  • Higher Total Physical Activity is Associated with Lower Arterial Stiffness in Swedish, Young Adults : The Cross-Sectional Lifestyle, Biomarkers, and Atherosclerosis Study

    • Purpose: Arterial stiffness describes the rigidity of the arterial walls and is associated withrisk factors for cardiovascular disease (CVD). Arterial stiffness predicts future events andmortality, and the predictive value is stronger in younger versus older subjects. The aims ofthe present study were, firstly, to present data on physical activity (PA) and time spentsedentary, in the population of Swedish, young adults. Secondly, to explore the associationbetween PA and arterial stiffness.Material and Methods: Self-reported healthy, non-smoking, Swedish, young adults, 18–25years old, participated in the cross-sectional Lifestyle, Biomarkers and Atherosclerosis(LBA) study. The daily PA was objectively measured with an accelerometer for 1 week.Of the 834 participants, 658 individuals had valid registrations. The arterial stiffness measures, pulse wave velocity (PWV) and augmentation index (AIx) were measured withapplanation tonometry.Results: Women were on overall more physically active than men, they spent 214 min/dayin light PA (LPA) compared to men who spent 202 min/day. Women took significantly moresteps per day than men, 7796 vs 7336 steps/day, and spent less time sedentary, 523 min/day,compared to men who spent 547 min/day sedentary. In total, 76% of the individuals spent onaverage at least 30 minutes per day in the recommended moderate and vigorous PA (MVPA).Lower arterial stiffness was associated with more MVPA and total PA in the total population.Conclusion: We conclude that in this age group of young, self-reported healthy adults18–25 years, it is important to highlight the health-enhancing possibilities of time spent inphysical activity on the vascular function, measured as PWV and AIx. It is of high relevancein a public health perspective to expand preventive efforts beyond the high-risk groups andencourage young adults to be physically active.

  • Exercise and internet-based cognitive-behavioural therapy for depression : Multicentre randomized controlled trial with 12-month follow-up

    • Background Evidence-based treatment of depression continues to grow, but successful treatment and maintenance of treatment response remains limited.Aims To compare the effectiveness of exercise, internet-based cognitive-behavioural therapy (ICBT) and usual care for depression.Method A multicentre, three-group parallel, randomised controlled trial was conducted with assessment at 3 months (posttreatment) and 12 months (primary end-point). Outcome assessors were masked to group allocation. Computer-generated allocation was performed externally in blocks of 36 and the ratio of participants per group was 1:1:1. In total, 945 adults with mild to moderate depression aged 18-71 years were recruited from primary healthcare centres located throughout Sweden. Participants were randomly assigned to one of three 12-week interventions: supervised group exercise, clinician-supported ICBT or usual care by a physician. The primary outcome was depression severity assessed by the Montgomery-Åsberg Depression Rating Scale (MADRS).Results The response rate at 12-month follow-up was 84%. Depression severity reduced significantly in all three treatment groups in a quadratic trend over time. Mean differences in MADRS score at 12 months were 12.1 (ICBT), 11.4 (exercise) and 9.7 (usual care). At the primary end-point the group x time interaction was significant for both exercise and ICBT. Effect sizes for both interventions were small to moderate.Conclusions The long-term treatment effects reported here suggest that prescribed exercise and clinician-supported ICBT should be considered for the treatment of mild to moderate depression in adults.

    • ABSTRACT: BACKGROUND: Gait-lines, or the co-ordinates of the progression of the point of application of the vertical ground reaction force, are a commonly reported parameter in most in-sole measuring systems. However, little is known about what is considered a "normal" or "abnormal" gait-line pattern or level of asymmetry. Furthermore, no reference databases on healthy young populations are available for this parameter. Thus the aim of this study is to provide such reference data in order to allow this tool to be better used in gait analysis. METHODS: Vertical ground reaction force data during several continuous gait cycles were collected using a Computer Dyno Graphy in-sole system(R) for 77 healthy young able-bodied subjects. A curve (termed gait-line) was obtained from the co-ordinates of the progression of the point of application of the force. An Asymmetry Coefficient Curve (AsC) was calculated between the mean gait-lines for the left and right foot for each subject. AsC limits of +/- 1.96 and 3 standard deviations (SD) from the mean were then calculated. Gait-line data from 5 individual subjects displaying pathological gait due to disorders relating to the discopathy of the lumbar spine (three with considerable plantarflexor weakness, two with considerable dorsiflexor weakness) were compared to the AsC results from the able-bodied group. RESULTS: The +/- 1.96 SD limit suggested that non-pathological gait falls within 12-16% asymmetry for gait-lines. Those exhibiting pathological gait fell outside both the +/- 1.96 and +/- 3SD limits at several points during stance. The subjects exhibiting considerable plantarflexor weakness all fell outside the +/- 1.96SD limit from 30-50% of foot length to toe-off while those exhibiting considerable dorsiflexor weakness fell outside the +/- 1.96SD limit between initial contact to 25-40% of foot length, and then surpassed the +/- 3SD limit after 55-80% of foot length. CONCLUSION: This analysis of gait-line asymmetry provides a reference database for young, healthy able-bodied subject populations for both further research and clinical gait analysis. This information is used to suggest non-pathological gait-line asymmetry pattern limits, and limits where detailed case analysis is warranted.

  • Daily eating events among co-living and single-living diseased older men

    • Objectives: To analyse, describe and compare the frequency and energy intake of eating events, including specific food items, among diseased older men living in ordinary housing. Design: Descriptive and explorative. Setting: Interviews were performed in the participants’ home. Participants: Thirty-five co-living and 26 single-living men, 64-88 years of age. Participants had one of three chronic diseases associated with difficulties in buying and preparing food and with difficulties related to the meal situation: Parkinson’s disease, rheumatoid arthritis or stroke. Measurements: A repeated 24-h recall was used to assess food intake and meal patterns. Results: Eating events were distributed over a 24-h period. Co-living men had a higher (p=0.001) number of eating events/day; both hot and cold eating events were consumed more frequently. There was no difference between groups concerning energy intake. Co-living men had more often hot eating events cooked from raw ingredients (p=0.001) and more various vegetables/roots (p=0.003) included in such eating events. Conclusion: Single-living men may constitute a vulnerable group from a nutritional perspective, while co-living men, besides the pleasure of eating together, seem to get support with food and eating events from their partners. Hence, the group of single-living men, particularly those with a disability, should receive particular attention with regard to possible food-related difficulties.

    • The aim of the present study was to address the question of how older men with somatic diseases living in ordinary housing approach the question of food-related activities. Further, any food-related activity adaptations consequent to effects of diseases and of alterred life circumstances were explored. Interviews were conducted with a purposeful sample of 18 co-living and singel-living men, 64-84 years old. They were diagnosed with Parkinson's disease, rhematoid arthritis, or stroke. In the analysis a thematic framework was used. The findings revealed three food-related approaches, namely 'Cooking as a pleasure', describing joy in cooking; 'Cooking as a need', indicating no habit or skills in cooking; and ' Food is served', that is being served meals by a partner. It was found that gender-related roles in particular, but also changed life circumstances, activity limitations, personal interests, and a wish to maintain continuity and independence, affected the men's approaches to these activities. This knowledge may be useful in attempts to facilitate and support food-related activities among elderly men with diseases. Health-care efforts in promoting food-related activities should preferably be individualised in respect to older men's approach to these activities.
