  • Style-transfer counterfactual explanations : An application to mortality prevention of ICU patients

    • In recent years, machine learning methods have been rapidly adopted in the medical domain. However, current state-of-the-art medical mining methods usually produce opaque, black-box models. To address the lack of model transparency, substantial attention has been given to developing interpretable machine learning models. In the medical domain, counterfactuals can provide example-based explanations for predictions, and show practitioners the modifications required to change a prediction from an undesired to a desired state. In this paper, we propose a counterfactual solution MedSeqCF for preventing the mortality of three cohorts of ICU patients, by representing their electronic health records as medical event sequences, and generating counterfactuals by adopting and employing a text style-transfer technique. We propose three model augmentations for MedSeqCF to integrate additional medical knowledge for generating more trustworthy counterfactuals. Experimental results on the MIMIC-III dataset strongly suggest that augmented style-transfer methods can be effectively adapted for the problem of counterfactual explanations in healthcare applications and can further improve the model performance in terms of validity, BLEU-4, local outlier factor, and edit distance. In addition, our qualitative analysis of the results by consultation with medical experts suggests that our style-transfer solutions can generate clinically relevant and actionable counterfactual explanations.

    • Just like a living organism, also the project has an anatomy, a life span and a purpose. A project has its given actors, given timeframe and phases, and a given goal. Towards the project’s completion, all these factors must intertwine perfectly, otherwise the project’s success will be challenged. Uncovering how these intricacies are held in place has been the epistemological foundation of this book. However, rather than relying on formal project descriptions, reports and evaluations, we chose a different way, autoethnography. By exploring the implicit knowledge that emerges during the process of running a complex 21st-century sustainability project, we wanted to better understand what makes it tick, halt or change its course. Taking cue from the various project actors’ personal reflections on their own role within the project has helped illuminate a complex transdisciplinary co-creation process from the perspective of the individual. We conclude that if we truly want to attain sustainability transitions, then the organisation, the methods and the modes of thinking utilised in projects must differ from the traditional ones. But reaching a breakpoint for behavioural change must be rooted in interactions where the participating individuals and organisations have a common understanding of the complex challenges that are entailed in running a sustainability project in the 21st century.

    • The application of palladium complexes of a modular series of axially chiral phosphinamine ligands, the Quinazolinaps, to the enantioselective alkylation of 1,3-diphenyl-2-propenyl acetate with dimethyl malonate and methyl dimethyl malonate is described. Complete conversions and enantiomeric excesses of up to 91% were obtained. To elucidate the solution structure of these complexes and their dynamic behaviour, 2D COSY and NOESY NMR experiments were carried out. An X-ray crystal structure of a palladacycle derived from 2-phenylQuinazolinap which possesses two Pd3Cl5 units is shown. Computational studies were also undertaken to allow qualitative predictions of diastereomeric ratios. The observed enantioselectivity was then rationalised in terms of combined spectroscopic and theoretical data. The catalytic results obtained are best interpreted by the reaction proceeding with nucleophilic attack on the allyl trans to the phosphorus donor atom of the major diastereomeric intermediate.     • This paper deals with Hans-Georg Gadamer’s theory of the way in which cognition depends on its historical context. In section I and II the historical background of this theory is briefly described. In section III Gadamer’s claim that context-dependence involves unreflected presuppositions (Vorurteile) is discussed; throughout the paper, different senses of this claim are distinguished. Section IV examines Gadamer's corresponding claim that these presuppositions are of a general kind, with the result that the manifold of seemingly opposed and more specific views which rely on them is overrated by individuals sharing a historical context.

    • This paper aims at providing a provisional classification of different types of multimodal metaphors belonging to the verbo-pictorial variety found in comics, based on the relation between written and visual language, as two modes of human communication commonly combined in everyday life. Starting from the theoretical background on multimodal metaphors and comics studies, and using a corpus comprising comics and graphic novels published by both mainstream and independent American houses, the authors propose the following three types of relations which occur in multimodal metaphors in this medium: (1) image-dominant metaphors, (2) text-dominant metaphors, and (3) complementary metaphors.

    • Objectives: Identification of genetic targets specific to Neisseria gonorrhoeae for use in molecular detection methods has been a challenge. The porA pseudogene in N gonorrhoeae has been commonly used but recently gonococcal isolates giving a negative result in these PCRs have been reported. Here we describe the characterisation of two such gonococcal isolates received by the reference service at the Health Protection Agency, London, England.Methods: Phenotypic characterisation was achieved using conventional biochemical and immunological tests, matrix-assisted laser desorption/ionisation time-of-flight mass spectrometry (MALDI-TOF-MS), antimicrobial susceptibility testing, serovar determination and detection of meningococcal PorA using monoclonal antibody 4BG4-E7. Genetic species confirmation was determined using commercial and in house PCRs and 16S rRNA gene sequencing. Molecular typing using the N gonorrhoeae multi-antigen sequence typing (NG-MAST) and multilocus sequence typing (MLST) was performed. The DNA sequence of the full-length gonococcal porA pseudogene was determined and compared with published sequences.Results: Both isolates were confirmed, biochemically and immunologically as N gonorrhoeae, but repeatedly gave negative results with two in house real-time PCR assays for the porA pseudogene. Further characterisation of these isolates identified the presence of a meningococcal porA sequence and showed these isolates belong to serovar Bropyst, and to NG-MAST sequence type (ST) 5967 and MLST ST1901.Conclusions: Gonococcal isolates that give false negative results with porA pseudogene PCR assays have now been identified in four countries, three of which are in Europe, and do not appear clonal. This report highlights the genetic diversity of N gonorrhoeae, which remains a challenge for the molecular detection methods.
