from the process, another central issue was that the women's relationships were frequently discussed; specifically their relationship to themselves, their environment, and with the surgeons. The findings suggest that Internet forums represent a channel through which posters can share values, feelings, and thoughts from the position of an agent of action as well as from a position as the object of action. These dual positions and the medium endow the women with a virtual nursing competence that would otherwise be unavailable. By introducing the concept of torrenting as a means of sharing important self-care information, the authors provide a concept that can be further explored in relation to post modern self-care strategies within contemporary nursing theories and practice.

Particle image velocimetry visualization and measurement of airflow over a wall-mounted radiator

- A common room-heating technique involves the use of a wall-mounted radiator without forced convection. The cold surrounding air passes adjacent to the warm surfaces of the radiator where it absorbs heat and gains momentum to rise along the wall surface and finally circulate in the entire room. Understanding the properties of heated airflows is important for several purposes. To understand the flow process it is important to identify where the transition from laminar to turbulent flow occurs and to quantify the turbulent fluctuations. With the objective to characterize the airflow in the vicinity of wall surfaces, the local climate over the radiator was visualized and measured using a two-dimensional particle image velocimetry technique. The PIV technique yields 2D vector fields of the flow. The resulting vector maps were properly validated and post-processed using in-house software to provide the average streamlines and other statistical information such as standard deviation, average velocity, and covariance of the entire vector field. The results show that, for a room with a typical heating power, the airflow over the radiator becomes agitated after an ordinate of N = 5 - 6.25 over the radiator upper level, in which N is the dimensionless length based on the thickness of the radiator. Practical problems encountered in near-wall PIV measurements include generating a homogeneous global seeding that makes it possible to study both plume and entraining region, as well as optical problems due to near-surface laser reflection that makes the measurement process more complicated.

- Background: Animal-Assisted Therapy using dogs have been described as having a calming effect, decrease sundowning and blood-pressure in persons with Alzheimer’s disease. The aim was to investigate how continuous and scheduled visits by a prescribed therapy dog affected daytime and night-time sleep for persons with Alzheimer’s disease.Methods: In this case study, registration of activity and sleep curves was conducted from five persons with moderate to severe Alzheimer’s disease living at a nursing home, over a period of 16 weeks using an Actiwatch. Data was analysed with descriptive statistics.Result: The study shows no clear pattern of effect on individual persons daytime activity and sleep when encounter with a therapy dog, but instead points to a great variety of possible different effects that brings an increased activity at different time points, for example during night-time sleep.Conclusions: Effects from the use of a Animal-Assisted Therapy with a dog in the care of persons with Alzheimer’s disease needs to be further investigated and analysed from a personcentred view including both daytime and nightime activities.
