  • Effects of oxygen on post-surgical infections during an individualised perioperative open-lung ventilatory strategy : a randomised controlled trial

    • Background: We aimed to examine whether using a high fraction of inspired oxygen (FIO2) in the context of an individualised intra- and postoperative open-lung ventilation approach could decrease surgical site infection (SSI) in patients scheduled for abdominal surgery. Methods: We performed a multicentre, randomised controlled clinical trial in a network of 21 university hospitals from June 6, 2017 to July 19, 2018. Patients undergoing abdominal surgery were randomly assigned to receive a high (0.80) or conventional (0.3) FIO2 during the intraoperative period and during the first 3 postoperative hours. All patients were mechanically ventilated with an open-lung strategy, which included recruitment manoeuvres and individualised positive end-expiratory pressure for the best respiratory-system compliance, and individualised continuous postoperative airway pressure for adequate peripheral oxyhaemoglobin saturation. The primary outcome was the prevalence of SSI within the first 7 postoperative days. The secondary outcomes were composites of systemic complications, length of intensive care and hospital stay, and 6-month mortality. Results: We enrolled 740 subjects: 371 in the high FIO2 group and 369 in the low FIO2 group. Data from 717 subjects were available for final analysis. The rate of SSI during the first postoperative week did not differ between high (8.9%) and low (9.4%) FIO2 groups (relative risk [RR]: 0.94; 95% confidence interval [CI]: 0.59-1.50; P=0.90]). Secondary outcomes, such as atelectasis (7.7% vs 9.8%; RR: 0.77; 95% CI: 0.48-1.25; P=0.38) and myocardial ischaemia (0.6% [n=2] vs 0% [n=0]; P=0.47) did not differ between groups. Conclusions: An oxygenation strategy using high FIO2 compared with conventional FIO2 did not reduce postoperative SSIs in abdominal surgery. No differences in secondary outcomes or adverse events were found.

  • Measurement of prompt J/psi and beauty hadron production cross sections at mid-rapidity in pp collisions at root s=7 TeV

    • The ALICE experiment at the LHC has studied J/psi production at mid-rapidity in pp collisions at root s = 7 TeV through its electron pair decay on a data sample corresponding to an integrated luminosity L-int = 5.6 nb(-1). The fraction of J/psi from the decay of long-lived beauty hadrons was determined for J/psi candidates with transverse momentum p(t) > 1,3 GeV/c and rapidity vertical bar y vertical bar < 0.9. The cross section for prompt J/psi mesons, i.e. directly produced J/psi and prompt decays of heavier charmonium states such as the psi(2S) and chi(c) resonances, is sigma(prompt J/psi) (p(t) > 1.3 GeV/c, vertical bar y vertical bar < 0.9) = 8.3 +/- 0.8(stat.) +/- 1.1 (syst.)(-1.4)(+1.5) (syst. pol.) mu b. The cross section for the production of b-hadrons decaying to J/psi with p(t) > 1.3 GeV/c and vertical bar y vertical bar < 0.9 is a sigma(J/psi <- hB) (p(t) > 1.3 GeV/c, vertical bar y vertical bar < 0.9) = 1.46 +/- 0.38 (stat.)(-0.32)(+0.26) (syst.) mu b. The results are compared to QCD model predictions. The shape of the p(t) and y distributions of b-quarks predicted by perturbative QCD model calculations are used to extrapolate the measured cross section to derive the b (b) over bar pair total cross section and d sigma/dy at mid-rapidity.

  • Underlying Event measurements in pp collisions at root s=0.9 and 7 TeV with the ALICE experiment at the LHC

    • We present measurements of Underlying Event observables in pp collisions at root s = 0 : 9 and 7 TeV. The analysis is performed as a function of the highest charged-particle transverse momentum p(T),L-T in the event. Different regions are defined with respect to the azimuthal direction of the leading (highest transverse momentum) track: Toward, Transverse and Away. The Toward and Away regions collect the fragmentation products of the hardest partonic interaction. The Transverse region is expected to be most sensitive to the Underlying Event activity. The study is performed with charged particles above three different p(T) thresholds: 0.15, 0.5 and 1.0 GeV/c. In the Transverse region we observe an increase in the multiplicity of a factor 2-3 between the lower and higher collision energies, depending on the track p(T) threshold considered. Data are compared to PYTHIA 6.4, PYTHIA 8.1 and PHOJET. On average, all models considered underestimate the multiplicity and summed p(T) in the Transverse region by about 10-30%.

    • Our understanding of the role of bottom-current and associated oceanographic processes (as overflows, barotropic currents, tides or intermittent processes like giant vertical eddies, deep sea storms, horizontal vortices, internal waves and tsunamis) is rapidly improving. But, most of these oceanographic deep-water processes remains poorly understood and have been rarely observed but believed to be capable of generating depositional and erosional features in many contexts at both short and long term. In present compilation we described these oceanographic processes including examples or their potential role in the sedimentation processes around the Iberia margin. Importance of these processes is highlighted and its implication discussed to demonstrate that these oceanographic processes are very important is affecting the shape and evolution of the Iberian continental margin and adjacent oceanic basins although is not still considered as important as other factors as mass-transport deposits and turbiditic processes. To have a better understanding on the implication of all these oceanographic processes connections have to be made between (contourite) sea-floor features, their spatial and temporal evolution, and the near-bottom flows that form them. The only way to achieve that in next decades is by a more intensive collaboration between specialist from Geology, Oceanography and Benthic Biology in new multi-disciplinary studies. Consequently, further multi-disciplinary approach uses oceanographic data, sea-floor morphology, sediments and seismic characterization will be essential for improve our knowledge of these permanent and intermittent processes around Iberia and evaluate their conceptual and regional role in the margins evolution over time and space.

  • Contourites and associated sediments controlled by deep-water circulation processes: State of the art and future considerations

    • The contourite paradigm was conceived a few decades ago, yet there remains a need to establish a sound connection between contourite deposits, basin evolution and oceanographic processes. Significant recent advances have been enabled by various factors, including the establishment of two IGCP projects and the realisation of several IODP expeditions. Contourites were first described in the Northern and Southern Atlantic Ocean, and since then, have been discovered in every major ocean basin and even in lakes. The 120 major contourite areas presently known are associated to myriad oceanographic processes in surface, intermediate and deep-water masses. The increasing recognition of these deposits is influencing palaeoclimatology & palaeoceanography, slope-stability/geological hazard assessment, and hydrocarbon exploration. Nevertheless, there is a pressing need for a better understanding of the sedimentological and oceanographic processes governing contourites, which involve dense bottom currents, tides, eddies, deep-sea storms, internal waves and tsunamis. Furthermore, in light of the latest knowledge on oceanographic processes and other governing factors (e.g. sediment supply and sea-level), existing facies models must now be revised. Persistent oceanographic processes significantly affect the seafloor, resulting in large-scale depositional and erosional features. Various classifications have been proposed to subdivide a continuous spectrum of partly overlapping features. Although much progress has been made in the large-scale, geophysically based recognition of these deposits, there remains a lack of unambiguous and commonly accepted diagnostic criteria for deciphering the small-scaled contourite facies and for distinguishing them from turbidite ones. Similarly, the study of sandy deposits generated or affected by bottom currents, which is still in its infancy, offers great research potential: these deposits might prove invaluable as future reservoir targets. Expectations for the forthcoming analysis of data from the IODP Expedition 339 are high, as this work promises to tackle much of the aforementioned lack of knowledge. In the near future, geologists, oceanographers and benthic biologists will have to work in concert to achieve synergy in contourite research to demonstrate the importance of bottom currents in continental margin sedimentation and evolution.
