Tanzanian Nursing Students' Experiences of Student Exchange in Sweden : A Qualitative Case Study

- IntroductionNeeds within healthcare are changing and nurses require new skills and knowledge in global nursing. Student exchange programs in global contexts provide an opportunity to develop the necessary skills. ObjectiveThe aim of this study was to describe Tanzanian nursing students' experiences of student exchange in Sweden. MethodsA qualitative design was used for this empirical study. Semistructured interviews were conducted with six Tanzanian nursing students who had participated in student exchange in Sweden. The participants were recruited by purposeful sampling. Inductive reasoning and qualitative content analysis were applied. ResultsFour main themes were formed; new roles, experience a new culture, establish new competencies, and global work ambitions. The findings revealed that the students experienced new approaches in Sweden, giving them new competencies and understanding. Furthermore, they increased their global perspectives on nursing and interest in working with global health issues, but they also experienced challenges in the new environment. ConclusionThe present study showed that Tanzanian nursing students benefitted from their student exchange, both personally, as well as for their future careers as nurses. More research is needed in examining nursing students from low-income countries participating in student exchange in high-income countries.

- Background: From a management point of view, there are many different approaches from which to choose to engage staff members in initiatives to improve performance.Objective: The present study evaluated how two different types of improvement strategies facilitate and encourage involvement of different professional groups in health-care organizations.Methods/Design: Empirical data of two different types of strategies were collected within an improvement project in a County Council in Sweden. The data analysis was carried out through classifying the participants' profession, position, gender, and the organizational administration of which they were a part, in relation to their participation.Setting: An improvement project in a County Council in Sweden.Participants: Designed Improvement Processes consisted of n=105 teams and Intrapreneurship Projects of n=202 projects.Intervention: Two different types of improvement strategies, Designed Improvement Processes and Intrapreneurship Projects.Main Outcome Measures: How two different types of improvement strategies facilitate and encourage involvement of different professional groups in healthcare organizations.Results: Nurses were the largest group participating in both improvement initiatives. Physicians were also well represented, although they seemed to prefer the less structured Intrapreneurship Projects approach. Assistant nurses, being the second largest staff group, were poorly represented in both initiatives. This indicates that the benefits and support for one group may push another group aside.Conclusions: Managers need to give prerequisites and incentives for staff who do not participate in improvements to do so. Comparisons of different types of improvement initiatives are an underused research strategy that yields interesting and thoughtful results.

- This study aimed to provide an understanding of nursing students’ self-reported eHealth literacy in Sweden and Poland. This cross-sectional multicentre study collected data via a questionnaire in three universities in Sweden and Poland. Descriptive statistics, the Spearman’s Rank Correlation Coefficient, Mann–Whitney U, and Kruskal–Wallis tests were used to analyse different data types. Age (in the Polish sample), semester, perceived computer or laptop skills, and frequency of health-related Internet searches were associated with eHealth literacy. No gender differences were evidenced in regard to the eHealth literacy. Regarding attitudes about eHealth, students generally agreed on the importance of eHealth and technical aspects of their education. The importance of integrating eHealth literacy skills in the curricula and the need to encourage the improvement of these skills for both students and personnel are highlighted, as is the importance of identifying students with lacking computer skills.

Experiences of long-term home care as an informal caregiver to a spouse : Gendered meanings in everyday life for female carers

- Background. One of the most common circumstances in which a woman gradually steps into a long-term caregiver role at home involves caring for a spouse suffering from dementia. Little attention has been paid to examining the experiences and motivations of such caregivers from a feminist perspective. Methods. Twelve women, all of whom were informal caregivers to a partner suffering from dementia, were interviewed on the following themes: the home, their partner's disease, everyday life, their relationship and autonomy. The results of these interviews were analysed in relation to gender identity and social power structures using a feminist perspective. Results. The findings of this study show that the informants frequently reflected on their caregiving activities in terms of both general and heteronormative expectations. The results suggest that the process of heteropolarisation in these cases can be an understood as a consequence of both the spouse's illness and the resulting caring duties. Also, the results suggest that the act of caring leads to introspections concerning perceived 'shortcomings' as a caregiver. Finally, the results indicate that it is important to recognise when the need for support in day-to-day caring is downplayed. Conclusions. Women view their caregiving role and responsibilities as paramount; their other duties, including caring for themselves, are deemed less important. We stress that the intense commitment and responsibilities that women experience in their day-to-day caring must be acknowledged and that it is important for healthcare professionals to find mechanisms for providing choices for female caregivers without neglecting their moral concerns. Implications for practice. Female carers face difficulties in always living up to gendered standards and this need to be considered when evaluating policies and practices for family carers.

His helping hands-adult daughter's perceptions' of fathers with caregiving responsibility

- Women's position as informal carers has been taken for granted in social policy and social professions, while relatively few discussions have elaborated on caring as a later life activity for men and the impact on family care. This study explores the processes connected to informal caregiving in later life through the position of adult daughters of older fathers engaged with long-term caregiving responsibilities for a partner. A sample of eight daughters, with fathers having primary caregiving responsibility for their ill partners was recruited and in-depth interviews were carried out and analysed according to qualitative procedures. The daughters' descriptions of their relationships with their fathers show that being an older man who engages in caring can have a positive outcome on relations. Even if some of the daughters have doubts about their fathers “masculine authenticity”, all of them appear to cherish “his helping hands” as a carer and closer more intimate relationships with their fathers. Caring for an old and frail spouse may potentially present alternative ways of being a man beyond traditional ‘male activities’ and that caring might also sometimes involve a re-construction of gender identities. It is suggested that social work professionals may use a gendered understanding to assess and work strategically with daughters and other family members who support caring fathers.

- The present article refers to a case study on the Kenyan Government’s Digital Villages Project (DVP). The Kenyan Government, together with external stakeholders and private contractors, is increasing their ICT investments to provide the entire population with information and communication regardless of demographic factors. In the Kenyan context, digital villages are what normally other countries refer to as telecentres, i.e. a centre that provides services with regard to Internet and telecommunication. In this case, the digital villages also offer education, learning, and e-Government. The present study wants to learn whether DVP is accessible, and appropriate to women and girls in resource-poor environments and, thus, successful. The following questions guided the study: 1. Who are the users of Pasha Centres? 2. How and for what purposes are Pasha Centres used? 3. In what way do Pasha Centres consider local needs (e.g. education, literacy, job, and diversity)? 4. What do users and managers do to encourage female users? The study is built upon observations and interviews. The results show that male users generally believe that women have a lack of knowledge and understanding of ICT. The results also show that what is said by the government is not fully implemented at the local levels. The authors believe, despite this, that DVP has the potential to serve the population in vulnerable areas and that the government should continue focusing on similar projects.

- This paper introduces the notion of community gardens as multipurpose, technological systems with dynamic boundaries. It also strives to revise the notion of food security and health in regards to such systems. This is a cross-cultural project where twelve community gardens located in urban regions within three different countries were examined. This paper explores common tendencies rather than diversities or differences. The paper describes a behavioural chain that depends on various factors and affects various types of health. It is suggested that a community garden has the potential to serve as a link between the past and the present. While a community garden may provide food, it is suggested that it may also encourage social interactions, knowledge sharing, and relaxation. The cultivation of food occurs, but it may not always be the sole purpose.

- This paper draws attention to the urgency of taking into consideration women’s genderspecific needs and interests in the implementation of community-based ICT projects in lifelong learning. We set out this research to understand the constraints of information and communications technology (ICT) in lifelong learning on disadvantaged women. National statistics and data from field studies were used. The constraints and their consequences at national level are often a result of national policies and regulations – or lack thereof – while the constraints and their consequences at local/regional level involve everyday-life occurrences that are present in women's immediate surroundings. Hence, an understanding of both levels is critical. This research is valuable for stakeholders delving into issues of development intervention using ICTs, not only in Kenya but in a broader, global perspective.

Factors affecting HIV disclosure among partners in Morongo, Tanzania

- Background: Human immunodeficiency virus (HIV) remains a major concern globally and locally. Married couples and those in stable relationships account for the highest percentage of new HIV and acquired immune deficiency syndrome (AIDS) infections. The rate of HIV disclosure among couples is low and affected by both known and unknown factors. The aim of this study was to describe the reasons for HIV status (non)disclosure among partners in Morongo. Methods: A sampling containing two stages was used in this quantitative, exploratory, and descriptive study to select the 100 participants. The location was a Care and Treatment Clinic in the Morogoro municipality. Results: The participants had a moderate level of knowledge about the importance of HIV serostatus disclosure. Female genital mutilation was the most mentioned (44%) custom affecting disclosure. The participants’ level of knowledge about their partner's HIV status was also moderate (28%). Nitty-six percent had not disclosed due to fear of divorce and 98% due to fear of loss of financial support. Cultural factors such as traditional practices (95%) were also a major reason that hindered disclosure. Conclusions: Interventions to address the negative attitudes are necessary to promote HIV disclosure and, in turn, better adherence to psychological adjustment therapy and reduction in the risk of HIV transmission among couples.
