  • Malpractice claims regarding calls to Swedish telephone advice nursing : what went wrong and why?

    • We analysed the characteristics of all malpractice claims arising out of telephone calls to Swedish Healthcare Direct (SHD) during 2003-2010 (n = 33). The National Board of Health and Welfare's (NBHW) investigations describing the causes of the malpractice claims and the healthcare providers' reported measures were analysed using Qualitative Content Analysis. The original telephone calls themselves, which had been recorded, were analysed using the Roter Interaction Analysis System (RIAS). Among the 33 cases, 13 patients died and 12 were admitted to intensive care. Failure to listen to the caller (n = 12) was the most common reason for malpractice claims, and work-group discussion (n = 13) was the most common measure taken to prevent future re-occurrence. Male patients (n = 19) were in the majority, and females (n = 24) were the most common callers. The most common symptoms were abdominal (n = 11) and chest pain (n = 6). Telenurses followed up on caller understanding in six calls, and mainly used closed-ended questions. Despite the severity of these malpractice claims, the measures taken mainly addressed active failure, rather than the latent conditions. Third-party communication should be regarded as a risk. When callers make repeated contacts, telenurses need to re-evaluate their need for care.

  • Use of technical skills and medical devices among new registered nurses : A questionnaire study

    • Background: One comprehensive part of nursing practice is performing technical skills and handling of medical equipment. This might be challenging for new registered nurses (RNs) to do in patient-safe way.Objectives: The aim of this study was to describe and compare the extent to which new RNs perform various technical skills and handle medical devices in different settings, and to investigate their possibility for continued learning in this respect. A further aim was to describe their perceptions of incident reporting related to technical skills and medical devices.Design: A cross-sectional study with descriptive and comparative design.Participants: RNs who recently graduated from a nursing programme at three Swedish universities and had worked as a RN for up to 1year were included in the study (n=113, response rate 57%).Method: Data were collected by means of a postal questionnaire.Results: Half of the RNs reported that they performed several of the listed tasks every day or every week, regardless of workplace. These tasks were most frequently performed in surgical departments. The majority of the participants (76%) stated a need of continued practical training. However, less than half of them (48%) had access to a training environment. Several participants (43%) had been involved in incidents related to technical skills or medical devices, which were not always reported. Nearly a third of the participants (31%) did not use the existing guidelines when performing technical skills, and reflection on performance was uncommon.Conclusions: This study highlights the importance of shared responsibilities between nurse educators and health care employers to provide learning opportunities for new RNs in technical skills, to maintain patient safety. To increase the safety culture where nursing students and new RNs understand the importance of using evidence-based guidelines and taking a reflective approach in the performance of technical tasks is needed.

  • Walking the bridge : Nursing students' learning in clinical skill laboratories

    • Despite an increasing focus on simulation as a learning strategy in nursing education, there is limited evidence on the transfer of simulated skills into clinical practice. Therefore it's important to increase knowledge of how clinical skills laboratories (CSL) can optimize students' learning for development of professional knowledge and skills, necessary for quality nursing practice and for patient safety. Thus, the aim was to describe nursing students' experiences of learning in the CSL as a preparation for their clinical practice. Interviews with 16 students were analysed with content analysis. An overall theme was identified walking the bridge in which the CSL formed a bridge between the university and clinical settings, allowing students to integrate theory and practice and develop a reflective stance. The theme was based on categories: conditions for learning, strategies for learning, tension between learning in the skills laboratory and clinical settings, and development of professional and personal competence. The CSL prepared the students for clinical practice, but a negative tension between learning in CSL and clinical settings was experienced. However, this tension may create reflection. This provides a new perspective that can be used as a pedagogical approach to create opportunities for students to develop their critical thinking.

    • The aim was to describe nursing home (NH) caregivers' work experiences while receiving education and clinical supervision for 2 years. Working in elder care seems to be losing its attraction especially with organizational changes, cutbacks and changes in work place conditions. Clinical supervision has been reported to increase job satisfaction and creativity. Semi-structured interviews from caregivers working at an NH in Sweden were conducted, at the start and again at 12 and 24 months. At about 12 months the caregivers were informed of planned cutbacks. Content analysis was the method used to analyse the interviews from seven caregivers who participated throughout the entire period. Findings show that the value of a caring milieu was one category generated by the subcategories: experiences related to work activities and changes, and experiences related to relationships. The value of knowledge was the other category that was influenced by the experiences related to the different backgrounds and the experiences related to increased knowledge gained from the support through education and clinical supervision. The categories contained positive as well as negative influences on care. The initial focus on practical duties associated with the opening of the NH shifted towards caregiver activities with the elders they spoke warmly about. After 2 years the caregivers' willingness to care continued despite their disappointment in the worsened working conditions. The main theme that resulted was: Despite shattered expectations a willingness to care for elders remained. Continued education and clinical supervision seems to be one factor behind the retained willingness. These findings demonstrate that support and caregiver involvement in educational programmes are important during times of change and when disappointments arise in the workplace.

    • Functionality for various services of scheduling algorithms is typically provided as extensions to a basic paradigm, intertwined in the kernel architecture. Thus, scheduling services come in packages around single paradigms, fixed to a certain methodology and kernel architecture. Temporal constraints of applications are addressed by a combination of scheduler and system architecture. Consequently, changing system architecture results in a complete rescheduling of all tasks, calling for a new cycle of analysis and testing from scratch, although a schedule meeting all temporal constraints already existed.We propose a component based architecture for schedule reuse. Instead of tying temporal constraints, scheduler, and system architecture together, we provide methods which allow for the reuse of existing schedules on various system architectures. In particular, we show how a schedule developed for table driven, dynamic or static priority paradigm can be reused in the other schemes.We address an architecture to disentangle actual scheduling from dispatching and other kernel routines with a small interface, suited for a variety of scheduling schemes as components.

  • The concept of welfare technology in Swedish municipal eldercare

    • Purpose:  An ageing population presents a challenge for municipal eldercare in Sweden due to difficulties recruiting staff and there being a strained economy. A strategy involving welfare technology is presented as one such solution. An important group to carry out this strategy involves those who work with welfare technology in municipal eldercare. In this paper we describe their perception of welfare technology, and the challenges and opportunities they perceive in utilizing it. Methods: A self-administered online questionnaire was distributed to all Swedish municipalities and answered by 393 respondents. Analyses show that the respondents were representative of the different professions who work with welfare technology within municipal eldercare.Results: Welfare technology was perceived as being more reliable and safer than humans with regards to supervisions and reminders. The respondents acknowledged factors that slowed down the implementation of welfare technology in municipal eldercare organizations, such as resistance to change, lack of finances, lack of supporting evidence, lack of infrastructure, high staff turnover, difficulties with procurement and uncertainties about responsibility and laws.Conclusions: We found that the people who work with and make decisions about welfare technology in municipal eldercare organizations were generally very positive about the deployment and use of such technology, but there appear to be problems within municipal eldercare organizations to realize this vision. The lack of structured implementation processes and coherent evaluation models indicates inequality of the access to welfare technology and, as a result, even though Swedish eldercare is publicly funded, the availability of welfare technologies and their usage differ between municipalities.

    • BACKGROUND: Despite the substantial number of older adults suffering from gastrointestinal (GI) symptoms little is known regarding the character of these complaints and whether they are associated with an altered intestinal barrier function and psychological distress. Our aim was to explore the relationship between self-reported gut health, intestinal permeability and psychological distress among older adults.METHODS: Three study populations were included: 1) older adults with GI symptoms (n = 24), 2) a group of older adults representing the general elderly population in Sweden (n = 22) and 3) senior orienteering athletes as a potential model of healthy ageing (n = 27). Questionnaire data on gut-health, psychological distress and level of physical activity were collected. Intestinal permeability was measured by quantifying zonulin in plasma. The level of systemic and local inflammation was monitored by measuring C-reactive protein (CRP), hydrogen peroxide in plasma and calprotectin in stool samples. The relationship between biomarkers and questionnaire data in the different study populations was illustrated using a Principal Component Analysis (PCA).RESULTS: Older adults with GI symptoms displayed significantly higher levels of both zonulin and psychological distress than both general older adults and senior orienteering athletes. The PCA analysis revealed a separation between senior orienteering athletes and older adults with GI symptoms and showed an association between GI symptoms, psychological distress and zonulin.CONCLUSIONS: Older adults with GI symptoms express increased plasma levels of zonulin, which might reflect an augmented intestinal permeability. In addition, this group suffer from higher psychological distress compared to general older adults and senior orienteering athletes. This relationship was further confirmed by a PCA plot, which illustrated an association between GI symptoms, psychological distress and intestinal permeability.

    • I det här kapitlet diskuterar vi lyssnandets roll i skolans musikundervisning. När människan lyssnar på musik, hör hon sällan bara det som klingar. Mer eller mindre medvetet associerar hon också till personliga minnen, särskilda platser eller musikaliska och sociala konventioner som finns i samhället. Det kan vara kulturella föreställningar, konnotationer, om hur en viss musik är förbunden med vissa (grupper av) människor – exempelvis kvinnor, män, svenskar, homosexuella. Det kan vara musikaliska konventioner som är knutna till en viss stil – tonartshöjning, ett särskilt ”sound” eller typiska ackordfärgningar. Sådana konnotationer och konventioner kan påverka hur musiken värderas, vilket i sin tur kan göra människor ”döva” för musik som känns främmande. Att öka sin kompetens att ta till sig sådan musik, är inte synonymt med att lära sig att ”tycka om”. Det handlar snarare om att kunna möta skilda musikstilar, utövare eller lyssnare med respekt. Varför och hur man arbetar med musiklyssning i undervisningen kan, och bör, variera. Vi har dock avgränsat den här texten till att handla om arbetssätt som är inriktade dels mot adekvata beskrivningar av hur den klingande musiken låter (som exempelvis ljudbild, tempo, rytm, instrument), dels mot diskussioner om vad i detta som gör att musiken kan upplevas som o/intressant, nyskapande eller förutsägbar. Vi menar att detta kan bidra till att elever kommer förbi för-givet-tagna föreställningar, vilket också har en viktig funktion att fylla i utformandet av en jämlik musikundervisning.Vårt resonemang tar sin utgångspunkt i att människor utvecklar kunskap och förståelse genom att tillägna sig och tolka redan existerande uppfattningar i ett samhälle. Som individer föds vi in i ett visst sammanhang som är betydelsefullt för våra tankar, idéer och föreställningar, men också för vad och hur vi lär. Detta innebär att våra erfarenheter, tolkningar och vårt lärande delvis är kollektivt präglade. Vår vardagliga förståelse och våra vardagliga tolkningar tas ofta för givna och blir sällan föremål för reflektion. Det betyder däremot inte att vi behöver vara fast i dessa för-givet-taganden. Skolan är en plats där våra erfarenheter, föreställningar och argument kan prövas och utmanas genom möten med andra/s erfarenheter och synsätt. Vår utgångspunkt och bärande tanke kan därmed benämnas ett hermeneutiskt bildningsperspektiv (Gustavsson 2009; Schuback Sá Cavalcante & Ruin 2006: 42).
