  • Comparison of Fenton's Reagent and Ozone Oxidation of Polycyclic Aromatic Hydrocarbons in Aged Contaminated Soils

    • Background, Aim and Scope: Polycyclic aromatic hydrocarbons (PAHs) are formed as a result of incomplete combustion and are among the most frequently occurring contaminants in soils and sediments. PAHs are of great environmental concern due to their ubiquitous nature and toxicological properties. Consequently, extensive research has been conducted into the development of methods to remediate soils contaminated with PAHs. Fenton's reagent or ozone is the most commonly studied chemical oxidation methods. However, the majority of remediation studies use soils that have been artificially contaminated with either one or a limited number of PAH compounds in the laboratory. Hence, it is essential to extend such studies to soils contaminated with multiple PAHs under field conditions. Objectives. The objective of this study is to investigate the capacity of Fenton's reagent and ozone to degrade PAHs in soils. The soils have been collected from a number of different industrial sites and, therefore, will have been exposed to different PAH compounds in varying concentrations over a range of time periods. The capacity of Fenton's reagent and ozone to degrade PAHs in industrially contaminated soils is compared to results obtained in studies using soils artificially contaminated with PAHs in the laboratory. Materials and Methods: Nine soil samples, contaminated with PAHs, were collected from five different industrial sites in Sweden. For the Fenton's reagent procedure, the pH of the soil slurry samples was adjusted to pH 3 and they were kept at a constant temperature of 70ºC whilst H2O2 was added. For the ozone procedure, soil samples were mixed with 50% water and 50% ethanol and kept at a constant temperature of 45 ºC. Ozone was then continually introduced to each soil sample over a period of four hours. Following the Fenton's reagent and ozone oxidation procedures, the samples were filtered to isolate the solid phase, which was then extracted using pressurized liquid extraction (PLE). The sample extracts were cleaned up using open columns and then analysed by gas chromatography-mass spectrometry (GC-MS). Results: The relative abundance of the detected PAHs varied between soils, associated with different industries. For example, low molecular weight (LMW) PAHs were more abundant in soil samples collected from wood impregnation sites and high overall PAH degradation efficiencies were observed in soils originating from these sites. In the contaminated soils studied, PAHs were more effectively degraded using Fenton's reagent (PAH degradation efficiency of 40-86%) as opposed to ozone (PAH degradation efficiency of 10-70%). LMW PAHs were more efficiently degraded, using ozone as the oxidizing agent, whereas the use of Fenton's reagent resulted in a more even degradation pattern for PAHs with two through six fused aromatic rings. Discussion: The degradation efficiency for both methods was largely dependent on the initial PAH concentration in the soil sample, with higher degradation observed in highly polluted soils. LMW PAHs are more susceptible to degradation than high molecular weight (HMW) PAHs. As a result of this the relative abundance of large (often carcinogenic) PAHs increased after chemical oxidation treatment, particularly after ozone treatment. Repeated Fenton's reagent treatment did not result in any further degradation of soil PAHs, indicating that residual soil PAHs are strongly sorbed. The effectiveness of the two oxidation treatment approaches differed between industrial sites, thus highlighting the importance of further research into the influence of soil properties on the sorption capacity of PAHs. Conclusions: This study demonstrates that the degree to which chemical oxidation techniques can degrade soil bound PAHs chemical degradation is highly dependent on both the concentration of PAHs in the soils and the compounds present, i.e. the various PAH profiles. Therefore, similarities in the PAH degradation efficiencies in the nine soil samples studied were observed with the two chemical oxidation methods used. However, the degradation performance of Fenton's reagent and ozone differed between the two methods. Overall, Fenton's reagent achieved the highest total PAH degradation due to stronger oxidation conditions. LMW PAHs showed higher susceptibility to oxidation, whereas high molecular weight (HMW) PAHs appear to be strongly sorbed to the soils and therefore less chemically available for oxidation. This study highlights the importance of including soils collected from a range of contaminated sites in remediation studies. Such soil samples will contain PAH contaminants of varying concentrations, chemical and physical properties, and have been aged under field conditions. In addition to the chemical and physical properties of the soils, these factors will all influence the chemical availability of PAHs to oxidation. Recommendations and Perspectives: We recommend including aged contaminated soils in chemical degradation studies. In future chemical remediation work, we intend to investigate the potential influence of the chemical and physical properties of PAHs and soil parameters potential influence on the chemical oxidation efficiency in aged contaminated soils. Due to the vast number of contaminated sites there is a great need of efficient remediation methods throughout the world. This study shows the difficulties which may be experienced when applying remediation methods to a variation of contaminated sites.

    • In this study, we investigated how the chemical degradability of polycyclic aromatic hydrocarbons (PAHs) in aged soil samples from various contaminated sites is influenced by soil characteristics and by PAH physico-chemical properties. The results were evaluated using the multivariate statistical tool, partial least squares projections to latent structures (PLS). The PAH-contaminated soil samples were characterised (by pH, conductivity, organic matter content, oxide content, particle size, specific surface area, and the time elapsed since the contamination events, i.e. age), and subjected to relatively mild, slurry-phase Fenton's reaction conditions. In general, low molecular weight PAHs were degraded to a greater extent than large, highly hydrophobic variants. Anthracene, benzo(a)pyrene, and pyrene were more susceptible to degradation than other, structurally similar, PAHs; an effect attributed to the known susceptibility of these compounds to reactions with hydroxyl radicals. The presence of organic matter and the specific surface area of the soil were clearly negatively correlated with the degradation of bi- and tri-cyclic PAHs, whereas the amount of degraded organic matter correlated positively with the degradation of PAHs with five or six fused rings. This was explained by enhanced availability of the larger PAHs, which were released from the organic matter as it degraded. Our study shows that sorption of PAHs is influenced by a combination of soil characteristics and physico-chemical properties of individual PAHs. Multivariate statistical tools have great potential for assessing the relative importance of these parameters.

    • Vi har bedrivit en forskningscirkel med syfte att utvärdera värmekameran som visualiseringsteknik i olika ämnen på naturbruksprogrammet genom att planera, genomföra och reflektera kring lektionsupplägg, där elever har fått möjlighet att använda värmekameror med olika grad av styrning. Resultatet visar att värmekameran har många skilda tillämpningsområden inom lantbruk och djurhållning och kan användas i undervisningen som ett verktyg för elevers praktiska undersökningar av t.ex. djurs fysiologi, eller som ett mätinstrument då tillfälle ges, t.ex. för att studera juverbölder hos suggor eller temperaturökning när en skruv tar fel gäng. En utmaning i undervisningen är att finna en balans mellan öppenhet och styrning, där eleverna upplever att de har möjlighet att undersöka fenomen de själva är intresserade av, men utifrån ett etablerat, systematiskt arbetssätt. Sammanfattningsvis har vi upplevt användning av värmekameror som ett effektivt och intresseväckande sätt att konkretisera och individanpassa undervisningen på naturbruksprogrammet, vilket vi kan rekommendera andra lärare att prova.

  • Smoking is associated with a worse self-reported health status in patients with psoriatic arthritis: data from a Swedish population-based cohort

    • The aim was to study possible associations between smoking habits and self-reported clinical features in a large population-based cohort of patients with psoriatic arthritis (PsA). All subjects with PsA who had sought health care in the period 2003-2007 were identified using a regional health-care register. In 2009, all those identified who were 18 years of age or more (n = 2,003) were sent a questionnaire with questions on smoking, health-related quality of life [EuroQol five-dimension (EQ-5D)questionnaire], function [Health Assessment Questionnaire (HAQ)], pain, fatigue, and global health. We performed age- and sex-adjusted regression analysis to compare health status outcomes in never and ever smokers. Altogether, 1,185 subjects (59 %) returned the questionnaire. Mean age was 57 years (SD 13.5), and 58 % were women; 38 % were never smokers and 62 % were ever smokers. Mean age at disease onset was 38.2 years (SD 13.2) and 41.2 years (SD 13.6), respectively (p = 0.001). In age- and sex-adjusted data, ever smokers reported worse EQ-5D (p = 0.009); worse reports of global health (p = 0.01), pain (p = 0.01), and fatigue (p = 0.04); and a higher number of painful body regions (p = 0.04) compared to never smokers. In this population-based PsA cohort, patients who were ever smokers reported worse health status than never smokers. Besides being a possible result of a worse PsA in ever smokers, impaired health status could also be an effect of unstudied comorbidities. Further longitudinal studies are needed to gain a better understanding of cause and effect. However, smoking cessation should be recommended because of general health considerations as well as disease-specific issues.

  • What is in the fish? Collaborative trial in suspect and non-target screening of organic micropollutants using LC- and GC-HRMS

    • A collaborative trial involving 16 participants from nine European countries was conducted within the NORMAN network in efforts to harmonise suspect and non-target screening of environmental contaminants in whole fish samples of bream (Abramis brama). Participants were provided with freeze-dried, homogenised fish samples from a contaminated and a reference site, extracts (spiked and non-spiked) and reference sample preparation protocols for liquid chromatography (LC) and gas chromatography (GC) coupled to high resolution mass spectrometry (HRMS). Participants extracted fish samples using their in-house sample preparation method and/or the protocol provided. Participants correctly identified 9–69 % of spiked compounds using LC-HRMS and 20–60 % of spiked compounds using GC-HRMS. From the contaminated site, suspect screening with participants’ own suspect lists led to putative identification of on average ∼145 and ∼20 unique features per participant using LC-HRMS and GC-HRMS, respectively, while non-target screening identified on average ∼42 and ∼56 unique features per participant using LC-HRMS and GC-HRMS, respectively. Within the same sub-group of sample preparation method, only a few features were identified by at least two participants in suspect screening (16 features using LC-HRMS, 0 features using GC-HRMS) and non-target screening (0 features using LC-HRMS, 2 features using GC-HRMS). The compounds identified had log octanol/water partition coefficient (KOW) values from −9.9 to 16 and mass-to-charge ratios (m/z) of 68 to 761 (LC-HRMS and GC-HRMS). A significant linear trend was found between log KOW and m/z for the GC-HRMS data. Overall, these findings indicate that differences in screening results are mainly due to the data analysis workflows used by different participants. Further work is needed to harmonise the results obtained when applying suspect and non-target screening approaches to environmental biota samples.

  • The Prognostic Importance of CD20+ B lymphocytes in Colorectal Cancer and the Relation to Other Immune Cell subsets

    • The anti-tumour immune response is critical to patient prognosis in colorectal cancer (CRC). The aim of this study was to investigate infiltration of B lymphocytes into CRC tumours, and their clinical relevance, prognostic value and relation to other immune cell subsets. We used multiplexed immunohistochemistry and multispectral imaging to assay the amount of infiltrating CD20+ B lymphocytes along with infiltration of CD8+ cytotoxic T cells, FOXP3+ T regulatory cells, CD68+ macrophages and CD66b+ neutrophils, in 316 archival CRC tissue specimens. A higher density of infiltrating CD20+ B lymphocytes was associated with tumours of the right colon (P = 0.025) and of lower stages (P = 0.009). Furthermore, patients whose tumours were highly infiltrated by CD20+ B lymphocytes had a significantly improved disease-specific survival (HR = 0.45, 95% CI 0.28-0.73, P = 0.001), which remained significant in multivariable analysis. CD20+ B lymphocytes were highly and positively associated with CD8+ T lymphocytes (P < 0.001), and part of the prognostic role was found to be a cooperative effect between these lymphocyte subsets. Our results support a favourable prognostic value of tumour-infiltrating CD20+ B lymphocytes in CRC. Furthermore, a cooperative prognostic effect between CD20+ B lymphocytes and CD8+ T lymphocytes is suggested.

  • Difficulties in diagnostics of lung tumours in biopsies : an interpathologist concordance study evaluating the international diagnostic guidelines

    • AIMS: Accurate and reliable diagnosis is essential for lung cancer treatment. The study aim was to investigate interpathologist diagnostic concordance for pulmonary tumours according to WHO diagnostic criteria.METHODS: Fifty-two unselected lung and bronchial biopsies were diagnosed by a thoracic pathologist based on a broad spectrum of immunohistochemical (IHC) stainings, molecular data and clinical/radiological information. Slides stained with H&E, thyroid transcription factor-1 (TTF-1) clone SPT24 and p40 were scanned and provided digitally to 20 pathologists unaware of reference diagnoses. The pathologists independently diagnosed the cases and stated if further diagnostic markers were deemed necessary.RESULTS: In 31 (60%) of the cases, ≥80% of the pathologists agreed with each other and with the reference diagnosis. Lower agreement was seen in non-small cell neuroendocrine tumours and in squamous cell carcinoma with diffuse TTF-1 positivity. Agreement with the reference diagnosis ranged from 26 to 45 (50%-87%) for the individual pathologists. The pathologists requested additional IHC staining in 15-44 (29%-85%) of the 52 cases. In nearly half (17 of 36) of the malignant cases, one or more pathologist advocated for a different final diagnosis than the reference without need of additional IHC markers, potentially leading to different clinical treatment.CONCLUSIONS: Interpathologist diagnostic agreement is moderate for small unselected bronchial and lung biopsies based on a minimal panel of markers. Neuroendocrine morphology is sometimes missed and TTF-1 clone SPT24 should be interpreted with caution. Our results suggest an intensified education need for thoracic pathologists and a more generous use of diagnostic IHC markers.

    • Background: The effect of the genetic imprint on the emergency presentation of colon cancer remains unclear. The disparity between tumours evolving along different carcinogenetic pathways has not been studied systematically. This retrospective multicenter cohort study evaluates the association between mismatch repair status and the risk for acute surgery of colon cancer.Patients and methods: A retrospective multicenter cohort study including in total 870 patients from three different countries. Scandinavian cohort (Finland and Sweden), including a total of 412 patients operated between January 1, 1995 and December 31, 2010, was validated against a cohort from the Czech Republic, including a total of 458 patients, operated between January 1, 2018 and December 31, 2019. The proficiency or deficiency of mismatch repair was determined by immunohistochemistry. Primary outcome was the risk for acute colon cancer surgery given as the Odds Ratio (OR) in the univariable and multivariable analyses. Acute colon cancer surgery was defined as surgery performed during the same hospital admission as when the diagnosis of colon cancer was made.Results: Of the 870 patients (399 females [46%]) included in the analyses, median age at surgery was 69 [interquartile range, 61–76] years, deficient Mismatch Repair (dMMR) status was found in 190 patients (22%), and 179 patients (21%) underwent acute surgery during the same hospital admission as when the diagnosis of colon cancer was made. In the Scandinavian cohort, a significant association between dMMR status and acute surgery was seen in both the univariable (OR 1.82, 95% CI 1.11–3.02, P = 0.017) and the multivariable (OR = 2.21, 95% CI 1.28–3.95, P = 0.005) analyses. This was confirmed in the Czech validation cohort in both the univariable (OR = 1.94, 95% CI 1.09–3.26, P = 0.022) and the multivariable (OR = 1.77, 95% CI 1.15–3.18, P = 0.021) analyses.Conclusion: This multicenter study reveals a strong association between acute colon cancer surgery and dMMR tumour status.

  • Sporadic deficient mismatch repair in colorectal cancer increases the risk for non-colorectal malignancy : a European multicenter cohort study

    • Background and Objectives: Disparities between tumors arising via different sporadic carcinogenetic pathways have not been studied systematically. This retrospective multicenter cohort study evaluated the differences in the risk for non-colorectal malignancy between sporadic colorectal cancer (CRC) patients from different DNA mismatch repair status.Methods: A retrospective European multicenter cohort study including in total of 1706 CRC patients treated between 1996 and 2019 in three different countries. The proficiency (pMMR) or deficiency (dMMR) of mismatch repair was determined by immunohistochemistry. Cases were analyzed for tumor BRAFV600E mutation, and BRAF mutated tumors were further analyzed for hypermethylation status in the promoter region of MLH1 to distinguish between sporadic and hereditary cases. Swedish and Finish patients were matched with their respective National Cancer Registries. For the Czech cohort, thorough scrutiny of medical files was performed to identify any non-colorectal malignancy within 20 years before or after the diagnosis of CRC. Poisson regression analysis was performed to identify the incidence rates of non-colorectal malignancies. For validation purposes, standardized incidence ratios were calculated for the Swedish cases adjusted for age, year, and sex.Results: Of the 1706 CRC patients included in the analysis, 819 were female [48%], median age at surgery was 67 years [interquartile range: 60–75], and sporadic dMMR was found in 188 patients (11%). Patients with sporadic dMMR CRC had a higher incidence rate ratio (IRR) for non-colorectal malignancy before and after diagnosis compared to patients with a pMMR tumor, in both uni- (IRR = 2.49, 95% confidence interval [CI] = 1.89–3.31, p = 0.003) and multivariable analysis (IRR = 2.24, 95% CI = 1.67–3.01, p = 0.004). This association applied whether or not the non-colorectal tumor developed before or after the diagnosis of CRC in both uni- (IRR = 1.91, 95% CI = 1.28–2.98, p = 0.004), (IRR = 2.45, 95% CI = 1.72–3.49, p = 0.004) and multivariable analysis (IRR = 1.67,95% CI = 1.05–2.65, p = 0.029), (IRR = 2.35, 95% CI = 1.63–3.42, p = 0.005), respectively.Conclusion: In this retrospective European multicenter cohort study, patients with sporadic dMMR CRC had a higher risk for non-colorectal malignancy than those with pMMR CRC. These findings indicate the need for further studies to establish the need for and design of surveillance strategies for patients with dMMR CRC.

  • Association between local immune cell infiltration, mismatch repair status and systemic inflammatory response in colorectal cancer

    • BACKGROUND: Systemic inflammatory response in colorectal cancer (CRC) has been established as a prognostic factor for impaired cancer-specific survival, predominantly in patients with right-sided tumors. On the other hand, defective mismatch repair (dMMR) tumors, primarily located in the right colon, are known to have favorable survival and dense local immune infiltration. The aim of this study was to see if there is any form of relationship between these seemingly diverse entities.METHODS: Complete clinical and long-term survival data were retrieved for 316 CRC patients operated at Helsinki University Hospital between the years 1998 and 2003. Tissue microarrays were prepared from surgical specimens and further processed and analyzed for local immune cell infiltration using multispectral imaging with a Vectra quantitative pathology imaging system and Inform software. Multiplex immunohistochemistry was applied using antibodies against CD66b, CD8, CD20, FoxP3, CD68 and pan-Cytokeratin. After exclusions, data on immune infiltration were available for 275 patients. Mismatch repair status was determined by immunohistochemistry.RESULTS: CRP was seen to be an independent predictor of cancer-specific survival but not overall survival in uni- and multivariable (HR 1.01 (1.00-1.02); p = 0.028) analyses of non-irradiated patients. There was no significant difference in CRP according to mismatch repair status, but all cases (n = 10) with CRP ≥ 75 mg/l had proficient mismatch repair (pMMR). There was a significant negative correlation between intratumor stromal infiltration by T-regulatory FOXP3+ cells and CRP (p = 0.006). There was significantly lower intratumor stromal infiltration by FOXP3+ cells (p = 0.043) in the right colon compared to the rectum, but no significant difference in CRP (p = 0.44). CRP was not a predictor of overall survival (HR 0.99, 95% CI 0.98-1.01) nor cancer-specific survival in irradiated patients (HR 0.94, 95% CI 0.94-1.02).CONCLUSIONS: There was a significant negative relationship between SIR, defined as an elevated CRP, and intratumor stromal infiltration by T-regulatory FOXP3+ cells. This and the fact that all cases with a CRP > 75 mg/l had pMMR suggests that SIR and dMMR are independent entities in CRC. Indeed, the general lack of difference in CRP between cases with dMMR and pMMR may be evidence of overlap in cases with a less pronounced SIR.
