  • Sensitivity to bone-conducted sound : excitation of the mastoid vs the teeth

    • The sensitivity of nine subjects to bone-conducted sound was measured at three positions: osseointegrated percutaneous titanium implants in the temporal bone, the skin-covered mastoid and the teeth. Voltage levels supplied to a bone-anchored hearing aid (BAHA) transducer and to an Oticon bone-transducer were measured and the thresholds obtained at the three positions were compared. Using the mechanical impedance of the teeth, the parameters of a first order model for the vibration transmission through the teeth was calculated. Also, the equivalent force thresholds were calculated from the voltage threshold levels. The sensitivity to bone-conducted sound, for both voltage and force thresholds, conformed fairly well at the three positions for frequencies below 1 kHz; however, above 1 M-Iz, bone-conducted sound applied at the titanium implant becomes mole sensitive than at the two other positions investigated. It was concluded that the teeth can be used for the application of bone-conducted sound, in particular for pre-operative assessment of a BAHA and to facilitate service and quality control of such a hearing device.

  • Bone Mass from Childhood to Adulthood

    • Attaining high peak bone mass (PBM), the highest bone mass value in life which is reached in young adulthood, is important as it reduces the risk of having low bone mass in old age69, 80. Low bone mass is associated with high fracture risk3, 60. Osteoporosis is the result of bone loss, a physiological process related to aging and/or low PBM. It would therefore be of great value to identify children at risk of reaching low PBM for possible interventions. But the level of correlation, in the thesis referred to as “tracking”, in bone mass from childhood to adulthood is unclear. Making predictions about adult bone mineral density (BMD) from childhood measurements is difficult as bone properties change rapidly during growth59. Most studies that have evaluated the question are either cross-sectional, have a short follow-up time or end close to the final growth spurt, making reliable predictions difficult. There are some reports suggesting that a childhood excess62, 76 or deficit77, 116 in BMD remains in adulthood, and the few prospective studies that have addressed the question infer that there is a partial “tracking” in BMD during growth. Longitudinal studies with serial measurements that cover both the pre- and post-pubertal phases and that follow the participants until peak bone mass (PBM) would provide data with a higher level of evidence and thereby increase our knowledge. In this thesis, with a long-term prospective study design, we have evaluated the “tracking” of bone mass from childhood to adulthood, and specifically evaluated two risk factors linked to low BMD. The first is a fracture in childhood which has been an event identified as associated with low BMD both in childhood31 and in adulthood54. The second is premature birth in relation to low birth weight, since both traits have been associated with low PBM67, 84. We invited subjects from three previous studies63, 86-87 published during 1981–1985 to be re-measured almost three decades after the initial measurement. The study subjects with a mean age of 10 years (range 3–17) at the first measurement were re-measured a mean 27 (range 25–29) years later. Bone traits were prospectively evaluated with singlephoton absorptiometry (SPA) in 214 individuals consisting of three cohorts: healthy control subjects, children with fracture during childhood and children born preterm, either small for gestational age (SGA) or appropriate for gestational (AGA). In the second cohort we evaluated bone traits prospectively by dual-energy X-ray absorptiometry (DXA) in 121 children from the Pediatric Osteoporosis Prevention (POP) study, an exercise intervention study that is primarily designed to assess 10 musculoskeletal development and fracture risk in response to increased physical education in school children. The study subjects with a mean age of 8 years (range 7– 9) at the first measurement were re-measured a mean 11 (range 10–12) years later. Our aim was to evaluate (i) whether a bone mass scan in childhood can be used to predict bone mass in adulthood, (ii) whether children who sustain a fracture are at increased risk of reaching low adult BMD and (iii) whether prematurely born children, either AGA or SGA, are at increased risk of reaching low adult BMD. The correlation coefficients (r) between pre-pubertal and young adulthood measurements for distal radius BMC and BMD varied between 0.35 and 0.64 and for femoral neck BMC, BMD and bone area it varied between 0.37 and 0.65. A childhood fracture in men was associated with a low BMC Z-score (–0.4 (95% CI –0.6, –0.1)) and low BMD Z-score (–0.4 (95% CI –0.7, –0.1)) at baseline and with a low BMC Zscore (–0.5 (95% CI –0.8, –0.2)) and low BMD Z-score (–0.4 (95% CI –0.7, –0.1)) at follow-up. Preterm-born children were still shorter in adulthood (p=0.03), they also had lower femoral neck (FN) BMC, FN BMD, tibial cortical BMD, tibial crosssectional area and SSI than controls (all p-values 0.001 to <0.05). The deficits were driven by lower bone traits in preterm SGA individuals, while no differences were seen in preterm AGA individuals compared to controls. This thesis shows that an individual pediatric bone mass scan, regardless of whether it is evaluated with SPA or DXA and independent of the measured skeletal region, has poor ability to predict an adult bone mass value. We also show that a childhood fracture in men was associated with low BMD and smaller bone size in young adulthood and that prematurity and being born SGA is another risk factor for low bone mass in young adulthood.

    • Cervical cancer has been associated with specific human leukocyte antigen (HLA) haplotypes/alleles and with polymorphisms at the nearby non-HLA loci TNF, LTA, TAP1 and TAP2. Distinguishing effects of individual loci in the major histocompatibility complex (MHC) region are difficult due to the complex linkage disequilibrium (LD) pattern characterized by high LD, punctuated by recombination hot spots. We have evaluated the association of polymorphism at HLA class II DQB1 and the TNF, LTA, TAP1 and TAP2 genes with cervical cancer risk, using 1306 familial cases and 288 controls. DQB1 was strongly associated; alleles *0301, *0402 and *0602 increased cancer susceptibility, whereas *0501 and *0603 decreased susceptibility. Among the non-HLA loci, association was only detected for the TAP2 665 polymorphism, and interallelic disequilibrium analysis indicated that this could be due to LD with DQB1. As the TAP2 665 association was seen predominantly in non-carriers of DQB1 susceptibility alleles, we hypothesized that TAP2 665 may have an effect not attributable to LD with DQB1. However, a logistic regression analysis suggested that TAP2 665 was strongly influenced by LD with DQB1. Our results emphasize the importance of large sample sizes and underscore the necessity of examining both HLA and non-HLA loci in the MHC to assign association to the correct locus.

    • Aim: To investigate whether nasopharyngeal suctioning produces a physiological and behavioural stress reaction in preterm infants and if a possible reaction can be dampened by sweet solution. Methods: Eleven preterm infants were randomly assigned to receive either 30% oral glucose or nothing prior to morning nasopharyngeal suctioning; the procedure was reversed in the afternoon. The study included a total of 44 samples from preterm infants evaluated with salivary cortisol, pain score (Visual Analogue Scale), heart rate, oxygen saturation and recovery time through the Newborn Individualized Developmental Care and Assessment Program. For reference, 44 samples from eleven full-term infants were evaluated for salivary cortisol. Results: Regardless of whether or not preterm infants received glucose before nasopharyngeal suctioning, no statistically significant difference was found in salivary cortisol reactivity, pain score, heart rate, oxygen saturation or recovery time. Nor were any statistically significant differences between salivary cortisol baseline and response values found in full-term infants after nasopharyngeal suctioning. Conclusion: In the present setting, nasopharyngeal suctioning was not stressful enough to increase salivary cortisol or pain score. Oral glucose did not alter salivary cortisol levels.

Available on heavyweight 180g LP, gatefold CD and digital download with bonus tracks on the CD only.Jones, who has helmed notable recordings by Josh Rouse, Hayes Carll and Jill Sobule, said: “My goal was to show David branching out from youthful power-pop to a more reflective, and more lasting sound.” Comparing Lucky Day to Myhr’s prior album Soundshine, Jones continues: “The idea was to create a warmer circle of sound around David, letting him sing in an easy and natural voice, to invite the listener into his world as it is now.” Ultimately, the album offers what Jones calls “a naturalistic kind of rehearsal-room energy.“

    • The main aim of my dissertation is to understand the different ways problem youngsters cope with reality, construct identity and make choices of lifestyles. From a common starting point, the assessment home, I have by means of continual interviews followed fourteen youngsters, six girls and eight boys, for almost three years in the boundary zone between "the normal life" and "the deviating behaviour". In the assessment home they where defined as socially vulnerable and problematical. I was able to establish co-operation, got close and through narratives, gain insight into the "order" they gave themselves for experiences, thoughts and identity in choices of lifestyles. My interest was focused on life historical narratives and their own version of events. Even if different choices and lifestyles are experienced as individual they are not only a personal construction. Choosing is also the obvious way of doing gender, making a place for one's own thread, weaving it into the collective warp. Gender and reality differences leads to gender specific choices. Females gave priority to Intimacy before Identity and the males gave priority to Identity before Intimacy. I also found two types of social inheritance and life history scenarios that gave structure to choices and orientations. Choices of lifestyles were related to social action, ways of doing gender, dealing with social inheritance in situational ways. In an individual way actions had much to do with "a play around self-esteem" in which habits and routinized practice provided security and self-confidence. The power of habit gave security to create and recreate known figures even if the figures generated fear and respect in others' eyes. Social choices are decisions not only about how to act but who to be and the main theme for all the youngsters was to be somebody and to fit in somewhere.

    • This dissertation studies the labor market development of fourteen Western European countries during the years 1970-90, focusing on the relatively low Swedish unemployment. The variation in unemployment over time and between countries has proved difficult to explain with the most influential theoretical models, which are usually categorized as New-keynesian and Post-keynesian respectively. The dissertation shows that the latter can to some extent explain the development over time for eachcountry, but leaves much unexplained in terms of differences between countries. To explain these differences, a structural analytical framework is constructed. The composition and growth of employment are analyzed for each country. It is shown that there are underlying structural explanations for the development of unemployment. Unemployment increased as a consequence of declining growth (in some cases negative growth) in total employment, which in turn is explained by the structural crisisgiving rise to a gap between the decline of the industrial sector and the service sector's ability to create new employment. This gap was not as pronounced in all countries and through analysis of the structural change, information is obtained on what explains variations in unemployment. It is shown that the relatively good Swedish development in the labor market during this period is partly explained by a relatively milder employment crisis in industry and a higher degree of compensatory employmentgrowth in the welfare services sector.
