  • Prevalence and risk factors of fatty liver in Portuguese adults

    • Background Prevalence of fatty liver (FL) and nonalcoholic fatty liver disease (NAFLD) depends mainly on obesity, diabetes and genetic factors. FL and NAFLD prevalence was evaluated in Portuguese adult population and correlated with several risk factors and related mortality data, within the same period. Materials and methods A cross-sectional, population-based multicenter study, voluntary and randomly selected in 834 Portuguese adults (18-79 years). Participants were evaluated after 12-hour fasting. Anthropometric data, past history including alcohol consumption, and associated diseases were registered. Blood samples were collected for biochemical testing. Dietary intake was evaluated using a semi-quantitative food frequency questionnaire. Presence of FL was evaluated using ultrasound, and NAFLD was diagnosed after exclusion of other causes for liver disease. Results Adjusted prevalence of FL and NAFLD was 37.8% and 17.0%, respectively. FL individuals were older, more frequently males, with increased probability of having obesity, diabetes or harmful alcohol consumption (HAC). NAFLD individuals were also older, but had a similar sex distribution and an increased probability of obesity and diabetes. In both groups, no differences were found regarding dietary pattern or physical activity. During the same time period, nonalcoholic steatohepatitis (NASH) liver-related deaths in Portugal were 0.105/100 000, while alcohol-related liver disease mortality was 6.790/100 000. Conclusion The large spectrum of FL was present in more than one third of the population, although only less than half could be classified as NAFLD. Other significant risk factors, such as HAC, are probably implicated in FL, explaining the low NASH-related mortality compared with the high alcohol-related mortality during the same time period.

  • "Utopias Are for Those Who Cannot Build": The Structural Philosophy of the Swedish National Board of Building

    • Structuralism in architecture was a widespread international phenomenon in the post-war decades. It was an avant-garde architecture, in many cases even utopian. In contrast to this, the structural philosophy of the Swedish National Board of Building was outspokenly pragmatic. This article, based on documents and interviews with the architects involved, gives the background of the National Board's interest in the chronological dimension of architecture. The National Board was the largest client in Sweden for design and building and experienced managers of a large building stock. In the mid-1960s, they developed, in cooperation with consultants, a building box for office buildings. They gladly showed a lack of interest or downright scepticism towards international structuralism: Utopias are for those who cannot build. Two of the main involved practices were A4 and ELLT, later merged into Coordinator architects, and from early on focused on an architecture of change. Two of their iconic projects from the late 1960s are the large office block Garnisonen and IBM Nordic Education Center. They are examples of a consequence architecture, very clearly ideas based. For a period around 1970 the pragmatic theory led to radical projects. Dogmatic theorising was part of the game as it was said about contemporary art.

  • Seminal Plasma : Relevant for Fertility?

    • Seminal plasma (SP), the non-cellular component of semen, is a heterogeneous composite fluid built by secretions of the testis, the epididymis and the accessory sexual glands. Its composition, despite species-specific anatomical peculiarities, consistently contains inorganic ions, specific hormones, proteins and peptides, including cytokines and enzymes, cholesterol, DNA and RNA-the latter often protected within epididymis- or prostate-derived extracellular vesicles. It is beyond question that the SP participates in diverse aspects of sperm function pre-fertilization events. The SP also interacts with the various compartments of the tubular genital tract, triggering changes in gene function that prepares for an eventual successful pregnancy; thus, it ultimately modulates fertility. Despite these concepts, it is imperative to remember that SP-free spermatozoa (epididymal or washed ejaculated) are still fertile, so this review shall focus on the differences between the in vivo roles of the SP following semen deposition in the female and those regarding additions of SP on spermatozoa handled for artificial reproduction, including cryopreservation, from artificial insemination to in vitro fertilization. This review attempts, including our own results on model animal species, to critically summarize the current knowledge of the reproductive roles played by SP components, particularly in our own species, which is increasingly affected by infertility. The ultimate goal is to reconcile the delicate balance between the SP molecular concentration and their concerted effects after temporal exposure in vivo. We aim to appraise the functions of the SP components, their relevance as diagnostic biomarkers and their value as eventual additives to refine reproductive strategies, including biotechnologies, in livestock models and humans.

    • Developing distributed real-time systems is a complex task that has historically entailed specialized handcraft. In this paper, we propose a retrospective on the (r)evolutionary changes that led to the transition from low-level programming to industrial full-fledged model-based development embodied by the Rubus Component Model and its tool-ecosystem. We focus on the needs, challenges, and solutions of a 15-year-long evolution journey of a software development approach that has gone from low-level and manual programming to a highly automated environment offering modeling, analysis, and development of vehicular software systems with multi-criticality for deployment on single- and multi-core platforms.

    • This study is based on an ethnography that was carried out in the Transport Programme (TP) in a Swedish upper secondary school (in this paper referred to as Rockmeadows High). The research is part of a larger project focusing on discourses on Intelligence in Swedish upper secondary school, and how these are produced and used in different educational contexts. The title of the article quotes Emily, a hardworking and high-achieving TP student. Emily is disappointed that the academic courses much of the time operate on a rote level, and that teachers' expectations on the students in the programme generally are quite low. The present study also indicates, in line with several studies of vocational education in Sweden, that academic courses in vocational programmes often seem to provide scarce opportunity for theoretical learning and higher order thinking. Others suggest that the vocational courses present better conditions for such learning. The present article explores learning and instruction in different subjects and the conditions that are structuring them. It discusses the possibility that performativity pressure is one of the structuring forces. 'Performativity' is used here to refer to the notion that individuals and systems are valued based on their measured performances in regard to standards, and identified by those standards. Thus ascribed values become ends in themselves and render the use value of knowledge subordinate to the exchange value. This means that not only knowledge but also the pedagogical interaction and relationships become commodified. This is exactly what happens when new managerialism and economic rationality are imposed on the education system, as it has been throughout the Western world in the last decades. Swedish educational policy has gone far down this road, focusing measurable outcomes, individual choice and competition on all levels as means for quality. Still, not all spheres of education are dominated by the economistic rationality, as this articles aims to demonstrate and discuss. © 2012 Taylor and Francis Group, LLC.

    • Gaussian Mixture Models (GMMs) have been the dominant technique used for modeling in speaker recognition systems. Traditionally, the GMMs are trained using the Expectation Maximization (EM) algorithm and a large set of training samples. However, the convergence of the EM algorithm to a global maximum is conditioned on proper parameter initialization, a large enough training sample set, and several iterations over this training set. In this work, a Sample Iterative Likelihood Maximization (SILM) algorithm based on a stochastic descent gradient method is proposed. Simulation results showed that our algorithm can attain high loglikelihoods with fewer iterations in comparison to the EMalgorithm. A maximum of eight times faster convergence rate can be achieved in comparison with the EM algorithm.
