  • Behandlingsfamiljer för ungdomar med allvarliga beteendeproblem – Treatment Foster Care Oregon. En systematisk översikt och utvärdering inklusive ekonomiska och etiska aspekter

    • Varje år placeras ungefär 2 000 ungdomar på institution på grund av allvarliga beteendeproblem. Ungdomarna placeras då antingen på särskilda ungdomshem eller på kommunala eller privata Hem för vård eller boende (HVB). En alternativ placering för dessa ungdomar är i en så kallad behandlingsfamilj. Det är en tidsbegränsad insats där utbildade familjehemsföräldrar tillsammans med ett team bedriver behandling. En version av behandlingsfamilj är Treatment Foster Care Oregon (TFCO). TFCO finns i begränsad omfattning i Sverige sedan början av 2000-talet. Slutsatser När ungdomar med allvarliga beteendeproblem placeras i TFCO, leder detta troligtvis till mindre fortsatt kriminalitet och färre placeringar på låst avdelning än när ungdomarna placeras på institution. Det är också möjligt att TFCO leder till färre kriminella kamrater, mindre användning av narkotika och bättre psykisk hälsa, än vid placering på institution. TFCO kostar i genomsnitt mindre än placering på särskilda ungdomshem och något mer än på HVB, om man räknar med att vårdtiderna är desamma. Väger man in antaganden om långsiktiga effekter är TFCO kostnadsbesparande för samhället jämfört med placering på särskilda ungdomshem eller HVB. Idag får cirka 30–40 ungdomar i Sverige årligen tillgång till TFCO. Om fler ska få tillgång måste fler behandlingsteam inrättas, vilket kortsiktigt medför kostnader för utbildning och certifiering. behandlingsfamiljer för ungdomar med allvarliga beteendeproblem – treatment foster care oregon Alternativet till behandlingsfamiljer är institutionsvård som ofta kompletteras med olika behandlingsmetoder. Kunskapen om metodernas nytta och risker är bristfällig eftersom det saknas studier. Ur ett etiskt perspektiv är det angeläget att få tillförlitlig kunskap om för- och nackdelar med de övriga kompletterande behandlingar som används.

  • Interventions in child welfare : A Swedish inventory

    • In Swedish child welfare, there are no mandatory guidelines on what interventions to use. Local authorities are able to set their own criteria for implementing or designing interventions. We carried out a survey to identify interventions in use in Children's Social Services and Child and Adolescent Psychiatric Care in Sweden. A total of 102 interventions were stated to have been in use, with between 31 and 45 different interventions for each of the four different child welfare populations. Of the 102 interventions, 56 were designed outside Sweden and later imported. Only 27 interventions were supported with some kind of research evidence. About half of the interventions targeted the child. Possible implications for practice and research are discussed.

  • Treatment Foster Care Oregon for seriously delinquent adolescents: A systematic review and assessment including economic and ethical aspects

    • Approximately 2,000 adolescents are annually placed in residential care in Sweden due to their behavior problems. Those adolescents also have substantially increased risks of mental and physiological problems at time of care entry, while in care and in adulthood. An alternative to residential care is Treatment Foster Care Oregon, TFCO (previously called Multidimensional Treatment Foster Care). TFCO has been available in Sweden since the beginning of the 2000s. This report reviews and synthesizes the evidence regarding TFCO for seriously delinquent adolescents.

    • This article takes its starting point in Sami and Roma exposure to racism by studying preconditions within the criminal law for dealing with racism against individual victims of crime. The purpose is to identify whether and how individual victims can obtain remedy and reparation when exposed to racism. In this article, that means that the racist act must be criminalized, that the criminal interest of crim- inalization is the individual victim, and that the racist nature of the crime has legal implications for classification, sentencing and pun- ishment. Methodologically, the study focuses on identifying possible challenges in the application of so-called hate crimes legislation. Two challenges to the application of individual remedies and reparations were identified. First is the difficulty in identifying and establishing (racist) motive. Second is the lack of clear statements from the courts about how assessments have been made in the use of aggravating and mitigating circumstances in sentencing, and if or how they have affected sentencing and punishment. The results of this study show that the legal conditions for individual victims to obtain criminal justice when they have been subjected to racism are insufficient. Problems are primarily due to deficiencies in the formulation of the legislation, but are also seen in its application. Despite a strong contemporary political focus on work to combat racism, no legal changes have been proposed. Based on the results of this study, we suggest the appli- cation of a holistic approach to the legislation on hate crimes. This would enable individual redress for the racism to which Sami and Roma, among others, are exposed.

  • Peak CKMB and cTnT accurately estimates myocardial infarct size after reperfusion

    • OBJECTIVES: To find the time-to-peak for creatine kinase MB(mass) (CKMB) and cardiac troponin T (cTnT) after acute reperfusion, to compare peak and cumulative values to estimate infarct size (IS), and to evaluate clinical routine sampling for assessment of IS. DESIGN: Acute primary percutaneous coronary intervention (PCI) was performed in 38 patients with first-time myocardial infarction. In 21 patients, CKMB and cTnT were acquired before PCI and at 1.5, 3, 6, 12, 18, 24, and 48 hours thereafter. In 17 patients, clinical routine samples were acquired at arrival, and at 10 and 20 h. IS was assessed by delayed contrast-enhanced MRI (DE-MRI). RESULTS: Time-to-peak was 7.6+/-3.6 h for CKMB and 8.1+/-3.4 h for cTnT. Peak values correlated strongly to cumulative values (r(s)=0.97-0.98) as well as to DE-MRI (r(s)=0.8-0.82). Clinical routine sampling showed lower rs values (0.47-0.60). CONCLUSIONS: Peak values are likely captured if CKMB and cTnT are acquired at 3, 6, and 12 h after acute PCI. These peak values can be used to estimate myocardial infarct size after acute PCI

  • Can the Human Resources Index (HRI) Be Used as a Process Feedback Measurement in a Structured Support Model for Systematic Work Environment Management?

    • The aim of the study was to estimate the level of the human resources index (HRI) measure among Swedish municipal employees, and to investigate the association between human resources index (HRI) and relational justice, short-term recovery, work environment-related production loss, and health-related production loss. A cross-sectional design was used with one sample of municipal employees (n = 6402). The results showed a positive association (r = 0.31) between human resources index (HRI) and relational justice; a positive (r = 0.27) association between HRI and short-term recovery; a negative association between HRI and work environment-related production loss (r = -0.37); and a negative association between HRI and health-related production loss (r = -0.23). The findings implicate that HRI captures important aspects of the work environment such as productivity, relational justice, and short-term recovery. The HRI measure is part of a support model used in workplaces to systematically address work environment-related issues. Monitoring changes in the HRI measure, it is possible to determine whether the measures taken effect production loss, perceived leadership, and short-term recovery in a work group. The support model using HRI may thus be used to complement traditional work environment surveys conducted in Swedish organizations as obliged by legal provisions.
