    • Geographical accessibility is fundamental when planning rescue services. At present there are several programs for computing the accessibility of stationary rescue units. This study suggests a method for evaluating geographic accessibility in scenarios containing also non-stationary units. The method supports the planning process by matching the risk of an incident occurring with the rescue units’ capabilities and accessibility. The method is implemented in the computer program Rescue Unit Planner. The result of the analysis is presented in thematic maps and graphs as level of coverage, mean response time and concentration. The method and the computer program have been evaluated in two case studies: one urban area in southern Sweden and one rural area in northern Sweden. The case studies show that that this method can be useful in the rescue service planning process.

    • The Wechsler Intelligence Scale for Children-Revised (WISC-R) was applied (in a Swedish version) in 120 children with Asperger syndrome, autistic disorder, and attention disorders. Using stepwise logistic regression analysis, the WISC's discriminating ability was investigated. The overall rate of correct diagnostic classification was 63%. Further, WISC profiles were analysed within each group. The group with autistic disorder was characterised by a peak on Block Design. The Asperger syndrome group had good verbal ability and troughs on Object Assembly and Coding. The group with attention disorders had troughs on Coding and Arithmetic. The results suggest that Kaufman's Verbal Comprehension, Perceptual Organisation and Freedom from Distractibility factors rather than verbal IQ and performance IQ account for the variance on the WISC. Furthermore, the Asperger syndrome and autistic disorder groups differed in respect of "fluid" and "crystallised" cognitive ability.

    • Systemet för Byggvarudeklarationen har idag en omfattande användning och är i jämförelse med andra länder ett unikt system för miljöinformation. Systemet har i dagsläget fler än 10 000 deklarerade byggvaror. Det är accepterat av både leverantörer och användare av byggvaror som ett överenskommet format för att lämna miljöinformation. Informationen används för att dokumentera och göra miljöanpassade materialval vid nyproduktion, drift och ändring av byggnader. Innehållet i deklarationerna garanteras av den enskilde byggvaruleverantören. I föreliggande projekt har Byggvarudeklarationen uppdaterats i enlighet med förändrad lagstiftning, nya marknadskrav samt anpassats för att informationen ska kunna hanteras digitalt genom förädlingskedjan inom samhällsbyggnadssektorn. Lagstiftning som har legat till grund för uppdateringen är: - Byggproduktförordningen och kraven på prestandadeklaration -REACH, CLP och RoHS - Direktivet om offentlig upphandling När det gäller marknadskraven är det främst de olika miljöcertifieringssystem (Miljöbyggnad, BREEAM, LEED) för byggnader som idag tillämpas i Sverige som har varit utgångspunkten för förändringarna. Det digitala formatet har utvecklats med fokus på att göra Byggvarudeklarationerna spårbara genom unika dokumentidentiteter dvs att enskilda artiklar har en koppling till en unik byggvarudeklaration. XML-schema även utvecklats utifrån det överenskomna innehållet så att all miljöinformation kan överföras digitalt. Vidare har en applikation och en databas utvecklats som ska användas av den som vill upprätta en digital byggvarudeklaration. Den nya byggvarudeklarationen innehåller följande 11 avsnitt: 1. Grunddata 2. Hållbarhetsarbete 3. Innehållsdeklaration 4. Råvaror 5. Miljöpåverkan 6. Distribution 7. Byggskede 8. Bruksskede 9. Rivning 10. Avfallshantering 11. Innemiljö

Design and Setting: We sought to develop a mortality model of the Swedish general population born between 1973 and 2010, using continuous-hazard functions adjusting for birth characteristics, sex, age intervals, and calendar year to estimate standardized mortality ratio (SMR) and to apply this model to assess expected deaths in Swedish rhGH-treated patients with idiopathic isolated GH deficiency (IGHD), idiopathic short stature (155) or born small for gestational age (SGA). Participants:The general population: Swedish Medical Birth Register (1973-2010: 1 880 668 males; 1 781 131 females) and Cause of Death Register (1985-2010). Intervention Population: Three thousand eight hundred forty-seven patients starting rhGH treatment between 1985 and 2010 and followed in the National GH Register and/or in rhGH trials diagnosed with IGHD (n = 1890), ISS (n = 975), or SGA (n=982). Main Outcome Measures: Death. Results: Using conventional models adjusting for age, sex, and calendar-year, the SMR was 1.43 (95% confidence interval, 0.89-2.19), P = .14, observed/expected deaths 21/14.68. The rhGH population differed (P amp;lt; .001) from the general population regarding birth weight, birth length, and congenital malformations. Application of an Advanced Model: When applying the developed mortality model of the general population, the ratio of observed/expected deaths in rhGH-treated patients was 21/21.99; SMR = 0.955 (0.591-1.456)P = .95. Model Comparison: Expected number of deaths were 14.68 (14.35-14.96) using the conventional model, and 21.99 (21.24-22.81) using the advanced model, P amp;lt; .001, which had at all ages a higher gradient of risk per SD of the model, 24% (range, 18-42%; P amp;lt; .001). Conclusions: Compared with the general Swedish population, the ratio of observed/expected deaths (21/21.99) was not increased in childhood rhGH-treated IGHD, ISS, and SGA patients when applying an advanced sex-specific mortality model adjusting for birth characteristics.

  • Influence of neighbourhood purchasing power on breastfeeding at four months of age : a Swedish population-based cohort study

    • BACKGROUND: Parental socioeconomic status (SES) is an important determinant in child health, influencing beneficial factors such as breastfeeding. A better understanding of the influence of neighbourhood-level SES measures, relating to spatial determinants, might lead to targeted actions to promote breastfeeding during infancy.METHODS: A cross-sectional study analysis the association between breastfeeding at four months of age and neighbourhood purchasing power, taking account of individual-level variables including maternal age, smoking and parental level of education. Data were obtained from a prospective population- based cohort study recruited from birth in 2007-2008 in the Halland region, southwestern Sweden. Questionnaire data on the individual-level variables and the outcome variable of breastfeeding at four months (yes/no) were used (n=2,407). Each mother was geo-coded with respect to her residential parish (there are 61 parishes in the region) and then stratified by parish-level household purchasing power. It emerged that four neighbourhood characteristics were reasonable to use, viz. <10%, 10-19%, 20-29% and ≥ 30% of the resident families with low purchasing power.RESULTS: The proportion of mothers not breastfeeding at four months of age showed a highly significant trend across the neighbourhood strata (p=0.00004): from 16.3% (< 10% with low purchasing power) to 29.4% (≥ 30% with low purchasing power), yielding an OR of 2.24 (95% confidence interval: 1.45-3.16). After adjusting for the individual-level variables, the corresponding OR=1.63 (1.07-2.56) was significant and the trend across the strata was still evident (p=0.05). A multi-level analysis estimated that, in the neighbourhoods with ≥ 30% of the families with low purchasing power, 20% more mothers than expected, taking account of the individual-level factors, reported no breastfeeding at four months of age (≥ 95% posterior probability of an elevated observed-to-expected ratio).CONCLUSION: The neighbourhood purchasing power provided a spatial determinant of low numbers of mothers breastfeeding at four months of age, which could be relevant to consider for targeted actions. The elevated observed-to-expected ratio in the neighbourhoods with the lowest purchasing power points toward a possible contextual influence. © 2013 Almquist-Tangen et al.; licensee BioMed Central Ltd.
