  • Review of PEEK implants and biomechanical and immunological responses to a zirconium phosphate nano-coated PEEK, a blasted PEEK, and a turned titanium implant surface

    • PURPOSE: To investigate the biomechanical and immunological reactions to coated and non-coated blasted PEEK implants in vivo after 12 weeks and review the associated literature. METHODS: Two osteotomy sites were performed in each proximal tibia of 10 lop-eared rabbits (n= 4 per rabbit). Each rabbit received a randomly placed (1) blasted zirconium phosphate nano-coated PEEK- (nano-ZrP), (2) blasted PEEK- (PEEK) and (3) titanium implant (Ti) and an empty sham site. At 12 weeks, removal torque of all implants and biological investigation with qPCR was performed. The implant surfaces were analyzed prior to insertion with interferometry, SEM and XPS. RESULTS: The interferometry analysis showed that there was no difference in roughness for the uncoated PEEK compared to the ZrP coated PEEK implants. The titanium implants were considerably smoother (Sa= 0.23 µm) than the uncoated Sa= 1.11 µm) and ZrP coated PEEK implants (Sa= 1.12 µm). SEM analysis on the PEEK implants corroborated the interferometry results; no difference in structure between the uncoated vs. the ZrP coated PEEK was visible on the micrometer level. At higher magnifications, the ZrP coating was visible in the SEM as a thin, porous network. All tested implants displayed osseointegration with the highest RTQ for nano-ZrP (18.4 Ncm) followed by PEEK (14.5 Ncm) and Ti (11.5 Ncm). All implants activated the immune system, with elevated macrophage and M2 macrophage qPCR markers at 12 weeks compared to the sham site. CLINICAL SIGNIFICANCE: Nano-ZrP coating improves osseointegration of blasted PEEK implants at 12 weeks of follow-up. Osseointegration of titanium, PEEK and nano-ZrP PEEK is not a normal bone healing process, but rather a shield-off mechanism that appears to be regulated by the innate immune system.   • Topographical characterisation of artificial femoral heads : a Benchmarking study

    • The main functional demands on artificial joints are that they last more than 20 years. This means optimising the friction coefficient to minimise forces transmitted to the surrounding bone and the wear of the contacting surfaces. Surface roughness has an important influence on friction and wear. Traditionally, the 2D Ra-parameter has been the preferred way to characterise the joint's topography. Today ongoing development of 3D techniques are available for the research, and the aim of the present paper is to highlight the importance and possibilities using high-resolution 3D atomic force microscope ( AFM) and the optical phase-shifting interferometer for texture characterisation of eight commercially available femoral heads. The results show a significant variation of topography and surface-lay for the different femoral heads in the study. The variation and difference between heads were further underlined by the use of different characterisation techniques and measuring strategy. Results highlight the importance of the selection of measurement strategy, parameters, instrumentation, and scale of measurement in order properly to characterise the femoral heads in this study.

  • Implant stability during initiation and resolution of experimental periimplantitis: an experimental study in the dog.

    • BACKGROUND: Histologic studies have demonstrated the possibility to reestablish direct bone-implant contacts after ligature-induced periimplantitis. The influence of the reosseointegration on the stability of implants is not known. PURPOSE: The aim of the present investigation was to study bone tissue and associated implant stability alterations that occurred during induction and resolution of periimplantitis using resonance frequency analysis (RFA), radiography, and histology. MATERIALS AND METHODS: Three implants with smooth (turned) or roughened (SLA) surfaces were placed in each side of the edentulous mandible of four dogs. Experimental periimplantitis was induced for 3 months. Five weeks later, the animals were treated with antibiotics and surgical therapy and were followed for another 6 months. Periapical radiographs and RFA were used to evaluate marginal bone levels and implant stability throughout the study period. After termination, the tissue-implant interface was evaluated by light microscopy in ground sections. RESULTS: There was a linear relationship between radiographic and RFA findings because continuous loss of marginal bone and a decrease in implant stability were observed for both implant surfaces during the periimplantitis period. Antibiotic treatment and surgical therapy resulted in some reosseointegration, which was more marked for the SLA surface. The resonance frequency values corresponded well to the histometric measurements because reosseointegration resulted in an increase in implant stability. CONCLUSIONS: The findings from the present study indicate a linear relationship between marginal bone level and resonance frequency value. It is suggested that the RFA technique is sensitive and may be used to detect even a minor change in the level of bone-implant contact.

  • An antibacterial surface on dental implants, based on the photocatalytic bactericidal effect

    • BACKGROUND: It is well known that the moderately roughened surfaces of dental implants enhance direct bone-implant contact. However, rough implant surfaces, as compared to smooth surfaces, are thought to pose a higher risk of bacterial infection when exposed to the oral cavity. PURPOSE: This study was focused on evaluating the photocatalytic bactericidal effects of anatase titanium dioxide (TiO(2)) on gram-negative anaerobic bacteria known to be associated with periimplantitis. MATERIALS AND METHODS: A film of photocatalytic anatase TiO(2) was added onto the surface of commercially pure titanium disks by plasma source ion implantation (PSII) followed by annealing. The photocatalytic properties of the film were confirmed by the degradation of methylene blue. Actinobacillus actinomycetemcomitans and Fusobacterium nucleatum cells were incubated anaerobically and seeded on the disk. The disks were then exposed to ultraviolet A (UVA) illumination from black light in an anaerobic environment. After illumination, the number of viable cells was counted in terms of colony-forming units. RESULTS: The anatase TiO(2) film added by the PSII method and annealing exhibited a strong photocatalytic reaction under UVA illumination. The viability of both types of bacteria on the photocatalytic TiO(2) film was suppressed to less than 1% under UVA illumination within 120 minutes. CONCLUSION: The bactericidal effect of the TiO(2) photocatalyst is of great use for sterilizing the contaminated surface of dental implants.

    • PURPOSE: This study was undertaken to investigate surface properties of surface-modified titanium implants in terms of surface chemistry, morphology, pore characteristics, oxide thickness, crystal structure, and roughness. MATERIALS AND METHODS: An oxidized, custom-made Mg implant, an oxidized commercially available implant (TiUnite), and a dual acid-etched surface (Osseotite) were investigated. Surface characteristics were evaluated with various surface analytic techniques. RESULTS: Surface chemistry showed similar fingerprints of titanium oxide and carbon contaminant in common for all implants but also revealed essential differences of the elements such as about 9 at% Mg for the Mg implant, about 11 at% P for the TiUnite implant and about 12 at% Na for the Osseotite implant. Surface morphology of the Mg and TiUnite implants demonstrated a duplex oxide structure, ie, an inner barrier layer without pores and an outer porous layer with numerous pores, whereas the Osseotite implant revealed a crystallographically etched appearance with pits. The diameter and depth of pores/pits was < or = 2 microm and < or = 1.5 microm in the Mg implant, < or = 4 microm and < or = 10 microm in the TiUnite implant, and < or = 2 microm and < or = 1 microm in the Osseotite implant, respectively. Oxide layer revealed homogeneous thickness, about 3.4 microm of all threads in the Mg implants. On the contrary, TiUnite showed heterogeneous oxide thickness, about 1 to 11 microm, which gradually increased with thread numbers. Crystal structure showed a mixture of anatase and rutile phase for the Mg implants. With respect to roughness, Sa showed 0.69 microm in the Mg implant, 1.35 microm in the TiUnite implant, and 0.72 microm in the Osseotite implant. CONCLUSIONS: Well-defined surface characterization may provide a scientific basis for a better understanding of the effects of the implant surface on the biological response. The surface-engineered implants resulted in various surface characteristics, as a result of different manufacturing techniques.

    • The early biological response at titanium (Ti), copper (Cu)-coated Ti and sham sites was evaluated in an in vivo rat model. Material surface chemical and topographical properties were characterized using Auger electron spectroscopy, energy dispersive X-ray spectroscopy and interferometry, respectively. The number of leukocytes, cell types and cell viability (release of lactate dehydrogenase) were determined in the implant-interface exudate. The contents of activated nuclear transcription factor NF-kappaB, interleukin-6 (IL-6) and interleukin-10 (IL-10) were determined by enzyme linked immunosorbent assay. An increase in the number of leukocytes, in particular, polymorphonuclear leukocytes, was observed between 12 and 48 h around Cu. A marked decrease of exudate cell viability was found around Cu after 48 h. The total amounts of activated NF-kappaB after 12 h was highest in Ti exudates whereas after 48 h the highest amount of NF-kappaB was detected around Cu. The levels of cytokine IL-6 were consistently high around Cu at both time periods. No differences in IL-10 contents were detected, irrespective of material/sham and time. The results show that materials with different toxicity grades (titanium with low and copper with high toxicity) exhibit early differences in the activation of NF-kappaB, extracellular expression and secretion of mediators, causing major differences in inflammatory cell accumulation and death in vivo.
