  • Sense of support within the family : a cross-sectional study of family members in palliative home care

    • BackgroundDespite evidence that family members’ support to each other can be of importance to its members, there are limited studies of factors related to family members’ sense of such support during palliative care.AimBased on the family systems approach, we evaluated which factors were associated with family members’ sense of support within their closest family in a palliative home care context and developed a model that predicts such sense of support.DesignA cross-sectional design was used. We interviewed 209 adult family members (69% of eligible) of adult patients with expected short survival receiving palliative home care.MethodsGeneralised linear models were used to evaluate individual factors related to family members’ sense of support within their closest family during palliative care. The Akaike Information Criterion (AIC) was applied in the model-building analyses.ResultsNineteen variables were identified that were significantly associated with the family members’ sense of support within the closest family. Model building selected six variables for predicting this sense of support (decreasing Wald values): family member perceiving support from other more distant family members; feeling secure with the provided palliative home care; possibility of respite if family member needed a break; family member living alone; being a child of the patient (inverse relationship); perceiving that the patient was supported by other family members.ConclusionsOur findings support clinical application of the Family Systems Theory in the context of palliative care. The factors identified may be of value in assisting practitioners in detecting and treating family members sensing a low level of support within the closest family.

  • What is a “secure base” when death is approaching? A study applying attachment theory to adult patients’ and family members’ experiences of palliative home care

    • ObjectiveAttachment theory has received much interest lately in relation to how adults cope with stress and severe illness. The aim of this study was using the experiences of patients and family members to explore palliative home care as a ‘secure base’ (a central concept within the theory).MethodsTwelve patients and 14 family members were interviewed during ongoing palliative home care. The interviews were analysed with deductive qualitative content analysis.ResultsInformants expressed the relevance of sensing security during palliative home care because death and dying were threats that contributed to vulnerability. Palliative home care could foster a feeling of security and provide a secure base. This was facilitated when informants had trust in staff (e.g. due to availability and competence in providing symptom relief), felt recognised as individuals and welcomed to contact the team in times of needs. Being comfortable, informed and having an everyday life also contributed to a perception of palliative home care as a secure base. Family members stressed the importance of being relieved from responsibilities that were too heavy. The underlying meanings of experiencing palliative home care as a secure base involved gaining a sense of control and of inner peace, perceiving that despite a demanding and changed life situation, one could continue partially being oneself and having something to hope for, even if this no longer concerned cure for the ill person.ConclusionsImportant aspects of palliative home care as providing a secure base were identified and these have implications for clinical practice.   • What to do when 'there is nothing more to do'? A study within a salutogenic framework of family members' experience of palliative home care staff

    • The aim of this study was to develop a theoretical framework of family members' experience of palliative home care staff based on a secondary analysis of four previous studies. A salutogenic framework was used, i.e. with the origin of health in focus. Data had been collected (semi-structured tape-recorded interviews and postal questionnaires with open-ended questions) from 469 family members of mainly cancer patients referred to advanced palliative home care. Walker and Avant's strategies for theory construction were used. The secondary analysis generated three theoretical blocks: (1) general components of staff input (including five generalized resistance resources (GRRs): competence, support, spectrum of services, continuity, and accessibility); (2) specific interactions with staff (including two GRRs: being in the centre and sharing caring); (3) emotional and existential consequences of staff support (including six health-disease continuums: security-insecurity, hope-hopelessness, congruent inner reality-chaos, togetherness-isolation, self-transcendence-feelings of insufficiency and retained everyday life-disrupted everyday life). It seems important that all three aspects of family members' experience of palliative care staff are to be considered in evaluations of palliative care, in goal-setting and in teaching role models. The study is specific to the Swedish model of palliative home care and replication of the work in other countries is recommended.

  • A burning desire for smoke? Sampling insects favoured by forestfire in the absence of fire

    • Fire-favoured insects are difficult to sampleexcept opportunistically after forest fires. Here, we tested ifsmoke from a small fire could be an efficient way to samplesuch insects. Insects were sampled over ca. 10 h hours, byhand-picking and netting on screens put up around the fire.Two specimens of the rare and redlisted Hormopeza spp.(Diptera, Empididae) were caught. Large numbers([20,000) of Microsania spp. (Diptera, Platypezidae) werecaught, but none in the absence of smoke. The numbers ofMicrosania spp. clearly peaked in late afternoon, and ashort sampling period would be sufficient if targeting onlythis taxon. Of the almost 200 species of Coleoptera, 17 %were considered as fire-favoured, contributing 9 % of thespecimens, suggesting low efficiency of the method for thisgroup. Using 23 sites differing in fire history, catches ofMicrosania spp. were unaffected by numbers and area offorest fire (preceding 5 years and within 10 km radius overthe sampling sites). In contrast, there was a weak trend forthe proportion of fire-favoured Coleoptera to increase withincreasing number of fires. To conclude, smoke as producedin our study can clearly attract fire-favoured Diptera,but smoke had only a weak effect on fire-favoured Coleopterain the study area. It is still likely that selectivelypicking specimens of species attracted to smoke is a morecost-efficient method than using, e.g., Malaise traps thatcatch indiscriminately.

  • Annual burning of semi-natural grasslands for conservation favours tall-grown species with high nectar production

    • Species-rich semi-natural grasslands in Europe are a main target for conservation efforts, and alternative methods to the traditional management of mowing or grazing would be welcome due to the difficulties in maintaining traditional management practices. One such method proposed is burning of grassland vegetation during late winter or spring. To evaluate the effects of annual spring burning vs annual mowing on semi-natural grassland vegetation, we compared the frequency of species in eleven field experiments in southern Sweden after ca 14 years. Out of the 88 species analyzed, five were more frequent in burnt plots compared with mowed plots (Vicia cracca, Cirsium arvense, Urtica dioica, Galium verum, Convallaria majalis). In contrast, 37 species were significantly less frequent in burnt plots compared with mowed ones, those with the largest differences being Ranunculus acris, Briza media, Veronica chamaedrys, Festuca ovina, Plantago lanceolata and Anthoxanthum odoratum. Tall-grown species and those with preferences for N-rich soils increased in frequency under an annual spring-burn regime, compared with annual mowing, as did species producing larger amounts of nectar. Hence, although vegetation composition becomes more trivial with annual spring burns, there might be long-term benefits for nectar-feeding insects.

This makes them a potential habitat for several groups of insects that typically use open habitats like grassland, including those species that visit flowers. In the current study, we used colour pan traps to catch flower-visiting species. Study sites were selected according to age (2-8 yrs since clear-cut) and land-use history (forest or meadow 150 yrs ago). We caught and identified solitary bees (395 specimens belonging to 59 species), social bees (831/16), other Hymenoptera (367/66), Syrphidae (256/31), and beetles (Lepturinae & Cetoniinae; 11,409/12). Age of the clear-cut strongly affected species composition as well as several groups and species, with most species caught mainly in the younger clear-cuts. Flower abundance statistically affected several groups and species, but inferring causation is difficult due to the flower-richness bias in pan trap catches. Bare soil and woody debris were important for the insect assemblage sampled, while bare rock was not. Although the majority of the insects caught were forest species, about one third of the species were associated with open, agricultural sites and hence seem to be able to locate and exploit resources in clear-cuts.

  • Assemblages of saproxylic beetles on large downed trunks of oak

    • Old living oaks (Quercus robur) are known as a very species-rich habitat for saproxylic beetles, but it is less clear to what extent such veteran trees differ from an even rarer feature: downed trunks of large oaks. In this study, we set out to sample this habitat, using window traps, with two aims: (1) to describe the variation of assemblages among downed trunks of different type and (2) to compare beetles on downed oaks with data from veteran standing trees. The results showed that trunk volume and sun exposure better explained assemblages as well as species numbers on downed trunks than did decay stage. Furthermore, species classified as facultative saproxylic species showed weak or no differentiation among downed trunks. Species with different feeding habits showed no apparent differentiation among downed trunks. Furthermore, species composition on dead, downed oak trunks differed sharply from that of living, veteran oaks. Wood or bark feeders were more common on veterans than downed trunks, but there was no difference for those species feeding on fungi or those feeding on insects and their remains. In conclusion, for a successful conservation of the saproxylic beetle fauna it is important to keep downed oak trunks, and particularly large ones, in forest and pastures as they constitute a saproxylic habitat that differs from that of living trees.
