    • Debatt: Vi vill att forskningsråden skapar tvärvetenskapliga regionala forskningscentrum för arbetslivsforskning, som är internationellt konkurrenskraftiga och ger nationellt och regionalt kunskapsstöd. Dessa centrum ska ge kunskaper för både befintliga och framtida utmaningar. Stora vinster kan fås om forskning om folkhälsa och yttre miljö samordnas i centrumen. Arbetslivet är grunden för hälsa, välstånd och ett välfungerande samhälle. För att säkra att framtidens arbetsliv bidrar till hälsa och välstånd behövs både kunskap om hur det ska utformas och en uthållig infrastruktur för forskning. Tyvärr saknas detta. Gammal kunskap faller i glömska.

    • DN DEBATT 15/3. Sedan industrialiseringens början har vi använt omkring fyra femtedelar av den mängd fossilt kol som får förbrännas för att vi ska klara Parisavtalet. Vi har bara en femtedel kvar och det är bråttom att kraftigt reducera utsläppen. Det har Greta Thunberg och de strejkande ungdomarna förstått. Därför stödjer vi deras krav, skriver 270 klimatforskare.

    • Requirements on well documented management systems for Nuclear Power Plants (NPPs) are relatively new and in different developmental phases. Thus there is a need of knowledge about well-functioning Safety Management Systems (SMSs) also considering other goals and restrictions. Since there ia a lack of mature management systems for NPPs it is important to learn from other NPPs, but also from other safety critical industries. The objectives of this paper are to identify and discuss some important elements that should be included and well managed in SMSs in the NPP sector. Information is taken from reports from major accidents, the scientific literature and interviews. The results are pointing at weaknesses in the SMS at the concern and site levels and a need for specific training for the top management. Abilities to anticipate and cope with foreseeable and unforeseeable events are stressed. Important topics in development are indicators and probabilistic risk analysis.

    • For ergonomics/human factors solutions to be beneficial for society and its members it is vital to apply a systems level perspective. Students who will be the future engineers, designers, and managers of socio-technical systems need to gain an understanding of how ergonomics/human factors can be integrated in the design of workplaces, organisations, products, and services. This paper gives examples of how the systems perspective is realised in courses on ergonomics/human factors at the Division of Ergonomics and Aerosol Technology (EAT) in the Faculty of Engineering at Lund University, Sweden. Examples are the application of theories and models of systems perspectives, guest lecturers from industry and other faculties, the use of supportive information technologies in teaching, arranging for student to visit and carry out projects in industrial settings, utilising peer assessment and written reflections to enhance learning, and communicating human factors research results. Strategies of the EAT educational environment that strengthen its teachers’ pedagogical abilities are also presented.

  • Efficient and effective learning for safety from incidents

    • Learning from incidents is important for improving safety. Many companies spend a great deal of time and money on such learning procedures. The objectives of this paper are to present some early results from a project aimed at revealing weaknesses in the procedures for learning from incidents and to discuss improvements in these procedures, especially in chemical process industries. The empirical base comes from a project assessing organizational learning and the effectiveness of the different steps of the learning cycle for safety and studying relations between safety-specific transformational leadership, safety climate, trust, safety-related behavior and learning from incidents. The results point at common weaknesses in the organizational learning, both in the horizontal learning (geographical spread) and in vertical learning (double-loop learning). Furthermore, the effectiveness in the different steps of the learning cycle is low due to insufficient information in incident reports, very shallow analyses of reports, decisions that focus at solving the problem only at the place where the incident took place, late implementations and weak solutions. Strong correlations with learning from incidents were found for all safety climate variables as well as for safety-related behaviors and trust. The relationships were very strong for trust, safety knowledge, safety participation and safety compliance.

(2012) European Journal of Cancer Care Characteristics of medication errors with parenteral cytotoxic drugs Errors involving cytotoxic drugs have the potential of being fatal and should therefore be prevented. The objective of this article is to identify the characteristics of medication errors involving parenteral cytotoxic drugs in Sweden. A total of 60 cases reported to the national error reporting systems from 1996 to 2008 were reviewed. Classification was made to identify cytotoxic drugs involved, type of error, where the error occurred, error detection mechanism, and consequences for the patient. The most commonly involved cytotoxic drugs were fluorouracil, carboplatin, cytarabine and doxorubicin. The platinum-containing drugs often caused serious consequences for the patients. The most common error type were too high doses (45%) followed by wrong drug (30%). Twenty-five of the medication errors (42%) occurred when doctors were prescribing. All of the preparations were delivered to the patient causing temporary or life-threatening harm. Another 25 of the medication errors (42%) started with preparation at the pharmacies. The remaining 10 medication errors (16%) were due to errors during preparation by nurses (5/60) and administration by nurses to the wrong patient (5/60). It is of utmost importance to minimise the potential for errors in the prescribing stage. The identification of drugs and patients should also be improved.

    • Abstract in UndeterminedThis paper describes a method for assessing the effectiveness in the steps of the learning cycle: the 1st loop with reporting - analysis - decision - implementation - follow-up, and the 2nd loop on an aggregated basis. For each step, the dimensions considered the most relevant for the learning process (scope, quality, timing and information distribution) and for each dimension the most relevant aspects (e.g. completeness and detail) were defined. A method for a semi-quantitative assessment of the effectiveness of the learning cycle was developed using these dimensions and aspects and scales for rating. The method will give clear indications of areas for improvement when applied. The results of the method can also be used for correlation with other safety parameters, e.g. results from safety audits and safety climate inquiries. The method is intended to be used on a sample of the broad range of incidents normally seen in process industry companies. The method was tested on a two-year incident reporting material from six companies from various types of process industries. It was found that the method and the tools worked very well in practice. The results gave interesting insights into the effectiveness of learning from the incidents.

    • The evolution of a system for the management of fatigue-related risks in airline and maintenance operations is at present a contentious issue. The aspirations for the integration of fatigue risk management processes into existing Safety Management System (SMS) guidelines need to be placed in the realities of the present economic and commercial environment – they therefore need to exhibit relevance and demonstrate cost effectiveness if they are to survive the commercial scrutiny of operators worldwide. Some operators (e.g. easyJet, Air New Zealand, DHL) have already faced the task of making FRMS (Fatigue Risk Management System) relevant to the operation. A recognized and accredited FRMS conversely allows an operator an exemption from the rigid FTL (Flight Time Limitations) rule sets and the flexibility to introduce safe and commercially beneficial rostering solutions. The key to application of an FRMS is defining the relationship between prescriptive FTL and FRMS for an operator that shows benefits over adherence in whole or part to prescriptive FTL. The principle that needs to be debated should be that the rigidity and scope of an operator’s prescriptive FTL requirements are proportionate to the credibility of their FRMS. This paper describes the development and implementation of a Fatigue Risk Management System (FRMS) in a major airline in Europe with a focus on management of fatigue-related risks. The FRMS example focuses on the principles of SMS by employing both Risk Management System (RMS) and Safety Assurance (SA) processes to support evidence-based crew scheduling practices, allowing the identification of structural factors related to consistency and standardisation and performance. This work has been realised as a part of an EU integrated project HILAS (Human Integration into the Lifecycle of Aviation Systems). In the development of the FRMS, the HILAS SMS working group adhered to the principles of Organisational Learning and Resilient Safety Culture.
