  • Suicidal expression in adolescents in Nicaragua in relation to youth self-report (YSR) syndromes and exposure to suicide
  • 2011
  • Ingår i: Clinical Practice and Epidemiology in Mental Health. - : Bentham Open. - 1745-0179. ; 7, s. 89-96
  • Tidskriftsartikel (refereegranskat)abstract
    • Background: Suicide and suicidal expressions among young people represent a major public health problem worldwide. Most studies are from high-income countries, and it remains unclear whether prevalence and risk factors show a similar pattern in other settings. This study aims to assess the prevalence of suicidal expressions and serious suicidal expressions (ideation, plans and attempts) among adolescents in Nicaragua, in relation to previously reported risk factors, such as exposure to suicide in significant others (parents, siblings, partners or friends) and mental health problems. Methods: 368 adolescents aged 15-18 years were randomly selected from public secondary schools in León, Nicaragua. Data was collected using Attitude Towards Suicide (ATTS) and Youth Self-Report questionnaires (YSR). Bivariate and multivariate analyses were conducted. Results: Suicide ideation prevalence in the past year was 22.6%, suicide plans 10.3%, and suicide attempts 6.5%. Girls were significantly more likely to report suicidal ideation. Multivariate analyses showed that anxious/depressed, somatic complaints and exposure to suicidal behavior in significant others were significantly associated with own serious suicidal expressions. Conclusions: The prevalence of serious suicidal expressions among young people in Nicaragua is within the range reported from Western high-income countries. An attempted or completed suicide in someone close, is associated with own suicidal expressions even in the absence of increased mental distress. Furthermore, somatic complaints should alert healthcare professionals of the possibility of increased suicide risk.

  • Gas measurements from the Costa Rica-Nicaragua volcanic segment suggest possible along-arc variations in volcanic gas chemistry
  • 2014
  • Ingår i: Earth and Planetary Science Letters. - : Elsevier BV. - 1385-013X .- 0012-821X. ; 407, s. 134-147
  • Tidskriftsartikel (refereegranskat)abstract
    • Obtaining accurate estimates of the CO2 output from arc volcanism requires a precise understanding of the potential along-arc variations in volcanic gas chemistry, and ultimately of the magmatic gas signature of each individual arc segment. In an attempt to more fully constrain the magmatic gas signature of the Central America Volcanic Arc (CAVA), we present here the results of a volcanic gas survey performed during March and April 2013 at five degassing volcanoes within the Costa Rica-Nicaragua volcanic segment (CNVS). Observations of the volcanic gas plume made with a multicomponent gas analyzer system (Multi-GAS) have allowed characterization of the CO2/SO2-ratio signature of the plumes at Pads (0.30 +/- 0.06, mean +/- SD), Rincon de la Vieja (27.0 +/- 15.3), and Turrialba (2.2 +/- 0.8) in Costa Rica, and at Telica (3.0 +/- 0.9) and San Cristobal (4.2 +/- 1.3) in Nicaragua (all ratios on molar basis). By scaling these plume compositions to simultaneously measured SO2 fluxes, we estimate that the CO2 outputs at CNVS volcanoes range from low (25.5 +/- 11.0 tons/day at Pods) to moderate (918 to 1270 tons/day at Turrialba). These results add a new information to the still fragmentary volcanic CO2 output data set, and allow estimating the total CO2 output from the CNVS at 2835 1364 tons/day. Our novel results, with previously available information about gas emissions in Central America, are suggestive of distinct volcanic gas CO2/S-T (= SO2 + H2S)-ratio signature for magmatic volatiles in Nicaragua (similar to 3) relative to Costa Rica (similar to 0.5-1.0). We also provide additional evidence for the earlier theory relating the CO2-richer signature of Nicaragua volcanism to increased contributions from slab-derived fluids, relative to more-MORB-like volcanism in Costa Rica. The sizeable along-arc variations in magmatic gas chemistry that the present study has suggested indicate that additional gas observations are urgently needed to more-precisely confine the volcanic CO2 from the CAVA, and from global arc volcanism.

  • Gift and Exchange in the Reciprocal Regime of the Miskito on the Atlantic Coast of Nicaragua, 20th Century
  • 2001
  • Doktorsavhandling (övrigt vetenskapligt/konstnärligt)abstract
    • This dissertation is a historical-comparative analysis of a conflict that has developed in the economic system of the Miskito in the 20th century, concerning the use of common property resources. The study is based on empirical material collected in a Miskito/Creole village on the Atlantic Coast of Nicaragua in the 1990s. During the enclave period (1860-1960) Miskito men earned money as wage labourers at foreign companies (lumber, mining and banana plantations), and sent home money and purchased goods to women, who worked in subsistence agriculture. At this time the communal resources were reserved mainly for subsistence and food gifts. This permitted a symbolic transformation of money and purchased goods, which were distributed in a wider network of kin-related women. There was a relatively stable coexistence of short-term exchange and long-term gift, which is referred to as a reciprocal regime. However, the transition to the commercial period (1960-) resulted in a commercial exploitation of communal resources, which seriously started to compete with subsistence and the custom of food gifts in the 1970s. A contradiction developed between short-term exchange and long-term gift, which stems from a double and contradictory coding of communal resources. This development was interrupted during the 1980s - the decade of the Sandinist revolution - but continued in the 1990s, although new aspects were added due to increasing population pressure, external exploitation and few opportunities for wage labour. In spite of instabilities in the reciprocal regime, the analysis suggests that there could be a conflictual coexistence of gift and exchange, which is potentially creative for the future.

  • Mental health in Nicaragua : with special reference to psychological trauma and suicidal behaviour
  • 2004
  • Doktorsavhandling (övrigt vetenskapligt/konstnärligt)abstract
    • This thesis explores mental health problems relating to war and natural disaster and suicidal behaviour in the Nicaraguan population. The more specific aims of the study were to assess the prevalence and sociodemographic correlates of mental disorder in a community-based study during time of war (Paper I), to assess the mental health impact of Hurricane Mitch in 1998 (Paper II), to assess the incidence of hospitalized parasuicide cases and groups at risk (Paper III), and to examine suicide intent among attempters relating to gender, suicide method and sociodemographic factors and identify predictors for repetition of an attempt (Paper IV).Method: Based on 4453 family food ration books for families living in an urban area of León, Subtiava, 219 families including 746 adults were selected through a systematic sampling procedure. The study was conducted in 1987 during the war. We were able to reach 584 adults for interview according to the Present State Examination for ICD-9 diagnoses and Self-Report Questionnaire (Paper I). In Paper II, 496 adult primary health care attendees were interviewed six months after Hurricane Mitch according to the Harvard Trauma Questionnaire and were diagnosed for post-traumatic stress disorder (PTSD) according to DSM-IV. In Papers III and IV, all cases from León city admitted to HEODRA Hospital for a suicide attempt over a three-year period (n=233) were interviewed regarding sociodemographic factors and method, time and place of the suicide attempt. A subgroup of 204 cases was interviewed using the Suicide Intent Scale (SIS). Out of those 106 cases were followed-up regarding repetition of attempt or completed suicide after a mean period of 1172 days. Results: In the Paper I study, the one-month prevalence of any mental disorder was 28.8% for men and 30.8% for women. Among men, alcoholism was the most common diagnosis, whereas neurosis, crisis reaction and depression were dominant among women. Alcoholism was scored as the second most severe disorder after psychosis in terms of functional level. In the Mitch study six months after the hurricane, traumatic events were common and 39% reported death or serious injury of a close relative as a result of the hurricane. The prevalence of PTSD ranged from 4.5% in the least damaged area to 9.0% in the worst damaged area. At the prolonged follow-up six months later, half of the cases still retained their diagnosis. Trauma-related symptoms were common and death of a relative, destroyed house, female sex, illiteracy and previous mental health problems were associated with a higher level of symptoms. Suicidal ideation was reported among 8.5% and was significantly associated with previous mental health problems and illiteracy. The studies regarding hospitalized parasuicides showed the highest rate among girls aged 15–19 years (302 attempts per 100 000 inhabitants and year). After drug intoxication, pesticide was the second most common method and most often used by men (23%). Half of the women had recent contact with health care services before attempting suicide. There were significant peaks regarding time of attempt in terms of seasonal and diurnal distribution. Overall scores regarding seriousness of the intent (SIS) were equal between the sexes, but the pattern of SIS items showed significant gender differences in terms of relation to background factors and method used. For women, having a child was one factor associated with higher seriousness. Factor analysis of SIS items revealed a four-factor solution, explaining 59% of the variance. Risk for fatal repetition was 3.2% after three years and for non-fatal repetition 4.8%. During follow-up, three men (11%) had completed suicide but no women. We failed to identify any predictors for repetition from background factors or SIS.Conclusion: The studies have identified different groups at risk for mental health problems relating to war and disasters. Parasuicide rates equalled those from European countries. Whereas young girls dominated, attempts among men were more severe in terms of the methods used and completed suicide at follow-up. SIS seemed to give a meaningful pattern among women but not for men. In our study, seriousness of attempt in terms of method or suicide intent did not predict repetition. Overall non-fatal repetition rate was very low as compared to other studies.

  • First Episodes of Norovirus and Sapovirus Gastroenteritis Protect Against Subsequent Episodes in a Nicaraguan Birth Cohort
  • 2022
  • Ingår i: Epidemiology. - 1044-3983 .- 1531-5487. ; 33:5, s. 650-653
  • Tidskriftsartikel (refereegranskat)abstract
    • Background: Norovirus and sapovirus cause a large burden of acute gastroenteritis (AGE) in young children. We assessed protection conferred by norovirus and sapovirus AGE episodes against future episodes. Methods: Between June 2017 and July 2018, we recruited 444 newborns in León, Nicaragua. Weekly household surveys identified AGE episodes over 36 months, and AGE stools were tested by reverse transcriptase-quantitative polymerase chain reaction (RT-qPCR) for norovirus genogroup (G)I/GII and sapovirus. We used recurrent-event Cox models and negative control methods to estimate protection conferred by first episodes, controlling for observed and unobserved risk factors, respectively. Results: Sapovirus episodes conferred a 69% reduced hazard of subsequent episodes using the negative control method. Norovirus GI (hazard ratio [HR] = 0.67; 95% confidence interval [CI] = 0.31, 1.3) and GII (HR = 0.20; 95% CI = 0.04, 0.44) episodes also appeared highly protective. Protection against norovirus GII was enhanced following two episodes. Conclusions: Evidence of natural immunity in early childhood provides optimism for the future success of pediatric norovirus and sapovirus vaccines.

  • Can community-based tourism contribute to development and poverty alleviation? Lessons from Nicaragua. In Current Issues in Tourism, vol 14 (8): 725-749
  • 2011
  • Ingår i: Current Issues in Tourism. - : Informa UK Limited. - 1368-3500 .- 1747-7603. ; 14:8, s. 725-749
  • Tidskriftsartikel (refereegranskat)abstract
    • Since the development of community-based tourism (CBT) governments, development agencies and NGOs have placed considerable emphasis on this development model. However, CBT has been strongly criticized with respect to low economic impact in terms of jobs and income, the result of small-scale interventions, its low life expectancy after external funding ends, the monopolisation of benefits by local elites, or the lack of business skills to make it operational. This article explores the viability of the CBT model to support socio-economic development and poverty alleviation via a Nicaraguan case study. The characteristics and effects of different modes of organising community tourism were examined, based on an impact assessment and lifecycle analysis of the CBT Nicaraguan Network. The results showed how traditional top-down CBT, created and fully funded by external organisations, reflected the general criticisms of the approach, while bottom-up CBT, borne as a result of a local initiative, demonstrated longer life expectancy, faster growth, and more positive impacts on the local economy. The findings suggest a shift is required in the attention of donors and policy-makers towards redistribution policies that strengthen the skills, resources, and conditions of micro, community-based and family entrepreneurship, together with a stronger orientation towards the domestic markets.

  • Pediatric norovirus diarrhea in Nicaragua
  • 2008
  • Ingår i: Journal of Clinical Microbiology. - 0095-1137 .- 1098-660X. ; 46:8, s. 2573-2580
  • Tidskriftsartikel (refereegranskat)abstract
    • Information about norovirus (NoV) infections in Central America is limited. Through a passive community and hospital pediatric diarrhea surveillance program, a total of 542 stool samples were collected between March 2005 and February 2006 in León, Nicaragua. NoV was detected in 12% (65/542) of the children; of these, 11% (45/409) were in the community and 15% (20/133) were in the hospital, with most strains (88%) belonging to genogroup II. NoV infections were age and gender associated, with children of <2 years of age (P < 0.05) and girls (P < 0.05) being most affected. Breast-feeding did not reduce the number of NoV infections. An important proportion (57%) of NoV-infected children were coinfected with diarrheagenic Escherichia coli. A significant proportion (18/31) of NoV-positive children with dehydration required intravenous rehydration. Nucleotide sequence analysis (38/65) of the N-terminal and shell region in the capsid gene revealed that at least six genotypes (GI.4, GII.2, GII.4, GII.7, GII.17, and a potentially novel cluster termed "GII.18-Nica") circulated during the study period, with GII.4 virus being predominant (26/38). The majority (20/26) of those GII.4 strains shared high nucleotide homology (99%) with the globally emerging Hunter strain. The mean viral load was approximately 15-fold higher in children infected with GII.4 virus than in those infected with other G.II viruses, with the highest viral load observed for the group of children infected with GII.4 and requiring intravenous rehydration. This study, the first of its type from a Central American country, suggests that NoV is an important etiological agent of acute diarrhea among children of <2 years of age in Nicaragua.

  • Bioaccumulation and translocation of field-weathered toxaphene and other persistent organic pollutants in three cultivars of amaranth (A. cruentus ‘R127 México’, A. cruentus ‘Don León’ y A. caudatus ‘CAC 48 Perú’) – A field study from former cotton fields in Chinandega, Nicaragua
  • 2018
  • Ingår i: Ecological Engineering. - : Elsevier BV. - 0925-8574 .- 1872-6992. ; 121, s. 65-71
  • Tidskriftsartikel (refereegranskat)abstract
    • Soils polluted by persistent organic pollutants threatens habitats for plants and animals as well as basic human needs such as food security and provision of potable water. Toxaphene is a persistent organic pollutant that was heavily used as a pesticide in Central America, Brazil, Soviet Union etc. until it was banned in 1993. The objective of this study was to determine the bioaccumulation and translocation characteristics of three different cultivars of amaranth in soils contaminated with field-weathered toxaphene and other POPs in former cotton fields in Chinandega, Nicaragua to identify safety issues for human consumption and/or potential for phytoremediation. The concentration of toxaphene and other POPs in the edible parts of the amaranth (leaves and seeds) exceeded the maximum residue level for human consumption established by the European Union for most of the tested compounds. Concentrations of toxaphene congeners and other POPs were found in all vegetative organs. Many substances were accumulated to concentrations more than 10 times higher than in the soil. Of the three cultivars, A. caudatus 'CAC48 Perú' and A. cruentus 'Don Leon' presented the highest average BAF. None of the 3 cultivars can be considered a panacea hyperaccumulator for either toxaphene alone or in conjunction with other POPs but since many agricultural soils comprise a range of different contaminants, the broad-spectrum bioaccumulating capacity of amaranth can make it an interesting candidate for phytoremediation.
